This article was based on the investigation of ethical research in traditional Chinese medicine (TCM) clini-cal trials among 240 ischemic stroke patients and 105 experts specialized in this field. Analysis was given on the re-search results. This article was intended to regulate the ethical review of ischemic stroke, and to protect the rights and interests of the subjects better.

Objective To observe the efficacy ofvitrectomy combined with internal limiting membrane (ILM) peeling and scleral shortening for myopic foveoschisis (MF).Methods Prospective and non-randomized concurrent control study.A total of 35 MF patients (35 eyes) were enrolled in this study. The patients were divided into 2 groups according to surgery, including group A (18 eyes) and group B (17 eyes),all received vitrectomy combined with ILM peeling, but group A also received scleral shortening. The best corrective visual acuity (BCVA) examination using the Snellen vision chart was converted to the minimum resolution logarithm (logMAR).Ocular axis length (AL) was measured by Zeiss IOL-Master or A-scan ultrasound (Quantel Medical,France). The maximal value of retinal foveoschisis (MxFT) was measured by frequency-domain optical coherence tomography (Heidelberg,Germany).Multifocal electroretinogram (mfERG) responses were obtained with the RETIscan system (Roland Consult,Gemany). There was no statistically significant difference between the two groups (P>0.05) in age (t=0.460),AL (t=1.520),diopter (t=0.020),logMAR BCVA (t=-2.280),MxFT (Z=-4.179) and b-wave ERG amplitude (Z=-0.198).The changes of BCVA,AL,MxFT and b wave amplitude were followed-up for 3-12 months. Results At the last follow-up,the height of MF was decreased in 18 eyes of group A,and MF was completely disappeared in 4 eyes. The logMAR BCVA (t=7.272,5.951),MxFT (Z=-3.724,-3.622) and b-wave ERG amplitude (Z=-3.223,-3.243) in both groups A and B were statistically improved (P=0.000,0.000,0.000,0.000,0.001,0.001) compared to pre-operational results. There was significant difference of logMAR BCVA (t=-2.280) and MxFT (Z=-4.179) between the two groups (P=0.029,0.000).But there was no significant difference in the amplitude of b-wave (Z=-0.198,P=0.843).The AL in group A was shortened after surgery, the difference was statistically significant (t=10.017,P=0.000).During the follow-up,there was no ocular hemorrhage, endophthalmitis and other complications. Conclusion PPV combined with ILM peeling and scleral shortening can shorten AL significantly for MF patients, and gain relative normal anatomical structure of the fovea, thus improve the vision.

Objective To evaluate the implant-bone interfaces on implants with active Zn coating in a rabbit model. Methods Pure titanium cylinder implants, fabricated by machining, were prepared by surface microarc oxidation (MAO).The MAO-Zn/Ca/P coated (experimental group) and the MAO-Ca/P coated (control group)implants were randomly placed in the rabbit mandible. The samples were harvested at 4 and 12 weeks postoperatively, histological analysis on the implant-bone interfaces of the two implants was performed, and the bone coverage was compared. Results The coatings of MAO presented micro-porous structure with dense and uniform features,in which size of micro-porous increased with zinc element. The histological examination of implant-bone contact interfaces showed that the bone tissues attached on the coatings increased with time.Further, the adhered bone tissue in the experimental group was thicker and denser and had a larger amount compared to that in the control group at 4 and 12 weeks postoperatively. The bone coverage in the experimental group (70.8%±13.6%) at 12 weeks postoperatively was significantly higher than that in the control group (55.9%±13.8%) (P<0.05).The results of bone interface element showed that the apatite layer was precipitated at 12 weeks. Conclusion The addition of active zinc element can improve the biological activity of the coating, enhance the osteogenesis ability of the coating, and accelerate the osseointegration of implant-bone interfaces.

Objective To examine the practical value of early detection of heart-type fatty acid binding protein (H-FABP)for risk stratification and prognosis assessment in cardiac troponin T (cTnT)-negative acute coronary syndrome(ACS)patients. Methods From March 2010 to March 2012,55 patients with chest pain and negative cTnT were selected from 232 ACS patients at the General Hospital of PLA.Expression levels of cTnT and H-FABP were detected within 6 h of the onset of clinical symptoms.H-FABP and cTnT values at 12,24,and 48 h from the onset of clinical symptoms were continuously measured to monitor the dynamic changes.Based on prognosis, patients were divided into two groups, levels of H-FABP were compared, and its predictive value for prognosis was assessed with the ROC curve. Results Within 6 h of the onset of clinical symptoms,cTnT levels in cTnT-negative ACS patients increased gradually as disease progressed and reached the peak value at 12 h before decreasing slowly and arriving at 50% of the peak value at 48 h.Meanwhile,HFABP levels reached the peak within 6 h,decreased slightly(12.8%) at 12 h,and then decreased rapidly at 48 h (about 79%).Of 55 patients,24 had acute myocardial infarction during hospitalization. The H-FABP level within 6 h was a good predictor for cTnT-negative ACS patients. The area under ROC curve was 0.946 and the cutoff value was 15.47 μg/L.The prediction sensitivity was 87.5 %,with a specificity of 90.3%.Eleven patients had cardiovascular events after a 12-month follow-up.Levels of H-FABP were different in patients with or without cardiovascular events,[(38.08±8.43) μg/L vs.(18.96 ± 2.85) μg/L (t =2.438,P<0.05)].ROC curve analysis showed that the area under the curve was 0.772 and the prediction cutoff value was 44.71 μg/L.The rates of cardiovascular events were markedly different between patients with high(≥44.71 μg/L)and those with 1ow(<44.71 μg/L)H-FABP levels(54.5% vs.11.4%).Conclusions For ACS patients with negative cTnT,H-FABP is a good index for early risk stratification and prognosis assessment.
