1.Concentrations of Morphine in Colostrum and Plasma During Postcesarean Epidural Anesthesia

Objective To study the effect of morphine used in epidural anesthesia on puerpera after cesarean section and the safety of morphine to neonate. Method One hundred puerpera undergone cesarean section were randomly divided into test group and control group with continuous 2% lidocaine epidural anesthesia, as soon as the operation were finished, 2 mg morphine was injected into vacum epidurale for test group, nothing for control group. Colostrum and plasma, urine samples of puerpera and neonate were collected, morphine and metabolite level were tested by GC-MS and FPIA. Result Morphine concentrations ranged from 0.05). Conclusion As the epidural analgesia medicine after cesarean section, morphine has no side-effect to neonate and is safe to neonate.

5.Levels and values of serum uric acid in patients with multiple system atrophy

Objective To investigate the level and value of serum uric acid (UA) in patients with multiple system atrophy (MSA).Methods One hundred and one MSA patients were selected as the MSA group and 101 age and sex-matched healthy controls as control group.The UA concentrations and other biochemical indexes were measured by an enzymatic method and carefully analyzed.By comparing the UA concentrations for different genders, subtypes, and different levels of certainty of the MSA group with their counterparts in the control group, we obtained the differences between these two groups and also discussed possible reasons and factors.Results The serum UA levels of MSA patients ((283.74 ± 87.76) μmol/L) were significantly lower than the controls ((317.86 ± 76.95) μmol/L;t =-2.94, P < 0.05).In addition, when the male and female groups were investigated separately, the decrease of serum UA was more significant in the male group compared to the female group (t =-3.88, P < 0.01).Furthermore, by comparing different subtypes of the MSA patients with the control group, we found that the serum UA levels in both P-type or C-type MSA patients were significantly lower than in controls (t =2.92, 2.02;all P < 0.05).By comparing different levels of MSA with the control group, we obtained that UA levels in both probable MSA patients and possible MSA patients were significantly lower than in controls (t =3.13, 2.09;all P < 0.05).By adjusting for the confounding factors, such as age, alanine aminotransferase, aspartate aminotransferase, urea nitrogen, creatinine, high density lipoprotein-cholesterol and low density lipoprotein-cholesterol, the multiple Logistic regression analysis showed that the numbers of MSA patients in the lowest level UA group (UA < 263.0 μmol/L) were higher than the high level UA group (UA > 370.5 μmol/L), though the difference did not reach statistical significance (P > 0.05).Nevertheless, the numbers of male MSA patients in the lowest level and the lower level UA groups (UA =263.0-309.0 μmol/L) were 11.5 and 10.8 times that in the high level UA group (P =0.001, P =0.003).No obviously linear relation between the level of UA and disease progression was found.Conclusions The level of serum UA is significantly decreased in MSA patients, especially in the male patients.The UA, as an antioxidant, may be beneficial to the neurodegenerative diseases.
6.Study of relationship between liver function and the enhancement degree of liver parenchyma in the hepatobiliary phase with Gd-BOPTA

Objective To explore the relationship between the MRI enhancement ratios of liver parenchyma in hepatobiliary phase with gadobenate dimeglumine (Gd-BOPTA)and liver function.Methods Fifty-nine patients who underwent Gd-BOPTA-enhanced MRI were retrospectively enrolled in the study.The enhancement ratio of signal to noise ratio and enhancement ratio of the contrast ratio were calculated.The relationships between the enhancement ratio and CTP grading and MELD score were analyzed.Results The signal enhancement ratios in hepatobiliary phase in patients with CTP A classification were higher than those with CTP B classi-fication (P <0.01).Meanwhile,the ratios in patients with MELD scores less than 10 points were higher than those with MELD scores more than 10 points (P <0.01).Conclusion The MR enhancement degree of liver parenchyma in the hepatobiliary phase with Gd-BOPTA may reflect the liver function.
7.A study of maxillary canine impaction and root resorption of adjacent incisor in three-dimensional space

Objective:To localize the impacted maxillary canine and to observe the root resorption of adjacent incisor in 3 dimen-sions using cone beam computed tomography(CBCT).Methods:92 impacted maxillary canines in 63 patients were scanned by CBCT.The three-dimensional images were obtained by multiplanar reconstructions(MPR).The cusp tip of each impacted canine was localized and set to the X,Y and Z planes after the observation of sagittal,coronal and axial views.The root resorption of adjacent in-cisors was observed.Results:In the X-axis,92% of impactions were mesial by 1 0.4 to 1 5.1 mm,8% distal by 2 to 5 mm;in the Z-axis,60% of impactions were palatal by 1 to 4 mm,40% buccal by 0 to 4 mm.41 .3% of the impacted canines were without root resorption of adjacent incisor,36.5% with slight,1 4.3% with moderate and 7.9% with severe root resorption of the adjacent inci-sor.Conclusion:The most frequent location of impacted maxillary canine is palatal and mesial with high incidence of root resorption of adjacent incisor.

10.Effect of dexamethasone on pulmonary diffusion in patients undergoing cardiac valve replacement under cardiopulmonary bypass

Objective To investigate the effect of dexamethasone on the pulmonary gas exchange in patients undergoing cardiac valve replacement under cardiopulmonary bypass(CPB).Methods Forty ASAⅡorⅢpatients aged 29-47 yrs weighing 50-69 kg undergoing cardiac valve replacement under CPB were randomly divided into 2 groups(n=20 each):dexamethasone group received dexamethasone 0.5 mg?kg~(-1) after induction of anesthesia and control group received normal saline(NS).Blood samples were taken before operation(T_0) immediately before CPB(T_1)and immediately after discontinuation of CPB(T_2)for determination of plasma total and active matrix metallo-proteinase-9(MMP-9)concentration(by enzyme-linked immuno-absorbent and fluorometric enzyme-linked immuno-absorbent assay respectively)and MMP-9 gene expression(RT-PCR).Blood samples were taken from radial artery at T_1 and T_2 for blood gas analysis.A-aDO_2 was calculated.Results MMP-9 gene expression and plasma total and active MMP-9 concentrations were significantly increased at T_2 as compared with those at T_0 in both groups and were significantly lower in dexamethasone group than in control group(P<0.05 or 0.01).The A-aDO_2 at T_2 was significantly smaller in dexamethasone group than in control group.Conclusion Dexamethasone can inhibit the increase in gene expression,protein synthesis and activation of MMP-9 and decrease in gas exchange across alveolar-capillary membrane caused by CPB and protect the lungs during open heart surgery performed under CPB.
