1.Evaluation of Preoperative Imaging in Total Mesorectal Excision for Rectal Cancer

Objective To study the value of multi-slice spiral CT(MSCT)and high-resolution MR in evaluating the feasibility of total mesorectal excision(TME)for rectal cancer before operation.Methods Sixty-three cases biopsy-proven rectal cancer underwent MSCT and high-resolution MR examination,including CT in 35 and MR in 28.The involvement of mesorectal fascia,lymph nodes extra-fascia and the curve distance between the lesions and the anal margin were observed and compared with that of operation and pathology.Results In evaluating the involvement of mesorectal fascia,two patients were oversteped by CT and one patient by MRI,and there was no significantly statistic difference between CT and MRI.The curve distance between lesions and anal margis could be measured accurately on sagittal section of MR images in all cases.Sixteen patients had been found having metastatic lymph nodes at extra-mesorectal fascia,twelve of them were biopsy-proven.Thirty-seven patients had been found having metastatic lymph nodes at intra-mesorectal fascia,but 4 of them were inflammatory biopsy-proven.Conclusion MSCT and high-resolution MR are of significant values in evaluating the feasibility of TME in the patients with rectal carcinoma.
2. Safety of laser treatment for peri-implantitis: a review

3.Incidence and risk factors for breast cancer-related lymphedema:a retrospective cohort study

Objective To determine the incidence of breast cancer?related lymphedema ( BCRL) in China and to analyze the associated risk factors. Methods A retrospective analysis was performed on the clinical data and the incidence of BCRL in 281 patients who were newly diagnosed with breast cancer and received surgery. The incidence of BCRL was evaluated using arm circumference measurement and Norman questionnaire. The risk factors for lymphedema were analyzed using chi?square test and logistic regression model. Results In all patients,the incidence rates of BCRL determined by arm circumference measurement and Norman questionnaire were 31?7% and 27?0%, respectively. The multivariate analysis showed that postoperative radiotherapy,a preoperative body mass index no less than 24 kg/m2 ,a large axillary lymph node dissection area,and a large number of positive axillary lymph nodes significantly increased the risk of BCRL (HR=2?87,P=0?042;HR=2?54,P=0?011;HR=1?97,P=0?037;HR=1?06,P=0?023). Moreover, patients with breast cancer and hypertension had 1?74?fold higher risk of BCRL than those with normal blood pressure. Conclusions The incidence of BCRL is still very high. However,most of patients only have mild edema. Postoperative radiotherapy, a large axillary lymph node dissection area, a large number of positive axillary lymph nodes,a high preoperative body mass index,and hypertension are risk factors for BCRL.
4.Clinicopathological features of metastatic castration-resistant prostate cancer

Objective To investigate the histopathological characteristics of metastatic castration-resistant prostate cancer.Methods Clinical findings,morphologic features,immunophenotypes of androgen receptor (AR),prostatic specific antigen (PSA),chromogranin A (CgA),and phosphatase and tensin homolog deleted on chromosome ten(PTEN)(by EnVision method) of 178 core biopsies from patients with metastatic castration resistant prostate cancer encountered from January 2010 to January 2015 from West Coast Dream Team were analyzed.Results In the 178 cases,51(28.7%)were diagnosed with typical adenocarcinoma,41 cases(23.0%)with intermediate atypical carcinoma and 24 cases(13.5 %)with small cell carcinoma.The expression of PSA was significantly lower in patients with intermediate atypical carcinoma or with small cell carcinoma(31/39,79.5 %) than in those with adenocarcinoma(20/20,100 %).The expression of PTEN was significantly higher in patient with intermediate atypical carcinoma or with small cell carcinoma (31/46,67.4%) than in those with adenocarcinoma (19/41,46.3%).Conclusions Three histopathological subtypes exists in metastatic castration-resistant prostate cancer,including adenocarcinoma,intermediate type atypical carcinoma and small cell carcinoma.They are entities with unique pathological features.The expression of PTEN is significantly lower in the patients with intermediate atypical carcinoma or with small cell carcinoma than in those with adenocarcinoma,indicating that intermediate atypical carcinoma and small cell carcinoma are different from typical adenocarcinoma in tumor occurrence and progression.
5.Application of in situ amniocyte culture for prenatal diagnosis.

OBJECTIVE:
To explore the value of in situ amniocyte culture for prenatal diagnosis.
METHODS:
2716 amniotic fluid samples were cultured in situ on slides. After the culture, the slides were stained, photographed and analyzed.
RESULTS:
All samples were successfully analyzed, with the success rates for primary culture and subculture being 98.42% and 1.58%, respectively. 224 samples (8.25%) were detected with chromosomal aberrations, which included 125 cases with trisomy 21, 31 with trisomy 18, 3 with trisomy 13, 4 with 45,X, 17 with 47,XXY, 5 with 47,XYY, 1 with 48,XXY,+18, 1 with 48,XXYY, 26 with structural chromosomal aberrations, and 11 with mosaicisms for aneuploidies.
CONCLUSION
In situ amniocyte culture is stable and has a high success rate, and is capable of identifying true and false mosaicisms, which can improve the accuracy of prenatal diagnosis.
6.Ultrasound-guided intra-articular injection of hypertonic glucose for treating knee osteoarthritis in plateau area

Objective To observe the value of ultrasound-guided intra-articular injection of hypertonic glucose(HG)for treating knee osteoarthritis in plateau area.Methods Ninety-one patients with knee osteoarthritis in plateau area(124 affected knee joints)were enrolled.The knee joints were divided into 20%HG group(n=67),25%HG group(n=42)or sodium hyaluronate group(n=15)based on the medication.Clinical and ultrasound scores were compared before and after treatment,and the efficacy of injection of HG was evaluated.Results At the 12th and 48th weeks after treatment,visual analog scale(VAS)scores decreased in all 3 groups(all P<0.05),and Lysholm scores of 20%HG group and 25%HG group increased compared to those before treatment(all P<0.05).The difference of Lysholm score before and in the 12th week after treatment,and of VAS score before and in the 48th week after treatment of 25%HG group were higher than those of 20%HG group(both P<0.05).The joint exudation score of 20%HG group decreased in the 48th week after treatment compared to that before treatment(P<0.05),and the synovial blood flow score decreased in the 12th and 48th weeks after treatment compared to those before treatment(both P<0.05).The joint exudation score of 25%HG group decreased in the 48th week after treatment compared to that before treatment(P<0.05),and the synovial hyperplasia and synovial blood flow score decreased in the 12th and 48th weeks after treatment compared to before treatment(all P<0.05).The joint exudation score of sodium hyaluronate group decreased in the 48th week after treatment compared to that before treatment(P<0.05),and the synovial hyperplasia score decreased in the 12th and 48th weeks after treatment compared to that before treatment(both P<0.05).Conclusion Ultrasound-guided intra-articular injection of HG,esp.25%HG,had certain value for treating knee osteoarthritis in plateau area.
7.Surgical plan selection and efficacy analysis in 32 cases of laryngotracheal stenosis

Objective:To explore the optimization of surgical procedures for laryngotracheal stenosis and its effect analysis.Methods:The data of 32 patients with acquired laryngotracheal stenosis who received surgical treatment from October 2015 to December 2021 were analyzed retrospectively. The age ranged from 19 to 72 years, with an average of (34.0±9.0) years. The medical history ranged from 1 to 32 months (median 3 months). As for etiology, there were 30 cases of iatrogenic laryngotracheal stenosis, including 20 cases of tracheal intubation and 10 cases of tracheotomy (7 cases of percutaneous tracheotomy and 3 cases of traditional tracheotomy). There were 1 case of laryngotracheal trauma and 1 case of airway Penicillium marneffei infection. According to Myer-Cotton grading system, grade Ⅳ stenosis was found in 14 cases, including 12 cases involving trachea and 2 cases involving trachea and subglottic area.There were 18 cases of grade Ⅲ, all of which involved the cervical trachea 5 cases failed in operation in other hospitals. According to stenosis grading, course of disease, primary disease control and the patient′s general condition, the surgical plan was determined individually. The operations of end-to-end anastomosis, circumferential tracheal partial resection, T-tube placement and CO 2 laser tracheal scar resection were performed respectively. The recovery of airway function and perioperative complications were observed one year after operation. Results:End-to-end anastomosis was performed in 16 cases, and partial circumferential tracheal resection in 2 cases, and tracheal granulation (scar) resection by CO 2 laser in 2 cases and T-tube insertion in 12 cases. Eighteen cases which performed end-to-end anastomosis, partial resection of circumferential trachea in and 2 cases which performed laser tracheal scar resection were all recovered airway function at one stage. After 1 year, 19 cases were cured and 1 case was effective. Of 12 patients with T tube implantation, 11 cases were successfully extubated after 6-12 months, 7 cases were cured after 1 year, 2 cases were effective and 3 cases were ineffective. Among the 3 cases of failure, 2 cases were successfully extubated by sleeve resection and end-to-end anastomosis in the second stage, and the other case refused to accept other treatment methods and the T-tube was placed again, and the tube was blocked and the patient survived. During the follow-up period, the total cure rate was 87.5%, the effective rate was 9.4%, and the total extubation rate was 96.9%.The most common complication was subcutaneous emphysema, accounting for 78% (25/32), but no serious mediastinal emphysema or pneumothorax occurred. In the T-tube implantation group, granulation tissue grew in different degrees around the neck wound after operation, and improved or disappeared after 6-9 months. Anterior cervical tracheal fistula occurred in 4 cases of T-tube implantation group after extubation, which were cured by sealing the stoma. There were no complications such as severe bleeding or perioperative death. Conclusion:When there were various factors, the optimization of the surgical plan according to the degree of stenosis, the course of disease, the control of primary disease and the general condition was an important guarantee to improve the curative effect of laryngotracheal stenosis.
8.Piperacillin and Its Compound Preparation-induced Anaphylactic Shock :A Case Report and Literature Analysis

OBJECTIVE:To i nvestigate the clini cal ch aracteristics of anaphylactic shock induced by piperacillin and its compound preparation ,and to provide reference for prevention and treatment of the severe ADR. METHODS :A case of anaphylactic shock induced by piperacillin and sulbactam were analyzed in our hospital ,meanwhile ADR literatures about piperacillin alone and its compound preparation-induced anaphylactic shock were collected from Medline ,CNKI,Wanfang database and VIP during the inception to Jul. 2020. Gender and age of patients ,allergic history ,primary disease and treatment ,skin test , administration route and dosage of piperacillin and its compound preparation ,occurrence time and main manifestations of anaphylactic shock ,treatment measure and prognosis were analyzed ,then prevention and treatment suggestions were put forward. RESULTS:The patient in this case was transferred to the ICU after partial hepatectomy. The use of piperacillin and sulbactam to prevent postoperative infection caused anaphylactic shock. A total of 28 literatures about anaphylactic shock induced by piperacillin and its compound preparations were collected from the database (involving 28 patients). Among totally 29 patients,there were 12 male and 17 female;the majority of patients were 50-59 years old (6 cases,20.69%). Three patients had allergic history (food, latex gloves ,etc.),and most of the primary diseases were infectious diseases or the drug used in perioperative period. Skin tests were carried out in 22 patients(75.86%)before medication and the results were negative. The possible allergenic drugs of 27 cases which were administered by intravenous route included piperacillin ,piperacillin sulbactam and piperacillin tazobactam. The dosage was different according to the primary disease and severity. 14 patients(55.56%)developed anaphylactic shock within 5 minutes after drug exposure. The main symptoms were systemic allergic reaction ,mainly involving the circulatory system. Except for two death cases ,the other patients ’symptoms were relieved after treatment. CONCLUSIONS :Allergic history and skin test results may have limitation in predicting anaphylactic shock induced by piperacillin and its compound preparation. Close monitoring needs to be taken in patients during these medications. Rescue therapy should be prepared in advance and countermeasures need to be carried out promptly in case of anaphylactic shock.

10.A case report of primary extubation by partial cricotracheal resection for severe subglottic stenosis.

This patient suffered from severe subglottic stenosis(grade Ⅳb). During partial cricotracheal resection, we cut through the cricothyroid membrane and the cricoid arch along the line from the lower edge of the thyroid cartilage to 5 mm of the inferior thyroid cartilage corner anteromedially. This can protect the cricothyroid joint, effectively protect the recurrent laryngeal nerve, and also support the airway. Strictly adhere to airway separation, avoid excessive separation of scars, and combine with reasonable postoperative management to achieve a safe extubation.
