1.The expression and clinical significance of p130Cas and c-erbB-2 in human breast cardnoma

Objective To explore the expression of p130Cas and c-erbB-2 in human breast carcinoma and the relationship between p130Cas and c-erbB-2 levels with clinical and pathological characteristics. Methods Immunohistochemistry SP staining was applied to detect the expression of p130Cas and c-erbB-2 in tumor tissues from 53 cases of human primary breast carcinoma,10 cases of breast fibroadenoma and in 10 cases of normal breast tissues. Results Breast carcinoma tissues showed higher levels of p130Cas and c-erbB-2 than normal and fibroadenoma tissues(both P<0.01).The expression of p130Cas was related with age, menopausal status, ER, PR status and histological grades, but not related with tumor size, lymph node status and pathological stages. The expression of c-erbB-2 was related with ER status, histological grades and lymph node status, but not related with age, menopausal status, PR status, tumor size and pathological stages. There were no significant correlations between p130Cas and c-erbB-2. Conclusion p130Cas and c-erbB-2 have relations with the malignant transformation and differentiation of breast carcinoma.c-erbB-2 is associated with metastasis of breast carcinoma.p130Cas and c-erbB-2 can be served as useful indicators in the prognosis of breast carcinoma.
2.Diagnostic value of 18F-FDG PET/CT delayed imaging after diuresis in patients with primary bladder tumor

Objective To explore the diagnostic value of 18F-FDG PET/CT delayed imaging after induced diuresis for primary bladder tumor.Methods Fourteen patients (12 males,2 females; age range 35-88 years) with pathologically confirmed primary bladder tumor (clinical stage T1.3N0M0) were retrospectively analyzed.All patients underwent standard 18F-FDG PET/CT followed by a delayed (2.5-3.0 h later)pelvic imaging post intravenous injection of 20 mg furosemide and oral intake of 600 ml water.A positivelesion was defined as the uptake of 18F-FDG greater than the urine radioactivity and negative as equaled to or less than the urine radioactivity.Diagnostic efficacy of 18F-FDG PET/CT was calculated with pathology as the gold standard.Mann-Whitney u test was used to analyze data.Results There were 12 malignant (11 urothelium carcinomas and 1 squamous cell carcinoma) and 2 benign tumors (papillary epitheliomas).On standard PET/CT imaging,3 of 12 malignant lesions showed increased metabolism of 18F-FDG and the other 9 were false negative; while the 2 benign tumors showed no abnormal 18F-FDG uptake.The sensitivity,specificity,and accuracy of 18F-FDG PET/CT were 3/12,2/2 and 5/14,respectively.Sizes of the 3 18F-FDG-avid malignant lesions were greater than those of other 9 18F-FDG-negative lesions (26,30,35 mm vs (15.6±6.3) mm; Z=-2.315,P<0.05).The sensitivity,specificity,and accuracy of delayed pelvic imaging were 11/12,2/2,13/14,respectively.Conclusion 18F-FDG PET/CT delayed imaging after diuresis has a high diagnostic efficacy for primary bladder tumor.
3.Clinical value of delayed 18 F-FDG PET/CT imaging after diuresis in male patients with locally re-current rectal cancer

Objective To explore the diagnostic value of delayed 18 F?FDG PET/CT pelvic imaging after forced diuresis in detecting male patients with locally recurrent rectal cancer. Methods Thirty?two male patients ( 32-79 years) with clinically suspicious recurrences of rectal cancer were retrospectively ana?lyzed. All patients underwent a standard 18 F?FDG PET/CT followed by a delayed pelvic imaging after 2.5 h of administration of 20 mg furosemide intravenously and extra water intake of 500 ml at least. Final diagnosis was made by pathology or clinical follow?up (>10 months ) . Diagnostic efficiency of 18 F?FDG PET/CT standard and delayed pelvic images were calculated and compared. χ2 test was used for data analysis. Re?sults A total of 25 patients were diagnosed with recurrent rectal cancer, including 10 cases with invasion of surrounding tissue, and 5 cases with metastases. Among the 7 patients without local recurrences, 2 had dis?tant metastases. The sensitivity, specificity, and accuracy of standard FDG PET/CT were 92.0%(23/25), 3/7, 81.2%(26/32), respectively. The corresponding values of the delayed imaging were 96.0%(24/25), 6/7, and 93.8%(30/32), respectively. The specificity of delayed imaging was higher than that of standard FDG PET/CT (χ2=14.333, P<0.05) , but no significant differences were found in sensitivity and accuracy between the delalyed imaging and standard FDG PET/CT (χ2=0.085 and 0.966, both P>0.05) . The urina?ry bladder activity reached or approached the background during the delayed imaging. Twenty?two malignant lesions of surrounding invasion were confirmed. The positive rate of delayed imaging was higher than that of standard FDG PET/CT:95.5%(21/22) vs 63.6%(14/22);χ2=6.400, P<0.05. Conclusions 18F?FDG PET/CT delayed pelvic imaging after diuresis and hydration is useful for eliminating the artifacts from urina?ry bladder activity, and for detecting the invasion to the rectal surrounding tissue more effectively. It could be a supplement to standard FDG PET/CT.
4.Mechanisms of plasmid-mediated quinolone resistance in Proteus mirabilis from urine

Objective To investigate the prevalence of plasmid-mediated quinolone resistance(PMQR) in Proteus mirabilis clinical isolated from urine. Methods Antimicrobial susceptibility test was performed using the microdilution method.And PMQR gene qnrA,qnrB,qnrC,qnrD,qnrS,aac(6′)-Ib-cr,qepA were amplified by PCR,then the PCR positive products were sequenced to identify their genotypes. Results In 41 strains of Proteus mirabilis from urine,the resistant rates to ciprofloxacin and levofloxacin were 41.5% and 29.3%,respec-tively.Among all clinical isolates,qnrA1 gene was detected in 2 strains,qnrB2 gene in 3 strains.PMQR gene qn-rB,qnrC,qnrD,qnrS,aac(6')-Ib-cr and qepA were not detected in all strains.Conclusions Clinical isolates of Proteus mirabilis from urine carry PMQR genes.The prevalent principal genotypes are qnrA1 and qnrB2 in these iso-lates.They are related to low levels resistance toquinolone.
5.Risk factors analysis and nursing strategies of coronary heart disease among young people

Objective To explore the risk factors of young patients with coronary heart disease ( CHD) , and the differences between the young patients and the elderly patients to provide a reference for the prevention of early CHD. Methods We retrospectively analyzed 437 cases of patients who came our hospital and had coronary artery bypass surgery from May 2009 to May 2012. The age of young people was less than 45 years wold patients (51 cases), and elderly patients (age greater than or equal to 65 years of age) were 386 cases. We compared and analyzed the indicators of the family history, lipid profile, blood glucose, blood pressure, heart function, postoperative infections, liver and kidney function, postoperative complications, between two groups of patients. Results Compared with the older patients, there were differences on the positive family history, smoking, obesity, dyslipidemia, cardiac function, angina classification, blood sugar, complications in young group (P<0. 05). Acute myocardial infarction was 33. 3% in young people, but it was 18. 1% in elderly patients (χ2 =2. 15,P<0. 05), old myocardial infarction was 7. 8% in young people, but it was 20. 7% in elderly patients (χ2 =2. 16,P<0. 05). Conclusions The major risk factors for CHD of young men are family history, smoking, being overweight, low HDL, high blood pressure, and the young patients would recovery faster than older patients after surgery. Early surgical treatment could prevent the occurrence of CHD and delay the progress.
6.Piperacillin and Its Compound Preparation-induced Anaphylactic Shock :A Case Report and Literature Analysis

OBJECTIVE:To i nvestigate the clini cal ch aracteristics of anaphylactic shock induced by piperacillin and its compound preparation ,and to provide reference for prevention and treatment of the severe ADR. METHODS :A case of anaphylactic shock induced by piperacillin and sulbactam were analyzed in our hospital ,meanwhile ADR literatures about piperacillin alone and its compound preparation-induced anaphylactic shock were collected from Medline ,CNKI,Wanfang database and VIP during the inception to Jul. 2020. Gender and age of patients ,allergic history ,primary disease and treatment ,skin test , administration route and dosage of piperacillin and its compound preparation ,occurrence time and main manifestations of anaphylactic shock ,treatment measure and prognosis were analyzed ,then prevention and treatment suggestions were put forward. RESULTS:The patient in this case was transferred to the ICU after partial hepatectomy. The use of piperacillin and sulbactam to prevent postoperative infection caused anaphylactic shock. A total of 28 literatures about anaphylactic shock induced by piperacillin and its compound preparations were collected from the database (involving 28 patients). Among totally 29 patients,there were 12 male and 17 female;the majority of patients were 50-59 years old (6 cases,20.69%). Three patients had allergic history (food, latex gloves ,etc.),and most of the primary diseases were infectious diseases or the drug used in perioperative period. Skin tests were carried out in 22 patients(75.86%)before medication and the results were negative. The possible allergenic drugs of 27 cases which were administered by intravenous route included piperacillin ,piperacillin sulbactam and piperacillin tazobactam. The dosage was different according to the primary disease and severity. 14 patients(55.56%)developed anaphylactic shock within 5 minutes after drug exposure. The main symptoms were systemic allergic reaction ,mainly involving the circulatory system. Except for two death cases ,the other patients ’symptoms were relieved after treatment. CONCLUSIONS :Allergic history and skin test results may have limitation in predicting anaphylactic shock induced by piperacillin and its compound preparation. Close monitoring needs to be taken in patients during these medications. Rescue therapy should be prepared in advance and countermeasures need to be carried out promptly in case of anaphylactic shock.

Immunization with regulatory T cell (Treg) epitope peptides to activate and induce Tregs, by which to suppress pathological autoimmune responses and reconstitute a new homeostasis, is a promising therapeutic regimen for autoimmune rheumatic diseases. However, it is usually hard to induce potent peptide-specific immune responses
