1.Treatment of single segment lumbar degenerative disease with Quadrant assisted MIS-TLIF single side pedicle screw system

Objective To investigate the clinical efficacy of unilateral small incision Quadrant channel assisted MIS?TLIF unilateral pedicle screw fixation system in the treatment of degenerative lumbar disease. Methods From January 2011 to December 2013,a total of 56 cases with low back and leg pain were selected in the People′s Hospital of Dongguan,including 25 cases with lumbar disc herniation,18 cases with lumbar tube stenosis,10 cases with discogenic low back pain,2 cases of recurrence after posterior lumbar spine surgery,1 case of recurrence after transforaminal endoscopic surgery. Unilateral pedicle screw fixation was performed in the treatment of MIS?TLIF with expandable pipeline system. VAS and Oswestry dysfunction index scoring system( ODI) were used to evaluate of pain and functional recovery in patients with preoperative and postoperative pain and functional recovery,the Suk method was used to observe the bone graft fusion. Results There were 5 cases of non operative side waist back pain after operation,and the waist circumference and anti?inflammatory pain relief were improved after treatment. One case of postoperative subcutaneous fat liquefaction, was cured by dressing change. One patient with recurrence of MED intraoperatie cerebrospinal fluid leakage,was cured after treatment by the bed,dehydration and others. Other complications such as infection,screw loosening, nerve root injury and other complications had no found. After 1 month,the VAS score from preoperative ( 6. 82 ±0. 92) points fell to (1. 95±0. 55) points,ODI score from preoperative (35. 21±2. 73) points fell to (10. 05 ±1. 72) points, significantly improved compared with the preoperative, the differences were statistically significant( t=36. 775,65. 858,P<0. 05) ,based on the fusion of Suk judgment method,2 cases of patients with possible fusion,the rest were fusion. Conclusion Unilateral small incision under the quadrant assisted MIS?TILF unilateral pedicle nail stick system has obvious advantages in treatment of degenerative lumbar spine disease,as long as we choose to suitable cases and most patients can obtain satisfactory results.
2.Significance of detection of EBV-encoded mRNA on bone marrow biopsy tissues of hemophagocytic syndrome

ObjectiveTo investigate the diagnostic significance of different pathology techniques (Immunohistochemistry and ISH)to detect EBV on bone marrow biopsy tissues of hemophagocytic syndrome. Methods Histological,immunohistochemicalandinsituhybridizationwereusedtostudythe hemophagocytic features and expression of LMP-1,EBER and other markers.Results25 out of 51 cases (49.0 %)showed an active proliferation growth pattern while other 12 cases(23.5 %)demonstrated a deterioration morphological character compared with their same age group. The left 14 cases (27.5 %) showed a normal myeloproliferative pattern. 91.4 % (32/35) expressed CD68/KP-1 and 89.5 % (17/19) cases expressed CD68/PG-M1, which showed an abnormal increase of monocytes. 26 out of 51 cases (51.0 %) were positive for EBER,while EBER and immunohistochemistry on LMP-1 were detected simultaneously in 41 cases.The EBER were positive in 19 cases(46.3 %)but none for LMP-1(P =0.000).ConclusionHypoplasia or hemophagocytic features could be detected on bone marrow biopsy tissues of hemophagocytic syndrome, which might be slightly difficult to diagnose. So immunohistochemistry would be essential. Meanwhile, the detection of EBER is more helpful than LMP-1 for diagnosis.
3.Identification and expression pattern analysis of FBXL gene family in Salvia miltiorrhiza

Objective To identify and analyze the bioinformatics and expression patterns of the F-box-LRR(FBXL)gene family of Salvia miltiorrhiza based on genomic data,and provide a foundation for further elucidating its gene functions.Methods The SmFBXL gene was identified from the Salvia miltiorrhiza genomic database.Its gene structure features,promoter cis-acting elements,physicochemical properties of encoded proteins,evolutionary relationships,and tissue expression were analyzed by bioinformatics methods and online tools.Results A total of 104 SmFBXL genes were identified from the Salvia miltiorrhiza genome,unevenly distributed on 8 chromosomes,with upstream promoters containing cis-acting elements related to plant stress resistance,growth and development,and hormone response.A phylogenetic tree of the FBXL family members of Salvia miltiorrhiza,Arabidopsis thaliana,and Glycine max was constructed,dividing the 104 SmFBXL genes into 7 subfamilies.Through homologous evolution analysis,it was speculated that SmFBXL36 might be involved in defense against pathogen invasion,SmFBXL86 and SmFBXL79 might play important roles in regulating lateral root growth in Salvia miltiorrhiza,and SmFBXL11 and SmFBXL40 might regulate hypocotyl growth.Transcriptome data showed differential expression of SmFBXL genes in different tissues of Salvia miltiorrhiza,with 13 SmFBXL genes showing higher expression levels in roots and leaves,serving as candidate genes for further research on the SmFBXL gene family.Conclusion The research results provided a reference for further elucidating the regulatory mechanisms of SmFBXL genes in stress response and secondary metabolite biosynthesis in Salvia miltiorrhiza.
4.Targeting metabolism to improve CAR-T cells therapeutic efficacy

Chimeric antigen receptor T (CAR-T) cell therapy achieved advanced progress in the treatment of hematological tumors. However, the application of CAR-T cell therapy for solid tumors still faces many challenges. Competition with tumor cells for metabolic resources in an already nutrient-poor tumor microenvironment is a major contributing cause to CAR-T cell therapy’s low effectiveness. Abnormal metabolic processes are now acknowledged to shape the tumor microenvironment, which is characterized by increased interstitial fluid pressure, low pH level, hypoxia, accumulation of immunosuppressive metabolites, and mitochondrial dysfunction. These factors are important contributors to restriction of T cell proliferation, cytokine release, and suppression of tumor cell-killing ability. This review provides an overview of how different metabolites regulate T cell activity, analyzes the current dilemmas, and proposes key strategies to reestablish the CAR-T cell therapy’s effectiveness through targeting metabolism, with the aim of providing new strategies to surmount the obstacle in the way of solid tumor CAR-T cell treatment.
5.Analysis of clinical characteristics of patients with hyperthyroidism combined with liver injury

Objective:To analyze, explore and evaluate the clinical characteristics, abnormal thyroid function and follow-up of anti-hyperthyroidism treatment mode in patients with hyperthyroidism (commonly abbreviated as HT) combined with liver injury.Methods:The clinical data of patients with hyperthyroidism combined with liver injury were retrospectively analyzed, and then patients were divided into treated and untreated group according to whether they received anti-hyperthyroidism treatment before the consultation. Patients’ thyroid and liver function test indicators at the time of treatment were analyzed to determine the main cause of liver injury. The characteristics of liver injury were analyzed in the treatment group. Patients with severe thyroid toxicity and hyperthyroidism combined with liver injury were followed-up with anti-hyperthyroid therapy, mainly low-dose methimazole (MMI) and radioactive iodine therapy to evaluate its efficacy and safety. The comparison between data groups was performed by t-test, rank sum test and χ2 test. Results:Among the 43 cases with hyperthyroidism combined with liver injury, 19 were males and 24 were females, aged 49.0 ± 14.6 years-old; 16 cases (16/43, 37.21%) aged 40 to≤60 years- old, and 15 cases (15/43, 34.88%) aged > 60 years-old. There were 22 untreated cases (untreated group, accounting for 51.16%), and 21 treated cases with anti-hyperthyroidism (treatment group, accounting for 48.84%) at the time of consultation. Thyroid function indicators (FT3, FT4, TSH) and liver function indicators (alanine aminotransferase, aspartate aminotransferase, alkaline phosphatase, γ-glutamyltransferase, total bilirubin) of the two groups were compared, and the difference was not statistically significant ( P > 0.05). The order of liver injury from mild to severe in patients with different treatment options were: methimazole (MMI) < propylthiouracil < radioactive iodine
6.Mining and analysis of adverse drug event signals related to mepolizumab

OBJECTIVE To provide reference for rational clinical use of mepolizumab. METHODS The reporting odds ratio method and Bayesian confidence propagation neural network method were used to conduct signal mining and analysis of adverse drug event (ADE) reports related to mepolizumab in the United States Food and Drug Administration Adverse Event Reporting System from the first quarter of 2016 to the third quarter of 2022. RESULTS A total of 57 501 ADE reports were extracted with mepolizumab as the primary suspect drug, involving 16 358 patients. Among these reports, the proportion of males (23.51%) was lower than females (50.48%). The reporting countries were primarily the United States (51.91%) and Canada (29.94%). Consumers (71.18%) constituted the main reporting population. A total of 172 ADE-positive signals were identified, mainly involving 13 system organ classes such as the respiratory, thoracic and mediastinal disorders (41.63%), as well as infectious and parasitic diseases (14.16%). There were 60 high-risk signals, including 15 that were explicitly mentioned or related to adverse reactions in the drug instructions of mepolizumab and 45 signals (such as asthmatic crisis, sputum discoloured, purulent sputum, sleep disorder due to a general medical condition) were newly identified high-risk signals. Among them, 11 high-risk signals exhibited gender or age differences. CONCLUSIONS When clinically using mepolizumab, in addition to the adverse reactions mentioned in the drug instruction, special attention should also be given to changes in the nature of sputum, painful respiration, and sleep disorders.
