1.Application of extubating with negative pressure suction in endotracheal tube extubation

ObjectiveTo investigate the feasibility,safety and efficacy of tracheal extubation with directly connected negative pressure suction. Methods86 patients with Ⅰ or Ⅱ degree of ASA,aged 18~65 years old,without history of respiratory disease,difficult airway and ventilation problems were involved in this study.All patients undergoing endotracheal general anesthesia were randomly divided into group A and B with 43 cases in each group.Group A used negative pressure directly connected to external side of the endotracheal tube before extubation.Group B used suction tube putting into the endotracheal tube before extubation.MAP,HR,ECG,SpO2,the condition of cough and expectoration and complicatons after extubation were recorded. ResultsIn group A the MAP,HR slightly increased,SpO2 slightly decreased,ECG showed no significant change,but the differences were not significant.In group B,MAP,HR,SpO2 showed significant difference compared with those before extubation and the corresponding period of group A. ConclusionsThe method that connecting negative pressure suction with endotracheal tube can effectively reduce the cardiovascular response during extubation,It also can avoid hypoxia,reduce the tracheobronchial injury and pulmonary complications.It's a practical good method in tracheal extubation.
2.Adverse Drug Reaction Induced by Cemetidine:Literature Analysis of 306 Cases

OBJECTIVE:To study the general pattern and characteristics of adverse drug reactions (ADR) induced by cimetidine. METHODS: A total of 306 ADR cases induced by cimetidine reported by domestic journals of 1983~2007 were analyzed statistically. RESULTS: The clinical manifestations of ADR induced by cimetidine were complicated and various and characterized by lesions of central nervous system (27.78%), cardiovascular system (21.24%), endocrine disturbance (8.82%) and hematological system (8.82%) etc. CONCLUSION: Great importance should be attached to the ADR induced by cimetidine to guard against the serious ADR such as aplastic anemia, cardiac arrest and allergic shock.

OBJECTIVE:To discuss the thoughts and methods for the formation of solid Chinese traditional medicine preparation.METHODS: The thoughts and methods for the formation of solid Chinese traditional medicine preparation were introduced and summarized.RESULTS & CONCLUSION: The design for the formation of solid Chinese traditional medicine preparation was an important part in the preparation process of Chinese materia medica preparations,which should be consummated in the future study so as to enhance the quality of the solid Chinese traditional medicine preparations and facilitate the modernization of traditional Chinese medicine.
4.Retrospective Analysis on the Adverse Drug Reactions(ADR) Induced by Second Generation Antihistamine Drugs

OBJECTIVE:To analyze the clinical features and correlation factors of ADR caused by second generation antihistamine drugs for references of clinical rational drug use.METHODS:A total of 128 cases ADR cases induced by second generation antihistamine drugs that met our criteria were obtained by retrieving the domestic literature database between 1986 and 2006,which were categorized and analyzed in accordance with the ADR categorization stipulated by WHO international drug monitoring cooperation center.RESULTS:Of the 128 ADR cases,more were seen in male than in female,38 presented cardiovascular system lesions,accounting for 29.69%,followed by lesions of skin and its appendants(28.13%) and systemic reactions(20 cases,15.63%).However,the prognosis of these ADR was satisfactory.CONCLUSION:The ADR caused by second generation antihistamine drugs are associated to many factors such as patients’ sex and physical condition as well as drug combination etc.Clinic should be alert to such ADR such as allergic shock and arhythmia in patients treated with second generation antihistamine drugs.
5.Anticoagulation of Warfarin Therapy in Patients After Mechanical Valve Implantation

OBJECTIVE:To discuss the influencing factors of anticoagulation with warfarin in patients who had undergone artificial mechanical heart valve replacement and the relation between dosage regimens and international normalized ratio(INR).METHODS:123 patients who had undergone artificial valve replacement patients were followed-up.The patients were treated with warfarin therapy for anticoagulation after surgery,and given a regular follow-up.Patients' age,gender,and INR value etc were recorded and the adverse events including bleeding,embolism and death were observed.RESULTS:No death case was reported;femoral vein thrombosis occurred in 1 case;gingival bleeding occurred in 11 cases and urinary tract bleeding was noted in 3 case.The incidence of anticoagulation-associated ADR was close to that reported in overseas literature.CONCLUSION:According to INR,individual dose of warfarin should be administered on day 5 after artificial valve replacement.Maintaning INR at 1.5~2.0 can achieve certain anticoagulation efficacy and is relatively safe.
6.Introduce of the Benchmarking Framework of Unit Dose Dispensing

OBJECTIVE:To supply the reference for benchmarking framework of unit dose dispensing(UDD).METHODS:To introduce the running module and the training mode for UDD pharmacists of the UDD management mode,and the working quality was controlled using 6? management pattern.RESULTS & CONCLUSIONS:The benchmarking framework of UDD is feasible in practice and it can help guarantee dispensing accuracy,improve the quality of pharmaceutical care.
7.Operation Mode for the Management of Donated Medicines

OBJECTIVE: To discuss the optimal operation mode for the management of donated drugs.METHODS: The possible problems existed in the management of donated medicines were found out through summarizing our practical experience in the inventory,storage and use etc of the donated drugs.RESULTS & CONCLUSIONS: In the management of donated medicines,emphasis should be attached to the inventory,storage and use of the donated medicines,meanwhile,a sound supervising system for the donation affairs should be set up to standardize the donation behavior,strengthen expiration date management and gradually establish the practical and effective work mode.
8.Analysis of Drug Utilization in 16 Hospitals of Chengdu Area During 2001~2006

OBJECTIVE:To analyze the utilization status and developing trend of drugs in Chengdu area. METHODS:Data regarding the drug variety,consumption quantity and sum of money in 16 hospitals of Chengdu area between 2001 and 2006 were analyzed retrospectively. RESULTS:The drug consumption sum in 16 Hospitals of Chengdu area from 2001 to 2006 increased by 19.56%,27.19%,17.84%,15.08% and 10.40%,respectively,assuming an downtrend. Among the top 14 chief drug categories on the list of consumption sum,the sequence of anti-infective agents,cardiovascular agents,antineoplastic agents and nervous system agents experienced no obvious changes. CONCLUSION:The drug utilization in Chengdu area tends to be rational and standard.
9.Assay Method for Sustained-Release Palmatine Tablet in Beagle Dog

OBJECTIVE:To establish an assay method for Sustained-release palmatine tablet in beagle dog. METHODS:Sample separation was performed on Diamonsil ODS C18 column(250 mm?4.6 mm,5 ?m) with mobile phase consisted of acetonitrile-water (1∶1,containing NaH2PO4 3.4 g and SDS 1.7 g in every 1 000 mL water,pH was adjusted to 3 by H3PO4) with a flow rate of 1.0 mL?min-1.The column was kept at 35 ℃ and the detective wavelength was set at 342 nm.RESULTS:The linear serum concentration range was 0.005~0.5 ?g?mL-1(r=0.999 9) and the recovery rate was(102.22?2.48)%. CONCLUSION:The method was proved to be simple,accurate,sensitive and reproducible,and it was up to the standards for the study of pharmacokinetics and bioavailability of Sustained-release palmatine tablet in beagle dog.
10.Using Six Sigma Method to Reduce the Defects in Drug Delivery

OBJECTIVE:To improve the drug delivery procedures and reduce the defects occurred in drug delivery.METHODS:The five procedures of six sigma including defining,measuring,analyzing,improving and controlling were applied to improve the drug delivery in our hospital's single dose dispensing(UDD)center.RESULTS:Due to the application of the six sigma method,the defect rate of drug delivery dropped from 25.1%(before the initiation of the six sigma method)to 5%(after the initiation of the six sigma method),which was significant lower as compared before the initiation of the six sigma method.CONCLUSION:Six sigma method contributed to the reduction of defect rate of drug delivery,so it deserves to be widely used in each management link in UDD center so as to improve the quality of dispensing.
