1.Clinical risk factors for central compartment lymph node metastasis in papillary thyroid cancer(cN0)

Objective To analyze the risk factors of central compartment lymph node (CCLN)metastasis in papillary thyroid carcinoma(PTC) without clinically suspected cervical lymph node metastasis (cN0).Methods Clinical data of 272 PTC (cN0) patients who underwent radical thyroidectomy and CCLN dissection between January 2008 and December 2012 in Huashan hospital were retrospectively analyzed.Patients were divided into CCLN positive group (115 cases)and CCLN negative group(157 cases) according to postoperative pathology.Chi-square test,wilcoxon test and multivariate logistic regression analysis were used to analyze risk factors.Results CCLN metastasis of papillary micro thyroid cancer (PMTC) and papillary thyroid cancer (PTC) was 34.9% and 48.3%,respectively.Tumor size (x2 =10.26,P < 0.01),position(x2 =13.87,P < 0.01),capsular invasion(x2 =20.19,P < 0.01),multifocal PTC(x2 =7.42,P < 0.01) and unmicro-carcinoma (x2 =5.12,P < 0.05) were significantly correlated to lymph node metastasis.Middle area or lower pole of thyroid carcinoma,capsular invasion and multifocal PTC were independent risk factors of CCLN metastasis in PTC.Conclusions The cN0 PTC has a high rate of pathological CCLN metastasis,it is imperative to conduct thyroidectomy with ipsilateral level CCLN dissection in PTC patients.
2.Performance Tests and Quality Control of Siemens ECAT EXACT HR+ PET

Objective To evaluate the performance parameters of the PET instrument, which has been used for years in our hospital, assuring the stability of image quality and performance parameters. Methods NEMA NU 2-1994 Standards was used as reference for performance measurement of PET, which were published by National Electrical Manufacturers Association(NEMA). The NEMA standard cylinder model, 18F radiation source, and 18F and 68Ge columniform radiation source were used. Results Projects for performance measurement and daily quality control were drawn up. Conclusion This project provides a reference and methods for the performance measurement of PET instrument and daily quality control. It better ensures the stability of image quality and performance parameters of the PET instrument.
3.Cytokines in different combination plus total saponins of panax ginseng for inducing the differentiation of CD34+ cells cultured in vitro

BACKGROUND: Hematopoietic growth factors (HGFs) can be involved in different hematopoietic cells and different phase of differentiation. Erythropoietin (EPO) can promote he proliferation and differentiation of erythroid progenitor cells in vivo and in vitro, Interleukin-3 (IL-3) appears to have both the characteristics of a differentiating hormone and the ability to generate proliferation of relatively early stem cells. IL-3 acts to enhance the effect on the proliferation and differentiation of various hematopoietic cells.OBJECTIVE: To investigate the effect of HGFs and total saponins of panax ginseng (TSPG) on erythropoietic differentiation of purified CD34+ cells from human bone marrow.DESIGN: An observational comparative experiment.SETTING: Chongqing Medical University.MATERIALS: This experiment was performed in the Department of Histology and Embryology, Institute of Basic Medical Research and Key Laboratory of Biochemistry and Molecular Pharmacology in Chongqing Medical University from July 2002 to January 2004. Human bone marrow was collected from extracted ribs of patients without hematopoietic system diseases in the Department of Chest Surgery, Daping Hospital, the Third Military Medical University of Chinese PLA. Informed contents were obtained from all the patients. TSPG (purified to more than 95%) was provided by Chongqing Institute of Chinese Traditional Medicine; Fetal bovine serum (FBS), fluorescein isothiocyanate (FITC)-conjugated anti-CD34 Ab from Becton Dickinson; FITC isotype-matched mouse IgG1 and rabbit anti-CD71 Ab from Santa Cruz Biotechnology Inc; Recombinant human erythropoietin (EPO) from Amgen. rHu Flt3 Ligand (FL), rHu thrombopoietin (TPO), rHu stem cell factor (SCF) and rHu interleukin-3 (IL-3) from Santa Cruz Biotechnology Inc.METHODS: CD34+ hematopoietic stem/progenitor cells (HSC/HPC) were isolated by StemsepTM according to the strategy of negative selection on immunomagnetic separation. The sorted CD34+ cells were incubated in various combinations with IL-3+EPO, SCF+IL-3+EPO and FL+TPO+SCF+IL-3+EPO. SCF+IL-3+EPO was taken as the control group, TSPG of 10, 20, 50, 70 and 100 mg/L, then the total cell number and ratio of CD71+ cells were detected. CD34+ culture systems were added by TSPG of different dosages, those un-added by TSPG as control group. Methylcellulose culture method was used to detect the capacity of TSPG in inducing the proliferation and differentiation of CD34+ HSC/HPC [burst forming unit-erythroid (BFU-E) and colony forming unit-erythroid (CFU-E)].MAIN OUTCOME MEASURES: ① CD34+ cell proliferation in vitro in different cytokine groups; ② Effects of TSPG on inducing CD34+ cells to differentiation into CD34+ erythroid lineage hemocytes; ③ Effects of TSPG on the ability of CD34+ cells in forming erythroid lineage progenitor cells.RESULTS: ①For FL+TPO+SCF+IL-3+EPO, the mean increase in overall cell number corresponded to 546.38-fold expansion, and the ratio of CD71+ cells was increased to (61.20±5.31)%. ② TSPG 20 mg/L was identified as the most potent concentration of those tested, and the ratio of CD71+ cells was increased from (48.39±5.07)% to (56.20±1.40)%. ③ The addition of TSPG(10-50 mg/L) increased the colony production rates of BFU-E and CFU-E.CONCLUSION: A combination containing FL+TPO+SCF+IL-3+EPO may obviously induce CD34+ cells to proliferate and differentiate to erythroid lineage; TSPG may promote CD34+ cells to differentiate into erythroid lineage by cooperating with HGFs.
4.Toll like receptor 4-a potential transmembrane receptor contributes to the cardiac remodeling of hypertension

AIM:To observe the expression of Toll like receptor 4 (TLR4) in the left ventricle of Goldblatt rats and to explore the role and mechanism of TLR4 in left ventricular remodeling of hypertension.METHODS:Goldblatt model of Two-kidney,one-clip (2K1 C) renovascular hypertension was induced in twenty-five rats(H group),and twenty rats served the sham-operated group(sham group).The tail cuff blood pressure was detected every week and echocardiogram was observed every other week.After eight weeks of operation,the rats were killed and the samples of the left ventricle were collected.The concentration of Ang Ⅱ in left ventricle was assessed by tadioimmunoassay.Western blotting and RT-PCR were used to exam the mRNA and protein expression of TLR4 in the left ventricle.Immunohistochemistry was adopted to exam the location of TLR4 in the myocardium.RESULTS:TLR4 mRNA and protein expression were consistently upregulated in the left ventricle of H group compared with sham group.In H group, predominantly sarcolemmal staining was observed,especially focal areas of intense TLR4 staining were found in juxtaposed regions of two or more adjacent myocytes;However,in sham group,TLR4 expression was diffuse and presumably cytoplasmic.Considerable correlation was found between blood pressure,MESS,LVMI,RWT,the concentration of Ang Ⅱ in left ventricle and the protein expression of TLR4 in myoeytes.CONCLUSION:During the development of left ventricular remodeling of Goldblatt rats,expression of TLR4 increases significantly.Enhanced expression of TLR4 locates in the sarcolemma,especially in juxtaposed regions of two or more adjacent myocytes.These indicate that TLR4 transmembrane receptor which is closely relative with inflammation and immunity probably contributes to the development of ventricular remodeling.[KEY WORDS]Hypertension;Ventricular remodeling;Receptors,Toll-like;Goldblatt rats
5.Study on the erythroid differentiation on K562 cells induced by angelica polysaccharides and its signal transdution pathway

Objective To explore the relative signal transduction pathway of angelica polysaccharide(APS) inducing K562 cells toward erythroid differentiation. Methods K562 cells were divided into control group and ASP group, the control group cells were routinely cultured and APS group cells were treated with APS(final concentration is 100-500mg/L). By using of benzidine staining and spectrophotometry, the characteristics of erythroid differentiation of K562 cell induced by APS were detected;By using of laser confocal microscopy, the distribution of JAK_2 and STAT_5 in K562 cells was observed;By using of Western blotting, the expression of JAK_2 and STAT_5 in nucleus and cytoplasm of K562 cells was detected, By using of imm~uno~pre~cipi~ta~tion, the phosphorylation change of JAK_2 in cytoplasm was tested. Results After being induced by APS, the benzidine staining positive rate of K562 was increased.With increasing the concentration of APS, hemoglobin synthesis in K562 cells was promoted accordingly. After being cultured for 12 hours, 24 hours and 48 hour, the expression of STAT_5 in nucleus of K562 cells induced by APS was significantly higher than that of control group, however, expression of STAT_5 in cytoplasm of K562 cell induced by APS was obviously lower. The expression of JAK_2 in K562 cells was not different between APS group and control group, but the JAK_2 phosphorylation level of APS group was much higher than that of control group.Conclusion APS can induce erythroid differentiation of K562 cells, and the mechanisms may be that APS can promote the phosphorylationthe of JAK_2, then stimulating nuclear translocation of STAT_5.
6.Protective immunity effects of co-immunization with P30 DNA vaccine and protein vaccine

Objective To study protective immunity effects of co-immunization with P30 DNA vaccine and protein vaccine. Methods Forty-eight 5-6 weeks old BALB/c female mice were divided into four groups (A,B,C,D), 12 mice of each group. In group A (control group) each mouse was immunized with 100 ?g pcDNA3.1 plasmid DNA by intramuscular (i.m.) for three times at week 0,2 and 4; in group B (P30 protein group) each mouse was immunized (i.m.) with 50 ?g rP30+50 ?g CFA for three times at week 0, 2 and 4; in group C (pcDNA3.1-P30 group) each mouse was immunized with 100 ?g pcDNA3.1-P30 plasmid DNA (i.m.) for three times at week 0, 2 and 4; in group D (P30 DNA+rP30 co-immunization group) each mouse was immunized with 100 ?g pcDNA3.1-P30 plasmid DNA (i.m.) for two times at week 0, 2 and immunized by subcutaneous with 50 ?g rP30+50 ?g CFA at week 4. Each mouse was infected with 100 tachyzoites of Toxoplasma gondii RH strain four weeks later after last immunization. The anti-P30 antibodies were detected with ELISA before the challenge. Results The P30 DNA vaccine was successfully constructed. High titers of anti-P30 antibodies were induced in each mouse immunized with DNA vaccine. The protective trial proved that there was no significant difference between control group and experimental group though the survival time of mouse from experimental group had been prolonged. Conclusion The P30 DNA vaccine could induced high titers of anti- P30 antibodies in immunized mice, and it may be a potential DNA vaccine candidate.

OBJECTIVE To implement the automatic management of medical equipment.METHODS Based on the platform of net,three layers of C/S were applied with database technology to realize one-time importation of the basic information and current information,it made the information function of quantity evaluation,quantity determination,schedule formation,automatic alarm,apparatus distribution list,statistical inquiry,collection analysis,and information feedback coming to true.RESULTS By using one year,the application of management information system of medical equipment could meet the requirement in scientific and standardized supply,and standard,application and management of medical equipment.CONCLUSIONS It can be able to improve the system efficiency,reduce the labor and mistakes,and also achieve the aim that cannot be done by manual labor.

Objective:To compare the nutritional status between alcoholic cirrhosis and viral cirrhosis,and to provide reference information for the nutritional support strategy.Methods:95 alcoholic cirrhosis and 260 viral cirrhosis patients were involved in the study.The patients were reviewed with NRS 2002 and SGA within 48 hours after admission.The general information and liver function parameters including gender,age,BMI,Child-Pugh score,ALB,PA and Hb were recorded.Results:The NRS 2002 nutritional risk rate of alcoholic cirrhosis and viral cirrhosis patients was 76.80% and 65.00%,respectively.The SGA malnutrition rate of alcoholic cirrhosis and viral cirrhosis patients was 67.40% and 61.90%,respectively.There was statistically significant difference between the two groups in gender,Child-Pugh A rate,ALB,PA,Hb and NRS 2002,while there was no significant difference in age,Child-Pugh B rate,Child-Pugh C rate,BMI and SGA.Conclusion:Nutritional dysfunction exists in both of the two types of liver cirrhosis.The nutritional risk rate and the anemia rate in alcoholic cirrhosis patients are significantly worse than those in viral cirrhosis patients.
10.Effects of acupuncture combined with individualized occupational therapy on the rehabilitation of unilateral spatial neglect after stroke

Objective To explore the effects of acupuncture and individualized occupational therapy on the rehabilitation of patients with unilateral spatial neglect ( USN ) after stroke.Methods Stroke patients with hemiplegia were evaluated by neuropsychological tests,including line bisection,a cancellation test,clock drawing,and figure and shape copying.Sixty-nine right hemispheric stroke patients identified with USN were randomly assigned to 3 groups:20 patients as a control group received conventional rehabilitation therapy ; another 22 patients as an occupational group received type-specific individualized occupational therapy combined with conventional rehabilitation therapy; the remaining 23 patients as an acupuncture combined with occupational therapy group received acupuncture in addition to both conventional rehabilitation therapy and occupational therapy.These three groups were evaluated with neuropsychological tests and the modified Barthel index (MBI) before and after treatment.Results ①MBI scores in the occupational therapy group and the combined group were significantly higher after treatment,but no significant difference was observed in the control group.②The increased MBI scores in the combined and occupational groups were higher than those in the control group.At the same time the increase in MBI scores in the combined group was significantly greater than in the occupational therapy group.③The assessment results with both line bisection and cancellation tests in all three groups were significantly different after treatment.The line bisection results in the occupational therapy group and the combined group were significantly better than in the control group.The cancellation test results in the combined group were significantly better than in the control group.Conclusion These findings show that acupuncture combined with occupational therapy can have a positive effect on neglect recovery and greatly improve the daily life of USN patients.
