2.A meta-analysis of unilateralversus bilateral balloon kyphoplasty in the treatment of osteoporotic vertebral compression fractures

BACKGROUND:Baloon kyphoplasty is effective in the treatment of osteoporotic vertebral compression fractures, but it is unclear that which one is proper, unilateral or bilateral approach, with better efficacy and fewer
complications.
OBJECTIVE:To assess the efficacy and safety of unilateralversus bilateral baloon kyphoplasty in the treatment of osteoporotic vertebral compression fractures.
METHODS: We searched the electronic bibliographic databases including Cochrane Library, PubMed, EMBASE, ISI Web of Knowledge, CBMdisc and other databases to colect clinical trials concerning unilateral versus bilateral baloon kyphoplasty. Two estimators independently evaluated the quality of these included studies and analyzed data by Cochrane Colaboration’s RevMan 5.2 software.
RESULTS AND CONCLUSION:Fourteen trials involving 876 patients were included. There were 442 cases of unilateral approach and 434 of bilateral approach. The meta-analysis showed that there were no significant
differences in pain score by visual analog scale, vertebral height, and kyphotic angle; while the unilateral approach had less operating time, lower amount of cement injected and lower risk of cement leakage
than the bilateral approach [mean difference (MD)=-19.33, 95% confidence interval (CI) (-24.42,-14.24);
MD=-2.07, 95%CI (-2.42,-1.71); odds ratio=0.47, 95% CI (-24.42,-14.240)]. These findings indicate that the unilateral baloon kyphoplasty can reduce the leakage rate of bone cement.
3.Clinical analysis of complex femoral and tibial fractures treated with Grosse-Kempf intramedullary interlocking nails

Objective To investigate the clinical results of the treatment of complex femoral and tibial fractures using Grosse Kempf intramedullary interlocking nails. Methods Grosse Kempf intramedullary interlocking nails were used to treat 38 cases of complex femoral and tibial fractures. 12 cases of them were opened fractures, and 26 cases were closed. There were 24 cases of femoral fractures and 14 cases of tibial fractures. The average age was 33.6 years. Most of them were reduced under x ray monitoring. CPM rehabilitation started after the operation to gain the funtional recovey. After two weeks walking with crutches began. Results After the operation, all the wounds healed by first intention. The average union time of the fractures was 10.5 weeks (8~12 weeks). The average follow up period was 14.5 months. With all the cases rated as good or excellent, the clinical results were satisfactory. Conclusion Grosse-Kempf intramedullary interlocking nails are an effective, strong, convenient and secure implant to treat complex femoral and tibial fractures.
4. Correlation analysis of inflammatory factor expression and pain degree in patients with Kashin-Beck disease after total knee arthroplasty

Objective:
To explore the relationship between the expression of inflammatory factors and the degree of pain in patients with Kashin-Beck disease after total knee arthroplasty.
Methods:
Forty-nine patients with Kashin-Beck disease and 55 patients with knee osteoarthritis who underwent total knee arthroplasty in the Department of Orthopaedics, Gansu Provincial People's Hospital from January 2016 to December 2018, were selected, as the Kashin-Beck disease group and osteoarthritis group, respectively, and 50 healthy subjects who underwent healthy physical examination at the same time were selected as the control group. At the 7th day after operation, 3 ml of fasting venous blood was collected from all subjects. The expression levels of interleukin (IL)-6, tumor necrosis factor (TNF)-α, and C-reactive protein (CRP) were measured by enzyme-linked immunosorbent assay (ELISA); the visual analogue score (VAS) and knee society score (KSS) were used to compare the pain degree and knee function of each group; correlation analysis between postoperative inflammatory factor levels and VAS, KSS scores in patients with Kashin-Beck disease were studied.
Results:
The expressions of IL-6 [(78.17 ± 13.28), (60.28 ± 9.92), (9.65 ± 3.12) ng/L], TNF-α [(126.67 ± 15.83), (105.29 ± 12.76), (21.09 ± 5.82) ng/L] and CRP [(11.23 ± 3.41), (8.31 ± 2.54), (2.14 ± 1.01) mg/L] in Kashin-Beck disease group, osteoarthritis group and control group were significantly different (
