1.Study on kidney injury in cyanosis and acyanosis congenital heart disease after cardiopulmonary bypass operation: a single-center retrospective analysis

Objective:To investigate the acute kidney injury(AKI) after cardiopulmonary bypass surgery for cyanotic congenital heart diseases(C-CHD) as well as and acyanotic congenital heart diseases(A-CHD)and its possible influencing factors.Methods:One hundred and three patients with CHD admitted to PICU of Xinhua Hospital Affiliated to Shanghai Jiaotong University were enrolled from July 1, 2017 to September 30, 2019.The patients were divided into C-CHD group( n=36)and A-CHD group( n=67). The preoperative general data and information related to the operation of two groups were recorded.Hemodynamic data, oxygen metabolism index, dose of vasoactive drugs during or after operation and liquid equilibrium were assessed after surgery.The incidence and stage of AKI were calculated.Multivariate Logistic regression was used to analyze the relationship between hemodynamic index and oxygen metabolism index and AKI.The predictive value of hemodynamics and oxygen metabolism for AKI after cardiac surgery was analyzed by receiver operating characteristic curve (ROC). Results:A total of one hundred and three patients were enrolled including sixty-two males and forty-one females, the median age was 7(4, 24) months old.The preoperative hemoglobin and hematocrit in the C-CHD group were higher than those in the A-CHD group( P<0.05). Compared with the A-CHD group, the C-CHD group had higher American Society of Anesthesiologists Grades and Risk Adjustment for Congenital Heart Surgery Grades, longer operation time and cardiopulmonary bypass time( P<0.05). Central venous pressure in C-CHD group was higher than that in A-CHD group, and C-CHD group had higher dose of vasoactive drugs after surgery( P<0.05). Compared with the C-CHD group, blood lactic acid was lower and arterial oxygen partial pressure was higher in the A-CHD group( P<0.05). Fifty patients developed AKI in the PICU, and the incidence of AKI in the A-CHD group was higher than that in the C-CHD, but there was no statistical significance(53.7% vs.38.9%, P>0.05). Multivariate Logistic regression analysis showed that partial arterial oxygen pressure and arterial oxygen satiety were independent risk factors for AKI( P<0.05). The large circulation index could not directly reflect the renal perfusion.Compared with using hemodynamic index or oxygen metabolism index alone to predict the occurrence of AKI 48 h after the operation, the combined application of the two methods had higher predictive value for AKI. Conclusion:Compared with patient with A-CHD, patients with C-CHD have higher tolerance to renal damage caused by surgery, and the kidney of patient with C-CHD have higher tolerance to postoperative fluid load and demand lower oxygen consumption.It is of great clinical significance to evaluate the changes of hemodynamics and oxygen metabolism after cardiac surgery for the prevention and treatment of renal injury.

3.Attentional Connectivity-based Prediction of Autism Using Heterogeneous rs-fMRI Data from CC200 Atlas

Autism spectrum disorder (ASD) is a developmental syndrome characterized by obvious drawbacks in sociality and communication. It has crucial significance to exactly discern the individuals with ASD and typical controls (TC). Previous imaging studies on ASD/TC identification have made remarkable progress in the exploration of objective as well as crucial biomarkers associated with ASD. However, glaring deficiency is manifested by the investigation on solely homogeneous and small datasets. Thus, we attempted to unveil some replicable and robust neural patterns of autism using a heterogeneous multi-site brain imaging dataset from ABIDE (Autism Brain Imaging Data Exchange). Experiments were carried out with an attention mechanism based on Extra-Trees algorithm, taking the study object of brain connectivity measured with the resting-state functional magnetic resonance imaging (fMRI) data of CC200 atlas. With cross-validation strategy, our proposed method resulted in a mean classification accuracy of 72.2% (sensitivity=68.6%, specificity=75.4%). It raised the precision of ASD prediction by about 2% and specificity by 3.2% in comparison with the most competitive reported effort. Connectivity analysis on the optimal model highlighted informative regions strongly involved in the social cognition as well as interaction, and manifested lower correlation between the anterior and posterior default mode network (DMN) in autistic individuals than controls. This observation is concordant with previous studies, which enables our proposed method to effectively identify the individuals with risk of ASD.
4.Assessment of skeletal muscle early myopathy with contrast‐enhanced ultrasound in rabbit model of diabetes mellitus

Objective To evaluate early skeletal muscle myopathy with contrast‐enhanced ultrasound ( CEUS) in diabetic rabbits . Methods Alloxan ( 100 mg/kg ) was given intravenously to 11 New Zealand w hite rabbits . Another 5 age‐matched normal rabbits were used as controls .CEUS was performed at baseline and 1 ,2 ,3 months after the establishment of diabetes ,respectively . T hen ,skeletal muscle samples were obtained for pathological observation . Additionally ,the diabetic rabbits were divided into 3 groups according to their pathologic findings : mild , moderate , and severe myopathy group . M icro‐perfusion parameters ,including artery‐to‐vein transit time ( A‐VT T ) ,muscle tissue mean transit time ( M‐M T T ) and muscle tissue peak intensity ( M‐PI) were calculated . Results T he diabetic rabbits exhibited a lower body weight increase and a decrease of muscle thickness .Plasma levels of triglyceride ,cholesterol and creatinine were significantly higher in diabetic group than those in control group ( P <0 .05) . T he A‐V T T and M‐M T T values of the diabetic rabbits significantly increased over time ( all P < 0 .05 ) ,whereas M‐PI value significantly decreased compared with that in control group ( P = 0 .013 ) . Besides ,the A‐V T T value was significantly higher in severe myopathy group than that in mild group ( P =0 .001) . T he M‐M T T values in both moderate and severe groups were significantly higher w hen compared to that in mild group ( all P <0 .05) . T he A‐VT T and M‐M T T values were correlated with the severity of diabetic myopathy( e=0 .898 , P <0 .001 ;e= 0 .527 , P = 0 .01 ) . Conclusions Diabetic rabbits have defective skeletal muscle micro‐ perfusion in the early stage . CEUS can quantify impaire muscle microcirculation ,and is a valuable tool for assessment of diabetic myopathy .
5. Monitoring center venous pressure to guide the treatment of acute circulatory failure: give up or persist

Central venous pressure (CVP) is one of the most commonly used parameters in hemodynamic monitoring, and monitoring CVP to guide the treatment of acute circulatory failure is a relatively common clinical method.However, many studies have pointed out that CVP is affected by many factors and has great limitations, which cannot provide help for the treatment of acute circulatory failure.This review summarized the analysis of the working principle of CVP, its value in treatment and influencing factors.The main purpose is to get a better understanding of CVP.
6.Genetic analyzing for a Chinese intellectual disability pedigree with ARX gene mutation

Objective:To investigate the molecular mechanism of the disease based on the clinical characterization and genetic mutation analysis in a family with intellectual disability.Methods:The proband with intellectual disability was diagnosed at Luohe Central Hospital in December 2019. Peripheral blood samples were collected from four family members. Whole exome sequencing (WES) was used to screen the pathological mutations. Then the PCR and Sanger sequencing were used to verify the selected mutations and combine the relevant database to analyze variation loci.Results:We infer that the ARX c.1162 A>G was co-segregated with the phenotype of the family based on the results of WES. The results of sanger sequencing and WES are consistent. The mother of the proband is the carrier of the mutation. There is no mutation frequency reported in the healthy population. The mutation of the ARX c.1162A>G is harmful inferred by a variety of bioinformatics software. Combined with the phenotypic analysis of OMIM database, we infer the phenotype caused by the mutation is consistent with the patients in the family.Conclusion:The mutation of the ARX c.1162 A>G may be the cause of the intellectual disability in the family affected. And the variant has not been reported in China.
7.The effect of nursing intervention on peri-operative percutaneous coro-nary angioplasty and stent implantation

Objective To observe the effects of percutaneous coronary angioplasty and stent implantation in periopera-tive nursing intervention effect. Methods From January 2012 to January 2014 year in our hospital, 64 cases of percuta-neous transluminal coronary angioplasty and stent implantation were observed, according to different nursing patients were randomly divided into intervention group (n=33﹚ and the control group(n=32﹚,the control group adopted random symptomatic care, the intervention group focused on the implementation of perioperative nursing, included preopera-tive preparation,and psychological nursing,observation and nursing cooperation during the operation, postoperative complications and nursing, observation of life, patients,the SAS score, SDS score, the incidence and nursing satisfac-tion, complications were compared between the two groups before and after treatment. Results After treatment, two groups of SAS, SDS scores were significantly decreased than before treatment(P<0.05﹚, and the SAS, SDS scores of pa-tients in the intervention group were lower than control group (P<0.05﹚. After interventional,nursing satisfaction rate was 93.75% in the intervention group,nursing satisfaction rate was 71.88%,was significantly higher than the con-trol group (χ2=6.348,P<0.05﹚. After intervention, the incidence of complications of intervention group was 12.50%, the control group was 31.25%, the incidence of postoperative complications of the intervention group was significantly lower than the control group(P<0.05﹚. Conclusion Percutaneous coronary angioplasty and stent implantation of perioper-ative nursing intervention measures can improve nursing satisfaction, improve the patients' negative psychological shape body, reduce the incidence of complications, is conducive to the recovery of patients with the disease.
8.Effects of an improved perioperative diet management based on enhanced recovery after surgery in patients undergoing total hip arthroplasty

Objective:To explore the effects of an improved perioperative diet management based on the concept of enhanced recovery after surgery (ERAS) in patients undergoing total hip arthroplasty (THA) .Methods:From May 2020 to May 2022, convenience sampling was used to select 320 patients who underwent their first unilateral THA at the First Affiliated Hospital of Zhengzhou University. According to the random number table method, patients were divided into control group ( n=160) and observation group ( n=160). The control group adopted a routine perioperative diet management, while the observation group improved the perioperative diet management based on the ERAS concept, and nurses performed preoperative fasting and postoperative diet and drinking water nursing according to the plan. We compared the preoperative hunger and thirst between two groups of patients, and recorded the gastrointestinal function such as postoperative anal exhaust and defecation time, bowel sound recovery time, postoperative nausea and vomiting degree, and post eating nausea and vomiting degree between the two groups. Results:The number of preoperative hunger and thirst patients in the observation group was less than that in the control group, and the postoperative exhaust time, defecation time, and bowel sound recovery time were shorter than those in the control group ( P<0.05). The degree of nausea and vomiting after eating after surgery was lower than that in the control group. The differences were all statistically significant ( P<0.05) . Conclusions:The improved perioperative diet management based on ERAS has good clinical effects in THA patients, reducing perioperative discomfort and promoting postoperative gastrointestinal function recovery.
9.Early clinical features of 312 children with coronavirus disease 2019 during the epidemic in Shanghai in 2022

Objective:To analyze the early clinical features of children with coronavirus disease 2019 (COVID-19) in order to further improve the understanding of the disease.Methods:A total of 312 children with COVID-19 under 16 years old who were first diagnosed in the children′s fever clinic at Xinhua Hospital Affiliated to Shanghai Jiaotong University School of Medicine from March to May 2022 were retrospectively analyzed.The epidemiology, early clinical features and blood routine results of children were described, and the blood routine results among different age groups were compared.Results:The median age of 312 children was 3.15(1.47, 6.51) years.The proportion of infection rate of children with different ages from high to low were 1~3 years old, > 6 years old, 3~6 years old and ≤ 1 year old.Only 17 (5.4%) cases had underlying diseases.Additionally, 70.2% patients had definite positive case reports in their residential communities, and 65.1% showed a familial cluster.Moreover, 96.2% patients had fever and 52.9% patients had respiratory symptoms.The main symptoms are fever (96.2%), cough (38.1%), runny nose (20.2%), vomiting (14.7%), sore throat (11.5%), poor appetite (6.7%), nasal congestion (4.5%), expectoration (4.5%), convulsion (4.2%), diarrhea (3.8%), etc.Among 309 children, 11.3% patients had increased white blood cell count, especially in children over 6 years old ( P=0.006); 31.7% patients had decreased lymphocyte count and 32.4% had increased C-reactive protein.The positive rate of point-of-care testing was as high as 99%. Conclusion:Children with COVID-19 in Shanghai had the characteristics of familial cluster.The main symptoms are fever and respiratory symptoms.Most of the blood routine leukocytes have no obvious changes, and a few have lymphopenia and C-reactive protein elevation.Point-of-care testing detection can help the children′s fever clinic to early screen COVID-19.
10.Role of carboxypeptidase E in promoting the migration of lymphocytes through vascular endothelial cells

Objective To study the mechanism of carboxypeptidase E ( CPE ) in promoting the migration of lymphocytes and their subsets through vascular endothelial cells. Methods CRISPR/Cas9 technology was used to prepare cpe gene-knockout MS1 (Cpe-/-MS1) cells. Adhesion ability of lymphocytes to MS1 and Cpe-/-MS1 cells was analyzed with adhesion assay. Expression of adhesion molecules on these cells were detected by RT-PCR and flow cytometry. Transwell model was used to compare the difference in the transmigration of lymphocytes and their subsets through MS1 and Cpe-/-MS1 cells. Results Cpe-/-MS1 cells were successfully obtained. Under the stimulation of TNF-α, the adhesion ability of lymphocytes to MS1 cells was much better than that of Cpe-/-MS1 cells. Moreover, adhesion molecules expressed on MS1 cells were significantly more than those on Cpe-/-MS1 cells. The percentages of lymphocytes and their sub-sets that transmigrated through MS1 cells were significantly higher than those through Cpe-/-MS1 cells. Con-clusion CPE involved in the adhesion of lymphocytes to vascular endothelial cells and the transmigration of them through vascular endothelial cells, which was of great significance for understanding the migration of lymphocytes across vascular endothelial cells to peripheral lymph nodes.
