2.Ophthalmic imaging features of posterior scleritis

AlM: To analyze, summarize and describe ophthalmic imaging features of posterior scleritis.
METHODS: Clinical data of 16 patients ( 21 eyes ) with posterior scleritis diagnosed in our hospital from October 2008 to June 2013 were retrospectively analyzed. The results of type-B ultrasonic, fundus chromophotograph, fundus fluorescein angiography, CT were recorded for comprehensive evaluation and analysis of ophthalmic imaging features of posterior scleritis.
RESULTS: All patients underwent type-B ultrasonic examination and manifested as diffuse and nodular types. The diffuse type showed diffusely thickened sclera and a dark hypoechoic area that connected with the optic nerve to form a typical “T”-shaped sign. The nodular type showed scleral echogenic nodules and relatively regular internal structure. FFA showed that relatively weak mottled fluorescences were visible in the arterial early phase and strong multiple needle-like fluorescences were visible in the arteriovenous phase, which were then progressively larger and fused; fluorescein was leaked to the subretinal tissue in the late phase;varying degrees of strong fluorescences with less clear or unclear boundaries were visible in the optic disk. CT results showed thickened eyeball wall.
CONCLUSlON: Posterior scleritis is common in young female patients, whose ophthalmic imaging features are varied and more specific in type-B ultrasonic. Selection of rational ophthalmic imaging examination method, combined with clinical manifestations, can accurately diagnose posterior scleritis and avoid the incidence of missed and delayed diagnosis.
3.Weight-reducing Drug-induced Adverse Drug Reactions:Analysis of 462 Cases

OBJECTIVE:To study the adverse drug reactions(ADRs)induced by weight-reducing drugs and to provide references for rational administration in clinics.METHODS:Classification statistic and analysis were conducted on462cases with weight-reducing drug-induced ADRs reported in62kinds of domestic and overseas medicinal periodicals published during the period of1980~2004.RESULTS:With pseudo-5-HTA appetite-suppressing drugs showing the high incidence of ADRs,which stood at36.36%of the total.Multi-systems were involved in ADRs,with the cardiovascular system showing the highest proportion(53.68%).The prognostic mortality accounted for18.61%of the total.CONCLUSION:ADRs induced by weight-reducing drugs were associated with many factors,to avoid the occurrences of which,medical workers should give more directions and monitoring on administration of drugs.

5.Clinical and epidemiological characteristics of influenza-associated respiratory infection in children hospitalized in Shanghai Children's Hospital during 1999-2008

Objective To study the clinical and the epidemiological features of hospitalized children with influenza virus infection. Methods Two hundred and fifty-three inpatients with laboratory-confirmed influenza virus infection from 1999 to 2008 were reviewed for analyzing the clinical and epidemiological characteristics. Type A and B influenza viruses in the nasopharyngeal aspirates were detected by immunofluorescence assay. Mann-Whitney U test were performed for comparing the median age and the length of hospitalization. Chi-square test was performed for comparing the proportion of patients with fever and cough. Results Among 253 hospitalized children aged between 5 days and 127 months, 182 (71.9%) were boys and 71 (28. 1%) were girls. The median age was 18 months. Fifty-three cases were infants younger than 6 months. 95 cases were children aged between 6 months and 2 years, 85 cases were aged between 2 years and 5 years and 20 cases were older than 5 years. The diagnosis of influenza-related admission included pneumonia (190 cases), bronchitis (49 cases) and upper respiratory tract infection (14 cases). Eleven cases developed febrile convulsion, 6 cases had acute exacerbation of asthma and 3 cases had concomitant viralencephalitis. Twenty-nine cases had basic diseases. Cough and fever were the most common symptoms. Two hundred and thirty-eight cases presenting cough and 209 case presenting fever. Sixty-seven percent (140/209) had high fever with body temperature higher than 39 ℃. The average duration of fever was (5. 0 ±2. 9) days. Fever and cough were both more common in children older than6 months (X2 = 22. 895,P<0. 01; X2 = 16. 992,P<0. 01, respectively). Febrile convulsion occurred in children older than 2 years. Fifteen point five (39/251) developed leukocytopenia. Conclusions Children younger than 5 years old are at high risk of influenza-related hospitalization. We should emphasize influenza vaccination in previously healthy children aged between 6 months and 5 years and children with underlying diseases.
6.Value of CT in the postoperative evaluation of patellar dislocation triple surgery

Objective To study the role of CT in the evaluation after patellar dislocation triple surgeries.Methods Retrospectively analysed the CT measurements of 60 recurrent patellar dislocation patients (61 knees),who had undergone patellar dislocation triple surgeries,within one month before and after procedures.The measurements included patellar tilt angle,lateral patellar displacement,trochlear con-gruence angle,Tibial Tuberosity-Trochlear Groove distance,Insall-Salvati Ratio and Caton-Deschamps Index.Moreover,the troch-lear dysplasia classifications of all patients were evaluated.In addtion,an analysis has also been completed for 40 (41 knees)of 60 patients who had second postoperative CT examinations as follow-up assessment 3 to 6 months after surgeries.Results All one-month postoperative measurements decreased obviously with statistical significance(P <0.05 ).Furthermore,one case of subluxa-tion,1 1 cases of abnormal Tibial Tuberosity-Trochlear Groove distance,13 cases of abnormal Insall-Salvati Ratio and 14 cases of Ca-ton-Deschamps Index abnormities were also observed.Besides that,it was also found that all patients had trochlear dysplasia and 5 cases were diagnosed with patellar subluxation or dislocation at the second postoperative follow-up assessment.Conclusion The patients with redislocation or risks for patellar instability can be detectecd by CT which provides objective evidence for postoperative evaluation and further treatments.
7.Evaluation of the written project approval indexes system of Capital Medical Development Research Foundation

Objective The purpose of this study was to evaluate the written project approval Indexes System of Capital Medical Ddevelopment Research Foundation.We analyzed the approval results of application projects and proposed measures to improve the index system.Methods We evaluated its reliability with Cronbach's alpha coefficient,split-half reliability; validity was evaluated with contract validity and content validity,discrimination was studies according to discretion grouping law,and feasibility was studied with the percentage-of-completion method.Results ① Reliability:Cronbach's alpha coefficient of factor and total scores'and split-half reliability were all over 0.80.②Contract validity:the standard factor loading coefficients of the index system were over 0.70 (range:0.57~0.86),indicating path performed well.All factor loading coefficients were statistical significant (T-values were 30.49~54.25,all were over 1.96).Fit Indexes (RMSEA =0.065、NNFI =0.99、IFI =0.99、CFI =0.99、GFI =0.95、AGFI =0.93、X2 =1028.21、X2/DF =11.82) indicated the system's goodness of fit was good.③ Content validity:Scale-level CVI,S-CVI was 0.97 and Interrater agreement was 0.93,over 0.90,which indicated that indexes system's whole Content validity performed well.Except for the item Academic thought item-level CVI being 0.57 all items' item-level CVI were 1.00.④Discrimination:Discrimination index of every primary index,secondary index and total score ranged from 0.51 to 0.76 (all were over 0.40),meaning that discrimination performed extremely well.Conclusion The indexes system's reliability,contract validity,content validity and discrimination conformed to the actual application projects appraisal results.
8.Sequence analyses of the Cps2J gene in 9 strains of Streptococcus suis serotype2 isolated in Zhejiang province

To explore the molecular characteristics of Streptococcus suis serotype 2(ss2) isolated in Zhejiang province by deciding the variation loci and its variation frequency of Cps2J gene.The total DNA of 9 strains of ss2 isolated in Zhejiang province were extracted and amplifed by PCR. Then,the Cps2J fragments were cloned into plasmid carrier and completely sequenced after purification.Finally,the sequence results of all 9 ss2 isolates were compared with those obtained by other studies around the world.It was found that the open reading fragments of Cps2J in 9 SS2 isolates encoding 333 amino acids were 999 bp in length.Comparisons of this region among ss2 isolates revealed a similarity of between 98.8% and 99.9%, while the homology to ss1 strains varied between 56.8% and 57.0%.Our study shows the sequences of complete Cps2J segment are fairly stable and all these 9 ss2 strains of different sources possibly have the same evolutionary origin.
9.Comparative study of primordial germ cells in male and female mouse embryos.

OBJECTIVETo investigate the differences in the development of primordial germ cells (PGCs) between male and female mouse embryos.
METHODSThe morphological changes of genital ridge development were detected in C57BL/6J mouse embryos of 11-13.5 days, and the changes of PGCs quantity and proliferation were compared between the male and female embryos using immunofluorescence histochemistry.
RESULTSThe PGCs was the most numerous at 13.5 days in male and female embryos, and the quantity of proliferating PGCs reached the maximum at 13 days. The quantity of PGCs and proliferating PGCs in male embryos at 13 days was significantly larger than that in female embryos.
CONCLUSIONThe development of PGCs is characterized by a gender differences in early development of mouse embryos (11-13.5 days).

10.Heat shock protein 70 upregulates interleukin-6 expression by fibroblasts from psoriasis vulgaris lesions

Objective To evaluate the in vitro effect of heat shock protein 70(HSP70)on interleukin-6 (IL-6)expression by cultured fibroblasts from psoriasis vulgaris lesions(PFbs).Methods Fibroblasts were isolated from the lesions of patients with psoriasis vulgaris and subjected to a primary culture.After 3 to 5 passages of culture,the fibroblasts were collected and used in the next experiment.Some PFbs were cultured with different concentrations(5,10,20,30 mg/L)of HSP70 for 48 hours,or with HSP70 of 30 mg/L for different durations(3,6,12,24,48,72 hours);some PFbs were incubated with HSP70 of 30 mg/L for 24 hours after pretreatment with pyrrolidine dithiocarbamate(PDTC,a specific inhibitor of nuclear factor-kappa B)for 30 minutes.PFbs receiving no treatment served as the control.Enzyme-linked immunosorbent assay(ELISA)and semi-quantitative reverse transcription PCR were performed to measure the IL-6 protein expression in culture supematant and IL-6 mRNA expression by PFbs,respectively.Differences in the expression of IL-6 protein and mRNA between PFbs receiving different treatment were analyzed by using t test and Dunnett's t test.Results HSP70 significantly increased both protein production and mRNA expression of IL-6 in a time(0-48 h)-and dose(5-30 mg/L)-dependent manner.The expression levels of supernatant IL-6 protein and IL-6 mRNA were significantly higher in the PFbs treated with HSP70 of 10 mg/L for 48 hours than untreated PFbs((75.2 ± 15.4)ng/L vs.(47.2 ± 10.6)ng/L,0.439 ± 0.093 vs.0.249 ± 0.069,both P < 0.05).A significant increase was observed as early as 6 hours in the level of IL-6 mRNA after the treatment with HSP70 of 30 mg/L,and 12 hours in the level of supematant IL-6 protein.Decreased supernatant IL-6 protein and IL-6 mRNA were noted for PFbs treated with PDTC and HSP70 of 30 mg/L compared with untreated PFbs((42.23 ± 9.41)ng/L vs.(68.40 ± 14.43)ng/L,0.144 ± 0.048 vs.0.295 ± 0.081,both P < 0.05).Conclusion HSP70 may increase the expression of IL-6 mRNA and protein by cultured PFbs via the nuclear factor-kappa B pathway.
