1.Effect of survivin shRNA on chemotherapy resistance in GBC-SD cells

Objective To investingate the effect of survivin shRNA on chemotherapy resistance in GBC-SD cells. Methods They were divided into three groups of GBC-SD, GBC-SD/EGFP and GBC-SD/survivin. MTT assay was used to detect cell viability in the 3groups. mRNA and protein of survivin were tested by RT-PCR and Western blot. Then the cells were treated with proper construction DDP. The cell survival rate and IC_(50) were determined with MTT, cell apoptosis detected by FACS and the alteration of nucleus observed by TUNEL. Meanwhile, caspase-3 activity was determined using the colorimetric method. Results Cell viability was reduced remarkably in GBC-SD/survivin and survivin expression was decreased obviously. After being treated with DDP, cell survival rate and IC_(50) were decreased obviously in GBC-SD/survivin, apoptotic rate elevated remarkably compared with other groups. There were brownly nucleuses in three groups. Caspase-3 activity increased first and then decreased, but it exceeded in GBC-SD/survivin than that in other two groups. Conclusion The survivin shRNA can down-regulate the expression of survivin in GBC-SD cells remarkably and improve the sensibility to chemotherapy.
2.Relapse after hematopoietic stem cell transplantation: Report of two cases and review of literature

Objective To evaluate the effect of cytokine-induced killer cells (CIKs) as an adoptive immunotherapy option for treatment of leukemia relapse after allo-hematopoietic stem cell transplantation (allo-HSCT).Methods Two cases of infusion of donor CIKs in patients with leukemia relapse after allo-HSCT were retrospectively analyzed.Patient one relapsed 986 days (+986d) after HLA-matched unrelated donor allo-HSCT.Applications of chemotherapy only resulted in short term remission,but allo-CIKs were successfully expanded from the patient's peripheral blood mononuclear cells of donor origin.Totally five cycles of CIKs infusion were infused as an alternative of adoptive immunotherapy.Patient two had recurrent in the + 158d after HLA-matched sibling alloHSCT.At + 204d and + 294d,two cycles of CIKs which were expanded from donor peripheral blood mononuclear cells were infused.Results One cycle of CIKs was given to patient one after the application of chemotherapy to reduce the tumor burden,and the patient successively achieved complete remission.Again after additional four cycles of CIKs infusion,consistent remission was maintained during the following seven months.Patient two who had relapsed disease posttransplantation,achieved cytological complete remission after withdrawal of immunosuppressants and undergoing chemotherapy combined with G-CSF mobilized stem cell infusion.However,at + 187d,the patient suffered from side-effect of acute graft versus host disease and extramedullary infiltration.The symptoms were alleviated markedly after one cycle of CIKs infusion at + 204d.Moreover,the pain disappeared after an additional infusion at + 294d.And up to the present,the bone marrow aspiration showed complete remission while the extramedullary disease vanished.Conclusion The use of CIKs in the treatment of leukemia relapse after allogeneic bone marrow transplantation can be feasible and well tolerated.
3.Cleavage activity of anti-transforming growth factor ?_1 RNA by U1 snRNA chimeric ribozyme in vitro

AIM: To determine the cleavage activity of anti-transforming growth factor ?1 hammerhead ribozymes which was inserted into U1 small nuclear RNA in cell-free system.METHODS: The hammerhead ribozyme targeting against transforming growth factor ?1 was designed through the analysis of computer software.The ribozyme fragments were synthesized and cloned into the U1 snRNA ribozyme vector pZeoU1EcoSpe,which contained U1 snRNA promoter/enhancer and terminator.TGF ?1 cDNA partial fragment was generated by RT-PCR,and then cloned into the T-vector at the downstream of T7 promoter.The transcripts of ribozyme and target RNA incorporated into isotope were transcribed in vitro and purified by denaturing polyacrylamide gel electrophoresis.-labeled U1 snRNA chimeric ribozyme transcripts were incubated with target-RNAs at different conditions and autoradiographed after running denaturing PAGE.RESULTS: U1snRNA chimeric ribozyme(U1Rz803) cleaved TGF?1 mRNA efficiently and specifically at 37 ℃,while the disable ribozyme(U1Rz803m) showed no cleavage activity,so these indicated the design of U1Rz803 was correct.CONCLUSION: U1Rz803 prepared in this study possesses the perfect specific catalytic cleavage activity in cell-free system.These results indicate that U1 snRNA chimeric ribozyme U1Rz803 may suppress the expression of TGF?1 in vivo,therefore it may provide a new means for exploring the role of TGF?1 in hematopoietic regulation in the future.

Objective To observe the clinical efficacy of stage treatment withTiao He Ying Wei(regulating Ying-nutritional and Wei-defensive qi) needling in treating insomnia.Method A hundred insomnia patients presenting difficulty falling asleep were randomized into group A1 and B1, 50 cases each; 100 insomnia patients presenting difficulty maintaining sleep were randomized into group A2 and B2, 50 cases in each group; 100 insomnia patients presenting early-morning awakening were randomized into group A3 and B3, 50 cases each. Group A1, A2 and A3 were treated withTiao He Ying Wei needling, while group B1, B2 and B3 were treated with conventional medication. The Symptoms score and cerebral blood flow indicators were observed before and after the intervention.Result After the treatment, the symptoms scores were significantly changed in each group (P<0.05). The improvement of symptoms score in group A1 was superior to that in group B1 (P<0.05); the improvement of symptoms score in group A2 was superior to that in group B2 (P<0.05); the improvement of symptoms score in group A3 was superior to that in group B3 (P<0.05). The cerebral blood flow indicators (middle cerebral artery, posterior cerebral artery, anterior cerebral artery, and basilar artery) were significantly changed after the treatment in group A1, A2 and A3 (P<0.05). After the treatment, there were significant differences in comparing the cerebral blood flow indicators between group A1 and B1, A2 and B2, and A3 and B3 (P<0.05).Conclusion Stage treatment withTiao He Ying Wei needling can improve the sleep quality of insomnia patients.
6.Clinical study of Jianpi-Yishen-Tongluo capsules combined with candesartan cilexetil in the treatment of diabetic nephropathy

Objective:To evaluate the effect of Jianpi-Yishen-Tongluo capsules combined with candesartan cilexetil in the treatment of diabetic nephropathy (DN). Methods:According to the random table method, 82 patients who met the inclusion criteria in our hospital from January 2015 to December 2018 were divided into two groups, 41 in each group. The control group was treated with candesartan cilexetil, while the research group was treated with Jianpi-Yishen-Tongluo capsules based on the control group. Both groups were treated for 1 month. Then, the 24-hour urinary protein quantity (24 hPro) in urine, serum creatinine (SCr), blood urea nitrogen (BUN) were measured by automatic biochemical analyzer. The serum levels of visfatin (VF), glycosylated hemoglobin (HbAlc), interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α) were measured by ELISA. The adverse reaction during the treatment was observed and the clinical effect was evaluated. Results:The total effective rate of the research group was 97.6% (40/41), and 85.4% (35/41) of the control group, the difference between the two groups was statistically significant ( χ2=3.905, P=0.048). After the treatment, the 24 hPro (64.35 ± 6.39 mg vs. 96.32 ± 8.74 mg, t=18.908) in urine, SCr (86.84 ± 9.30 μmol/L vs. 124.34 ± 13.11 μmol/L, t=14.939), BUN (5.41 ± 0.59 mmol/L vs. 7.58 ± 0.71 mmol/L, t=15.052) in the research group were significantly lower than those of the control group ( P<0.01). The levels of VF and HbAlc in the research group were significantly lower than those of the control group ( t values were 10.595 and 13.140, respectively, all Ps<0.01), the serum levels of IL-6 and TNF-α were significantly lower than those of the control group ( t values were 10.071 and 6.969, respectively, all Ps<0.01). During the treatment, the incidence of adverse reactions was 12.2% (5/41) in the control group and 4.9% (2/41) in the research group, with no significant difference between the two groups ( χ2=1.406, P=0.236). Conclusions:The Jianpi-Yishen-Tongluo capsules combined with candesartan cilexetil can improve the renal function of DN patients, reduce the inflammatory reaction and improve the clinical effect.
7.The Predictive Value of Pre-therapeutic Serum Gamma-glutamyl transferase in Efficacy and Adverse Reactions to Neoadjuvant Chemotherapy among Breast Cancer Patients

Purpose:
Gamma-glutamyl transferase (GGT) has been reported as being involved in tumor progression. Previous studies documented a potential relationship between serum GGT level and survival outcome in several types of human malignancies. However, the association between serum GGT levels and response to neoadjuvant chemotherapy (NAC) has not yet been reported. The present study aimed to evaluate the association between pre-therapeutic serum GGT level and the efficacy, long-term survival, and adverse reactions of NAC and to investigate its role in predicting NAC sensitivity in patients with breast cancer.
Methods:
A total of 129 patients were recruited and stratified into 2 groups according to serum GGT level (< 29 U/L and ≥ 29 U/L). The association between pre-therapeutic serum GGT levels and clinicopathological parameters was examined. The correlation between pre-therapeutic serum GGT levels and pathological complete response (pCR) was analyzed using univariate and multivariate logistic regression. Survival analyses of relapse-free survival (RFS) and disease-free survival (DFS) were performed. Pearson's χ 2 test and multivariate logistic regression model were used to analyze the correlation between pre-therapeutic serum GGT levels and adverse reactions.
Results:
Pre-therapeutic serum GGT levels were associated with pCR among breast cancer patients treated with NAC. Multivariate analysis showed that low-level GGT significantly increased pCR rate. Patients in the high-level GGT group had poorer survival than those in the low-level GGT group. Subgroup analysis demonstrated that serum GGT level was potentially related to RFS and DFS in the hormone receptor-positive group. Low levels of GGT are significantly associated with a higher incidence of neutropenia.
Conclusion
Pre-therapeutic serum GGT level is an independent and novel biomarker for predicting the efficiency, prognosis, and adverse reactions to NAC in breast cancer patients.Patients with low pre-therapeutic serum GGT levels are more likely to have higher pCR rates, better RFS and DFS, and higher hematologic toxicity.
8.Epidemiological analysis of jellyfish stings in coastal bathing beaches in Qinhuangdao City from 2017 to 2019

Objective:To analyze the distribution and composition characteristics of jellyfish stings in various coastal baths in Qinhuangdao City from 2017 to 2019, and to provide scientific basis for the prevention, control and early warning of jellyfish stings.Methods:Statistics and analysis of the age, gender, time of stings, location of injury, first symptoms, and playing time in the sea at the time of the sting, etc. of people with jellyfish stings in various bathing beaches along the coast of Qinhuangdao from 2017 to 2019 (July to August) were conducted.Results:The number of jellyfish stings in the coastal bathing beaches of Qinhuangdao City in 2017, 2018, and 2019 was decreasing year by year, with 1 890, 492, and 171 cases respectively. Among them, Qianshuiwan Bathing Beach and Dongshan Bathing Beach had more stings (60.90% and 35.08% respectively in 2017, 24.39% and 64.23% respectively in 2018, 16.96% and 16.42% respectively in 2019). There was no significant change in the gender and age distribution of jellyfish stings each year [57.99% males in 2017, with a median age of 13 (8, 31) years old; 63.21% males in 2018, with a median age of 25 (8, 29) years old; and 59.65% males in 2019, with a median age of 12 (7, 31) years old]. Stings were mainly located at the lower limbs (the proportion of lower limb injuries: 46.54% in 2018, 45.61% in 2019), followed by upper limbs (upper arm, elbow, forearm), trunk, etc. The first symptom was mainly pain (89.43% in 2018, 38.29% in 2019), followed by rash (64.43% in 2018, 59.43% in 2019), numbness, blisters, etc. Sting incidents mainly occurred from 13:00 to 17:59 (the proportion of sting incidents in this time period in 2018 and 2019 were 68.09% and 52.63%, respectively).Conclusions:Jellyfish stings in coastal baths in Qinhuangdao City are mainly distributed in Qianshuiwan Baths and Dongshan Baths. The management of these sea areas should be strengthened, and scientific publicity and medical rescue should be strengthened to prevent jellyfish stings in peak hours and related baths.
9.Prevalence of diabetes mellitus and associated risk factors in Uygur population in Kashgar area of Xinjiang Uygur Autonomous Region

Objective To investigate the epidemiologic characteristics of diabetes mellitus (DM) in Uygur residents in Kashgar of Xinjiang.Methods The survey was conducted among the Uygur residents aged ≥ 18 years selected through stratified cluster sampling in Kashgar by means of questionnaire survey,physical examination and basic laboratory test.The prevalence of different groups were calculated and risk factors of DM was analyzed by logistic vegression model.Results A total of 4 608 adults were surveyed.The prevalence of DM was 11.31% (standardized prevalence:10.59%) and the prevalence was 13.65% (standardized prevalence:12.34%) in males and 10.04% (standardized prevalence:9.83%) in females.The prevalence increased with age.The prevalence of DM was higher than the prevalence of impaired fasting glucose (IFG) in people aged >60 years,especially in females.The rates of awareness,treatment and control of DM were 28.02%,21.31% and 5.57%,respectively.Multivariate logistic regression analysis indicated that people aged 45-55,55-65 and >65 years had higher risk of DM and the odds ratio were 2.08 (95%CI:1.24-3.48),2.73 (95%CI:1.63-4.56) and 3.90 (95% CI:2.24-6.78) for men and 2.63 (95% CI:1.71-4.02),3.14 (95% CI:2.00-4.94) and 5.56 (95%CI:3.47-8.92) for woman,respectively.Family history of DM (OR=2.88 for men,95%Cl:1.45-5.72;OR=2.52 for women,95%CI:1.49-4.26) and BMI≥28.0 kg/m2 (OR=1.77 for men,95%CI:1.19-2.64,OR.=1.80 for women,95%CI:1.30-2.50) were also risk factors for DM.Conclusion The prevalence of DM was high in Uygur residents in Kashgar,but the rate of awareness,treatment and control of DM were low.It is necessary to improve the detection rate of DM and conduct targeted prevention and control of DM.
10.Prevalence of dyslipidemia and normal blood lipids level in Uygur population in Kashgar area of Xinjiang Uygur Autonomous Region

Objective To understand the prevalence of dyslipidemia and normal blood lipids level in Uygur diabetes patients in Kashgar prefecture in southern area of Xinjiang.Methods A total of 5 078 local residents aged ≥ 18 years (42.56% were men) selected through cluster random sampling in Kashgar were surveyed by means of questionnaire survey,physical examination and laboratory test,and 521 diabetes patients were screened.Results The overall prevalence of dyslipidemia in diabetes patients was 59.50% (310/521) with adjusted rate as 49.39%.Age ≥65 years,overweight,obesity and abdominal obesity increased the risk for dyslipidemia by 0.771 times (95% CI:1.015-3.088),1.132 times (95%CI:1.290-3.523),1.688 times (95%CI:1.573-4.592) and 0.801 times (95%CI:1.028-3.155) respectively.Compared with males,female was a protective factor for dyslipidemia (OR=0.507,95%CI:0.334-0.769).The overall normal rate of blood lipids level including total cholesterol (TC),triglycerides (TG),high-density lipoprotein cholesterol (HDL-C) and low-density lipoprotein cholesterol (LDL-C) for type 2 diabetes patients was 11.13%.Female,higher BMI and abdominal obesity were the factors influencing the overall normal blood lipids level.The normal rate of LDL-C level decreased with increase of age,BMI and waist circumferences (trend test x2=18.049,P< 0.001;trend testx2=10.582,P=0.001;x2=19.081,P<0.001),but increased with educational level (trend testx2=9.764,P=0.002).Conclusion The prevalence of dyslipidemia in Uygur diabetes patients in Kashgar was high,however,the overall normal rate of blood lipid level was relatively low.Obesity was the most important risk factor for dyslipidemia in this area.More attention should be paid to dyslipidemia prevention in women.
