1.A clinical study on postoperative knee pain following treatment of tibial shaft fracture by intramedullary nailing

Objective To study the incidence and etiology of knee pain in patients with tibial shaft fracture after intramedullary nailing treatment. Methods From Jan 1997 to Dec 2002, 1332 patients with tibial shaft fracture were treated with intramedullary nailing in our hospital. Their clinical data were retrospectively studied. All the patients were followed up at least once after fracture healing, and the average clinical follow up duration was 27 months. Results 409 of the 1332 cases experienced knee pain. 31.9 percent of the patients treated with transtendinous nailing reported anterior knee pain, and 28.9 percent of the patients treated with paratendinous nailing reported knee pain. Conclusion Compared with the transpatellar tendon approach, the paratendinous approach for nail insertion does not reduce the incidence of anterior knee pain.
2.The impact of HCC family history on the prognosis of HCC patients after curative hepatectomy

Objective To evaluate HCC family history on recurrence and survival of HCC patients after curative hepatectomy. Methods The family history,postoperative recurrence rate,1,2 and 5 year survival rate of 79 HCC patients who received surgical resection were collected.Patients were divided into two groups:34 cases in familial aggregation HCC group (FH group ) and 45 cases in non- familial aggregation HCC group (NH group).All these HCC patients were followed up for 3 months to10 years after surgery,and the median follow-up period was 48.5 months.We analyzed the survival rate of HCC patients by Kaplan -Meier survival curve to compare the postoperative recurrence rate and survival rate between two groups. Results 34 cases of HCC probands came from 33 families,and there were 104 cases of HCC patients in these families,an average of 3.2 cases per family.There were 6 families in which 3 or more cases developed HCC,and there were 13 families in which father or (and) mother had HCC accounting for 39.4%.The six months,1 year,2 year posthepatectomy recurrence rate in FH group was 15.1%,22.3%,40.1%,significantly higher than NH group of 10.3%,17.4%,25.6% (x2 =5.762,6.434,5.987,P <0.05),and 1,2,5 year's survival rate in FH group was 57%,46%,40.3%,much lower than NH group of 85.3%,75.2% and61.5% (x2 =8.986,9.765,7.634,P<0.05). Conclusions Family history of HCC affects the prognosis of HCC patients after surgery. Patients with HCC family history have higher postoperative recurrence rate and worse prognosis than that of patients who do not have HCC family history.
3.The 40 Hz auditory steady-state response predicting the malignant process of middle cerebral artery territory infarction: a retrospective case series study

Objcctive To investigate the predictive value of the 40 Hz auditory steady-state response (ASSR) in patients with the malignant process of middle cerebral artery territory infarction.Methods The40 Hz ASSR and brainstem auditory evoked potential (BAEP) were performed within 72 hours after patients with middle cerebral artery terrtory infarction admitted in the neuro-intensive care unite (NICU).At the same time,the National Institutes of Health Stroke Scale (NIHSS) scores were assessed.Multivariable logistic regression analysis was used to deterrmine the influencing factors of the malignant process.The relevant indicators of the malignant process were analyzed by the receiver operating characteristic(ROC) curve in order to clear the predictive value of 40 Hz ASSR in the malignant process of middle cerebral artery territory infarction.Results A total of 104 patients with supratentorial middle cerebral artery territory infarction were included.They were divided into the malignant process group (n=59) or the non-malignant process group (n=45).There were significant differences in the baseline NIHSS scores (17.25 ± 7.23 vs.20.40 ± 8.09; t =- 2.055,P =0.043),infarct volume (105.85 ± 73.37 mm3 us.179.15 ± 144.38 mm3; t =-3.011,P =0.004),leukocyte count ([ 10.26 ±3.14] × 109/L vs.[ 13.45 ±5.42] × 109/L; t =-3.336,P =0.001),40 Hz ASSR (Z =-3.237,P =0.001),and short-latency somatosensory evoked potentials (Z =-3.130,P =0.002) grade between the malignant process group and the non-malignant process group.Multivariate logistic regression analysis showed that the40 Hz ASSR (odds ratio [OR] 3.347,95% confidence interval [CI] 1.630 -6.872; P=0.014),infarct volume (OR 1.006,95% CI 1.001 - 1.012,P=0.003),and leukocyte count (OR 1.277,95% CI 1.074 - 1.402; P =0.001) were the independent predictors in patients with the malignant process of middle cerebral artery territory infarction.When the 40 Hz ASSR was grade 3,the sensitivity and specificity of predicting malignant process were 39.5% and 94.4%.Conclusions The 40 Hz ASSR has an important predictive value in patients with the malignant process of middle cerebral artery territory infarction.
4.Genetic diversities of different geographical populations of Aedes albopictus based on mitochondrial gene COI

In order to figure out phylogenetic relationship and genetic diversity of different geographical populations,genetic analyses of Aedes albopictus were performed based on mitochondrial gene COI.Based on samples collected from most distribution regions in China,mitochondrial gene Cytochrome C Oxidase Subunit I was obtained through PCR and DNA sequence.Together with some COI sequences downloaded from GenBank,60 COI sequences with the final length of 598 bp were used for subsequent analyses.Results showed that there was no obvious divergence according to phylogenetic analyse,all sequences were clustered together in Maximum Likelihood tree.Sixteen haplotypes were detected,and four of them shared haplotypes.Haplotype diversity (Hd) was 0.737,nucleotide diversity (π) was 0.20 %.Population genetic differentiation analyses demon strated that Hainan population showed obvious divergences.In the network of haplotypes,H1 and H6 was found to be the primary haplotypes,and they formed two radical centers.All these results indicate that A.albopictus populations of China are expanding presently,and Hainan population become differential with other geographical populations,which probably attribute to geographical isolations.
5.Relationship between the nutritional status of vitamin A and HBV immune effect in infants

ObjectiveTo explore the relationship between the status of vitamin A and the level of AntiHBs in infants.Methods One hundred and ninty eight infants met the criteria were recruited into an investigation through local vaccination service.Two milliter venous blood was drawn from each studied child,and the infants' feeding information of successive 72 hours was collected through the 24-hour dietary recall method and 2-day diet records method.The nutrients intakes were analyzed with a NCCW software for calculation.The concentration of serum vitamin A and Anti-HBs was analyzed by HPLC and ECLIA,respectively.The logarithm of anti-HBs concentrations was compared among sub-clinical vitamin A deficiency (87 cases ),suspicious subclinical vitamin A deficient group (51 cases )and normal group (60 cases )and the relationship between the logarithm of anti-HBs concentrations and vitamin A were analyzed.ResultsThere were only 19.7% of children whose average daily intake of vitamin A reached the 80% of recommended nutrient intakes(RNI).The incidence rate of vitamin A malnutrition was 67.2%.The serum Anti-HBs GMC was 402.8 U/L The positive rate was 98.0%.There were significant differences on Anti-HBs Ig GMC between Sub-clinical vitamin A deficiency (SVAD) group,suspicious SVAD group and normal group( F =15.88,P < 0.001 ).The correlation coefficient between the level of serum vitamin A and Anti-HBs lg GMC was 0.441 ( P =0.01 ).ConclusionThe results show that the level of serum vitamin A may have influence on the maintenance of an effective anti-HBs level against hepatitis B virus.
6.The time-concentration relationship of serum bisphenol A in male rats.

OBJECTIVETo investigate the relationship of time-concentration of bisphenol A (BPA) in male Sprague-Dawley (SD) rats after single oral BPA administration.
METHODSA total of 66 specific pathogen free (SPF) SD male rats were divided into 10 experimental groups and control group (n = 6). The experimental group rats were treated with BPA of 300 mg/kg by oral gavage and blood samples were taken from one group at 0.5, 1, 2, 4, 6, 12, 24, 36, 60, 84 h time point after oral administration, respectively. The serum BPA concentration was determined by fluorescence-high performance liquid chromatography (FL-HPLC) analysis.
RESULTSAfter oral administration of 300 mg/kg, the total serum BPA concentration of 17.13 microg/ml was the highest in rats at 1 h, then decreased, but it increased to 15.18 microg/ml again at 24 h, then gradually decreased to 0.51 microg/ml at 84 h. The level of serum free BPA was lower than that of total serum BPA after oral administration, the serum free BPA was 0.57 microg/ml at 0.5 h after oral administration. The serum free BPA level decreased to 0.06 microg/ml at 1 h, 0.03 microg/ml at 4 h, 0.01 microg/ml at 36 h after oral administration. The free BPA was only 4.15% (0.57/13.73) in total BPA in serum at 0.5 h after oral administration of 300 mg/kg BPA.
CONCLUSIONThese results suggested that conjugated BPA was the main metabolite of BPA in rat serum after single oral administration. Enterohepatic circulation of BPA glucuronide in rats may results in two peak levels of total BPA in serum.

7.The association of paraoxonase 2 gene C311S variant with ischemic stroke in Chinese type 2 diabetes mellitus patients.

OBJECTIVETo investigate the association between the C311S polymorphism of paraoxonase 2 (PON2) gene and ischemic stroke in Chinese type 2 diabetes mellitus (T2DM) patients.
METHODSA case-control study of 279 Chinese subjects (including 162 T2DM with or without ischemic stroke and 117 non-diabetic control) was performed. Genotype frequencies of C311S polymorphism were studied by polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP) analysis with DdeI digestion.
RESULTSC311S polymorphism of PON2 gene was detected in Chinese with the C/S allele frequencies 0.145 and 0.855. The frequency distribution showed significant difference between Chinese and Asian Indian. Furthermore, the genotype distribution (SS, CS and CC) of the PON2 C311S gene polymorphism exhibited a significant difference between T2DM patients complicated with ischemic stroke and T2DM without ischemic stroke, the former had a significantly higher C allele frequency(P<0.05).
CONCLUSIONThe above data indicate that the polymorphism at codon 311(Cys --> Ser)in the PON2 gene is associated with ischemic morbidity in Chinese T2DM patients and C allele might be a risk factor.

OBJECTIVETo investigate the association between plasma adiponectin and insulin resistance in OLETF rats.
METHODSTwenty male Otsuka Long-Evans Tokushima Fatty (OLETF) rats and 10 male Long-evans Tokushima Otsuka (LETO) rats underwent oral glucose tolerance test (OGTT) at 13 and 40 weeks of age. At 8, 32 and 40 weeks of age, the rats were sacrificed to measure the blood glucose, plasma insulin and adiponectin levels, and serum levels of TG, CHOL and FFA.
RESULTSThe plasma adiponectin level was significantly decreased in 8-week-old OLETF rats compared with that of LETO rats (P<0.05). The plasma insulin level, TG, CHOL, and FFA were significantly higher in OLETF rats than in LETO rats at 32 and 40 weeks of age.
CONCLUSIONA decreased plasma level of adiponectin preludes insulin resistance and is inversely correlated to insulin sensitivity. Hypoadiponectinemia may be an important reason leading to insulin resistance.

10.Adiponectin decreases insulin receptor substrate-1 phosphorylation in the liver of OLETF rats possibly through nuclear factor-κB signaling pathway.

OBJECTIVETo investigate the effect of adiponectin (APN) on the insulin pathway in the liver of OLETF rats and explore its molecular mechanism.
METHODSTwenty male OLETF rats and 10 male LETO rats were sacrificed at 8 and 32 weeks of age to examine the fasting blood glucose, serum insulin, adiponectin and blood lipid profiles. The APN, phosphotyrosine of insulin receptor substrate-1 (IRS-1), IKKβ and nuclear-κB (NF-κB) in the liver tissue were determined using ELISA, Western blotting or immunohistochemistry.
RESULTSThe plasma adiponectin level in OLETF rats was significantly lower than that of LETO rats since 8 weeks of age (P<0.01). At 32 weeks of age, the blood lipid levels of OLETF rats increased significantly (P<0.05) with inverse correlations to plasma adiponectin (P<0.01). The liver APN, py-IRS-1, IKKβ and NF-κB levels in OLETF rats differed significantly from those of LETO rats at both 8 and 32 weeks. At 32 weeks of age, the APN level of both rats were correlated to the levels of NF-κB and py-IRS-1 (P<0.01).
CONCLUSIONAPN may decrease tyrosine phosphorylation of IRS-1 via the IKK/NFκB pathway and inhibit insulin signaling pathway in the liver, which contributes to hyperlipidemia, hyperglycemia and development of type 2 diabetes.
