2.Preliminary application of rep-PCR in epidemiological research of extended spetrum ?-lactamase(ESBL)+ Escherichia coli

Objective To establish better reaction factors for rep-PCR and to investigate if the extended spectrum ?-lactamase(ESBL)+ Escherichia coli isolated from different patients in the respiratory ward have the same origin by using rep-PCR.Methods The ESBL confirmation was taken by double disc confirmatory test.The susceptibility testing was performed with K-B test.By applying the widespread enterobacter repetitive intergenic consensus as a primer,the stains were typed by rep-PCR following electrophoresis in agarose gel.Results The analysis of the PCR productions indicated that it would create useful DNA ban-prints and all these 22 ESBL(+)Escherichia coli strains were of three origins.Conclusion The method of rep-PCR is practical for epidemiological research in nosocomial infection.

Objective: To translate Millon Clinical Multiaxial Inventory Ⅲ (MCMI-Ⅲ) into Chinese, and then test its reliability and validity. Methods: MCMI-Ⅲ was administered to patients with mental disorders and normal people. Some subjects completed SAS, SDS, HAMA and HAMD simultaneously. 7-14 days later, some subjects completed the Chinese version of MCMI-Ⅲ again. Results: Cronbach’s alpha and the split-half reliability of MCMI-Ⅲ was 0.957 and 0.919, respectively. 4.Effects of zinc on carbon and nitrogen metabolism in leaves of Pinellia ternata and their yield

Objective To find out the proper concentration of ZnSO4?7H2O as foliar fertilizer used for Pinellia ternata, and to provide reference for high yield of P.ternate,effects of zinc on the carbon and nitrogen metabolism in leaves of P.ternata and the yield were studied.Methods Seedlings of P.ternata were employed as the experiment material.At the stage of full seedling,ZnSO4?7H2O of different concentration was sprayed on the leaves,the indexes of carbon and nitrogen metabolism were determined on the day 10,the weight of tubers and bulbils were measured at the sprout tumble stage.Results The yield of P.ternata has an exceedingly significant positive correlation with the contents of soluble sucrose and starch in the leaves,a significant positive correlation with the content of photosynthetic pigment and the aldolase activity,and a negative correlation with the amylase activity.In addition,the yield shows both an exceedingly significant positive correlation whith the nitrate reductase activity and a significant positive correlation with the content of protein.At the stage of full seedling,zinc sprayed on leaves can increase the content of photosynthetic pigment and improve the capability of photosynthesis.Meanwhile,spraying zinc can also promote the accumulation of soluble sucrose and starch in the leaves because of its stimulation on aldolase and inhibition on amylase.Besides,spraying zinc on leaves could improve the activity of nitrate reductase and the content of protein,and finally increase the yield of P.ternata.Conclusion The concentration of 600 mg/L is the best for ZnSO4?7H2O used as foliar fertilizer for P.ternate at the stage of full seedling.

Objective Try to know the prevalence rate of ESBL-s produced by Eseherichia coil in our hospital from 2003 to 2007.Methods Antibiotic susceptibility were determined by K-B disc diffusion.Double-disk synergy tests performed on Mueller-Hinton agar plates was used for ESBL confirmation.Results There was 23.8% of our isolates can produce ESBLs in 2003,25.4% in 2004,31.3% in 2005,36.6% in 2006 and 44.9% in 2007,respectively.The situation of antibiotic resistance is more severe.Conclusion The antibiotic resistance should be well surveyed and the ESBLs must be routinely assayed.
6.Change of ESBLs-KPN and ESBLs-ECO after antimicrobial intervention

Objective To evaluate the change of extended spectrum β-lactamase (ESBLs) Producing Klebsiella Pneumoniae (ESBLs-KPN) and Escherichia coli (ESBLs-ECO) causing nosocomial infection after antimicrobial intervention. Methods We regularly monitored the data on the yearly consumption [defined as daily dose (DDD) per 1 000 patient-days] of frequently used antibiotics from Dec. 2004 to Dec. 2007. From Jan. 2005 to Dec. 2007, we monitored the resistance of frequently used antibiotics and the timely integrative antimicrobial intervention was based on the outcome of antimicrobial resistance. We also monitored the isolation rate of ESBLs-KPN and ESBLs-ECO causing nosocomial infection. The departments studied were the experimental group and other comparable medical departments were the control group(ICU was excluded).Results The isolation rate of ESBLs-KPN ((43.90%)) and ESBLs-ECO (45.83%) in the experimental group was higher than that in the control group (28.04% and 24.90%, respectively) before the intervetion (P<0.05). The isolation rate of ESBLs-KPN decreased (from 26.47% to 17.65%) in the experimental group and that in the control group increased ( ESBLs-KPN: from 34.18% to (52.94%;) ESBLs-ECO: from 47.13% to 63.78%) from 2005 to 2007 (P<0.05). The isolation rate of ESBLs-KPN and ESBLs-ECO in the experimental group was lower than that in the control group after the antimicrobial intervention (P<0.05). Usage of ceftazidime and cefoperazone/sulbactam and imipenem was reduced and the consumption of cefepime was increased in the experimental group ((P<0.05)). Consumption of ceftazidime and cefoperazone/sulbactam and cefepime was increased. Conclusion The prevalence of ESBLs-KPN and ESBLs-ECO may be decreased after the integrative antimicrobial intervention.
7.Role of enhancement features of cerebral venous sinus in the diagnosis of cerebral venous sinus thrombosis: a MRI study

Objective To describe the changes of thrombosed venous sinus on MRI after administration of contrast material and evaluate the sensitivity and specificity of thread-like enhancement around sinus to diagnose thrombosis in the corresponding sinus.Methods Patients with cerebral venous sinus thrombosis (CVST) admitted to our department from January 2005 to December 2014 and undergone a MRI scan with administration of contrast material were included in this study.The enhancement features of venous sinus were studied in the plane parallel to the interested sinus.The features of enhancement were classified as peripheral thread-like enhancement, partial enhancement and complete enhancement.The proportion of these three type of enhancement in thrombosed sinuses and normal sinuses were described and compared.The sensitivity and specificity of peripheral thread-like enhancement to diagnose thrombosis in corresponding sinus were calculated.The proportion of each type of enhancement was also described and compared in acute (≤ 7 d), subacute (8-30 d) and chronic (≥ 31 d) stage after onset of symptoms.Results Peripheral thread-like enhancement, partial enhancement and complete enhancement were all found in both thrombosed and normal sinuses.There was a significant difference of enhancement features between normal and thrombosed sinus in superior sagittal sinus (100% (30/30) vs 60% (27/45), x2 =13.789, P =0.001), left trans verse sinus, and right sigrnoid sinus.The sensitivity and specificity of peripheral thread-like enhancement to diagnose thrombosis in the corresponding sinus were 10.5%-44.4% and 53.3%-76.7% respectively.There was no significant difference of contrast features at different stage after onset.Conclusion The value of peripheral thread-like enhancement to diagnose CVST is limited because of low sensitivity and specificity.
8.Compar Bon of sensitivity and specificity of specific IgM detected by indirect-ELISA and capturing-ELISA

Objective To assess the differences in the specificity and the sensitivity in detecting specific IgM by indirect ELISA and capturing-ELISA methods,and to analyze the possible causes of those differences.Methods The HBc-IgM and EHF-IgM level in serum samples from activity Hepatitis B patients and hemorrhagic fever patients with renal syndrome were determined by both indirect-ELISA and capturing-ELISA methods,and the differences in the positive rate,positive threshold value and the specificity between those two methods were compared. Results Specific murine IgM diluted in PBS were detected by indirect-ELISA and capturing-ELISA,and the specificity and sensitivity of those two methods were similar.Results showed that sensitivity of indirect-ELISA was lower than capturing-ELISA in detecting specific IgM in serum from patients with activity Hepatitis B and hemorrhagic fever patients with renal syndrome.The IgM level in hepatitis B and hemorrhagic group treated with thioglycol became negative detected by those two methods,suggesting that both of them have the anti-IgM epitope-specificity.Cross reaction results demonstrated that the reagent detecting IgM from the two methods had the specificity to the specific antigen. Conclusion It is recommended to detect IgM level by capturing-ELISA method for the early diagnosis of acute infectious disease,pathological changes,immune reaction and prognoses of chronic continuous infectious disease.Applying the indirect-ELISA method to detect IgM level to diagnose acute infectious disease is to discuss in the future.

10.Stability tests and action limits setting for central frequency and transmitter gain of MRI scanners

Objective To test the central frequency (CF) and transmitter gain(TG) of MRI scanners,and set up their action limits.Methods Three different MRI devices (GE 3.0T HD,GE 1.5T HDi and GE 3.0T 750W) were tested by scanning American College of Radiology (ACR) phantom with the axial T1WI series.In the pre-scanning of T1WI series for the ACR phantom,the CF and TG were recorded.It was tested for eight times when MRI scanners were in good condition.The action limits of CF and TG were calculated based on mean values and standard deviations.Results The mean values of CF for three devices were (127 725 772.38±39.68)Hz,(63 875 740.13± 34.15)Hz,and (127 771 958.38±12.19)Hz,respectively.Their action limits were ≤119.04 Hz,≤68.30 Hz,and ≤36.57 Hz,respectively.The mean values of TG for three devices were (125.25±1.28)dB,(101.75±1.98)dB,and (113.25±0.89)dB,respectively.Their action limits were (125.25±2.56)dB,(101.75±3.96)dB,and (113.25±1.78)dB,respectively.Conclusion The CF and TG for three MRI scanners are all consistent with the action limits in this study.The CF and TG action limits will provide criterions for the clinical quality control.
