1.Study on the characteristics of blood pressure levels in patients with primary hypertension and coronary heart disease

Objective To study and analyze the 24 h ambulatory blood pressure and ambulatory electrocardiography in pa-tients with primary hypertension and coronary heart disease,the characteristics of blood pressure level of patients with hypertension and coronary heart disease and its correlation with myocardial ischemic burden in these patients were explored.Methods Selected by coronary angiography examination,139 cases of patients with high blood pressure,according to the results of coronary artery ex-amination were divided into high blood pressure with coronary heart disease group(observation group)74 cases and simple hyper-tension group(control group)65 cases.Two groups all were used 24 hours ambulatory blood pressure monitoring and ambulatory ECG monitoring.The relevant data were analyzed.Results (1)Observation group nocturnal systolic blood pressure was higher than the average in control group,the time of diastolic pressure was lower than the control group,24 hours and the average artery pres-sure was lower than the control group,the difference was statistically significant(P <0.05).(2)Total ischemia burden(TIB)and the corresponding systolic pressure,diastolic pressure,pulse pressure levels existed significant correlation(respectively r =0.409, P =0.000;r=-0.306,P =0.008;r =0.61 5,P =0.000);there was a significant positive correlation between day TIB and corre-sponding systolic pressure,pulse pressure level(r=0.41 7,P =0.000;r=0.402,P =0.001);there was a significant positive correla-tion between TIB and the corresponding nighttime systolic blood pressure,pulse pressure level(r=0.5 1 1,P =0.000;r=0.596,P =0.000).Conclusion The levels of nocturnal systolic pressure,diastolic pressure and the time of 24 hours are higher,and the average artery pressure levels are relatively low.There is a significant correlation with the myocardial ischemic burden.
2.Effect Evaluation of Spironolactone in the Treatment of Hypertensive Heart Disease by the Real Time 3 -dimensional Echocardiography

Objective To investigate the effect evaluation of spironolactone in the treatment of hypertensive heart disease by the real time 3-dimensional echocardiography .Methods Totally 104 patients with hypertensive heart disease were in the study from Jan 2011 to Jan 2012 of our department and divided into objective group and control group .Conventional treatments were put in all the patients and the spironolactone was used in objective group patients only .The results of ventricular morphology and function indexes were compared be -tween the two groups, and then investigate the relationship between cystatin C (CycC) and left ventricular remodeling index (LVRI) at the same time.Results There were significant differences in the comparison of ventricular morphology and function indexes .The left ventric-ular end-diastolic volume(LVEDV), left ventricular end systolic volume (LVESV) and left ventricular ejection fraction (LVEF) of ob-jective group were more advantageous than flowse of control group for prognosis .The same results were observed in the comparison of Cys C and LVRI.Moreover, Cyc C and LVRI had certain correlation in the study .Conclusion The spironolactone can play a positive role in the treatment for the patients with hypertensive heart disease .In the meantime, the RT-3DE is a good evaluation method for the develop-ment of ventricular morphology and function .
3.Application of CARTO3 Fast Anatomical Mapping During Radiofrequency Ablation in Patients With Paroxysmal Atrial Fibrillation

Objective: To investigate the safety and efficacy of CARTO3 fast anatomical mapping during radiofrequency ablation in patients with paroxysmal atrial ifbrillation (PAF). Methods: A total of 120 PAF patients treated in our hospital from 2013-01 to 2015-07 were enrolled. All patients received CARTO3 system for mapping and they were randomly divided into 2 groups: Control group, the patients had selective pulmonary vein angiography, followed by conventional point by point method to reconstruct left atrial model for guiding the ablation of PFA and Treatment group, the patients had selective pulmonary vein angiography followed by fast anatomical mapping to build left atrial model for guiding the ablation of PFA; the rest operational steps such as trans-septal and circumferential pulmonary vein ablation were the same.n=60 in each group. The times of operation, X-ray exposure and the rates of success, complication occurrence were compared between 2 groups. Results: All patients were successfully completed radiofrequency ablation for PAF. Compared with Control group, Treatment group had increased modeling time (8.5 ± 3.6) min vs (5.2 ± 2.3) min, while decreased pulmonary vein ostium determing time (12.0 ± 5.6) min vs (25.0 ± 8.4) min, circumferential pulmonary vein ablation time (95.0 ± 22.0) min vs (115.0 ± 25.0) min and X-ray exposure time (15.0 ± 6.3) min vs (24.0 ±5.5) min, allP<0.05. The rates of success and complication occurrence were similar between 2 groups, P>0.05. Conclusion: CARTO3 fast anatomical mapping is safe and effective for guiding radiofrequency ablation in PAF patients, it may decrease the X-ray exposure time and operation time which were important for treating the relevant patients.
4.Application of self-made fixed support in postoperative nursing of abdominal flap repair

Objective:To explore the effect of self-designed fixed support in the postoperative nursing of abdominal flap repair.Methods:From January 2016 to December 2019, 47 patients with hand tissue defects undergoing abdominal skin flap repair in Shajing People′s Hospital, Guangzhou Medical University were divided into control group (23 cases) and observation group (24 cases) by random digits table method. The control group was treated with traditional methods, and the observation group was treated with self-designed fixed support. The wound healing of abdominal flap, the wound tearing after flap operation, pedicle ulcer after flap operation and the patients' nursing satisfaction were compared between the two groups.Results:There were 7 cases of primary healing and 16 cases of secondary healing in the control group, 18 cases of primary healing and 6 cases of secondary healing in the observation group. The difference between the two groups was statistically significant ( χ2 value was 9.368, P<0.01). The incidence rate of the wound tearing after flap operation was 26.1% (6/23) in the control group and 0 (0/24) in the observation group. The difference between the two groups was statistically significant ( χ2 value was 7.177, P<0.05). The incidence rate of pedicle ulcer after flap operation was 69.6% (16/23) in the control group and 33.3% (6/23) in the observation group. The difference between the two groups was statistically significant ( χ2 value was 6.170, P<0.05). The patients' nursing satisfaction was (76.8±8.4)points in the control group and (90.4±6.5) points in the observation group.The difference between the two groups was statistically significant (t value was -4.640, P<0.01). Conclusions:The use of fixed support after the operation of abdominal flap is helpful to the wound healing, reduce the occurrence of wound tear and pedicle ulcer, and improve the satisfaction of nursing, which is worth popularizing.
5.Left Atrial and Pulmonary Vein Rotational Angiography Embedded With Real-time X-Ray Fluoroscopy System Guiding Radiofrequency Catheter Ablation in Treating the Patients With Atrial Fibrillation

Objective: To investigate the safety and effectiveness of three-dimensional (3-D) rotational angiography reconstruction of left atrial and pulmonary vein stereo image embedded with real-time X-ray fluoroscopy system for guiding radiofrequency catheter ablation (RFCA) in treating the patients with atrial ifbrillation (AF).
Methods: A total of 60 consecutive AF patients who received RFCA in our hospital from 2011-04 to 2013-04 were studied. The patients were randomly divided into 2 groups and 3-D Carto3 mapping system was applied for guiding RFCA in both groups. n=30 in each group. Treatment group, the patients received Siemens ARTIS Zeego digital subtraction system for left atrial and pulmonary vein rotational angiography, then, stereo images were reconstructed and embedded with real-time X-ray lfuoroscopy for RFCA guidance. Control group, conventional left and right pulmonary venography was conducted for RFCA guidance. The procedural and X-ray exposure times, rates of success and complications were recorded and compared between 2 groups.
Results: All 60 patients had successful RFCA, compared with Control group, the patients in Treatment group had obviously less procedural time and X-ray exposure time, while the success rate and complications were similar between 2 groups.
Conclusion: 3-D rotational angiography reconstruction of left atrial and pulmonary vein stereo image embedded with real-time X-ray lfuoroscopy system is safe and effective for guiding RFCA in treating the AF patients, which may reduce the procedural and X-ray exposure times.
6.Development and research of temporary demand pacemaker with electrocardiosignal display.

A temporary demand pacemaker with electrocardiosignal display is introduced in this paper. Double way low-noise electrocardiosignal preamplifier, amplitude limiter, high and low pass filter, 50 Hz notch filter, TTL level generator and stimulating pulse formation circuit are components of the hardware electrocircuit. The demand pacing and the electrocardiosignal display are separately controlled by the software in which the double microcontrollers communications technique is used. In this study, liquid crystal display is firstly used in body surface electrocardiosignal display or intracardial electrophysiologic signal display when the temporary demand pacemaker is installed and put into use. The machine has proven clinically useful and can be of wide appliation.

Polygraph has become a necessary instrument in interventional cardiology and fundamental research of medicine up to the present. In this study, a LabView development system (DS) (developed by NI in U.S.) used as software platform, a DAQ data acquisition module and universal computer used as hardware platform, were creatively coupled with our self-made low noise multi-channels preamplifier to develop Multi-channels electrocardiograph. The device possessed the functions such as real time display of physiological process, digit highpass and lowpass, 50Hz filtered and gain adjustment, instant storing, random playback and printing, and process control stimulation. Besides, it was small-sized, economically practical and easy to operate. It could advance the spread of cardiac intervention treatment in hospitals.

8.Application of self-designed hip intramuscular injection positioning device in papaverine injection after finger replantation

Objective:To explore the clinical effect and nursing experience of papaverine intramuscular injection after finger replantation with self-designed hip intramuscular injection positioning device.Methods:Totally 216 cases of patients treated with palavering for anti-vasospasm after of severed fingers from January 2018 to December 2019 were randomly divided into control group (108 cases) and observation group (108 cases) by random number table method. The control group received intramuscular injection of buttock muscle according to the conventional method, and the observation group used the auxiliary intramuscular injection with self-designed glutei muscle positioning device. The incidence of postoperative hip indurations and ecchymosed, survival rate of and buttock were analyzed Pain scores were compared.Results:Totally 76 cases (70.37%) and 62 cases (48.15%) in the control group, 12 cases (11.11%) and 6 cases (5.56%) in the observation group. The difference between the two groups was statistically significant ( χ2 values were 78.545, 67.307, P<0.01). 78 cases (72.22%) survived in the control group and 96 cases (88.89%) survived in the observation group ( χ2 value was 11.416, P<0.01). After papaverine intramuscular injection, there was no significant difference in hip pain score between the two groups on the first and second day ( P>0.05), but there was significant difference between the two groups on the third to seventh day ( t values were 17.17 to 45.97, P<0.05). Conclusion:Palavering intramuscular injection can reduce the incidence of hip indurations and ecchymosed, relieve buttock pain and improve the survival rate of amputated finger, which is worthy of clinical application.

ObjectiveTo evaluate the knowledge, attitude and practice (KAP) of medication use and safety among the patients with acquired immune deficiency syndrome (AIDS) in a 3A-grade hospital of Guangzhou city, and to provide scientific basis for AIDS prevention and treatment. MethodsA questionnaire survey was conducted among AIDS patients aged 18 years and above in our hospital to investigate their KAP regarding medication use and safety. ResultsA total of 549 questionnaires were collected, of which 503 were valid, with an effective recovery rate of 91.6%. The average scores of KAP were (14.58 ± 8.49) , (25.21 ± 6.92) and (47.58 ± 3.33) , respectively, with the scoring rates of 36.46%, 63.02%, and 95.16%, respectively. There were statistically significant differences (P<0.05) in knowledge scores among people with different ages, education levels and occupations. Multiple linear regression showed that education level and medical insurance status had most significant impact on knowledge scores (P<0.05). Significant differences were found in attitude scores among people with different education levels (P<0.05), as well as in practice scores among people with different occupations (P<0.05). Multiple linear regression revealed that age, occupation, knowledge score and attitude score had a significant impact on practice scores (P<0.05). Patients expected to receive pharmaceutical care services from the pharmacists via face-to-face communication, network platform and telephone consultation on medication knowledge such as adverse drug reactions and response measures, drug-drug interactions, missed medication and response measures, medication adherence measures, etc. ConclusionsAIDS patients in this hospital have a good awareness of medication safety, but their knowledge of medication use needs improvement. Some bad habits may affect their compliance, resulting in safety hazards. Therefore, there is an urgent demand for pharmaceutical care services related to rational drug use.
