1.Progress of early diagnosis of kidney damage caused by Henoch-Schonlein purpura
International Journal of Pediatrics 2013;40(4):341-345
Henoch-Schonlein prpura(HSP) is systemic vasculitis, which involves multiple body organs. The main lesion is small vascular infammation.Kidney belongs to the organ composed of rich blood vessel,so it is involved easily. It is so difficult to find early slight damage of kidney that the treatment can't be given timely.Kidney biopsy and urine routine test and urine microscale protein detection is the main means of detection.Investigating diagnostic indicators of the early renal damage caused by HSP is critical.

Objective To investigate the clinical implication value of using Doppler ultrasound to detect renal hemodynamic changes in children with Henoch-Schonlein purpura. Methods Color Doppler ultrasound was used to detect renal hemodynamic changes of main renal arteries and intedobar renal arteries and arcs renal arteries in 40 cases of Henoch-Schonlein purpura, whose urine routine, microalbuminuria were also tested, and 32 healthy children served as control group. Results Maximum crest flow rate (Vmax) during systole, minimum crest flow rate(Vmin) during diastole and resistance index(RI) of main renal arteries and interlobar renal arteries of children in HSP group were all higher than those of control group, while Vmax,Vmin and RI of arcs renal arteries were almost the same.Color doppler ullrasound showed that 28 cases were abnormal in renal flow, which accounted for 70%.Fourteen cases with abnormality were detected in routine urine and 18 cases in microalbuminutia, which accounted for 35% and 45% of total cases respectively.Sensitivity of color doppler ultrasound was higher than microalbuminuria and routine urine, while the sensitivity difference was not statistically different between microall uminuria and routine urine. Conclusions Renal hemodynamics is a sensitive index to find out renal lesion of early stages in children with Henoch-Schonlein purpura.

Objective To investigate the relationship among Machiavellianism, loneliness and coping style in adolescents. Methods 850 junior middle school students were recruited and assessed with Kiddie Machiavellianism Scale,Self-esteem Scale, and Strengths and Difficulties Questionnaire (for students).Mediating effect was analyzed by multiple regression analysis. Results ① The scores of Machiavellianism, loneliness, positive coping and negative coping were (37.35±7.52),(30.21 ± 10.72),(21.07±6.12) and (9.44± 3.84) respectively.② Machiavellianism was positively correlated with negative coping style (r=0.192,P<0.01) and loneliness (r=0.263,P< 0.01),as well as negatively correlated with positive coping style (r=-0.244,P<0.01).③ Positive coping style partially mediated the effect of Machiavellianism on loneliness, and the mediating role accounted for 37.72% for the whole effect. Conclusion The higher the Machiavellianism is,the more serious the loneliness is.Positive coping style partially mediates the relations between Machiavellianism and loneliness.

Human kallikrein-1 (hK1) gene was cloned from kidney tissues cDNA, it was inserted into the plasmid pPICZalphaA, then the yeast expression vector pPICZalpha-hK1 was constructed. After transformed into Pichia pastoris host X33, high-level expression transformants were screened by escalating the concentration of Zeocin (from 500 to 700 microg/mL) of YPD plate and medium. When temperature was 30 degrees C, pH 6.0 with induction duration of 64 hours in the 30 L fermenter, the highest yield can reach about 6500 u/L (1.25 g/L). The variation of glycosylation resulted in two kinds of molecules, i.e. rhK1-H with a heavy molecular weight and rhK1-L with a light one. rhK1 was purified from the supernatant through Phenyl hydrophobic interaction, Cu(2+)-charged Chelating and Anion-exchange chromatography. 0.28 g rhK1-H and 0.62 g rhK1-L can be purified from one liter supernatant. The yield recovery was 72% with a purity of > 96%. So far our yield of rhK1 is superior than known recombinant expression method reported by other researchers.
