1.Antitumor effects of photodynamic tumor cell lysates on rat epithelial ovarian cancer in vivo

Objective To study the antitumor effects of photodynamic therapy (PDT)treated by ovarian cancer cell lysates in rat epithelial ovarian cancer in vivo. Methods Female Fischer344 rats of 6 -8 weeks were allocated to four groups ( n = 8 each) : PDT group ( inoculated intraperitoneal with PDT tumor cell lysates), freeze/thaw group ( inoculated intraperitonealy with freeze-thaw tumor cell lysates), normal saline group (inoculated intraperitoneal with normal saline) and control group. Rat epithelial ovarian cancer NuTu19 cells were injected into all rats by intraperitoneal at day 7,while injected with normal saline in control group. The number of tumor specific interferon-γ (IFN-γ) secreting splenocytes was quantified by enzyme linked immunospot(ELISPOT) assay, the cytotoxic T lymphocyte(CTL) activity of splenocytes was measured by lactate dehydrogenase(LDH) analysis and tumor growth and the survival time of rats were also observe& Results Stimulated by PDT tumor cell lysates, the number of tumor specific IFN-γ secreting splenocytes in PDT group, freeze/thaw group, normal saline group and control group were 448. 8±34. 2, 211.2±47.9,43.3 ± 11.1,16.1 ± 2.4 respectively, which were significant differences among of them ( P < 0.05). Stimulated by freeze/thaw tumor cell lysates, the number of tumor specific IFN-γ, secreting splenocytes in four groups were 151.7 ± 22.6,188.7 ± 53.0, 18.2 ± 12.2,8.8 ± 7.7 respectively, which were not significant differences among of them ( P>0.05 ). Cytotoxicity of splenocytes of PDT group increased significantly than that in other three groups(P <0.05). Except rats in control group were all alive until the experiment ended, the mean survival time of other rats were 234 d in PDT group, 171 d in freeze/ thaw group and 168 d in normal saline group, which in PDT group was significantly higher than those in freeze/thaw group and normal saline group ( P<0.05 ). Conclusions Rats treated by PDT tumor cell lysates could produce antitumor effects in vivo, which shown that induce tumor-specific immune response and prolong the life span.
2.Expression and purification of asprosin and its effects on cardiac function in mice

Objective To purify asprosin protein expressed in Escherichia coli expression system and to study its effect on cardiac function.Methods Coding sequence of asprosin was obtained from GenBank.Codon optimization was performed according to the codon preference of E.coli.After gene synthesized, recombinant plasmid was made.Asprosin was then induced and purified by Ni-affinity purification.The mouse model of impaired cardiac function was established by ligating and relaxing the left anterior descending coronary artery.30 mice were randomly divided into 3 groups: sham operation group (sham), cardiac dysfunction group (MI/R) and cardiac dysfunction plus injection of recombinant asposin protein group (MI/R+rAsp).The left ventricular function was detected by echocardiography to determine the improving effect of recombinant asprosin protein on cardiac function.Results After prokaryotic expression and purification, the purity of the target protein was higher than 95%, and the endotoxin content was less than <0.1 EU/μg protein, which was suitable for cell and animal studies.After the recombinant asprosin protein was given, the left ventricular function of the mice was improved significantly (P<0.05).Conclusions Asprosin acts as a myocardial protective molecule to improve cardiac function.
3.Effects of acupuncture in combination with exercise on learning, memory and the expression of microtubuleassociated protein-2 in the hippocampal CA3 region after experimental focal cerebral infarction in rats

Objective To study the expression of microtubule-associated protein-2 (MAP-2) in the CA3 region of the hippocampus and any effect of acupuncture combined with exercise training on the recovery of learning and memory after experimental cerebral infarction.Methods Eighty Wistar rats were randomly divided into a sham-operation group (n = 8 ) and an operation group (n = 72 ) subdivided randomly and equally into a model group, an exercisetraining group and an acupuncture plus exercise training group, with 24 rats in each group. After right middle cerebral artery occlusion or a mock operation, the expression of MAP-2 in the CA3 region of the hippocampus was detected with immunohistochemistry 1, 3 and 5 weeks after the operation. Learning and memory function was assessed at the 5th week post operation.Results MAP-2- positive fibers were arranged in an orderly way and distributed densely in the shamoperation group. After cerebral infarction, number of MAP-2 positive neurons and dendrite fibers decreased in the CA3 region of the affected side. A week after the operation, MAP-2 positive fibers had increased significantly in the acupuncture plus exercise group compared with the model group and the exercise group. At the 3rd and 5th week post operation, expression of MAP-2 and optical density had increased significantly compared with the model group and the exercise training group. At the 5th week post operation, Y maze discrimination and learning tests showed that learning and memory had improved significantly more in the acupuncture plus exercise training group compared with the model group and the exercise training group.Conclusions Exercise training combined with acupuncture can promote changes in dendritic plasticity and increase MAP-2 expression in the CA3 region after cerebral infarction. This may be correlated positively with the recovery of learning and memory after cerebral infarction, at least in rats.
4.Enzyme-catalyzed synthesis of ASGPR ligand-targeted modifier in non-aqueous medium.

The asialoglycoprotein receptor (ASGPR) was used to mediate drug carrier for hepatic targeted drug delivery, this article showed the enzyme-catalyzed esterification of galactose and vinyl stearate and a kind of ASGPR ligand-targeted which was used to insert the surface of liposome has been synthesized. The structure of product has been confirmed by TLC, ESI-MS and 1H NMR. The factors of types and quantity of enzyme, organic solvents, molar ratio of substrate, temperature and time of reaction have been studied. Results showed when using acetone as reaction medium, the quantity of Novozym 435 immobilized lipase was 30 mg mL(-1), molar ratio of galactose to vinyl stearate was 1:5, and reacted at 60 degrees C for 12 h, the transformation of vinyl stearate reached more than 70%. This study provides a novel and efficient route to the synthesis of ligand-targeted modifier.
5.Effect of Multidisciplinary Rehabilitation on Diabetic Foot

Objective To observe the clinical effect of multidisciplinary rehabilitation on diabetic foot.Methods98 diabetic foot patients were divided into the routine treatment group (50 cases), treated only with conventional treatment, and the multidisciplinary rehabilitation group (48 cases) added with multidisciplinary rehabilitation besides conventional treatment.ResultsAfter one month treatment, blood flow parameters of the patients with 0 grade diabetic foot in the multidisciplinary rehabilitation group were markedly improved showed by color Doppler ultrasonograghy of lower limbs ( P<0.05); after two months treatment, the effect of the multidisciplinary rehabilitation group was better than that of the routine treatment group ( P<0.05).ConclusionThe multidisciplinary rehabilitation has obvious effect on diabetic foot and can improve disease recovery earlier.
6.The clinical value of continuous increase of blood lactic acid in prognosis evaluation of patients with sepsis

Objective:To investigate the clinical value of continuous increase of blood lactic acid (Lac) in prognosis evaluation of patients with sepsis.Methods:From January 2016 to December 2018, 84 patients with sepsis in the Department of Critical Medicine of Lanzhou First People's Hospital were retrospectively analyzed. According to the final outcome, the patients were divided into death group and survival group. The general condition, initial Lac, Lac at 6, 12, 18, 24 h, and the duration of Lac>2 mmol/L (T Lac>2) were compared between the two groups. Receiver operating characteristic curve (ROC) was used to analyze the sensitivity and specificity of gender, age, acute physiology and chronic health evaluation Ⅱ (APACHE Ⅱ), initial lac, 6, 12, 18, 24 h lac, T Lac>2 in evaluating the prognosis of patients with sepsis. At the same time, the relationship between these parameters and the prognosis of patients was analyzed by Cox regression analysis to evaluate the clinical value of the time of continuous increase of blood lactate in the prognosis of patients with sepsis. Results:There was no significant difference in age, gender, APACHE Ⅱ score and initial lac between the two groups ( P>0.05). The Lac of death group was higher than that of survival group at 6, 12, 18 and 24 h after treatment ( P<0.05), and T Lac>2 in death group was significantly longer than that in survival group ( P<0.05). Cox regression analysis showed that age, gender, APACHE Ⅱ score, initial Lac, 6, 12, 18, 24 h lac had no significant effect on the prognosis of sepsis patients, while T Lac>2 was significantly correlated with survival rate and death risk. The longer T Lac>2 was, the lower the survival rate and the higher the risk of death. At the same time, ROC curve analysis showed that gender, age, APACHE Ⅱ score and area under the curve (AUC) of initial lac showed that these indicators could not effectively evaluate the prognosis of patients ( P>0.05). The area under the curve of T Lac>2 was the largest, and the evaluation of prognosis was the best, followed by 24 h Lac and 18 h, 12 h and 6 h Lac. In addition, the sensitivity of T Lac>2, 24, 18, 12, 6 h for sepsis mortality risk assessment were 90.9%, 81.8%, 81.8%, 81.8%, 88.6%, and the specificity were 71.4%, 52.5%, 52.5%, 47.7% and 25.2%, respectively. Conclusions:Transient increase of lactic acid can not evaluate the prognosis of patients with sepsis, but the duration of lactic acid increase has a significant impact on the prognosis of patients with sepsis. The longer the increase of Lactic acid (T Lac>2) is, the lower the survival rate and the higher the risk of death. The sensitivity and specificity of lactic acid duration in evaluating the risk of death were higher than those of other parameters, and the prognostic efficacy was the best.
7.Application of secondary triage management system based on JCI standard in outpatient clinic of maternal and child health care hospital

Objective To discuss the establishment and application effects of secondary triage management system based on JCI standard in outpatient clinic of maternal and child health care hospital. Methods In July 2015,secondary triage mode was implemented and secondary triage management team was established in our hospital under the JCI standard basis. Nursing staff were trained. The secondary triage management system in outpatient clinic,job and responsibilities of nurses and work flow of secondary triage mode were developed. Satisfaction of patients and doctors in outpatient clinic and waiting time of patients before (January 2014 to June 2015) and after (July 2015 to October 2016) the implementation of secondary triage mode were compared.Results After the implementation of secondary triage mode,the satisfaction of patients increased from (85.76±15.24) to (96.38±12.91),while satisfaction of patients increased from 88.80% to 97.20%. The waiting time of patients decreased from (59.64±12.38) min to (40.55±10.96) min. The differences were statistically significant (t/χ2=15.634,13.550,9.915;P<0.001).Conclusions The secondary triage management model can reduce the waiting time of patients and improve their satisfaction.
8.Risk factors of medication no-adherence in elderly patients with hypertension:a Meta-analysis

Objective To investigate risk factors of medication non-adherence in elderly patients with hypertension through Meta-analysis.Methods Literatures on medication non-adherence in elderly patients with hypertension were systematically reviewed across various databases,from inception to September 1st 2023,including CNKI,VIP,Wangfong Database,SinoMed,PubMed,Web of Science,Cochrane Library、EMbase.Meta-analysis was conducted using RevMan 5.4.Results A total of 19 articles were included,comprising a total of 18 220 patients in sample size.Meta analysis identified 10 risk factors,showed significant association(P<0.05)with:age[OR=1.36,95%CI=(1.00,1.86)],place of residence[OR=1.91,95%CI=(1.33,2.75)],method of payment for medical expenses[OR=1.60,95%CI=(1.38,1.86)],income level[OR=1.68,95%CI=(1.45,1.95)],occupational status[OR=1.79,95%CI=(1.37,2.36)],social support[OR=1.43,95%CI=(1.27,1.60)],comorbidity[CI=0.33,95%(0.19,0.58)],frequency of blood pressure measurement[CI=1.53,95%(1.14,2.05)],disease awareness[OR=1.70,95%CI=(1.44,2.00)]and dedication frequency[OR=1.44,95%CI=(1.16,1.79)].In addition,the number of influencing factors such as gender,marital status,depression,cognitive impairment,and complications was included in<3 articles,and only descriptive analysis was conducted.Conclusion Healthcare providers should focus on the identification and management of the risk factors associated with medication non-adherence in elderly patients with hypertension,therefore to reduce the incidence of medication non-adherence.
9.Construction of nursing quality evaluation index system for pediatric orthopedics

Objective:To construct a representative index system for evaluating pediatric orthopedic nursing quality, providing a basis for hospital pediatric orthopedic nursing quality assessment and monitoring.Methods:From April to July 2023, using the "structure-process-outcome" three-dimensional quality structure model as the theoretical framework, a literature review was conducted, and an item pool was formulated. Through two rounds of Delphi method expert consultations, the hierarchical analysis method was finally employed to determine the indicators and their weights at each level.Results:The effective recovery rates of the questionnaire of the two rounds of expert consultations were 100% (20/20), the authority coefficients of experts were 0.87 and 0.88, the coefficients of variation were 0.00 to 0.27 and 0.00 to 0.24. The Kendell harmony coefficients of the second and third indicators in the two rounds of inquiry were 0.140, 0.166 and 0.192, 0.161(all P<0.05). The final pediatric orthopedic nursing quality evaluation index system included 3 primary indicators, 21 secondary indicators and 83 tertiary indicators. Among the primary indicators, the weight of process quality was the highest at 0.493 4, followed by outcome quality at 0.310 8, and the lowest was structural quality at 0.195 8. In the secondary indicators, "assessment criteria of limb blood circulation" had the highest weight at 0.099 8. Conclusions:The constructed pediatric orthopedic nursing quality evaluation index system covers key aspects and is more operationally feasible. It provides better guidance for nursing interventions and quality control.
10.Development and evaluation of the children′s postoperative health-related quality of life scale for thyroid cancer

Objective:To develop the children′s postoperative health-related quality of life scale for thyroid cancer and to test its reliability and validity.Methods:The first draft of the scale was developed through literature search, focus group meetings, and a pre-survey, and 116 children (76 for testing and 40 for external validation) with thyroid cancer attending Beijing Children′s Hospital of Capital Medical University were selected to answer the scale, to screen and categorize the questions and to form the final scale with multiple dimensions.Results:The children′s postoperative health-related quality of life scale for thyroid cancer contained 5 dimensions and 29 questions. Exploratory factor analysis showed that the cumulative variance explained by the 5 factors was 64.343%. Confirmatory factor analysis showed correlations between the questions and dimensions of this scale, fair convergent validity for the scale, and good discriminant validity. The validity of the validity scale showed that there was a existing correlation between the questions and the validity scale of this scale. The item-dimension correlation coefficients showed that the questions in each dimension were well differentiated. The total Cronbach′s α coefficient of the scale was 0.930, the folded half reliability was 0.843, and retest reliabilities at 2 weeks, 1 month, and 3 months after the initial test were respectively 0.936, 0.922, and 0.910.Conclusion:The developed children′s postoperative health-related quality of life scale for thyroid cancer has good reliability and validity and can be used to assess the health-related quality of life of children after thyroid cancer surgery.
