1.Research progress of octamer-binding transcription factor 4 and sal-like gene 4 in germ cell tumors

Tumor marker research is a hot field of oncology.Previous studies of tumor markers such as placental alkaline phosphatase,alpha-fetoprotein,c-kit and CD30 and so on,lack sufficient sensitivity and specificity for the early diagnosis of germ cell tumor.Recent research demonstrated that octamer-binding transcription factor 4 (OCT4),sal-like gene 4 (SALL4) can be used as new germ cell tumor markers.This paper reviewed the research progress on the OCT4 and SALL4 in recent years,to provide the reference about the early diagnosis,tumor typing,condition estimation,prognosis and targeted therapy of germ cell tumors.
2.Characteristics of secondary osteoporosis in stroke patients with hemiplegia

BACKGROUND: Hemiplegia caused by stroke or brain injury may lead to secondary osteoporosis. Under the circumstance, bone mineral density (BMD) and Bone Mineral Content (BMC) are two effective factors for predicting the risk of osteoporosis-related fracture OBJECTIVE: To investigate the characteristics of secondary osteoporosis in stroke patients with hemiplegia by measuring and analyzing BMD and BMC.DESIGN: A case study.PARTICIPANTS: From March 1999 to May 2002, 156 hemiplegia patients at the Neurorehabilitation Department of Peking Boai Hospital were selected, 87 males and 69 females, with an age ranging from 17 to 65 years old, in average of (47.0±5.3) years old and having a course of diseases ranging (5.0±3.5) months. The types of their illness: 89 had cerebral infarction, 67 had cerebral hemorrhage. Hemiplegia sides: 79 patients had hemiplegia in the left and 77 in the right.METHODS: Using LUNAR Dual Energy X-Ray densitometers, all the patients underwent a fro-top-to-toe scan to assess the BMD and BMC status.MAIN OUTCOME MEASURES: ① The occurrence rate of osteoporosis in patients with various course of hemiplegia. ② The BMC in patients with hemiplegia in various part.RESULTS: All the 156 patients entered the statistical analysis procedure.① The occurrence rate of osteoporosis was highest in patients with 3 to 6 months of hemiplegia (44%, 16/36) and was lowest in those with 9 to 12 months of disease (21%,7/33). The total occurrence rate of osteoporosis was 31% (48/156). ② For those with a less-than-3-month course of hemiplegia: Of all the 47 patients, 9 had dystonic hemiplegia, all suffering from osteoporosis and 2 of whom having fracture. For those with a 3-to-6-month course of hemiplegia: Of all the 36 patients, 3 had dystonic hemiplegia and osteporosis as well. ③ BMC in the affect side of upper limb was signifi cantly lower than that in healthy side [(154.76±43.91) g, (172.59±43.78) g,t=3.591, P< 0.001]. BMC in the affect side of lower extremity was close to that in healthy side [(463.41±30928) g, (464.11±86.45) g, t=0.027, P> 0.05].CONCLUSION: In patients with hemiplegia, changes in bone mass fluctuate with the length of hemiplegia. BMC in the affect side of upper limb is low while that in the healthy side is nearly normal. It may be explained that upper extremities start the functional exercises at an early time. This indicated that bone mass has a close relationship with excercises.
3.Accuracy comparison of enhanced dynamic wedge modles among Pinnacle3 9.0 ACA and Eclipse7.3 AAA, PBC algorithm

ObjectiveTo compare the accuracy of enhanced dynamic wedge (EDW) models of adaptive convolution algorithm (ACA) in Pinnacle3 9.0 and anisotropic analytical algorithm (AAA),and pencil beam convolution (PBC) algorithms in Eclipse7.3 treatment planning systems (TPS).MethodsTo evaluate the accuracy of the three algorithm models,we compared actual measurement values with TPS calculation values of EDW wedge factors under for different fields in which Varian-21EX 6 MV X-ray was applied,and also compared the actual dose distribution profile with that of TPS.ResultsThe deviations of EDW wedge factors of symmetry fields and asymmetric fields are within 2.8% and 19.4% for ACA in Pinnacle3 9.0.Meanwhile,the deviations are 1.0% and 2.0% for AAA,1.2% and 3.0% for PBC in Eclipse7.3.The deviations between measurement and calculation of all fields profile for ACA is within 3% and within 2.7% for AAA within 4.0% for PBC in wedge direction.For the dose distributions,we evaluated the pass rates of three algorithms using gamma analysis.The gamma pass rates among all the three algorithms in symmetry and asymmetric fields are above 87% and 85% respectively.After the removal of the penumbra zone,the pass rates among all the three algorithms are above 96% in symmetry fields,and above 95% in asymmetric fields,respectively.Conclusions AAA and PBC algorithms in symmetric and asymmetric fields can meet the need of clinical applications.While,wedge factor of ACA should not be used in clinical due to its greater error in asymmetric fields.
4.Clinical research on feeding effect of semi-demand feeding and abdominal massage in preterm infants

Objective To evaluate the feeding effect of semi-demand feeding and abdominal massage on the feeding process, feeding performances and clinical safety (including the time of returning to birth weight, feeding tolerance, apnea). Methods A total of 120 preterm infants that met inclusion criteria of the study were randomly assigned to three groups, and each group had 40 cases. Control group:received standard feeding care. Semi-demand feeding group: received 5-10 minutes of oral motor intervention every time before feeding(including oral stimulation and non nutritional sucking), then decided feeding methods according to its consciousness behavior state. Semi-demand feeding combined abdominal massage group: received both semi-demand feeding and abdominal massage before feeding. Observed and recorded the three groups′ transition time, whether there were statistically significant differences in feeding performance and clinical safety. Results The feeding transition time was (11.55± 4.69), (10.98±3.85), (8.50±3.87)d in control group, semi-demand feeding group and semi-demand feeding combined abdominal massage group, and there were significant differences (F=6.088, P=0.003). In terms of feeding performance, the feeding efficiency were (7.27 ± 2.65), (9.74 ± 2.63), (9.52 ± 2.13) ml/min, the feeding proficiency were (66.31 ± 14.51)%, (76.64 ± 12.46)%, (82.26 ± 10.65)%, the volume transfer were (63.99 ± 11.02)%, (80.16 ± 14.41)%, (84.97 ± 11.16)% in control group, semi-demand feeding group and semi-demand feeding combined abdominal massage group, and there were significant differences (F=12.189, 16.383, 31.959, all P < 0.01).Among the clinical safety, the time of restoration of birth weight were (10.55 ± 1.99), (9.28 ± 1.77), (10.05 ± 2.46) d in control group, semi-demand feeding group and semi-demand feeding combined abdominal massage group, and there were significant differences(F=3.759, P=0.026). The incidence of feeding intolerance were 50.0%(20/40), 40.0%(16/40), 22.5%(9/40), and there were significant differences(χ2=6.613, P=0.037). The incidence of apnea were 37.5%(15/40), 20.0%(8/40), 15.0%(6/40), and there were significant differences(χ2=6.093, P=0.048). Conclusions The combined intervention methods could accelerate the feeding process, improve feeding performance as well as reduce the incidence of feeding intolerance and apnea. Hence, semi-demand feeding combined abdominal massage is a recommended choice for clinical use.
5.The relation between endothelium-dependent vasodilator function and platelet L-arginine/nitric oxide pathway in patients with percutaneous coronary intervention

Objective To investigate alteration and cross link of the vascular endothelium dependent diastolic function (FMD) and platelet endogenous L-arginine/NOS/NO pathway before and after percutaneous coronary intervention (PCI) .Methods The study included two groups :normal control group(17 cases) and PCI group(20 cases) .FMD of brachial artery and NO production , NOS activity ,L-arginine transport in platelets were examined respectively before ,immediately ,24 h and 72 h after PCI and the rela-tionships between them would subsequently be analysed .Results The preoperative FMD ,content of NO2 - ,NOS activity and L-ar-ginine of PCI group significantly decreased than the normal control group (P<0 .01) ,and further decreased in the immediate post-operative(P<0 .01) .Gradually recovered 24 h after surgery ,72 h after surgery ,the FMD ,NO2 - ,L-arginine transport ,NOS activity restored to the preoperative level (P>0 .05) .The analysis showed the obvious positive correlation between FMD and content of NO2 - or NOS activity ,or L-arginine transport of platelet ,respectively(P<0 .01) .Conclusion The pathway of platelet L-arginine/NOS/NO was significantly positively correlated with FMD and the detection of platelet L-arginine/NOS/NO can reflect the changes of vascular endothelial function with PCI .
6.The clinical characteristics of the secondary osteoporosis in hemiplegic patients

Objective To characterize the secondary osteoporosis in hemiplegic patients. Methods By use of dual energy X ray absorptometry, the bone density and bone mineral quantity of 156 hemiplegic inpatients were tested and analyzed. Results The incidence of osteoporosis was 44.4% during 3~6 months after stroke and 21.1% found during 9~12 months after stroke. The average incidence was 30.8%. The osteoporosis was more common in patients with flaccid hemiplegia than in those with spastic hemiplegia. The bone mineral quantity in the affected upper limbs was significantly lower than that in nonaffected side ( P 0.05). Conclusion The bone mineral quantity of hemiplegic patients varies with the duration of disease. The reduction of bone mineral quantity is regular and reversible.
7.Posology study on 3DCRT and IMRT for lung cancer with mediastinum lymphonode metastasis

Objective To provide a reference to the clinical application by comparing the posology difference of 3DCRT(three-dimensional conformal radiation therapy) and IMRT(intensity modulated radiation therapy) for lung cancer with mediastinum lymphonode metastasis. Methods From Aug.2007 to Feb.2008,11 patients of lung cancer with mediastinum lymphonode metastasis were treated by IMRT. IMRT plan and 3DCRT plan were designed by CT-simulator image. The conformity index, PTV uniformity and the organs at risk were compared by the target isodose curve and dose volume histogram(DVH) in two plans. The prescribe dose was 66 Gy/33 fracinations in 6 weeks, and 95 % dose curve covered 95 % target volume. The data were analyzed by t-test in SPSS 14.0. Results The conformity index in IMRT was superior than 3DCRT (P<0.05).PTV above 110 % volume in IMRT was reduced than 3DCRT. On the protection of lung tissue,the volumes of V20,V30,V40 in IMRT were obviously less than 3DCRT (P<0.05). Conclusion IMRT is a better selection to larger volume and irregular shape of CTV for lung cancer with mediastinum lymphonode metastasis.
8.A dosimetric comparison of volumetric modulated Arc therapy with conventional intensity-modulated radiotherapy for breast cancer radiotherapy after breast-conserving surgery

Objective To compare the dosimetric differences between two plans of RapidArc and 5F-IMRT for breast cancer radiotherapy after breast-conserving surgery.Methods Eight female patients with left-sided breast cancer after breast-conserving surgery were selected.A dose of 50 Gy in 25 fractions was prescribed for plans of RapidArc and 5F-IMRT.Target conformity index (CI),homogeneity index (HI),target coverage,exposure dose volume delivered to organ at risk were compared between two plans.At the same time,treatment delivery time and monitor units (MU) were also compared.Results The target conformity index (CI) in RapidArc plan (·0.88 ±0.03) was higher than that in 5F-IMRT plan (0.79 ±0.02,t =8.28,P < 0.05).The homogeneity index (HI) in RapidArc plan 9.01 ± 0.73 was significantly lower than that in 5F-IMRT plan10.44 ± 1.08 (t =-2.73,P <0.05).For the dose volume delivered to the ipsilateral lung in two plans,the values of V10,V20,V30,and Dmean in RapidArc plan were lower than those in 5F-IMRT plan(t =-7.53,-7.20,-8.39,-7.80,P < 0.05).However,the value of V5 in RapidArc plan was higher than that in 5F-IMRT plan (t =5.67,P <0.05).For the heart,the values of V5,V10 and Dmean in RapidArc plan were higher than those in IMRT plan(t =10.46,28.76,5.40,P < 0.05),while the value of V30 in RapidArc plan was lower than that in 5F-IMRT plan(t =-6.12,P <0.05).The values of V5 in contralateral lung and breast were higher in RapidArc plan than those in 5F-IMRT plan(lung:t =21.50,P <0.05;breast:t =5.44,P <0.05).The MU in RapidArc plan was decreased by 25%,and the average treatment delivery time was saved by 60%,compared with that of 5F-IMRT plan.Conclusions During breast cancer radiotherapy after breast-conserving surgery,compared with 5F-IMRT plan,the RapidArc plan could improve the target HI,and reduce both the irradiated dose in high-dose volume and MU,and shorten the treatment time,but increased the exposed volume in low-dose volume of normal tissues.
9.Therapeutic effect of adipose-derived mesenchymal stem cells on radiation enteritis

Objective To evaluate the therapeutic effect of adipose-derived mesenchymal stem cells on radiation enteritis.Methods A total of 52 male Sprague-Dawley rats were used in the present study.Herein,46 rats were randomly selected and irradiated with a dose of 15 Gy at their abdomens.Two hours post-irradiation,23 rats were randomly selected and infused intraperitoneally with adipose-derived mesenchymal stem cells in passage 6 from young-female donor.The other 23 rats were intraperitoneally infused with PBS.The rest 6 rats were set as normal control.During the first 10 days post-irradiation,peripheral blood-samples from irradiated rats were harvested for testing the levels of IL-10 in serum using ELISA assay.Additionally,after isolating the thymic cells and peripheral blood mononuclear cells,the percentages of CD4/CD25/Foxp(3)-positive regulatory T cells in thymus and peripheral blood were tested by flow-cytometry.Finally,infiltration of inflammatory cells and deposition of collagens within irradiated small intestine were analyzed by H&E staining and Masson Trichrome staining,respectively.Based on the MPO-immunohistochemistry staining,the type of infiltrated cells was identified.The Kaplan-Meier method was used for analyzing the survival rate of irradiated rats.Results During a period of 30 days post-irradiation,the irradiated rats receiving adipose-derived mesenchymal stem cells survived longer than those receiving PBS (t =4.53,P < 0.05).Compared to the irradiated rats with PBS-treatment,adipose-derived mesenchymal stem cells could elevate the level of IL-10 in serum (7 d:t =13.93,P < 0.05) and increase the percentages of CD4/CD25/Foxp(3)-positive regulatory T cells in both peripheral blood (3.5 d:t =7.72,7 d:t=11.11,10 d:t =6.99,P <0.05) and thymus (7 d:t =16.17,10 d:t =12.12,P< 0.05).Moreover,infiltration of inflammatory cells and deposition of collagens within irradiated small intestine were mitigated by adipose-derived mesenchymal stem cells.Conclusions Adipose-derived mesenchymal stem cells were capable of curing radiation enteritis.
10.Adipose-derived mesenchymal stem cell therapy for radiation-induced vascular injury in small intestine of rat

Objective To assess the therapeutic effect of human adipose-derived mesenchymal stem cells on radiation-induced vascular injury in the small intestine of rat. Methods A total of 34 male Sprague-Dawley rats were enrolled in this study. To establish a model of radiation-induced intestinal injury, each rat was irradiated with 15 Gy in whole abdomen. 17 rats were randomly selected and infused intraperitoneally with passage 6 ( P6 ) Ad-MSCs, and the other 17 rats that received PBS were set as control. 10 days post-irradiation, the number of CD31+ endothelial cells in the small intestine villus was measured by flow-cytometry, the expressions of CD31, CD105 and isolectin-B4 in the na?ve endothelial cells with detected by IHC-staining, and the vascular integrity was evaluated by measuring VE-Cadherin. The origination of na?ve endothelial cells within injured intestine was also analyzed. In addition, total mRNA were extracted from irradiated small intestine to assay the expressions of VEGF, bFGF, Flk-1 and SDF-1 using quantitative Real-time PCR. Results Compared to the control, the amount of CD31-postive endothelial cells within irradiated intestine was significantly increased after Ad-MSCs infusion ( t=12?15, P<0?05). The microvascular density in the injured sites was also significantly increased by the infusion of Ad-MSCs (20 d:t=10?33, P<0. 05;30 d:t=32?85, P<0?05). Moreover, the expressions of VEGF, bFGF, Flk-1 and SDF-1 were significantly up-regulated after delivery of Ad-MSCs ( VEGF:t =10?34, bFGF:t=11?25,Flk-1:t=6?73, SDF-1:t=6?73, all P<0?05), which was beneficial in maintaining the integrity of intra-villus blood-vessels as well as promoting neovascularization in the injured sites. Conclusion Ad-MSCs had potentials in healing radiation-induced vascular injury in rat small intestine.
