1.Effect of Soft Tissues Injury around Atlantoaxial Joint on Memory Abilities of Mice

Objective To observe the effect of the soft tissues injury around atlantoaxial joint on memory abilities of mice.Methods 44 mice were divided into the model group and control group with 22 animals in each group.The soft tissues around atlantoaxial joint were injured in the model group by surgery between the first and the second cervical vertebrae.The changes of learning and memory of mice were observed with step-down avoidance test,water-maze test and hole-board test;the expression of caspase-3 in hippocampus was observed by immunohistochemical staining.Results Compared with the control group,escape latency was longer and wrong times were more in water-maze test of the model group(P<0.01). In step-down avoidance test of the model group,the reaction period prolonged and wrong times increased obviously(P<0.01).In hole-board test,the latency period prolonged and the times of extending the holes decreased significantly(P<0.01).Degenerated or dead neuron in hippocampus of surgery-treated mice was found scatteredly or continuously with HE staining,the expression of caspase-3 protein was showed positive with immunohistochemical staining.Conclusion The soft tissues injury arround atlantoaxial joint can induce abnormality of the stability of cervical vertebra,which aggravated the aging process of the mice and decreased abilities of learning and memory.
2.Role of post-hemorrhagic shock mesenteric lymph in enhancement of vas-cular permeability

AIM: To investigate the role of post-hemorrhagic shock mesenteric lymph (PHSML) in the enhancementof vascular permeability .METHODS: Eighteen Wistar rats were randomized into sham group , shock group,and shock plus mesenteric lymph drainage (shock +drainage) group.The rats in shock group and shock +drainagegroup were routinely subjected to hemorrhagic shock and hypotension [(40 ±2) mmHg] was maintained for 90 min, andthen the fluid resuscitation was performed.Mesenteric lymph was drained in the rats in shock +drainage group from resuscitationfinished to 6 h, for the observation of PHSML drainage on the vascular permeability in multiple tissues of hemorrhagicshock rats.Afterwards, human umbilical vein endothelial cells (HUVECs) were incubated with the PHSML in vitro to observethe effects of PHSML on the morphology and permeability of HUVECs .RESULTS: The degree of blue color and concentrationsof Evens blue in the lung, myocardium, kidney, liver, spleen and small intestine were significantly increased inthe shocked rats than that in sham group, while the ratios of the dry weight to the wet weight were decreased .The mesentericlymph drainage reversed these changes .Meanwhile, 4% and 10% of PHSML at 0 ~3 h and 3 ~6 h after resuscitation,and lipopolysaccharide (10 mg/L) all caused the damage of HUVECs, decreased the viability and trans-endothelial electricalresistance of HUVECs, and increased the permeability of HUVECs to fluorescein isothiocyanate -labeled albumin. CONCLUSION: PHSML is a vital factor in the enhancement of vascular permeability .
3. MRI of foot pain in amateur marathoners and analysis of related factors

Objective:
To study the MRI manifestations of foot injuries in amateur marathoners and the diagnostic value of MRI for foot injuries.
Methods:
Forty-eight amateur marathoners were prospectively recruited from October 2018 to March 2019. Participants were grouped according to the FASS criteria. All participants underwent bilateral foot MRI examinations. The MRI features of foot in amateur marathoners with and without foot pain were analyzed.The association with gender, age, time to participate in marathon, amount of running per week, number of times to complete the whole marathon and the symptoms of foot pain was studied.
Results:
There were 28 sides in FASS in grade 0, including 8 males and 20 females, 8 hydrocele of the first metatarsophalangeal joint, 6 edema of bone marrow, 4 edema of soft tissue, 4 hydrocele of tendon sheath, and 6 without abnormal manifestations of MRI. There were 68 sides in FASS grade 1, including 42 males and 26 females, 38 Achilles tendinitis, 14 plantar fasciitis, 12 Achilles tendon bursitis and 4 posterior ankle impingement syndrome. 4.Development of Portable Anesthesia Video Laryngoscope Based on ARM

In the process of tracheal intubation, the anesthesia video laryngoscope is used to lift up the patient's epiglottis to expose the glottis, and thus guiding the medical staff to perform anesthesia intubation accurately. This paper describes the method and significance of video laryngoscope in the process of guiding anesthesia intubation, introduces the overall structure and function of portable anesthesia video laryngoscope, the design is mainly focused on image acquisition module, core board circuit, video decoding circuit, lithium battery charging circuit and external storage circuit, at last briefly introduces work process of the video laryngoscope.

The incidence of foot pain in amateur marathoners was related to gender(OR=1.060, P=0.043), unrelated to age(OR=0.248, P=0.078), number of marathons completed(OR=1.006, P=0.956), time to participate in marathon(OR=1.008,P=0.944), amount of running per week (OR=0.971,P=0.407). Conclusion Among amateur marathon runners, the foot pain is mainly caused by Achilles tendon and plantar fascia, while those without foot pain are mainly characterized by joint effusion, tendon sheath effusion and bone marrow edema. Gender is the only factor affecting the incidence of foot pain in amateur marathoners. MRI examination is an effective method to determine foot injury and has important clinical application value in the diagnosis and treatment of foot injury.
6.Development and validation of a nomogram for predicting the survival of patients with non-metastatic nasopharyngeal carcinoma after curative treatment

Background:The TNM staging system is far from perfect in predicting the survival of individual cancer patients because only the gross anatomy is considered. The survival rates of the patients who have the same TNM stage disease vary across a wide spectrum. This study aimed to develop a nomogram that incorporates other clinicopatho-logic factors for predicting the overall survival (OS) of non-metastatic nasopharyngeal carcinoma (NPC) patients after curative treatments. Methods:We retrospectively collected the clinical data of 1520 NPC patients who were diagnosed histologically between November 2000 and September 2003. The clinical data of a separate cohort of 464 patients who received intensity-modulated radiation therapy (IMRT) between 2001 and 2010 were also retrieved to examine the extensibil-ity of the model. Cox regression analysis was used to identify the prognostic factors for building the nomogram. The predictive accuracy and discriminative ability were measured using the concordance index (c-index). Results:We identiifed and incorporated 12 independent clinical factors into the nomogram. The calibration curves showed that the prediction of OS was in good agreement with the actual observation in the internal validation set and IMRT cohort. The c-index of the nomogram was statistically higher than that of the 7th edition TNM staging sys-tem for predicting the survival in both the primary cohort (0.69 vs. 0.62) and the IMRT cohort (0.67 vs. 0.63). Conclusion:We developed and validated a novel nomogram that outperformed the TNM staging system in predict-ing the OS of non-metastatic NPC patients who underwent curative therapy.
7.Teaching reform and practice of pathophysiology guided by the cultivation of "clinical competence"

In order to adapt to the development of higher medical education, it is imperative to carry out a student-centered teaching reform of pathophysiology guided by the cultivation of "clinical competence". The main contents of this teaching reform of pathophysiology include: the teaching content layout has been reconstructed to promote classroom teaching with clinical manifestation, new media means such as blackboard online teaching system, WeChat public platform and Chaoxing virtual classroom have been applied to assist students in their learning, and students are encouraged to conduct literature retrieval and reading, and then make presentation PPT for a literature of interest and report to all students to cultivate students' independent learning and sustainable development ability via flipped classroom. The practice shows that the student-centered teaching reform of pathophysiology guided by the cultivation of "clinical competence" has gained successful results.
8.Real-world research of vonoprazan and amoxicillin dual therapy for Helicobacter pylori eradication

OBJECTIVE To explore the efficacy, safety and economics of a dual therapy consisting of conventional dose of vonoprazan combined with conventional dose of amoxicillin in patients with primary treatment of Helicobacter pylori (HP) infection. METHODS Using a prospective cohort study, the patients diagnosed with HP infection and receiving initial treatment in Chengdu Xinhua Hospital from July 2021 to July 2022 were collected according to inclusion and exclusion criteria. The patients were given vonoprazan/amoxicillin dual therapy (i.e. VA group, Vonoprazan fumarate tablets 20 mg, once a day+Amoxicillin capsules 1.0 g, twice a day, 14 days) and bismuth-containing quadruple therapy (i.e. LJAF group, Rabeprazole sodium enteric- coated tablets 20 mg, twice a day+Colloidal bismuth pectin capsules 200 mg, twice a day+Amoxicillin capsules 1.0 g, twice a day+ Furazolidone tablets 100 mg, twice a day, for 14 days) according to the patient’s medication willingness. Four weeks after the end of the treatment, HP eradication rates of the two groups were compared by using intention-to-treat (ITT), modified intention-to- treat (MITT) and per-protocol (PP) analysis. The occurrence of adverse drug reactions (ADR) was recorded, and an economic evaluation was performed for them. RESULTS Among the 58 patients in VA group, 55 completed the trial, 2 were lost to follow- up and one withdrew due to rash; among the 62 patients in LJAF group, 57 completed the trial, 3 were lost to follow-up and 2 withdrew due to rash. Results of ITT, MITT and PP analysis showed that HP eradication rates of VA group were 86.2%, 89.3% and 90.9%, and those of LJAF group were 87.1%,91.5% and 94.7%, respectively; there was no statistical significance among different groups (P>0.05). The incidences of ADR in VA group and LJAF group were 6.9% and 14.5%,which were not significantly different (P>0.05). The result of cost minimization analysis showed that the treatment cost of VA group was 340.9 yuan, which was lower than 373.5 yuan of LJAF group. CONCLUSIONS In patients with primary treatment of HP infection, the efficacy and safety of dual therapy of conventional dose of vonoprazan combined with conventional dose of amoxicillin is equivalent to the bismuth-containing quadruplex therapy with low cost.
