BACKGROUND: Transient ischemic attack is a risk factor in the causation of complete stroke. Early detection of transient ischemic attack by the examinations of imaging and ultrasonography together with timely interventions can prevent it from developing to complete stroke.OBJECTIVE: To compare the values of early transcranial Doppler ultrasonography and CT examination in the evaluation of early transient ischemic attack.DESIGN: A case analysis.SETTING: Department of Neurology, CT Room and Transcranial Doppler Ultrasonography Room in a municipal hospital.PARTICIPANTS: Thirty in- and out-patients with transient ischemic attack (22 males and 8 females), at the age of 26 to 85 with an average of 59.6 years old in the Yuanbaoshan District Hospital of Chifeng City between January 2001 and April 2003 were involved in the study.INTERVENTIONS: After admission, all the 30 patients with transient ischemic attack were given routine lose of medication for promoting blood circulation and invigorating pulse-beat, and heparin or low molecular hepacarin calcium; Cranial CT examination was performed for all selected patients, and the transcranial Doppler ultrasonic diagnostic system was also used to detect the related dynamic parameters of systolic and diastolic flow rate, mean flow velocity and pulse index of bilateral internal carotid artery and vertebral basilar artery. The occurrence of cerebral infarction was observed through follow up for 1 month and 1 year.MAIN OUTCOME MEASURES: ① Results of cranial CT examination;② Results of transcranial Doppler ultrasonography; ③ The occurrence of cerebral infarction in the 30 patients after follow up for 1 month and 1 year.RESULTS: All the 30 patients were followed up for 1 month and I year after discharge. ① Focal changes were detected with CT examination in 10 cases (34%, 10/34); ② Abnormal changes of hemorheology were detected with transcranial Doppler ultrasonography in 24 cases (80%, 24/30). ③ Among the 30 patients treated with heparin or low molecular hepacarin calcium, cerebral infarction occurred in 4 cases within 1 month, and 6 cases between 1 month and 1 year.CONCLUSION: For patients with transient ischemic attack, the hemorheological abnormality can be detected earlier with transcranial Doppler ultrasonography than with CT examination; treatment for anti-coagulation should be given as early as possible after the transient ischemic attack to reduce the reoccurrence rate and prevent the occurrence of complete stroke.

Objective To investigate the effect of ligustrazine combined with phentolamine in the treatment of pregnancy induced hypertension.Methods 90 patients with pregnancy induced hypertension were randomly divided into the observation group and the control group according to the random number table,45 cases in each group.The control group was treated with phentolamine and magnesium sulfate.The observation group was given Chuanqiongqin injection,phentolamine and magnesium sulfate for 10 days.The effects of the two groups were compared before and after treatment,and the changes of mean arterial pressure,24h urine protein and perinatal adverse reaction were compared.Results After treatment,the mean arterial pressure [(99.7 ± 36.2) mmHg vs.(108.4 ± 35.1) mmHg] and 24h urinary protein [(1.41 ± 0.45)g/L vs.(2.51 ± 0.82)g/L] were significantly decreased in the two groups.Compared with before treatment,the differences were statistically significant (t =3.534,3.765,5.646,6.543,all P ＜ 0.05).And the improvement of the observation group was significantly better than those of the control group,the differences were statistically significant (t =3.365,3.986,all P ＜ 0.05).The cure rate (71.1％ vs.44.4％) and total effective rate (95.6％ vs.82.2％) in the observation group were higher than those in the control group,the differences were statistically significant (x2 =6.559,4.050,all P ＜ 0.05).The rate of termination of pregnancy of the observation group was significantly lower than that of the control group(40.0％ vs.62.2％),the incidence rates of premature delivery (2.2％ vs.15.6％) and postpartum hemorrhage(6.7％ vs.22.2％) of the observation group were significantly lower than those of the control group,the proportion of neonatal asphyxia (4.4％ vs.17.8％) of the observation group was lower than that of control group,the differences were statistically significant (x2 =4.447,4.939,4.406,4.050,all P ＜ 0.05).Conclusion Ligustrazine combined with phentolamine in the treatment of pregnancy induced hypertension can significantly improve the average arterial pressure and 24h urinary protein,has good efficacy,less adverse reactions during perinatal period.

Objective To observe the effects of anti lipopolysaccharide (LPS) immunoglobulin Y (IgY) on LPS in vitro . Methods The direct in vitro neutralization of different concentrations of IgY and LPS was conducted for the detection of the remained amount of LPS to observe the dose effect relationship. The primary cultured human umbilical vein endothelial cells (HUVECs) were cultured for 48-72 h after addition of certain concentrations of fluorescein isothiocyanate LPS (FITC LPS) and IgY. Changes of the fluorescent intensity in HUVECs were observed at different time points. After addition of certain concentrations of LPS and IgY for incubation with HUVECs for 4, 8, and 12 h, the changes of lactate dehydrogenase (LDH) activity were measured. Results The dose effect relationship was satisfactory when the ratio was 16︰1. At 4, 8, and 12 h, fluorescent intensity was somewhat strong in FITC LPS group, but weak in IgY group, especially at 8 and 12 h. Objective To investigate the effect of angelica polysaccharide (APS) on bioactivity of bone marrow macrophage (BMM?) and its relationship to hematopoietic regulation, for clarifying the hematonic mechanism of Angelica sinensis. Methods The techniques of hematopoietic progenitor cell culture and BMM? culture in vitro, biological assay of hematopoietic growth factor (HGF) in culture media of BMM?, immunocytochemistry, and nucleic acid in situ hybridization were used. Results The culture supernatant of BMM? induced by APS can enhance the CFU-Mix, CFU-E, CFU-GM; the expression of erythropoietin (EPO), GM-CSF, IL-3, IL-6 protein in BMM? induced by APS was much stronger than that in the control group at different levels; the expression of EPO mRNA and GM-CSF mRNA in BMM? induced by APS were intensified. Conclusion APS may directly and/or indirectly stimulate the BMM? in hematopoietic inductive microenvironment to accelerate the synthesis and secretion of hematopoietic regulation factors on the basis of gene and protein level, such as EPO, GM-CSF, IL-3, IL-6, which in turn to promote the proliferation and differentiation of CFU-Mix, CFU-E, and CFU-GM.

OBJECTIVE:To observe the efficacy and safety of ornidazole combined with Jinzhi jieyin gargle in the treatment of acute wisdom tooth pericoronitis. METHODS:110 patients with acute wisdom tooth pericoronitis were randomly divided into control group and observation group. Control group was given one Ornidazole tablet,ground into fine powder in sterile bottle. Checking oral cavity carefully before treatment,Hydrogen peroxide solution (3%) and 0.9% Sodium chloride injection were in sterile syringe to repeatedly and alternatively flushing peripheral tissue to clean the food residuals,bacteria and pus in gingival pock-et,until spilled solution was clear and no purulent. Sterile cotton swabs were used to blot up and separate it from the wet. After sep-arating gingival valve by probe,ornidazole fine powder was put into gingival pocket and pressed into deep area by tampons,and kept tampons,once a day + 2 Amoxicillin capsules,3 times a day. Observation group was additionally given 10 ml Jinzhi jieyin gargle in mouth for 3 min after the spilled solution was clear and no purulent,for 2 times,then ornidazole fine powder was put in-to blind pouch. 10 ml was taken in mouth for 3 min after three meals per day. The treatment course for both groups was 4 d. Clini-cal efficacy,main symptom and total scores of main symptoms,secondary symptom and total scores of secondary symptoms be-fore and after treatment,and incidence of adverse reactions in 2 groups were observed. RESULTS:The total effective rate in obser-vation group was significantly higher than control group,the difference was statistically significant (P<0.05);after treatment, main symptom and total scores of main symptoms,secondary symptom and total scores of secondary symptoms in 2 groups were significantly lower than before,and observation group was significantly lower than control group,the differences were statistically significant(P<0.05). And there was no significant difference in the incidence of adverse reactions between 2 groups(P>0.05). CONCLUSIONS:Ornidazole combined with Jinzhi jieyin gargle has better efficacy than only ornidazole in the treatment of acute wisdom tooth pericoronitis,it can significantly reduce the main symptom scores and secondary symptom scores,with similar safety.

Objective To explore the application efficacy of scalp nerve block combined with sevoflurane in craniotomy.Methods Fifty-five patients undergoing craniotomy in pingdingshan second people's hospital of henan province from Mar 2013 to Mar 2015 were randomly divided into observation group (n =28) and control group (n =27).The scalp nerve block combined with sevoflurane was performed in observation group and the sevoflurane intravenous inhalational combined with sufentanil was performed in control group.The changes of MAP and HR were observed before induction,5min after induction,skin incision,break the meninges,operation end and 5rmin after catheter removal.The agitation scale after operation,anesthesia duration,awaking time and dosage of sufentanil were compared.Restults MAP and HR at skin incision,break the meninges,operation end and 5min after catheter removal in control group were significantly increased than those before operation,and significantly higher than those in observation group at same stage of operation,with statistically significance differences (P ＜0.05).The agitation scale of observation group was lower than that of control group,with statistically significance difference (P ＜0.05).There was no significance difference between two groups in anesthesia duration (P＞0.05),the awaking time in observation group was significantly shorter than that in control group and the dosage of sufentanil was significantly reduced in observation group (P＜0.05).Corclusion Scalp nerve block combined with sevoflurane used in craniotomy has advantages in stable vital signs,rapid awaking,less dosage of sufentanil and lower score of agitation,which is suitable for clinical application.

Objective:To establish a determination method for sulfadoxine, sulfathiazole and sulfadiazine in animal derived prod-ucts by ultra-performance liquid chromatography-tandem mass spectrometry ( UPLC-MS/MS) . Methods:Trichloroacetic acid and ace-tonitrile were used in the extraction and removal of proteins. A Waters Acquity UPLC BEH C18 (100 mm × 2. 1 mm,1. 7 μm) column was used as the separation column. The mobile phase was 0. 1% formic acid in water-methanol with gradient elution. The three sulfona-mides residues were detected with multiple reaction monitoring( MRM) mode in multi-period. Results:Under the conditions,the linear range of sulfadoxine, sulfathiazole and sulfadiazine was 1. 0-200. 0 ng·ml-1 ,the linear correlation coefficient was above 0. 99,the re-covery was more than 70%,the detection limit was 0. 5 ng·ml-1 , and the quantification limit was 1. 0 ng·ml-1 . Conclusion: The sample preparation method is simple and fast, and the method can be used to analyze sulfadoxine, sulfathiazole and sulfadiazine in ani-mal derived food efficiently and sensitively.

Objective To investigate the manifestations of indocyanine green angiography (ICGA) of the choroidal neovascular membrane (CNV) and the relationship with histopathological changes in patients with age-related macular degeneration (AMD). Methods Twenty-one eyes of 21 patients with AMD diagnosed by ICGA were classified into three types based on ICGA findings: developing type in 9 eyes, degenerating type in 9, and stabilizing type in 3. CNV was extracted by vitrectomy and the histopathological characteristics of CNV was observed under the light and electron microscope. Results The histopathological characteristics of the specimens of developing type revealed abundant CNV partly enwrapped with non-pigmental cells and fibrous tissue or a few pigmental cells; degenerating type revealed reduced activation duration of CNV, many pigmental cells and a little fibrous tissue; stabilizing type revealed a mass of fibrous tissue,few CNV and nonexistence of pigmental cells. Conclusion The histopathological characteristics of exudative AMD may be related to the manifestations of ICGA.

ObjectiveTo investigate the possible relationship between choroidal neovascularization (CNV) and macular choroidal watershed zones (CWZ) in patients with age-related macular degeneration (AMD). MethodsFifty-seven selected indocyanine green video angiograms (ICGA) of 57 patients (57 eyes) with AMD were evaluated, and 35 ones of the healthy fellow eyes of 35 patients with unilateral non-AMD fundus diseases were selected as age-matched control. The video angiograms were evaluated for investigating the relationship between CNV and macular CWZ. Objective To explore the quality of life(QOL) of stroke patients and the influence factors.Methods The QOL of stroke patients was measured with Shot Form 36 Health Survey Questionnaire(SF-36) and questionnaire survey before discharge and six months after discharge;the patients' state of life was analyzed with the ordinal Logistic regression method to find out influence factors.Results The QOL of the stroke patients decreased obviously with an average sphere score between 38.18～67.57.Sphere scores increased during the six months of post-discharge,but only those of physical functioning(PF),role limitations due to physical health(RP),bodily pain(BP),general health perceptions(GH) and vitality(VT) rose significantly.Some common factors that would affect physical and mental health of the patients included: marriage state,economic conditions,medical care assurance,health care,psychological obstacles,rehabilitation and viability.Besides,age,the features of pathological changes and the complication had significant effect on the physical health of the patients,while children and aphasia affected mental health.Conclusion Early and canonical rehabilitation physical care,language and psychological training and so on,as well as family caring and nursing all are contributive to improve the QOL of the stroke patients.
