Objective To evaluate the relationship between ultrasound-assayed detrusor thickness and bladder outlet obstruction (BOO) in old men with benign prostatic hyperplasia (BPH).Methods The 106 BPH patients underwent the urodynamic examination on which the diagnosis of BOO was dependent. The obstruction was defined as the Abrams-Griffiths nomogram (A-G index)≥40 and the grade of linear passive urethral resistance relation (LinPURR)≥Ⅱ . When bladder capacity reached 150 ml, the detrusor thickness was measured by abdominal ultrasound. Results Compared with unobstructed group, the maximum flow rate and average flow rate were both lower in obstructed group [(10.1±3.0) ml/s vs. (17.4±3.1) ml/s, (5.5±2.2) ml/s vs. (11.2±2.2) ml/s, t= 10.26and 11.03, both P<0.01]. And the residual urine volume and maximum detrusor pressure were significantly higher in obstructed group than in unobstructed group [(47.6 ± 24.3) ml vs. (17.0 ±5.6) ml, (39.3±14.4) cm H2Ovs. (26.8±8.0) cm H2O, t=6.32 and 4.07, P<0.01 or 0.05].Detrusor thickness was positively correlated with maximum detrusor pressure (r= 0.419, P<0.01),but negatively correlated with maximum flow rate (r =- 0.749, P< 0.01 ), mean flow rate (r=-0.853, P<0.01) and voided volume (r=-0.556, P<0.01). There was significant difference in detrusor thickness between obstructed group and unobstructed group [(3.0± 0.2) mm vs. (2.5 ±0.2) mm, t= 11.2,P<0.05]. According to the diagnostic standard of detrusor thickness≥3.0 mm,it had a sensitivity of 90% and a specificity of 84.6%, a positive predictive value of 93.1% and a negative predictive value of 78.6%. Conclusions Detrusor thickness of 3.0 mm or greater has a certain predictive value for BOO in old men.

Objective To get well-informed of the current status of emergency training in primary hospitals in Sichuan Province and to find weaknesses in the system in order to provide scientific basis for emergency knowledge and skills training. Methods A stratified random cluster sampling method as adopted and an anonymous questionnaire survey method was conducted to investigate the basic circumstances of the current training and the training needs of 1 000 emergency medical staff in 9 regions throughout Sichuan Province. Results The scope of the survey covered emergency doctors, nurses and technicians in hospitals of Grade Ⅱ,Grade Ⅲ and below.A total of 1 000 surveys were sent out and 983 were recovered. The questionnaire completion rate was 98.3%.The respondents mainly worked for more than 10 years, most of which with Bachelor degree, junior and intermediate titles. Results of the survey showed,63.1% Most respondents have only one or two training opportunities per year. The training methods are mainly continuing education (59.0%,580 trips),training organized within the unit (58.5%,575 trips),and training organized by health administrative units (39.3%,386 trips). The main contents of the training are:basic professional skills learning (66.7%,655 trips),basic professional theoretical study (59.2%,582 trips),and new emergency technology (42.5%,418 trips),only 32.0% of the respondents believe that these methods meet the practical needs. The main factors that influence participation in the training were:not be arranged by employer (36.2%,356 trips);the training was different from actual work (31.6%,311 trips);no replacement for work so they couldn't join training (29.8%,293 trips). The survey showed that the most desirable way of training is:practical application (60.1%,591 trips) and the most desirable training content is:emergency first aid knowledge (73.7%,724 trips). There were significant differences in case discussion, technical observation, academic lectures, special study trainingand learning methods in hospitals of different levels (P<0.05),but there was no significant difference in the choice of professional practice methods (P>0.05),and the selection of training contents was in emergency first aid knowledge, general medical knowledge and prevention and treatment of chronic diseases. There was no statistical difference in the selection of training contents of knowledge, infectious disease knowledge and department management knowledge (P>0.05).There were significant differences in the choice of nursing knowledge training (P<0.05).Conclusion The current situation showed a lack of first-aid knowledge and skills training for emergency personnel in Sichuan Province. The present situation of skill training can't satisfy the need of their desire to participate in training. It is urgent to develop a standardized, systematic and scientific training mode to improve the emergency first aid ability of primary medical and emergency personnel.

To investigate the awareness of cognitive impairment disorders among residents of the Xizang Autonomous Region and its influencing factors, thereby providing a basis for targeted prevention and treatment efforts. From April to December 2024, a questionnaire survey was conducted among permanent residents aged ≥18 years (residing in the Xizang Autonomous Region for 180 days or more). The survey was primarily conducted online, supplemented by QR code distribution during community medical outreach by healthcare workers. Demographic information and data on awareness of cognitive disorders were collected, and an ordered Logistic regression model was used to analyze influencing factors in the overall population and stratified by occupation. A total of 327 questionnaires were collected, with 14 excluded (13 for not meeting residency requirements and 1 for self-reported diagnosis of cognitive impairment), leaving 313 valid questionnaires. The average age of respondents was 42.0±11.9 years; 108 (34.5%) were male, and 205 (65.5%) were female. Most respondents were from Lhasa (78.6%, 246/313); 179 (57.2%) were healthcare workers, and 134 (42.8%) were non-healthcare workers. Regarding awareness of cognitive impairment disorders, 7.3% (23/313) were "unaware", 75.7% (237/313) were "partially aware", and 16.9% (53/313) were "well aware".Ordered Logistic regression analysis revealed that education level of high school or below ( Awareness of cognitive impairment disorders among residents of the Xizang Autonomous Region needs improvement. Educational level, occupation, and prior contact with cognitive impairment patients significantly influence disease awareness. Enhancing overall education levels and using vivid clinical case presentations in health education and public outreach are key strategies to improve public awareness of cognitive impairment disorders.
