1.Clinical application of hysteroscopic on intrauterine benign lesions treatment and its safety evaluation

Objective To investigate the hysteroscopy application on uterine cavitary benign and its safety.Methods Two hundred and ten patients with benign lesions in the uterine cavity from Jan 2010 to Jan 2013 were selected as our subjects.They were underwent hysteroscopy and electric section,including 72 cases with uterine polyps resection,70 cases with uterine fibroids resection,and 68 cases with pregnancy tissue residue removal surgery.The follow up periods were 4 months to 2 years and the therapy effects were recorded.Results Two hundred and ten patients were got one successful operation.Operation period was ranged from 11 to 39 minutes and average was 15 minutes.The intraoperation bleeding volumes was ranged from 5 to 70 ml and no intraoperation complications occurred.During the follow-up periods,94.1% (191/203) patients were recovered to regular menstruation and dysmenorrhea improvement rate was 87.70% (178/203) at 1 month after operation.At 6 months after operation,96.1% (195/203) and 91.6% (186/203) patient were with the regular menstruation and dysmenorrhea improvement.Two cases with uterine cavitypolyp were reoccurred after operation and cured by electron section.Conclusion Hysteroscopy in treatment of benign uterine lesions is showed the satisfactory curative effect in terms of keep uterus integrity with little injury,slight pain.It is proved to be as the prefer methods to treatment for benign uterine cavity lesions.
2.Effect of vascular endothelial growth factor on collagen Ⅱ expression in rat articular chondrocytes

ObjectiveTo investigate the effect of vascular endothelial growth factor (VEGF) on collagen Ⅱ expression in rat articular chondrocytes in vitro.MethodsChondrocytes were isolated and cultured.Then rats were divided into 4 groups:group A(control):without any intervention; group B:10 ng/ml VEGF was added; group C:10 ng/ml IL-1β was added; group D:10 ng/ml VEGF and 10 ng/ml IL-1β were added.Messenger RNA (mRNA) expression of collagen Ⅱ was detected by using real time polymerase chain reaction (real time PCR),and the protein expression level of collagen Ⅱ was detected by Western blotting.Comparisons between groups were performed by one-way ANOVA.ResultsThe collagen Ⅱ mRNA expression levels of group B (0.78+0.07),group C (0.67+0.06) and group D (0.57+0.04) were significantly lower than those of the group A (1.00±0.08),and there was significant difference between B and D,C and D.Compared with group A(0.95+0.21),the expression of collagen Ⅱ protein in group B(0.71+0.14),group C(0.60±0.11) and group D(0.31 +0.09) was significantly suppressed.The expression of collagen Ⅱ protein in group D was significantly lower than those of group B and C.ConclusionVEGF can significantly suppress the expression of collagen II in rat articular chondrocytes.VEGF may play an important role in the development of osteoarthritis.
3.Study on the senescence of rat glomerular mesangial cells induced by Tert-Butyl hydroperoxide in vitro and the influnce of probucol on its senescence

Objective To investigate the senescence of rat mesangial cells induced by Tert-Butyl hydroperoxide (tBHP) and the protective effect of probucol on senesecence. Methods Human glomerular mesangial cells(hGMC) were cultured in vitro and intervened by tBHP. The cell survival rate was observed by methyl thiazolyl tetrazolium( MTT). β-gal staining and cell cycle analysis were used to identify cell senescent status;transmission eletric microscopy was used to evaluate the ultra-microstructure of hGMC. Senescent-related indexes were detected after treatment with probucol. Results The cell survival rate with 30 μmol/L tBHP was (80. 12 ± 3. 25 ) % , the positive rate of β-gal staining was significantly higher in tBHP-induced cells (about 81% )than that of the control cells( P <0. 01). 86% of the cells was arrested at G0-G1 phase. Invagination of nucleus membrane and chromatin condensation at the nuclear margin in tBHP-induced cells was observed through transmission eletric microscopy. In the probucol intervented cells, the cell survival rate was higher than that of tBHP-induced cells (92. 68 ± 5.03) % vs. ( 80. 12 ± 3. 25) % (P < 0. 05 ). The positive rate of β-gal staining decreased to 45. 2%. The proportion of cell cycle stage was similar to the control cells.The change of morphous and ultrastructure was relieved. Conclusions tBHP can induce hGMC senescence in vitro and probucol may play a role in preventing hGMC senescence.
4.Non-motor symptoms in Parkinson' s disease: three years follow-up

Objective To assess the development,progression and change of nonmotor symptoms in patients with Parkinson' s disease and its impact on patients' quality of life.Methods Eighty-seven consecutive patients with idiopathic Parkinson' s disease were studied.Parkinsonian status was assessed at baseline and 3 years follow-up using Unified Parkinson' s Disease Rating Scale (UPDRS) part Ⅲ & Ⅳ,Nonmotor Symptoms Questionnaire (NMSQuset),Parkinson-related quality of life (PDQ) scales.Paired ttest,Chi-square test,Spearman rank order correlation and hierarchical regression of the major statistical procedures were employed.Results At 3 years follow-up,compared to baseline,the UPDRS Ⅲ score (22.21 ±11.31 vs 30.49± 11.68),UPDRS Ⅳ score(1.00±1.54 vs 2.94±3.12),NMS score (7.98±3.96 vs 12.35 ± 5.12) and PDQ score (28.11 ± 22.88 vs 36.65 ± 26.95) were significantly higher ( t =- 5.54,- 5.75,- 6.46,- 5.29,all P =0.000,respectively).The aggravation of motor and nonmotor symptoms caused the decline of quality of life.The prevalence of constipation,problem of remembenng thing,nocturia ranked tops,and depression,and anxiety were still in the middle,compared with baseline.The prevalence of pains,sweating,dribbling,sense of incomplete emptying etc were significantly increased during the follow-up,△R2 were 21.6% and 23.4% respectively,resulting in the deterioration of quality of life.Conclusions PD nonmotor symptoms appear from the early stage.The motor and nonmotor symptoms aggravate over time.
5.A 1ongitudinal study of the progression of motor symptoms and risk factors of motor complications in Parkinson' s disease

Objective To observe progression of motor symptoms and occurrence of motor complications in parkinsonian patients and investigate the rate of progression of motor symptoms and risk factors of motor complications.Methods One hundred and thirty patients diagnosed with PD in 2007 in Department of Neurology,Xinhua Hospital were followed up for 3 years.The Unified Parkinson' s Disease Rating Scale (UPDRS) and H-Y staging were used to assess and follow up motor symptoms and occurrence of motor complications,and analyze the rate of progression of motor symptoms and risk factors of motor complications with statistics.Results ( 1 ) Mean annual growth in H-Y staging was 2.5%,and UPDRS motor scores was 3.1% ; the incidence of dysphagia at endpoint in patients was increased by 23.0% compared with baseline; incidence of falls was increased by 16.7%;(2)Daily levodopa dose at endpoint ( OR =1.004,95% CI 1.001—1.006,P =0.008 ) was independent risk factors with dyskinesia; While duration ( OR =1.637,95% CI 1.083—2.473,P =0.019 ),levodopa treatment duration ( OR =0.698,95% CI 0.494—0.987,P =0.042 ),daily levodopa dose at haseline ( OR =1.005,95% CI 1.001—1.010,P =0.016) and at endpoint ( OR =1.014,95 % CI 1.001 —1.027,P =0.032 ) were risk factors with motor fluctuations.Conclusions As the disease progresses,motor function in parkinsonian patients gradually worsens,the incidence of swallowing difficulty and of falls is increased,and the incidence of motor complications is increased.The total exposure to levodopa in parkinsonian patients is predictor for motor complications.
6.A 3-year clinical prospective investigation: establishment of anassessment questionnaire for Parkinson's disease with motor complications

Objective To derive a questionnaire to assess the risk of developing motor complications through a 3-year prospective investigation on 71 patients of Parkinson' s disease (PD) in the out clinic at our hospital.Methods Three years after the first assessment,71 PD out patients were reassessed using various scales,including Unified Parkinson Disease Rating Scale,Hoehn-Yahr grade,Mini Mental State Exam,Hamilton Depression Scale and Hamilton Anxiety Scale.Results The incidence of motor complications was 43.6% (31/71).Logistic regression analysis showed that the prognostic factors for motor fluctuation were age of onset ≤ 54 (OR =6.4,95% CI 1.7-24.5,P =0.006),the occurrence of swallowing difficulty (OR =3.8,95 % CI 1.0-14.1,P =0.04) and depression (OR =4.0,95 % CI 1.1-13.7,P =0.03),and the prognostic factors for dyskinesia were age of onset ≤54 (OR =48.5,95% CI 1.9-121.0,P:0.02),the occurrence of falling (OR =64.1,95% CI 2.9-142.2,P =0.008) and the daily levodopa dosage > 600 mg(OR =17.5,95% CI 1.1-276.2,P =0.04).Based on the regression model,the assessment questionnaire for motor complicationsincludes the followings:the questionnaire for motor fluctuations:the age of onset ≤54,2 points; the occurrence of swallowing difficulty,1 point; the occurrence of depression,1 point; the questionnaire for dyskinesia:the onset age ≤54,2 points; the occurrence of falling,3 points; daily levodopa dosage > 600 mg,2 points.In all patients in this study,21.7% (10/46) was asscssed to a total scorc of 0-1 which is associated with a low risk of motor fluctuation,8/16 had a score of 2 which is associated with intermediate risk and 8/9 got a score of 3-4 associated high risk; 10.2% (5/49) had a score of 0-2,a low risk of dyskinesia,4/13 had a score of 3-4,a intermediate risk and 7/9 got a score of 5-7 which is associated with a high risk.Conclusions Age onset ≤54,the occurrence of swallowing difficulty,falling and depression,daily levodopa dosage > 600 mg were considered to be the prognostic factors of motor complications in PD.The questionnaire may help to stratify PD patients into low-risk,medium-risk and high-risk groups for motor complications and the higher the score in the questionnaire is related to the higher risk of motor complications.

8.Investigation and demand analysis of training status of emergency personnel in primary hospitals in Sichuan Province

Objective To get well-informed of the current status of emergency training in primary hospitals in Sichuan Province and to find weaknesses in the system in order to provide scientific basis for emergency knowledge and skills training.Methods A stratified random cluster sampling method as adopted and an anonymous questionnaire survey method was conducted to investigate the basic circumstances of the current training and the training needs of 1 000 emergency medical staff in 9 regions throughout Sichuan Province.Results The scope of the survey covered emergency doctors,nurses and technicians in hospitals of Grade Ⅱ,Grade Ⅲ and below.A total of 1 000 surveys were sent out and 983 were recovered.The questionnaire completion rate was 98.3%.The respondents mainly worked for more than 10 years,most of which with Bachelor degree,junior and intermediate titles.Results of the survey showed,63.1% Most respondents have only one or two training opportunities per year.The training methods are mainly continuing education (59.0%,580 trips),training organized within the unit (58.5%,575 trips),and training organized by health administrative units (39.3%,386 trips).The main contents of the training are:basic professional skills learning (66.7%,655 trips),basic professional theoretical study (59.2%,582 trips),and new emergency technology (42.5%,418 trips),only 32.0% of the respondents believe that these methods meet the practical needs.The main factors that influence participation in the training were:not be arranged by employer (36.2%,356 trips);the training was different from actual work (31.6%,311 trips);no replacement for work so they couldn't join training (29.8%,293 trips).The survey showed that the most desirable way of training is:practical application (60.1%,591 trips) and the most desirable training content is:emergency first aid knowledge (73.7%,724 trips).There were significant differences in case discussion,technical observation,academic lectures,special study trainingand learning methods in hospitals of different levels (P<0.05),but there was no significant difference in the choice of professional practice methods (P>0.05),and the selection of training contents was in emergency first aid knowledge,general medical knowledge and prevention and treatment of chronic diseases.There was no statistical difference in the selection of training contents of knowledge,infectious disease knowledge and department management knowledge (P>0.05).There were significant differences in the choice of nursing knowledge training (P<0.05).Conclusion The current situation showed a lack of first-aid knowledge and skills training for emergency personnel in Sichuan Province.The present situation of skill training can't satisfy the need of their desire to participate in training.It is urgent to develop a standardized,systematic and scientific training mode to improve the emergency first aid ability of primary medical and emergency personnel.
9.Construction of dengue virus-specific full-length fully human antibody libraries by mammalian display technology.

OBJECTIVETo construct dengue virus-specific full-length fully human antibody libraries using mammalian cell surface display technique.
METHODSTotal RNA was extracted from peripheral blood mononuclear cells (PBMCs) from convalescent patients with dengue fever. The reservoirs of the light chain and heavy chain variable regions (LCκ and VH) of the antibody genes were amplified by RT-PCR and inserted into the vector pDGB-HC-TM separately to construct the light chain and heavy chain libraries. The library DNAs were transfected into CHO cells and the expression of full-length fully human antibodies on the surface of CHO cells was analyzed by flow cytometry.
RESULTSUsing 1.2 µg of the total RNA isolated from the PBMCs as the template, the LCκ and VH were amplified and the full-length fully human antibody mammalian display libraries were constructed. The kappa light chain gene library had a size of 1.45×10(4) and the heavy chain gene library had a size of 1.8×10(5). Sequence analysis showed that 8 out of the 10 light chain clones and 7 out of the 10 heavy chain clones randomly picked up from the constructed libraries contained correct open reading frames. FACS analysis demonstrated that all the 15 clones with correct open reading frames expressed full-length antibodies, which could be detected on CHO cell surfaces. After co-transfection of the heavy chain and light chain gene libraries into CHO cells, the expression of full-length antibodies on CHO cell surfaces could be detected by FACS analysis with an expressible diversity of the antibody library reaching 1.46×10(9) [(1.45×10(4)×80%)×(1.8×10(5)×70%)].
CONCLUSIONUsing 1.2 µg of total RNA as template, the LCκ and VH full-length fully human antibody libraries against dengue virus have been successfully constructed with an expressible diversity of 10(9).

10.Wenyang-Shengji ointment regulates GRP78/CHOP pathway to inhibit excessive endoplasmic reticulum stress for facilitating diabetic refractory wound repair

AIM:To study the regulation of endoplasmic reticulum stress(ERS)using the glucose regulated protein 78(GRP78)/CCAAT/enhancer binding protein homologous protein(CHOP)pathway and explore the related mech-anism of Wenyang-Shengji ointment in facilitating the repair of diabetic refractory wounds.METHODS:To establish a rat model of diabetic refractory wound repair,Sprague-Dawley(SD)rats were fed a high-fat diet and intraperitoneally in-jected with streptozotocin.Subsequently,full-thickness skin defects were induced in the dorsal region of the rats.The ex-periment included 4 groups:normal,model(diabetic refractory wounds),Wenyang-Shengji ointment,and Beifuxin(re-combinant bovine basic fibroblast growth factor gel)groups.The normal and model groups were treated with normal saline after disinfection.In the Wenyang-Shengji ointment and Beifuxin groups,the wounds were topically treated with the re-spective ointments once daily.After 14 d of treatment,wound healing was assessed and quantified using the wound healing rate.Hematoxylin-eosin(HE)staining was employed to examine the micromorphology of the wound tissue.Western blot analysis was performed to measure GRP78,CHOP and caspase-12 levels in the wound tissue.Immunohistochemical analy-sis was used to detect the expression and distribution patterns of GRP78,CHOP and caspase-12 in the wounds.Transmis-sion electron microscopy was used to observe reticulum numbers and swelling.Enzyme-linked immunosorbent assay was used to determine interleukin-1β(IL-1β)level as a pro-inflammatory factor within the wound.RESULTS:Indexes of each group were assessed 14 d after the corresponding intervention.Compared with normal group,the rats in model group exhibited a significant decrease in the wound healing rate(P<0.01),accompanied by increased inflammatory exudation and poor granulation tissue growth.Additionally,there were increases in the expression levels of GRP78,CHOP and cas-pase-12 proteins(P<0.01),as well as a significant elevation in the content of inflammatory factor IL-1β(P<0.01).In contrast,compared with model group,treatment with Wenyang-Shengji ointment resulted in a significant improvement in wound healing rate(P<0.01),reduction in inflammatory exudation,and enhanced granulation tissue growth(P<0.01).Furthermore,there was a notable decrease in the protein expression of GRP78/CHOP/caspase-12 within the wound tissue following treatment with Wenyang-Shengji ointment(P<0.01).The levels of inflammatory factor IL-1β also showed a sig-nificant decrease(P<0.01).CONCLUSION:Wenyang-Shengji promotes the healing of diabetic refractory wounds,which may be associated with the downregulation of the GRP78/CHOP pathway,inhibition of excessive ERS,and reduc-tion in the level of wound cell apoptosis.
