1.Comparative study on home-based care effects of disabled centenarians

Objective To evaluate the effect of home-based care alongside with health promotion on Wechat towards disabled centenarians, to provide the basis for improving the home and community based care towards the oldest old. Methods A total of 158 indigious centenarians in Hainan were surveyed and 78 of them who were considered disabled by Barthel criteria were selected and then divided into intervention group (with health education on Wechat platform and nursing visit) and control group (with nursing care handbook) by the serial number of them. The living quality,incidence of adverse health events and positive feeling of family care givers of two group were evaluated and compared after 6 months intervention. Results After 6 months of home- care intervention, significant statistical differences in incidence of adverse health events were found between intervention group and control group, which compromise falls (2 cases vs. 13 cases, χ2 = 8.91, P = 0.00), pain (1 case vs. 17 cases, χ2 = 17.28, P =0.00) and constipation (6 cases vs. 15 cases, χ2 = 4.75, P = 0.03). When it came to positive feeling of care givers, the scores of intervention groupwere (33.44±3.72), (36.46±4.06) points, control groupwere (28.10± 3.25), (28.15±3.15) points, significant differences were found at 3rd and 6th month′s follow-up between two groups, which t value represent 4.955 (P=0.00) and 10.142 (P=0.00), respectively. It also suggested that the differences in living quality of the elderly were statistical significant when refered to discomfortble or pain scored 1.58±0.50 and 2.37±0.63 (t=-6.154,P=0.00), and anxiety or depression scored1.73±0.64 and 2.32 ± 0.70 (t=- 3.889,P=0.00). Conclusions Health education on Wechat platform with nursing visit can enhance the effect of home- based care by improving the living quality, incidence of adverse health events of the elderly and positive feeling of family care givers.
2.Surgical treatment for pulmonary carcinoid tumors: a single-center analysis of 62 patients

ObjectiveTo evaluate prognostic factors and the surgical results of pulmonary carcinoid tumors.Methods We retrospectively reviewed the medical records of 62 patients who were diagnosed as pulmonary carcinoid tumors between January 2000 and October 2010 at Department of Thoracic Surgery,Shanghai Chest Hospital.The following information was available for each of the 62 patients:age,sex,pathological type,and TNM stage.ResultsThere were no operative death.The 3-year and 5-year survival rates after surgery were 92.1% and 77.8%,respectively.Of the 62 patients,42 were diagnosed as typical carcinoid tumor,and among them,4 patients (8.3%) had lymph node metastases.Their 3-year and 5-year survival rates were 97.8% and 94.7%,respectively.The remaining 20 patients were diagnosed as atypical carcinoid tumor,and among them,6 patients (37.5%) had lymph node metastases.Their 3-year and 5-year survival rates were 84.4% and 58.8%,which were statistically significant compared with typical carcinoid tumor( P =0.0047 ).There was significant difference in survival rate between the patients with lymph node metastases and the patients without lymph node metastases (P =0.0048).CondusionThe main risk factors affecting survival rate of those patients who were diagnosed as pulmonary carcinoid tumors were pathological types and lymph node metastases.
3.Expression and significance of estrogen receptor alpha in renal tissue of IgA nephropathy

Objective To investigate the expression and significance of estrogen receptor alpha in renal tissue of immunoglobulin A (IgA) nephropathy.Methods Fifty renal tissue samples including forty five cases of IgA nephropathy and five cases of normal expression were collected.The expression of estrogen receptor αt (ERα) was detected by immunohistochemistry method,and its relationship with clinical parameters and the degree of glomerular damage were analyzed.Results ERα was located in the glomerular and renal tubules.With the severity of the lesion,the expression of ERα in renal tissue of IgA nephropathy decreased gradually (P =0.001) and its expression was associated with estimated glomerular filtration rate (eGFR),serum creatinine (Scr),and pathologic grade of IgA nephropathy (r =0.876,-0.818,and -0.736,P < 0.05),The expression of ERα was significantly decreased in hypertensive patients with IgA nephropathy,and the difference was statistically significant compared with that in non-hypertensive group (P =0.011).Conclusions The expression of ERα in renal tissue of IgA nephropathy was significantly decreased and there was a correlation between the degree of renal tissue damage,suggesting that ERα might play a role in the development of renal disease.
4.Study of regulation of miR-21 on ADAMTS-1 expression

Objective To investigate the regulatory effect of miR-21 on ADAMTS-1 expression in pulmonary fibrosis model. Methods A mouse model of pulmonary fibrosis was established using bleomycin (BLM) anda model of pulmonary fibrosis was constructed in vitro , the expression level of miR-21 was measured by quantitative real-time polymerase chain reaction (qRT-PCR), while the protein expression of ADAMTS-1 was measured by Western blot. NIH3T3 were transfected with miR-21 mimics and inhibitor in vitro and the cellular expression of ADAMTS-1 was measured by Western blot. Results Compared with the blank group , in mouse models of pulmonary fibrosis , the miR-21 expressions in lung tissues at three time points after BLM-treatment were significantly up-regulated while an evident decrease in ADAMTS-1 expressions were observed (P < 0.01). In vitro pulmonary fibrosis model , NIH3T3 cells after TGF-β1 in concentration 5 μg/L stimulation down-regulated ADAMTS-1 expression and up-regulated miR-21 expression (P < 0.01). NIH3T3 transfected with miR-21 mimics and inhibitor, up-regulated miR-21 expression, while down-regulated ADAMTS-1 protein expression. Conclusions Up-regulation of miR-21 and Down-regulation of ADAMTS-1 might be involved in the progression of pulmonary fibrosis model; miR-21 could negatively regulate ADAMTS-1 expression.
5.Treatment of radial neck fracture in children with elastic stable intramedullary nails.

OBJECTIVETo investigate effects of elastic stable intramedullary nails for the treatment of radial neck fracture in children.
METHODSFrom July 2006 to December 2011, 25 children with radical neck fractures, which included 16 males and 9 females aged from 7 to 15 years old (means 10.7), were treated with elastic stable intramedullary nails. According to Judet classification, 6 cases were type II, 17 cases were type III and 2 cases were type IV (including 1 case with type IVa and 1 case with type IVb). The fracture healing, pain, deformity and range of motion of elbow were recorded.
RESULTSAll patients were followed up for 6 to 24 months with an average of 14 months. Twenty-five patients were obtained bone healing. According to Tibone and Stoltz evaluation standard, 18 cases got excellet results, 4 cases in good and 3 cases in moderate.
CONCLUSIONElastic stable intramedullary nails for the treatment of radial neck fracture in children has advantages of simple operation,less trauma and good results.

6.LncRNAs expression profile of ankylosing sporidylitis revealed by microarray

Objective To investigate the expression profile variation of long non-coding RNAs (lncRNAs) in ankylosing sporidylitis (AS) and explore the role of lncRNAs in the pathogenesis of AS.Methods The peripheral blood mononuclear cells of AS patients and health controls (HC) were used to detect for differently expressed lncRNAs by microarray.The roles of lncRNAs were predicted with GO and pathway analysis.The results were verified by real time-polymerase chain reaction (PCR).Results A total of 148 lncRNAs and 134 mRNAs were detected,which had more than 2-fold differentially expressed in AS patients.Bioinformatics analysis found that GO term enrichment included protein binding,regulation of transcription,metabolism,signal transduction,et al.and might involve in toll-like receptor pathway,protein kinase,complement pathway,notch signaling pathway and so on.The expressions of three lncRNAs were estimated by real time-PCR which found that consistent with that of microarrays.Among these,D90064 was the most aberrantly expressed lncRNAs.Conclusions Several lncRNAs expression was changed significantly in AS patients in comparison with HC,which implies that those different lncRNAs may have an important role in the development and progression of AS.
7.Effects of thrombopoietin and thrombopoietinⅡ on human platelet activation

AIM:To investigate effects of thrombopoietin(TPO) and TPOⅡ on human platelet activation in vitro. METHODS:Human platelets were incubated in the phosphate-buffered saline containing rhTPO or TPOⅡ at the concentration of 100 μg/L for five minutes. In order to determine the rate of platelet activation. The CD62P and CD41 expressions on platelets were analysed by flow cytometry using fluorescence labelled monoclonal antibody to CD62P and CD41. RESULTS:The results demonstrated that expression of CD62P on platelets which were incubated with rhTPO or TPOⅡ didn't increase compared with that of contrast group. CONCLUSION:Both rhTPO and TPOⅡdidn't cause the disorder of platelet activation.

9.Effect of chaishu sijun decoction on the gonad axis of gan-qi stagnation, Pi deficiency, and gan-qi stagnation pi deficiency model rats.

OBJECTIVETo observe changes of gonad functions of Gan-qi stagnation (GS), Pi deficiency (PD), Gan-qi stagnation Pi deficiency (GSPD) model rats, and the effect of Chaishu Sijun Decoction (CSD) on them.
METHODSRats were randomly divided into 7 groups according to romdom digit table, i.e., the normal control group, the GS model group, the GS medication group, the PD model group, the PD medication group, the GSPD model group,and the GSPD medication group, 10 in each group. Rats in the GS model group, the PD model group, and the GSPD model group were treated with chronic restraint, improper diet +excessive fatigue, chronic restraint +improper diet +excessive fatigue. The model was established for 4 successive weeks. Starting from the 15th day of modeling, CSD at the daily dose of 3.57 g/kg was given by gastrogavage to them for 14 successive days. Equal volume of distilled water was given by gastrogavage to rats in each model group and the normal control group for 14 successive days. The blood contents of gonadotrophin releasing hormone (GnRH), follicular stimulating hormone (FSH), luteinizing hormone (LH), estradiol (E2), and testosterone (T) were detected in rats of each group.
RESULTSCompared with the normal control group, there was statistical difference in GnRH, T, E,, and FSH in the GS, PD, and GSPD model groups (P < 0.05, P < 0.01). The content of LH was elevated in the GS model group (P < 0.05) and declined in the GSPD model group (P < 0.01). Compared with the GS model group, the contents of FSH, LH, and T decreased and E2 increased in the PD model group (all P < 0.05); the contents of FSH and LH also declined in the GSPD model group (P < 0.05). Compared with the PD model group, the T content increased and FSH decreased in the GSPD model group (all P < 0.05). Compared with each corresponding model group, the FSH content decreased (P < 0.01) and LH increased in the GS medication group; the T content increased, E2 and LH decreased (P < 0.05, P < 0.01) in the PD medication group; the T content decreased (P < 0.01), GnRH, E2, FSH, and LH increased (P < 0. 05, P < 0.01) in the GSPD medication group.
CONCLUSIONSThere exist different degrees of abnormal function of the gonad axis in the GS, PD, and GSPD models. CSD had certain regulatory effect on the 3 syndromes. Of them, it showed a more comprehensive role in improving the gonad function axis. Results of this experiment had provided the experimental evidence for higher correlation between CSD and GSPD syndrome.
