1.Significance of KAI1 protein expression in tissues of papillary thyroid cancer and follicular thyroid cancer

Objective To investigate the expression of KAI1 protein in tissues of papillary thyroid cancer (PTC) and follicular thyroid cancer,and the effect of KAI1 protein in invasion and metastasis of PTC.Methods The clinical data of 58 patients with PTC (PTC group),17 patients with follicular thyroid cancer (follicular cancer group) and 30 patients with benign thyroid tumor (control group) were retrospectively analyzed from January 2006 to August 2009.The expression of KAI1 protein in PTC tissues,follicular thyroid cancer tissues and benign thyroid tumor tiasues were detected by immunohistochemistry method.Results The positive expression rate of KAI1 protein in PTC group was 48.3%(28/58),in follicular cancer group was 47.1%(8/17),in control group was 86.7 %(26/30).There was significant difference between PTC group,follicular cancer group and control group (P< 0.01).There was no significant difference between PTC group and follicular cancer group (P > 0.05).There was no significant difference in the positive expression rate of KAI1 protein of PTC tissues among the different sex,age and size of the primary lesion (P> 0.05).The positive expression rate of KAI1 protein in the TNM Ⅰ was 88.9%(8/9),the TNM Ⅱ was 60.9%(14/23)and the TNM Ⅲ-Ⅳ was 23.1% (6/26),there was significant difference in the positive expression rate of KAI1 protein among the different TNM stage (P < 0.01).The positive expression rate of KAI1 protein in the lymph node metastasis patients was 32.3% (10/31),the non-lymph node metastasis patients was 66.7% (18/27),there was significant difference (P < 0.05).Conclusion The expression of KAI1 protein may reflect the PTC metastasis and the clinical evolution of tumors,the KAI1 protein may inhibit the invasion and metastasis of PTC.

3.Comparison of the Efficacy of Lichtenstein and Transabdominal Preperitoneal Patch Repair for Primary Inguinal Hernia in Adults

Objective To compare the efficacy of Lichtenstein and transabdominal preperitoneal patch repair(TAPP) in the treatment of primary inguinal hernia in adults. Methods The clinical data of 223 patients who received Lichtenste and 121 patients who underwent TAPP in our hospital between January 2002 and January 2006,were analyzed retrospectively.The efficacy and complications of the two methods were compared.Results The rate of intraoperative injury of the inner organs in the TAPP group was significantly higher than that in the Lichtenstein group [5.8%(7/121) vs 0.4%(1/223),?2=7.626,P=0.006],while no significant difference was detected in the rate of abdominal organ injury between the two groups [0.8%(1/121) vs 3.1%(7/223),?2=0.969,P=0.325].The rate of postoperative complications in the TAPP group was significantly lower than that in the Lichtenstein group [14.0%(17/121) vs 36.3%(81/223),?2=18.881,P=0.000].In the TAPP group,during a follow-up of(14?7) months,one case of recurrence was found(0.8%).Whereas in the Lichtenstein group,the patients were followed up for(16?8) months,two of them had recurrence(0.9%).No significant difference existed in the recurrence rate between the two groups(?2=0.000,P=1.000).Conclusions Both the Lichtenstein and TAPP have benefits and drawbacks.Individulized therapy is recommended for the patients with inguinal hernia.It is important for surgeons to know the indications of the operations.
4.Retrospective study on the materials used in the reconstructoin of mandibular defects

objective: To evaluate the clinical effect of various materials used in the reconstruction of mandibular defects retrospectively. Methods: 467 patients with mandibular defects were treated with various implantation materials during last 30 years and 395 of them were followed up.Results: 217 were satisfied with their mouth opening, occlusion and maxillofacial contour after sugery, 189 of them had bone transplanation and 28 had non biomateria transplantaion; 91 were partly satisfied, 72 of them had bone trasplantation and 19 had non biomateria; 87 were not satisfied, 11 of them had bone trasplantation and 76 had non biomaterial. Conclusion: Bone especially auto bone transplantation is more effective than non biomaterial in the reconstruction of mandibular deffects.
5.The effect of mental nursing intervention on gastric cancer with liver metastasis patients treated with TACE sequential chemotherapy

Objective To explore the effect of mental nursing intervention on gastric cancer with liver metastasis patients treated with TACE sequential chemotherapy.Methods Sixty four gastric cancer patients with liver metastasis,no hopes of accepting operations,were included.All patients were divided into control group (32 cases) and observation group (32 cases).The former received routine nursing care and the later received extra mental nursing intervention.The anxiety,depression and quality of life in both groups were evaluated and compared.Results After extra mental nursing intervention,the SDS and SAS scores of the observation group [(41.6 ± 1.8) points and (41.6 ± 2.3) points] were significantly lower than those of the control group [(50.9 ± 1.9) points and (53.0 ± 3.5) points,P =0.040,P =0.036,respectively].The quality of life scores,including the physiological function,body pain,general self rating,vitality,social function,emotional function and mental health score were (45.26 ± 38.26)points vs (35.17 ± 33.65) points (P =0.044),(71.98 ± 17.36) points vs (63.01 ± 18.41) points (P =0.039),(55.59 ± 12.78) points vs (49.67 ± 12.43) points (P =0.048),(74.82 ± 14.23) points vs (55.61 ± 10.58) points (P =0.027),(75.47 ± 12.15) points vs (57.22 ± 14.78) points (P =0.031),(70.24 ± 22.00) points vs (45.92 ± 34.76) points (P =0.004),(78.31 ± 19.11) points vs (67.23 ± 11.96) points (P =0.037),respectively,the scores in the observation group were significantly higher than those in the control group (all P < 0.05).Conclusion The extra mental nursing intervention on gastric cancer with liver metastasis patients,treated with TACE sequential chemotherapy,can not reduce the patients'negative mood,improve the patients'compliance with chemotherapy and the confidence to treatment,but improve the quality of patients'life effectively.
6.Effect of miR-20a on pulmonary surfactant synthesis of alveolar epithelial cells A549 and its mechanism

Objective To explore the role of miR-20a on pulmonary surfactant synthesis of alveolar epithelial cells A549 and its potential mechanism.Methods Lentivirus miR-20a overexpression vector(miR-20a group) or lentivirus no-load vector(no-load group) was transfected into A549 cells,and the expression of green fluorescent protein(GFP) was observed to determinate the transfection effficiency;cell proliferation was detected by using 3-(4,5-Dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT);the bioinformatics software and database were applied to predict and analyze the target genes of miR-20a about lung development;expressions of miR-20a,pulmonary surfactant-associated protein A(SP-A),pulmonary surfactant-associated protein B(SP-B),pulmonary surfactant-associated protein C(SP-C) and pulmonary surfactant-associated protein D(SP-D) mRNA were detected by using quantitative real-time PCR(qPCR);the expressions of SP-A protein,SP-B protein,SP-C protein,SP-D protein and protein signal transducers and activators of transcription 3 (STAT3) were detected by using Western blot.Results Observation of GFP expression under a fluorescent microscope indicated similar transfection efficiency,and real time-PCR showed that the expression of miR-20a increased after being transfected with lentivirus miR-20a overexpression vector(3.85 ± 0.18)compared with the normal group (0.99 ± 0.04)and the no-load group (1.21 ± 0.12),and the differences were significant(t =10.85,9.64,all P <0.001).As a result,lentivirus miR-20a overexpression vector was constructed successfully.Online software predicted that STAT3 gene was likely to be the target gene of miR-20a.Compared with the normal group (24 h,48 h,72 h:0.23 ± 0.01,0.39 ± 0.01,0.56 ± 0.03) and the no-load group (24 h,48 h,72 h:0.25 ± 0.01,0.44 ± 0.05,0.59 ± 0.01),miR-20a did not change the cell proliferation at different time points(24 h,48 h,72 h:0.26 ± 0.01,0.41 ± 0.02,0.58 ± 0.02) (all P > 0.05).Compared with the normal group (1.00 ± 0.05,1.24 ± 0.20,1.31 ± 0.09,0.89 ± 0.12) and the no-load group (0.76 ± 0.10,1.31 ± 0.13,1.50 ± 0.11,1.01 ± 0.11),miR-20a up-regulated the mRNA expressions of SP-A,SP-B,SP-C and SP-D (2.05 ± 0.17,2.14 ± 0.10,2.84 ± 0.09,1.66 ± 0.08),and the differences were significant (all P < 0.05).Compared with the normal group (0.46 ± 0.01,0.27 ± 0.03,0.69 ± 0.01,0.43 ± 0.01) and no-load group (0.43 ± 0.01,0.21 ± 0.01,0.79 ± 0.02,0.44 ± 0.02),miR-20a also increased the protein expressions of SP-A,SP-B,SP-C and SP-D (0.55 ±0.01,0.47 ±0.05,0.96 ±0.02,0.59 ±0.03),the diffe-rences were statistically significant (all P <0.05).The expression of STAT3 in miR-20a group(0.37 ±0.05) was significantly lower than that in the normal group(0.60 ±0.04) and the no-load group (0.68 ±0.06),and the differences were statstically significant (all P < 0.05) in A549.Conclusions STAT3 is a downstream target gene of miR-20a.miR-20a can promote pulmonary surfactant synthesis of alveolar epithelial cells A549 by inhibiting STAT3.
7.Surgical treatment of early stage gastric cancer in elderly patients

Objective To explore the clinicopathological characteristics of early gastric cancer (EGC),surgical treatment, and prognosis in elderly patients. Methods The clinicopathological and follow-up data in 83 elderly patients with EGC treated by gastrectomy from 1989. 8-2004. 8 were studied retrospectively. Results All of 83 patients were resectable, and there was no operative death nor surgical complications. The 5-, 10-, and 15- year survival rate was 98%,95%,and 92%, respectively. Sixty-one cases (74%) were mucosal cancer without lymph node metastasis, 22 cases were submucosal cancer with node metastasis in 4 cases. 7 cases were multiple primary carcinoma. Conclusions Local resection and partial gastrectomy are not suitable for multiple primary carcionoma in elderly patieats with EGC. Subtotal gastrectomy, and total gastrectomy should be the procedure of choice when foci were scattered about. Lymph node metastasis,multiple primary cancer foci and invasion of submucosal layer are all among factors impacting postoperative prognosis.

9.Effect of comfortable nursing on cancer related fatigue and quality of life in patients with gastric cancer

Objective To study the effect of comfortable nursing of cancer related fatigue and quality of life in patients with gastric cancer. Methods A total of 76 cases in our hospital for gastric cancer radical operation of gastric cancer patients from January 2013 to January 2014 as the observation objects, randomly were divided into observation group and control group, 38 cases in each group. The control group underwent conventional symptomatic care, the intervention group mainly implemented comfortable nursing, the quality of life of fatigue of RPFS scores and QOL scores were com-pared between the two groups before and after intervention. Results There were no significant differences in behavior, emotion, body tired, cognitive RPFS scores between intervention group and control groups before intervention (P>0.05). After the intervention, the behavior fatigue, emotional fatigue, body fatigue and cognitive RPFS scores in interven-tion group and control group were significantly reduced compared with before intervention, and there was statistically significant difference between two groups (P<0.05). The physical function, role function, cognitive function, emotional function, social function of QOL scores of intervention group and control groups had no significant difference before in-tervention (P>0.05). After the intervention, the physical function, role function, cognitive function, emotional function, social function and QOL scores in intervention group and control group increased significantly compared with before in-tervention, there was statistically significant difference between two groups (P<0.05). Conclusion After radical opera-tion of gastric cancer patients, comfortable nursing intervention can reduce patient cancer-related fatigue, improve the quality of life of patients, conducive to the resumption of the disease.
10.The evaluation of health-related quality of life for prostate cancer patients receiving androgen deprivation therapy

Objective To evaluate the health-related quality of life (HRQOL) for prostate cancer patients receiving androgen deprivation therapy (ADT).Methods We studied 200 patients with prostate cancer who were treated with primary ADT in Peking University People's Hospital from June 2014 to June 2015.The patients'average age was 73.9 years.The mean PSA level was 21.57 ng/ml when they were diagnosed with prostate cancer.Of these 200 patients,79% (158/200) were localized and seclected ADT therapy due to age,body condition,basic diseases or individual will.21% (42/200) were locally advanced or metastasic,which accord with the indication of ADT therapy.The scales,including the MOS item short from health survey (SF-36),Functional Assessment of Cancer Therapy-General module (FACT-G),Functional Assessment of Cancer Therapy-Prostate instrument (FACT-P),Self-Rating Depression Scale (SDS) and Self-Rating Anxiety Scale (SAS) were used to evaluate their HRQOL.Results According to results of SF36,the physical health component score and mental health component score was 67.41 ± 16.39 and 64.81 ± 17.51,respectively.They indicated that the overall quality of life of these patients was at an acceptable level.And the average score of all domains exceeded 60 except general health domain,which the score was only 40.03 ± 21.89.When it comes to FACT-P,the sum score,emotional well-being score and prostate cancer component score were 8.15 ± 3.72,12.30 ± 4.04 and 77.41 ± 9.95,respectively,which were less than half of their respective top score.However the physical well-being score was 20.41 ±4.29,which was a relatively satisfactory value.In addition,61% (121/200) patient selected not at all in item I am able to have and keep an erection.The score of SDS and SAS was 46.76 ± 8.29 and 43.25 ±9.69,respectively.And there were 23.0% (46/200) and 21.5% (43/200) patiens exhibited depression and anxiety,respectively.Conclusion In the present study,the prostate cancer patients receiving ADT showed acceptable HRQOL,but some patients sufferd from depression,anxiety and erectile dysfunction.
