Objective; To investigate the immunolocalization and significance of Notch-2 expression in the process of dental pulp repair after injury. Methods: An experimental animal model of injury-induced pulpitis was established to observe the time-sequenced alteration of the expression of Notch-2. Results: Three days post-operation, weak positive staining of Notch-2 was observed in pulp mesenchyme cells and pulp fibroblasts but not in vascular endothelial cells or odontoblasts. Five days post-operation, strong Notch-2 reactivity was found in subodontoblasts as well as newly bom capillary endothelial cells. Seven days after cavity preparation, Notch-2 staining became weaker in pulp mesenchyme cells and capillary endothelial cells, but stronger positive staining was found in odontoblasts. Two weeks post-operation, weak Notch-2 staining was seen in pulp mesenchyme cells and subodontoblastic layer cells and was absent from odontoblasts. Notch-2 immunoreactivity was completely absent in intact rat dental pulp. Conclusion: Notch-2 is strikingly up-regulated in dental pulp mesenchyme cells in the early days after dental injury and then shows progressively up-regulation in odontoblasts. Properly regulated activation of Notch signaling pathway is important for controlling cell fate and maintaining the correct balance among cell proliferation, differentiation and apoptosis during dental pulp repair process.

Objective To detect mechanism of action mode of Jiuxieling Granules in spleen and kidney yang deficiency ulcerative colitis. Methods The perfusion of Rhei Radix et Rhizoma Decoction plus intramuscular injection of hydrocortisone and combined with TNBS and ethanol enema were employed to establish UC animal model. Ninety rats were randomly divided into blank group, model group, SASP group and Jiuxieling Granules 7 days, 14 days and 21 days groups. All treatment groups received relevant medicine intervention. The levels of IL-1, IL-6, TNF-α, and IFN-γin serum and colon tissue were detected by ELISA. Results Compared with the blank group, the levels of IL-1, IL-6, TNF-α, and IFN-γ in serum and colon tissues of rats in model group increased (P<0.05);compared with the model group, the levels of IL-1, IL-6, TNF-α, and IFN-γin serum and colon tissues of rats in treatment groups were reduced greatly (P<0.05), among which Jiuxieling Granules 21 days group showed the most obvious effects (P<0.05). Conclusion Jiuxieling Granules can regulate the normal secretion of the levels of IL-1, IL-6, TNF-α, and IFN-γin serum and colon tissue of model rats, and inhibit inflammation and protect colonic mucosa.

Objective To evaluate the risks of malnutrition and nutritional status of patients with esophageal cancer during radiotherapy , so as to explore nursing countermeasures to improve their nutritional status. Methods 60 inpatients with esophageal cancer receiving routine radiotherapy from June 2012 to April 2013 were investigated with anthropometric indicators and laboratory data for weekly nutritional evaluation on admission, during radiotherapy, and after radiotherapy.Meanwhile, the patients were conducted with Nutritional Risk Screening ( NRS2002 ) per week .Results NRS2002 score showed that the malnutrition risk values of patients with esophageal cancer gradually increased after radiotherapy .The score was (2.06 ±1.22) after 2 weeks of radiotherapy, suggesting that the difference was statistically significant (t=-2.457, P<0.05). After the radiotherapy , the scores of body mass index ( BMI ) , triceps skinfold ( TSF ) , arm circumference (AC), total protein (TP), albumin (ALB) and total lymphocyte count (TLC) were (20.21 ±2.69)kg/m2, (7.27 ±3.20)mm, (24.71 ±2.68)cm, (65.50 ±5.25)g/L, (38.48 ±5.00)g/L and (6.55 ±3.42) × 109/L, respectively, compared with those of admission , they decreased gradually in the subsequent weekly examinations and had statistical significance ( t=3.204,2.777, 3.053,7.174,5.685,12.508, respectively;P<0.05).After 2 weeks of radiotherapy, the incidence of malnutrition increased compared to that of admission, and the scores of TSF, TP and TLC were 95.00%, 11.67% and 98.33% respectively, which showed the statistically significant difference (χ2 =11.800, 7.792, 73.024, respectively; P <0.05 ). Conclusions Patients with esophageal cancer gradually have higher risk of malnutrition and worse nutritional status during radiotherapy and their incidence of malnutrition increased after 2 weeks of radiotherapy .

BACKGROUND:The abnormal gait of stroke patients seriously affects their propulsive force during walking, which subsequently reduces their walking speed, walking distance, and stability, increases their risk of falls, and seriously affects their quality of life. OBJECTIVE:To review the relevant research on propulsive force deficits in stroke patients with hemiplegia,to summarize the understanding of existing researchers on propulsive force deficits,to analyze the relationship between propulsive force and gait, and finally to explain and compare the latest rehabilitation technologies used to improve propulsive force deficits, providing reference for clinical treatment. METHODS:Relevant literature was retrieved from WanFang,CNKI,PubMed,and Web of Science Core Collection through computer search. The Chinese and English search terms were"propulsive force OR propulsive, stroke OR cerebral infarction OR hemiplegia, walk* OR gait."The search time limit was from 2003 to 2023,and 71 articles were finally included for review and analysis. RESULTS AND CONCLUSION:Training targeting the hip and ankle joints may be more effective for patients'walking function, especially training with the application of flexible exoskeleton robots, but more sufficient evidence is still needed to use propulsion as a prognostic indicator of walking function in stroke patients.Biomechanical variables related to propulsive force include:the hip joint extension angle at terminal stance, ankle joint dorsiflexion torque, and knee joint extension.Damage to the corticospinal tract,cerebellar-cortical pathways, and the reticulospinal tract in hemiplegic patients are associated with reduced propulsive force and gait asymmetry.Propulsive force is crucial for the stability of healthy gait, and a decrease in propulsive force is unfavorable for gait stability.Gait symmetry is correlated with propulsive force, stride length symmetry, trunk displacement, and lower limb swing ability, with propulsive force being a key factor.Propulsive force can serve as a quantitative indicator for assessing the gait of hemiplegic patients, and evaluation of gait using propulsive force is beneficial for the long-term development of walking ability.Main rehabilitation techniques for improving propulsive force include:lower limb exoskeleton robot walking training, treadmill training combined with functional electrical stimulation, adaptive speed treadmill training, biofeedback technology, and whole-body vibration training.Among them,whole-body vibration training and biofeedback technology are more effective. The specific contributions and mechanisms of the hip, knee, and ankle joints in improving propulsive force are still controversial, but it is expected that the contributions of the hip and ankle joints are greater.Focusing on the improvement of propulsive force as a rehabilitation goal may yield more sustainable advancements in walking function. However, several current challenges persist in this field:understanding the neurobiological basis of propulsive force deficits in stroke patients, assessing the long-term efficacy of current rehabilitation techniques for enhancing propulsive force, and determining the most suitable patient populations for the application of major rehabilitation techniques aiming at improving propulsive force. These areas require further exploration by subsequent researchers.
