International Journal of Biomedical Engineering 2011;34(6):357-361
Optical coherence tomography (OCT) which was brought into existence in early 90s of 20 century is a noninvasive technique for creating cross-sectional images of transparent or semi-transparent internal biological tissue structure. In recent years applicability research in the field of stomatology has made great progress. The micro structure changes of surface layer of the lesions in oral hard and soft tissues could be detected by this technique, with its unique feature to detect insidious secondary caries beneath dental restorations that couldn't be found by current clinical techniques easily. In this review, secondary caries etiopathogenisis and diagnosis, principle were discussed firstly and then imaging observation of OCT to detect early second caries around dental restoration and monitoring on risk factors inducing secondary caries are reviewed. It indicated that OCT technique has great potential on diagnosis of secondary caries.
2.Research progress on photodynamic therapy in dentistry
International Journal of Biomedical Engineering 2009;32(3):166-171
As a new technique, photedynamic therapy was introduced in the end of 1970s and has been put into clinical application from experimental tests in recent years. Its mechanisms was established on the basis that a series of special typed cells and tissues could selectively absorb photosensitive drugs and generate photore-sponsive effects by light irradiation of definite wavelengths. Currently, the photodynamic theraputics is applied mainly in non surgical treatment of tumor betided in body superfacial and hollow viscus. Its application area has been expanded unceasingly in modern clinic due to the discovery of the selective aggregation of photoresponsive drugs to many pathological tissues and pathogenic microorganisms other than tumors, and the development of new photoresensitizer. In this paper, review is given on the effect mechanisms, the light sources and photosensitizer, as well as the application of photodynamic treatment of tumors in head and neck and its treatments of mucosal dis-ease and oral pathogenic microorganism infected diseases.

Objective To study the preventive effect and mechanism of aesculin on intestinal mucosa in rats with experimental ulcerative colitis (UC) . Methods Forty specific-pathogen free SD rats were randomly divided into normal group, model group, salicylazosulfapyridine (SASP, 600 mg/kg) group and aesculin (EH, 100 mg/kg) group, 10 in each group. Rats in model group, SASP group and EH group were given enema with trinitrobenzene sulfonic acid ( TNBS, 100 mg/kg) for the establishment of UC model. The rats in SASP group and EH group were given gastric gavage of SASP and aesculin respectively. At the end of experiment, the serum levels of tumor necrosis factor alpha ( TNF-α) and interleukin 10 ( IL-10) were detected by enzyme-linked immunosorbent assay (ELISA) . The general state, histological features of intestinal mucosa and serum TNF-αand IL-10 levels of rats in each group were compared. Results Aesculin significantly improved the general state and relieved the inflammation of the colonic mucosa of UC rats. The disease activity index ( DAI) scores and tissue damage index (TDI) scores in the model group were significantly higher than those in the normal group ( P<0.01) . The DAI scores and TDI scores in the medication groups were significantly lower than those in the model group (P<0.01) . The serum TNF-αlevel was significantly higher and IL-10 level was significantly lower in the model group than the normal group ( P<0.01) . After treatment, TNF-α was decreased and IL-10 was increased in SASP group and EH group as compared with the model group (P<0.01) . Conclusion Aesculin has certain therapeutic effect on TNBS-induced UC in rats through significantly relieving the symptoms of UC rats. The mechanism may be related with the inhibition of TNF-α secretion and the increase of IL-10 expression, and then improving the disorder of intestinal immune function.

Objective To study the technique and clinical effect of endoscopic surgery using high frequency electrotome with suction device for nasopharyngeal angiofibroma. Methods In this study, we retrospective analyzed the clinical data of 10 patients treated between January 2009 and December 2015. All these patients were treated by endoscopic surgery using high frequency electrotome with suction device. The average blood loss during surgery, tumor residual, recurrent tumor and duration of recovery were evaluated. Results The tumor was completely removed in all 10 patients. The average blood loss was 465 ml, there was no postoperative complication, all patients had no residual tumor, and there was no recurrent tumor over a follow-up of 6~18 months. Conclusion Though it was not preoperative transcatheter arterial embolization, the endoscopic surgery using high frequency electrotome with suction device can be used to treated nasopharyngeal angiofibroma which either limited to nasopharyngeal cavities and paranasal sinus with intraoperative blood pressure control.

Objective To examine the effects of a functional polymorphism of the monoamine oxidase A (MAOA) gene on spontaneous brain activity in healthy male adolescents. Methods Resting-state fMRI was performed on 31 healthy male adolescents with the low-activity MAOA genotype (MAOA-L) and 25 healthy male adolescents with the high-activity MAOA genotype (MAOA-H).The amplitude of low-frequency fluctuation (ALFF) of the blood oxygen level-dependent (BOLD) signal was calculated using REST software, and was compared between two genotype groups. The region ROIs showed significant difference. The ALFF data in ROIs were related to BIS scores. Results Compared with the MAOA-H group, the MAOA-L group showed a significant decrease of ALFF (P<0.001) in the pons (MNI coordinates:-6,-19,-23;6,-16,-17;-6,-25,-32).In addition, the BIS scores were positively correlated with ALFF in pons in the MAOA-L group (r=0.398,P=0.02),but not in the MAOA-H group. Conclusions There exists relevance between the polymorphism of MAOA and the spontaneous brain activity in pons. And the lower activity of spontaneous brain activity in pons may be a key risk factor for impulsivity and aggression.

Background:Capsule endoscopy( CE)has been widely used for the diagnosis of small intestinal diseases. How to enhance the complete examination rate( CER)has attracted more and more attention in recent years. Aims:To investigate the effect of different intervention on gastric transit time( GTT)and small bowel transit time( SBTT)in CE. Methods:Ninety patients undergoing CE from January 2012 to May 2013 at Changhai Hospital were enrolled and randomly divided into control group, right lateral position group and motion group. The patients in control group were allowed to keep quiet, patients in right lateral position group were asked to lie on their right side, and patients in motion group were asked to take rapid pace walking after swallowing the capsule until the capsule passing pylorus. GTT,SBTT,CER and disease diagnosis rate among the three groups were compared. Results:Compared with control group,GTT was significantly decreased in right lateral position group(P﹤0. 05),however,no significant difference in SBTT was found between right lateral position group and control group(P ﹥0. 05). GTT and SBTT were both significantly decreased in motion group than in control group(P﹤0. 05). No significant differences in CER and disease diagnosis rate were found among the three groups(P﹥0. 05). Conclusions:Right lateral position can shorten GTT,rapid pace walking can shorten GTT and SBTT,and both these two intervention have substantial clinical values in CE.

Objective To evaluate the clinical application and safety of CT-guided transthoracic cutting needle lung biopsy in diagnosing diffuse lung diseases. Methods By using automatic biopsy gun (14 & 16 gauge), CT-guided transthoracic cutting needle lung biopsy was performed in 29 cases with diffuse lung diseases. The samples obtained were sent for pathological and immunohistochemical examination. The sampling successful rate, the diagnostic accuracy and the occurrence of complications were analyzed. Results Technical success rate was 100%, and large size of sample enough for pathological and immunohistochemical examination was obtained in all 29 cases. Definite pathological diagnosis could be made in 25 cases, with the positive diagnostic rate of 82.8%. The main complications included pneumothorax and pulmonary hemorrhage. The symptoms in most cases were not severe and disappeared within one week after the treatment. Conclusion For the diagnosis of diffuse lung diseases, CT-guided transthoracic cutting needle lung biopsy is a safe, easy, effective and reliable method with high successful rate, high diagnostic value and fewer complications, in these respects this technique is superior to transbronchial lung biopsy, open lung biopsy and video-assisted thoracoscopic lung biopsy. Therefore, this technique should be popularized in clinical practice.
