1.Diagnosis and treatment of rectal stromal tumor

Gastrointestinal stromal tumor (GIST) is a group of tumors which is independently derived from the mesenchymal stem cells of the digestive tract,and consists of undifferentiated or multi-functional spindle or epithelioid cells.Most of the rectal stromal tumor located at the middle or lower part of the rectum,and presented with exogenous growth outside of the rectum cavity.Rectal stromal tumor accounted for 5% of GIST.Rectal stromal tumor is easily missed or misdiagnosed because of insidious onset and non-typical clinical manifestations.The diagnosis of rectal stromal tumor relies on immunohistochemical staining (CD117 and CD34) and pathological examination.Surgical resection is the first choice for the treatment of local primary rectal stromal tumor.Protection of the integrity of the tumor is important during operation,and routine lymph node dissection is not recommended.Molecular targeted drug mesylate imatinib is adopted in the pre-and postoperative adjuvant therapy.However,due to the rarity of rectal stromal tumor,the treatment strategies need further investigation.
2.Ultrasound Observe the Relationship between Fetus Cerebral Lateral Fissure Depth in Middle Pregnancy and Gestational Weeks

[Objective] To observe the relation between the fetus cerebral lateral fissure depth in middle pregnancy and gestational weeks and offer foundation for whether or not normal growth of fetus cerebral cortex in antenatal early evaluation. [Method] Observe the shape of fetus cerebral lateral fissure of 18~24w pregnant women 559 cases with ultrasound, and measure the depth at cerebel um cross section. [Result] The 559 cases of pregnant women ’s fetus cerebral lat-eral fissure depth increases with gestational weeks, the shape changes blunt from smooth. [Conclusion] The ultrasound observation of fetus cerebral lateral fis-sure depth which increases with gestational weeks can be taken to offer relative basis for early evaluation of whether or not normal cerebral cortex growth.
3.A co-word analysis of the research progress in feeding behavior in China

Objective The study implemented the co-word approach to analyze Chinese literatures on feeding behavior published over the recent fifteen years, aiming to explore the research status and progress of the field and provide a foundation for further studies. Methods Literatures on the Chinese National Knowledge Integrated Database were searched using feeding behavior as theme and limiting published time fromJanuary 1,2001toJanuary 18,2017.those fitting in the criteria were included into the study. Keywords statistics and analysis were conducted by using Excel, and a co-occurrence analysis diagram was drawn by using Ucinet 6.237 and Netdraw to visualize the co-occurrence between frequently occurred keywords. Results A total of 467 literatures were included and 28 frequently occurred keywords were extracted for co-word analysis, revealing the correlations between the keywords. The results showed that the studies on feeding behavior in China had focused on infant, nutritional status, breastfeeding, complementary feeding and so on. The feeding difficulty, eating behavior, feeding behaviors for sick children had remained to be studied. Conclusion Domestic research on the current feeding behavior of caregivers is not yet comprehensive .There is an urgent need for a major parental feeding behavior scale for infants and young children, which is the main tool for assessing the feeding behavior of infants and young children. It provides scientific basis for feeding behavior problems and intervention.
4.Bacteriologic investigation and its clinical significance in patients with upper urinary tract calculi

Objective To investigate the correlation between bacteria and upper urinary tract calculi (UUTC). Methods 530 cases of UUTC were enrolled in the study.Mid-stream urine culture was obtained before operation.Intraoperative pelvic urine culture,cotton swab culture of stone-incarcerated mucosal surface,pre- and post-sterilized stone culture were also carried out. Results Among 530 cases of UUTC,the stone culture showed positive (infectious stone) in 120 cases (22.6%),more prevalent in females than in males. Staphylococcus epidermidis and Escherichia coli were the predominant bacteria, accounting for 60.1% and 32.5%,respectively.The infection rate with which a stone size was ≥1.0 cm with coarse surface,loose density or radiolucent was much higher than that of size

6.Suitable Hospital Infection Control Measures in Health Centers of Poverty-striken Villages

OBJECTIVE To explore the suitable hospital infection control measures in health centers of poverty-striken villages,in order to improve the management of hospital infection,decrease hospital infection rate and protect the health of medical staff and patients.METHODS The status quo of hospital infection in health centers of poverty-striken villages,was investigated in 20 small towns health centers with were randomly divided into two groups:test group(n=15)and control group(n=5).The suitable hospital infection control measures were explored from 5 points.The effect of infection control by before-after controlled study of experimental group and randomized controlled study of control group was anal yzed.RESULTS The rate of hospital infection in test group was decreased from 7.60% to 1.98% and at in control group didn't change,the difference was significant.CONCLUSIONS The managements of establishment of the suitable hospital infection control measures in health centers of poverty-striken villages have been put into practice and gained good result.
7.Study on correlation between plasma NT-proBNP with essential hypertension complicating left ventricular dysfunction

Objective To investigate the clinical values of NT-proBNP in left ventricular enlargement(LVE) and left ventricular dysfunction(LVD) of the patients with essential hypertension(EH) to provide a diagnostic basis for their diagnosis .Methods Plas-ma NT-proBNP concentrations in 120 patients with EH and in 29 normal controls were measured ,then the echocardiography exami-nation was performed to determine the left ventricular ejection fraction ( LVEF) ,left ventricular end-diastolic diameter(LVEDD) , left atrium(LA) and left ventricular systolic diameter(LVSDD) .The correlation between plasma NT-proBNP with LVE and LVD was analyzed .The diagnostic accuracy of NT-proBNP was analyzed by using receiver operating characteristic (ROC) curve .Results There were no statistically significant differences in the aspects of age ,sex and serum creatinine between the EH group and con-trol group ,but plasma NT-proBNP level of the former was significantly higher than that of the latter .The NT-proBNP level in the patients with LVE was significantly higher than that in the patients with normal left ventricle (P<0 .05) .The NT-proBNP level in the LVD patients was significantly higher than that in the patients with normal left ventricular function (P<0 .05) .The plasma NT-p roBNP level was positively correlated with LA ,LVEDD and LVSDD(r=0 .518 ,0 .58 ,0 .48 ,P<0 .01) while negatively crrelated with LVEF(r= -0 .61 ,P<0 .01) .The ROC curve showed that when the NT-proBNP was set at 380 pg/mL ,the sensitivity and specificity of NT-proBNP for diagnosing LVE were 80 .6% and 72 .4% ;which for diagnosing LVD were 80 .8% and 77 .4% ,re-spectively .Conclusion NT-proBNP is closely correlated with multiple ultrasonic indicators reflecting the left ventricular function and its level can reliably reflect the left ventricular contraction function ,which can serve as the marker for screening LVE and LVD in the patients with EH .
8.Study on aminoglycosides resistance and its resistance genes in multidrug resistant Acinetobacter baumannii

Objective To investigate the resistance of multi-drug resistant Acinetobacter baumannii against aminoglycosides and its resistance genes.Methods The minimal inhibitory concentrations (MICs) of seven antimicrobials (gentamicin, amikacin, tobramycine, netilmicin, neomycin, streptomycin and kanamycin) against 20 strains of multi-drug resistant Acinetobacter baumannii were detected by agar dilution method. Meanwhile, two resistance genes of aminoglycoside nucleotidyltransferase were amplified by PCR and vertified by DNA sequencer.Results It was found that MIC50 and MIC90 of gentamicin, amikacin, streptomycin and kanamycin in 20 strains of Acinetobacter baumannii were all above 1 024 mg/L, and their resistant rates were 95%, 95%, 100% and 90% respectively, while the resistant rates of tobramycin, netilmicin and neomycin were 85%, 90% and 40% respectively. Two aminoglycoside-modification genes ant(2″)-Ⅰ and ant(3″)-Ⅰ were detected in 20 strains, with 55% and 80% of positive rate respectively. The double positive rate of two resistance genes was 50%.Conclusion The resistance of multi-drug resistant Acinetobacter baumannii against aminoglycosides was closely associated with ant(2″)-Ⅰ and ant(3″)-Ⅰ genes.
9.Locking plate internal fixation for minimally invasive treatment of intra-articular calcaneal fractures

Objective To summarize and evaluate the technique and clinical outcome of limited tarsal sinus incision plus locking plate internal fixation for minimally invasive treatment of intra-articular calcaneal fractures. Methods Between February 2010 and February 2011,16 cases of intra-articular calcaneal fractures were treated in a minimally invasive manner in our department. All cases were evaluated carefully with routine X-rays and CT scans preoperatively to define the type of fracture and the involvement of articular surface.Open reduction and locking plate internal fixation with percutaneous screws were performed via a limited tarsal sinus approach 3 to 6 days after injury (average,4 days).Regular X-ray follow-ups were conducted to measure the Bohler's and Gissane angles.Overall functional evaluation was carried out according to Visual Analogue Scale (VAS),the Hind-foot Score by American Orthopaedic Foot and Ankle Society (AOFAS) and Short Form 36 Health Survey Questionnaire (SF-36).Complications were recorded as well.Results The 13 cases were followed up for a mean duration of 18 months (from 12 to 24 months).There were no wound infection,skin and flap necrosis or implant failure.Bone union was achieved at an average of 10 weeks (from 8 to 12 weeks) post-operatively.The average Bohler's angle was improved significantly from 13.4° ± 3.4° (from 8° to 19°) pre-operatively to 26.5° ± 4.5° ( from 21° to 38°) post-operatively ( t =9.781,P < 0.001 ).The average Gissane angle was improved significantly from 88.1° ± 7.6° (from 77° to 100°)pre-operatively to 116.2°±7.5° (from 100°to 124°) post-operatively (t =12.934,P <0.001).On average,the VAS score was 1.5 ± 1.7,the AOFAS score was 84.2 ± 5.9 and the SF-36 score was 79.5 ± 8.1 at the final follow-up.The follow-ups revealed no post-traumatic arthritis. Conclusion Open reduction and locking plate fixation with percutaneous screws via a limited tarsal sinus incision is a safe and reliable treatment for intra-articular calcaneal fractures,because it has the advantages of direct reduction of the articular surface,solid fixation,and limited soft tissue complications.
10.Clinical Analysis of Treating the Delayed Meconium Exclusin in Premature Infants by High-frequency Ultrasound

ObjectiveTo explore the infulence and to look for an optimal management of delayed meconium exclusion to the premature infant,in order to offer the dependence of diminished neonatal complications.Methods 23 premature infants with delayed meconium were randomly assigned into treatment group and control group.12 premature infants with normal meconium were selected as a normal group.Using the 6F silicone rubber stomach tube help the premature infants exclusing meconium by high-frequency ultrasound in treatment group.2-3ml glycerin mixed with 10-20ml warmed normal saline was given every two days or when vomit,abdominal distension appeared in control group.ResultsThere are no significant differences in serum bilirubin level,recovering days of weight,occurence rate of vomiting,abdominal distension between the treatment group and the normal premature infants group (P > 0.05 ).Oppositively,there were significant differences between the control group and the treatment group ( all P < 0.01).ConclusionUsing high-frequency ultrasound to treat the delay meconium exclusion is an optimal management,it may reduce the complications and recovery the weight of the premature infants with delayed meconium exclusions.
