Objective To investigate the prevalence of plasmid-mediated quinolone resistance(PMQR) in Proteus mirabilis clinical isolated from urine. Methods Antimicrobial susceptibility test was performed using the microdilution method. And PMQR gene qnrA,qnrB,qnrC,qnrD,qnrS,aac(6′)-Ib-cr,qepA were amplified by PCR,then the PCR positive products were sequenced to identify their genotypes. Results In 41 strains of Proteus mirabilis from urine, the resistant rates to ciprofloxacin and levofloxacin were 41.5% and 29.3%,respec-tively.Among all clinical isolates,qnrA1 gene was detected in 2 strains,qnrB2 gene in 3 strains.PMQR gene qn-rB,qnrC,qnrD,qnrS,aac(6')-Ib-cr and qepA were not detected in all strains. Conclusions Clinical isolates of Proteus mirabilis from urine carry PMQR genes. The prevalent principal genotypes are qnrA1 and qnrB2 in these iso-lates.They are related to low levels resistance toquinolone.

Objective To evaluate the effects of continued grateful emotion after discharge on the long-term positive experience and care burden of spouses of patients with brain tumors. Methods Totally 114 patients with brain tumor were selected as the research objects from October 2015 to October 2016. Random number table was used and all patients were randomly divided into the control group (n=56) and the observation group (n=58). Patients in the control group received routine health education and guidance, and two emotional emotional interventions were given to patients during the hospital; while patients in the observation group received increased emotional interventions according to the care burden of spouses of patients. The positive experience and care burden of two groups of spouses were evaluated by Caregiver Positive Experience Scale (PAC) and Caregiver's Burden Scale (CBI) 2 months, 3 months, and 6 months after discharge. Results There was no significant difference in positive experience and care burden of spouses between two groups (P>0.05); 3 months and 6 months after discharge, the scores of positive experience and care burden of spouses in the observation group were [(36.76±4.13), (36.71±4.21)]and [(27.07±4.62), (27.62±4.47)], and they were significantly better than those of the control group [(54.5±3.25),(25.36±3.17)and (54.75±6.27), (54.01±6.21)], the differences were statistically significant (t=7.847-8.587,P<0.05).Conclusions Emotional intervention during hospitalization can effectively improve the positive experience and care burden of spouses of patients. After discharge, emotional intervention can provide outpatient nursing care for the spouses of patients. There is a better long-term effect in improving the positive experience and care burden of spouses of brain tumor patients.

Objective:To construct a knowledge graph,to design an intelligent risk assessment model for adverse events and a risk factor analysis control system for the disinfection supply of stomatological medical devices, and improve the quality of disinfection supply of stomatology instruments through system application. Methods:The automatic collection, conversion and loading of stomatological medical devices disinfection supply data were realized by constructing data access services, the knowledge graph of adverse events of stomatology disinfection supply was constructed by using failure mode and effect analysis(FMEA)and dynamic risk assessment theory, and the risk prediction model and root cause analysis method of adverse events of disinfection supply were constructed by using recurrent neural network(RNN),and the risk assessment and root cause analysis of adverse events of stomatology medical devices disinfection supply were carried out. Results:The top four risk factors of the adverse events of"cleaning"in disinfection supply were incorrect cleaning parameter settings, stains on the instrument after cleaning, substandard lumen cleaning after cleaning, failure to meet the standard of luminal cleaning after cleaning, and scale presence in the instrument after cleaning, respectively, and the systematic prediction probabilities were 4.67%,2.33%,1.50%,and 1.39%,respectively,which were consistent with the actual probability of occurrence of 2.14%,1.85%,0.75%,and 0.50%.The top four risk factors for the adverse event of"water stains on the surface of disinfection equipment"were too high temperature setting of disinfection equipment, too long disinfection time of disinfection equipment, residual dirt on the surface of disinfection equipment and scale residue of heating pipe, which were consistent with the composition order of the first four risk factors in the actual statistical results. Conclusion:The application of the risk assessment model of adverse events and the risk factor analysis and control system of intelligent stomatological medical device disinfection supply can timely discover the quality hidden dangers in the operation of stomatology device disinfection supply and implement risk control, and provide reference for the construction of related business systems.

Journal of Shanghai Jiaotong University(Medical Science) 2024;44(11):1347-1358
Objective·To explore the effects of the cancer-testis antigen(CTA)family member CT63 on proliferation, differentiation, and tumorigenicity of chronic myeloid leukemia(CML)cells, and uncover the underlying molecular mechanisms. Methods·The link between CT63 expression and the prognosis of myeloid leukemia patients was analyzed using bioinformatics methods(TCGA database).A K562 cell line with CT63 knockdown was established. The knockdown efficiency of CT63 was confirmed by qRT-PCR and Western blotting.Live-cell imaging and CCK-8 methods were adopted to evaluate the inhibitory effect of CT63 knockdown in CML cells.A subcutaneous tumorigenesis assay in nude mice was conducted to examine the effects of CT63 on tumorigenesis, tumor growth, and differentiation of K562 cells in vivo.Phorbol 12-myristate 13-acetate(PMA)-induced monocyte/macrophage differentiation experiment was carried out to investigate the role of CT63 in the differentiation of K562 cells in vitro.Mitochondrial function was assessed to determine the impact of CT63 on CML cells both in vivo and in vitro. Results·The Kaplan-Meier survival curve indicated that low expression levels of CT63 were correlated with longer survival in patients with myeloid leukemia.Down-regulation of CT63 in K562 cells inhibited proliferation and promoted differentiation.Live-cell imaging and CCK-8 assays displayed that knockdown of CT63 inhibited cell proliferation and extended cell doubling time in K562 cells. In the subcutaneous xenotransplantation model,down-regulation of CT63 inhibited tumor growth in nude mice.K562 cells expressing lower levels of CT63 were more prone to differentiate into monocyte/macrophage both in vivo and in vitro under PMA exposure condition.Knockdown of CT63 suppressed the activity of mitochondrial respiratory chain complex Ⅳ.This led to decreased expression of mitochondrial markers, including cytochrome C oxidase Ⅳ(COX Ⅳ),pyruvate dehydrogenase, succinate dehydrogenase A(SDHA),and voltage-dependent anion channel(VDAC),thus affecting the mitochondrial metabolic activity of K562 cells.Conclusion·CT63 is related to the prognosis of myeloid leukemia patients.CT63 plays an important role in promoting proliferation and inhibiting differentiation of K562 cells in vivo and in vitro.CT63 serves as a switch to regulate the balance between proliferation and differentiation of CML cells via the modulation of mitochondrial activity.
