1.Analysis and forecast of the disease burden of schistosomiasis in China from 1992 to 2030

Objective To analyze the trends in the disease burden of schistosomiasis in China from 1992 to 2021, and to project the disease burden of schistosomiasis in China from 2022 to 2030, so as to provide insights into the elimination of schistosomiasis in China. Methods The prevalence, age-standardized prevalence, disability-adjusted life year (DALYs) rate and age-standardized DALYs rate of schistosomiasis, as well as the years lost due to disability (YLDs) rate and age-standardized YLDs rate of anemia attributable to Schistosoma infections in China, the world and different socio-demographic index (SDI) regions were captured from the Global Burden of Disease Study 2021 (GBD 2021) data resources, and the trends in the disease burden due to schistosomiasis were evaluated with estimated annual percentage change (EAPC) and its 95% confidence interval (CI). In addition, the age, period and cohort effects on the prevalence of schistosomiasis were examined in China using an age-period-cohort (APC) model, and the disease burden of schistosomiasis was predicted in China from 2022 to 2030 using a Bayesian age-period-cohort (BAPC) model. Results The age-standardized prevalence and DALYs rate of schistosomiasis, and the age-standardized YLDs rate of anemia attributable to Schistosoma infections were 761.32/105, 5.55/105 and 0.38/105 in China in 2021. These rates were all lower than the global levels (1 914.30/105, 21.90/105 and 3.36/105, respectively), as well as those in the medium SDI regions (1 413.61/105, 12.10/105 and 1.93/105, respectively), low-medium SDI regions (2 461.03/105, 26.81/105 and 4.48/105, respectively), and low SDI regions (5 832.77/105, 94.48/105 and 10.65/105, respectively), but higher than those in the high SDI regions (59.47/105, 0.49/105 and 0.05/105, respectively) and high-medium SDI regions (123.11/105, 1.20/105 and 0.12/105, respectively). The prevalence and DALYs rate of schistosomiasis were higher among men (820.79/105 and 5.86/105, respectively) than among women (697.96/105 and 5.23/105, respectively) in China in 2021, while the YLDs rate of anemia attributable to Schistosoma infections was higher among women (0.66/105) than among men (0.12/105). The prevalence of schistosomiasis peaked at ages of 30 to 34 years among both men and women, while the DALYs rate of schistosomiasis peaked among men at ages of 15 to 19 years and among women at ages of 20 to 24 years. The age-standardized prevalence of schistosomiasis showed a moderate decline in China from 1992 to 2021 relative to different SDI regions [EAPC = -1.51%, 95% CI: (-1.65%, -1.38%)], while the age-standardized DALYs rate [EAPC = -3.61%, 95% CI: (-3.90%, -3.33%)] and age-standardized YLDs rate of anemia attributable to Schistosoma infections [EAPC = -4.16%, 95% CI: (-4.38%, -3.94%)] appeared the fastest decline in China from1992 to 2021 relative to different SDI regions. APC modeling showed age, period, and cohort effects on the trends in the prevalence of schistosomiasis in China from 1992 to 2021, and the prevalence of schistosomiasis appeared a rise followed by decline with age, and reduced with period and cohort. BAPC modeling revealed that the age-standardized prevalence and age-standardized DALYs rate of schistosomiasis, and age-standardized YLDs rate of anemia attributable to Schistosoma infections all appeared a tendency towards a decline in China from 2022 to 2030, which reduced to 722.72/105 [95% CI: (538.74/105, 906.68/105)], 5.19/105 [95% CI: (3.54/105, 6.84/105)] and 0.30/105 [95% CI: (0.21/105, 0.39/105)] in 2030, respectively. Conclusions The disease burden of schistosomiasis appeared a tendency towards a decline in China from 1992 to 2021, and is projected to appear a tendency towards a decline from 2022 to 2030. There are age, period and cohort effects on the prevalence of schistosomiasis in China. Precision schistosomiasis control is required with adaptations to current prevalence and elimination needs.
2.Collection, storage and utilization of lung transplant tissue samples

After continuous development and improvement, lung transplantation has become the preferred means to treat a variety of benign end-stage lung diseases. However, the field of lung transplantation still faces many challenges, including shortage of donor resources, preservation and maintenance of donor lungs, and postoperative complications. Lung tissue samples removed after lung transplantation are excellent clinical resources for the study of benign end-stage lung disease and perioperative complications of lung transplantation. However, at present, the collection, storage and utilization of tissue samples after lung transplantation are limited to a single study, and unified technical specifications have not been formed. Based on the construction plan of the biobank for lung transplantation in the First Affiliated Hospital of Xi'an Jiaotong University, this study reviewed the practical experience in the collection, storage and utilization of lung transplant tissue samples in the aspects of ethical review, staffing, collection process, storage method, quality control and efficient utilization, in order to provide references for lung transplant related research.
3.Association between screen time and anxiety-depression symptoms and their comorbidity among middle school students in Taiyuan City

Objective:
To investigate the association between screen time (ST) during leisure time and anxiety-depression symptoms among middle school students, so as to provide a basis for formulating relevant intervention measures.
Methods:
From November to December 2023, a stratified cluster random sampling method was used to select 2 542 students from junior and senior high school in Taiyuan City. A self designed questionnaire, the Generalized Anxiety Disorder Scale (GAD-7), and the Patient Health Questionnaire (PHQ-9) were used to investigate ST and anxiety/depression symptoms among middle school students. The Logistic regression model was used to explore the association of ST with symptoms of anxiety and depression, as well as with anxiety and depression comorbiditles (CAD).
Results:
The detection rates of anxiety symptoms, depression symptoms, and CAD were 13.69%, 15.77%, and 10.11%, respectively. The median ST was 2.00 h/d [interquartile range ( IQR =2.38) for weekly averages], with 0.33 h/d ( IQR =1.67) on work days and 5.00 h/d ( IQR=5.50) on rest days. Logistic regression analysis indicated that the total ST of mobile phones during rest days ( OR =1.07, 1.10, 1.11) and the averages ST of mobile phones over a week ( OR = 1.20 , 1.22, 1.29), as well as the total ST of all screen types during rest days ( OR =1.04, 1.04, 1.05) and the averages ST of all screen types over a week ( OR =1.08, 1.09, 1.21) were positively correlated with anxiety symptoms, depression symptoms, and CAD (all P <0.01).
Conclusions
Among middle school students in Taiyuan City, screen time is positively correlated with symptoms of anxiety or depression and the comorbidity of anxiety and depression, especially smartphone screen time and weekend screen use. Therefore, measures should be implemented to reduce unnecessary screen time among middle school students, especially the use of mobile phones, in order to mitigate the occurrence of anxiety and depression.
4.Cost-effectiveness analysis of insulin degludec and insulin aspart in Chinese patients with type 2 diabetes mellitus

OBJECTIVE To evaluate the long-term cost-effectiveness of insulin degludec and insulin aspart (IDegAsp) in patients with type 2 diabetes mellitus (T2DM) in China. METHODS A cost-effectiveness analysis was conducted from the perspective of the Chinese healthcare system, using the CORE diabetes model to simulate long-term (20-year) health and economic outcomes. Baseline cohort characteristics and treatment effect data were derived from the CREATE study. The prices of glucose- lowering drugs were obtained from medical insurance payment standards and the average winning bid prices in the follow-up round of the specialized centralized procurement for insulin, while the daily dosages were derived from the CREATE study. The costs of complications and utility values were obtained from published literature, with a discount rate of 5%. One-way sensitivity analysis, scenario analysis, and probabilistic sensitivity analysis were performed to verify the robustness of the results. RESULTS Patients switching from previous once-daily basal insulin regimens to IDegAsp therapy gained an incremental 0.190 quality-adjusted life year (QALY) with direct medical cost savings of 42 163.58 yuan. For those switching from premixed insulin therapies, IDegAsp treatment provided 0.130 incremental QALY and reduced direct healthcare costs by 41 129.11 yuan. The outcome was significantly influenced by the discount rate and the cost of complications. Probabilistic sensitivity analysis and scenario analysis confirmed the robustness of these findings. CONCLUSIONS Switching from previous daily basal insulin or premixed insulin regimens to IDegAsp in Chinese patients with T2DM can improve patients’ long-term health outcomes and achieve cost savings, making it a more cost-effective treatment option.

Traditional development of small protein scaffolds has relied on display technologies and mutation-based engineering, which limit sequence and functional diversity, thereby constraining their therapeutic and application potential. Protein design tools have significantly advanced the creation of novel protein sequences, structures, and functions. However, further improvements in design strategies are still needed to more efficiently optimize the functional performance of protein-based drugs and enhance their druggability. Here, we extended an evolution-based design protocol to create a novel minibinder, BindHer, against the human epidermal growth factor receptor 2 (HER2). It not only exhibits super stability and binding selectivity but also demonstrates remarkable properties in tissue specificity. Radiolabeling experiments with 99mTc, 68Ga, and 18F revealed that BindHer efficiently targets tumors in HER2-positive breast cancer mouse models, with minimal nonspecific liver absorption, outperforming scaffolds designed through traditional engineering. These findings highlight a new rational approach to automated protein design, offering significant potential for large-scale applications in therapeutic mini-protein development.
6.Qishen Granule protects against myocardial ischemia by promoting angiogenesis through BMP2-Dll4-Notch1 pathway.

OBJECTIVE:
Therapeutic angiogenesis has become a promising approach for treating ischemic heart disease (IHD). The present study aims to investigate the effects of Qishen Granule (QSG) on angiogenesis in myocardial ischemia (MI) and the potential mechanism.
METHODS:
In vivo study was conducted on rat model of myocardial infarction. QSG was performed daily at a dose of 2.352 g/kg for four weeks. Cardiac function was assessed by echocardiogram and pro-angiogenic effects were evaluated by Laser Doppler and CD31 expression. Oxygen-glucose deprivation (OGD) was applied in cultured human umbilical vein endothelial cells (HUVECs). Cell viability, wound healing and tube formation assay were used to test functions of HUVECs. ELISA and Western blots were used to assess protein expressions of bone morphogenetic protein 2-delta-like 4-notch homolog 1 (BMP2-Dll4-Notch1) signaling pathway.
RESULTS:
The results showed that QSG improved heart function, cardiac blood flow and microvessel density in myocardial ischemic rats. In vitro, QSG protected HUVECs by promoting the cell viability and tube formation. QSG upregulated bone morphogenetic protein-2 (BMP2) and downregulated delta-like 4 (Dll4) and notch homolog 1 (Notch1) expressions both in rats and HUVECs.
CONCLUSION
QSG protected against MI by promoting angiogenesis through BMP2-Dll4-Notch1 pathway. BMP2 might be a promising therapeutic target for IHD.
7.Discovery of E0199: A novel compound targeting both peripheral NaV and KV7 channels to alleviate neuropathic pain.

This research study focuses on addressing the limitations of current neuropathic pain (NP) treatments by developing a novel dual-target modulator, E0199, targeting both NaV1.7, NaV1.8, and NaV1.9 and KV7 channels, a crucial regulator in controlling NP symptoms. The objective of the study was to synthesize a compound capable of modulating these channels to alleviate NP. Through an experimental design involving both in vitro and in vivo methods, E0199 was tested for its efficacy on ion channels and its therapeutic potential in a chronic constriction injury (CCI) mouse model. The results demonstrated that E0199 significantly inhibited NaV1.7, NaV1.8, and NaV1.9 channels with a particularly low half maximal inhibitory concentration (IC50) for NaV1.9 by promoting sodium channel inactivation, and also effectively increased KV7.2/7.3, KV7.2, and KV7.5 channels, excluding KV7.1 by promoting potassium channel activation. This dual action significantly reduced the excitability of dorsal root ganglion neurons and alleviated pain hypersensitivity in mice at low doses, indicating a potent analgesic effect without affecting heart and skeletal muscle ion channels critically. The safety of E0199 was supported by neurobehavioral evaluations. Conclusively, E0199 represents a ground-breaking approach in NP treatment, showcasing the potential of dual-target small-molecule compounds in providing a more effective and safe therapeutic option for NP. This study introduces a promising direction for the future development of NP therapeutics.
8.Analysis of Risk Factors for Gastrointestinal Acute Graft-versus-host Disease after Allogeneic Hematopoietic Stem Cell Transplantation

Objective To investigate the clinical features,associated risk factors,and prognosis of gastrointestinal acute graft-ver-sus-host disease(GI-GVHD)after allogeneic hematopoietic stem cell transplantation(allo-HSCT).Methods The clinical data of 201 patients who underwent allo-HSCT in Affiliated Hospital of Xuzhou Medical University from January 2017 to July 2020 were retro-spectively analyzed,and grouped according to the occurrence of GI-GVHD.The factors associated with the occurrence of GI-GVHD were evaluated by univariate Logistic regression analysis,and the independent risk factors of their onset were evaluated by multivariate Logistic regression analysis,and further analyzed the effects of GI-GVHD on survival and prognosis.Results GI-GVHD occurred in 36 cases(17.9%)of the 201 patients who received allo-HSCT,with a median time to occur of 34(9-88)days and a median survival time of 228(21-1759)days,with an overall survival rate of 36.1%.The overall survival(OS)was significantly lower in patients who developed GI-GVHD at follow-up(36.1%vs 75.0%)compared with those without GI-GVHD,and the difference was statistically significant(P=0.004).Patients with GI-GVHD were characterized by persistent nausea and vomiting,abdominal pain,diarrhea,and gastrointestinal bleeding,often accompanied by damage to target organs such as the skin and liver.The results of univariate Logistic re-gression analysis showed that donor-recipient gender relationship(P=0.012),graft type(P=0.054),human leukocyte antigen(HLA)locus discrepancy(P=0.015),use of carbapenem antibiotics during pretreatment(P=0.029),use of carbapenems for more than 7 days during pretreatment(P=0.007),and early bloodstream infection(BSI,P=0.023)were influential factors in the occurrence of GI-GV HD.The results of multivariate Logistic regression analysis showed that female donor to male recipients(P=0.009,OR=8.866),non-related incompatible grafts(P=0.043,OR=16.532),carbapenem use for more than 7 days during pretreatment(P=0.023,OR=0.079),and early BSI(P=0.008,OR=0.165)were independent risk factors for the occurrence of GI-GVHD.Recipi-ent age,disease type,presence of underlying disease,graft type,duration of transplantation,the addition of antithymocyte globulin(ATG)to the pretreatment regimen,GVHD prophylaxis regimen,number of mononuclear cells(MNC)and CD34+cells returned,gran-ulocyte and megakaryocyte lineage time to reconstitution,and the occurrence of other types of GVHD were not risk factors for the occur-rence of GI-GVHD.The results of survival analysis showed that OS in the GI-GVHD group was significantly lower than that in the non-GI-GVHD group(36.1%vs 75.0%),and the difference was statistically significant(P=0.004).Conclusion Female donor to male recipients,non-related incompatible grafts,carbapenem use for more than 7 days during pretreatment,and early BSI may be the major risk factors for GI-GVHD,and the survival rate of patients with GI-GVHD after transplantation will be reduced.

Aneurysmal subarachnoid hemorrhage(aSAH)is a common critical condition in neurosurgery,which is easily complicated by cerebral vasospasm and delayed cerebral ischemia,leading to a high morbidity and mortality rate.Currently,there is a lack of objective and specific medical methods for the early recognition of these two complications.The authors summarized the methods of early identification of cerebral vasospasm and delayed cerebral ischemia,which have attracted much attention,and relate them to current clinical practice with a view to providing references to early identification and intervention in high-risk patients,and thus to improving patients'neurological prognosis.
10.Latent profile analysis of home-based functional exercise adherence in patients undergoing total knee arthroplasty

Objective:To explore the different categories of home-based functional exercise adherence in patients undergoing total knee arthroplasty (TKA), identify the influencing factors, and provide a reference for the development of personalized intervention strategies.Methods:A cross-sectional study was conducted using convenience sampling to select 320 patients who underwent unilateral primary TKA at The First Affiliated Hospital of Shandong First Medical University between November 2022 and January 2023. The Orthopedic Functional Exercise Compliance Scale, Rehabilitation Self-Efficacy Scale, and Social Support Rating Scale were used for data collection. Latent profile analysis was employed to identify the latent characteristics of home-based functional exercise adherence in TKA patients. Univariate analysis and logistic regression were used to explore the influencing factors among different subgroups.Results:A total of 320 questionnaires were distributed, with 307 valid responses, yielding a recovery rate of 95.94%. The 307 patients were classified into three latent profile categories based on their characteristics: low adherence group (29.32%, 90/307), moderate adherence group (46.58%, 143/307), and high adherence group (24.10%, 74/307). Logistic regression analysis revealed that the number of comorbidities, marital status, home rehabilitation environment, pain score, education level, social support, and rehabilitation self-efficacy were influencing factors for home-based exercise adherence in TKA patients ( P< 0.05) . Conclusions:Home-based functional exercise adherence in TKA patients can be categorized into three latent profiles. Healthcare providers should implement targeted interventions based on the specific characteristics of each patient, with a focus on those with low adherence, to enhance their compliance with home-based exercises.
