1.Predictive value of serum procalcitonin for the localization of acute digestive tract perforation

Objective:To investigate the predictive value of serum procalcitonin (PCT) for the localization of acute digestive tract perforation.Methods:This retrospective study included 88 patients from the Department of Gastrointestinal Surgery of the First Affiliated Hospital of Southwest Medical University who were diagnosed as acute digestive tract perforation between January 2015 and January 2018. According to the intraoperative diagnosis and postoperative pathological reports, the enrolled patients were divided into the upper digestive tract perforation group (45 cases) and the lower digestive tract perforation group (43 cases) (above or below Treitz ligament). Preoperative serum PCT, white blood cell, neutrophil rate, C-reactive protein (CRP), albumin (ALB), C-reactive protein to albumin (CRP/ALB) ratio were measured and compared between the two groups. Univariate analysis and multivariate logistic regression analysis were used to analyze the independent risk factors of the lower digestive tract perforation group, and the receiver operating characteristic curve was used to analyze the predictive value of the above mentioned markers in the localization of acute digestive tract perforation.Results:Univariate analysis showed that patients in the lower digestive tract perforation group exhibited significantly higher levels of serum PCT, CRP, ALB, and CRP/ALB ratio than patients in the upper digestive tract perforation group ( P<0.05). Multivariate logistic regression analysis showed that serum PCT, CRP and CRP/ALB ratio were independent risk factors for the diagnosis of lower digestive tract perforation [PCT: odds ratio ( OR)=1.241, 95% confidence interval (95% CI): 1.098~1.403, P = 0.001; CRP: OR= 0.95, 95% CI: 0.912~0.99, P = 0.014; and CRP/ALB ratio: OR= 35.104, 95% CI: 3.889-316.885, P = 0.002]. The area under curve of serum PCT, CRP, CRP/ALB ratio to distinguish upper or lower digestive tract perforation were 0.932 (95% CI: 0.879~0.985), 0.77 (95% CI: 0.667~0.872), and 0.898 (95% CI: 0.827~0.969), respectively. The optimal cutoff value of PCT in differential diagnosis of upper or lower digestive tract perforation was 16.595 ng/mL, with a sensitivity of 86.0% and a specificity of 91.1%. The optimal cutoff value of CRP was 55.4 mg/mL, with a sensitivity of 76.7% and a specificity of 80.0%. The optimal cutoff value of CRP/ALB ratio was 1.45 and its sensitivity and specificity were 83.7%, 88.9%, respectively. Conclusions:Serum PCT, CRP, and CRP/ALB ratio are helpful to predict the localization of acute digestive tract perforation and can improve the diagnostic accuracy. The diagnostic efficiency of PCT is better than CRP and CRP/ALB ratio, exerting excellent clinical value.
2.Disease burden of influenza in schools and child care settings in rural areas of Hangzhou, 2016-2018

Objective The objective of this study was to analyze the disease burden of influenza in schools and child care settings in rural areas of Hangzhou. Methods Hospital visit influenza cases aged 3-17 years in hospitals that reported based on influenza surveillance system from 2016 to 2018 in Chun′an county, Hangzhou city were selected as study subjects and a total of 294 confirmed cases of influenza were selected using system sampling method. Questionnaires were designed to investigate the basic information and data on inpatients and outpatients among, health care and life quality, etc.. Direct and indirect economic burden and disability adjusted life year (DALY) were analyzed and compared among different age groups. Results The mean age of investigated subjects was (8.88±3.92) years. A total of 143 (48.64%) investigated cases were male. In total of 283 (96.26%) cases were outpatients. The total economic burden was 124 743.95 CNY. The mean economic burden was 424.30 CNY per person. The mean direct and indirect economic burden was 361.33 and 62.97 CNY per person respectively. The difference of the mean direct, indirect and total economic burden per person between different age group was statistically significant (P<0.001). The 3-5 years age group showed the highest economic burden with the median value of direct, indirect and total economic burden per person being 276.24, 50.98 and 321.26 CNY, respectively, while the 12-17 years age group showed the lowest values with 175.30, 26.54, 201.79 CNY, respectively. The DALY of 294 influenza cases was 1.18, and the median of burden strength was 3.21 DALY/thousand. The difference of the burden of strength between different age group influenza case was statistically significant (P<0.001), of which the 12-17 years age group showed the highest value with 4.25 DALYs/thousand while the 3-5 years age group showed the lowest value with 2.60 DALY/thousand. Conclusion The disease burden of influenza was heavy in schools and kindergartens in rural areas of Hangzhou city, with the cases aged from 3 to 5 years showing higher economic burden and cases aged from 12 to 17 years showing greater burden strength.

5.Effect of Oleanolic Acid on Abnormal Water Metabolism of Mice with Water-dampness Retention Caused by Spleen Deficiency

ObjectiveTo explore the effect of oleanolic acid (OA) on water metabolism in mice with water-dampness retention caused by spleen deficiency and the mechanism. MethodThe 60 SPF Kunming (KM) mice were randomized into blank group (n=10) and modeling group (n=50). Through long-term living in damp place and irregular diet, water-dampness retention caused by spleen deficiency was induced in modeling mice. Then the model mice were randomly classified into model group, natural recovery group, and low-dose, medium-dose, and high-dose OA groups. The mice in the blank group, model group, and natural recovery group were given (ig) 10 mL·kg-1·d-1 normal saline, and mice in the low-dose, medium-dose, and high-dose OA groups received 50, 100, 200 mg·kg-1·d-1 OA, respectively. The intervention lasted 7 days. Before and after modeling and administration, the general conditions of the mice were observed and body weight of mice was measured. The water content in feces and tissues was detected with the oven-drying method, and water load index and organ coefficient were measured with the weighing method. Enzyme-linked immunosorbent assay (ELISA) was employed to detect the urinary D-xylose excretion, serum gastrin (GAS), total protein (TP), albumin (ALB), total cholesterol (TC), high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C), interleukin-6 (IL-6), antidiuretic hormone (AVP), aquaporin 1 (AQP1) in renal medulla, and liver Na+-K+-ATPase. At the same time, OA was docked with ALB, IL-6, AQP1, and Na+-K+-ATPase. ResultCompared with the blank group, the model group showed withered hair, emaciation, laziness, bradykinesia, slow weight growth, infrequent spontaneous activities, high water content in feces and tissues, low weight loss after water loading, high coefficient of each organ (P<0.05, P<0.01). Moreover, the model group had less urinary D-xylose excretion, lower serum levels of GAS, TP, ALB, and HDL-C, higher levels of TC, LDL-C, AVP, and IL-6, lower expression of Na+-K+-ATPase in the liver, and higher expression of AQP1 in renal medulla than the blank group (P<0.05, P<0.01). The three OA groups demonstrated better general conditions, faster weight gain, more frequent spontaneous activities, lower water content in feces and tissues, larger weight loss after water loading, and lower coefficient of each organ than the model group (P<0.05, P<0.01). Moreover, compared with the model group, the three OA groups had high D-xylose excretion, high serum levels of GAS, TP, ALB, and HDL-C, low serum levels of TC, LDL-C, AVP, and IL-6, high expression of Na+-K+-ATPase in liver, and low expression of AQP1 in renal medulla (P<0.05, P<0.01). The recovery in each OA group was better than that in natural recovery group. Molecular docking results also confirmed that OA had high binding affinity with ALB, IL-6, AQP1, and Na+-K+-ATPase. ConclusionOA can alleviate the abnormal water metabolism in mice with water-dampness retention caused by spleen deficiency, which lays a basis for its potential clinical application.
