1.Hospital Infection in Fracture Patients and Corresponding Strategy

OBJECTIVE To investigate the prevention from hospital infection and therapy in fracture patients.METHODS We analyzed the situation of hospital infection in the fracture patients ranged from 2000 to 2005 with reviewing and perspective statistical method.RESULTS Totally 43 cases were infected from 1345 fracture patients,taken 3.2%.It was the highest with infection rate under the age of 5(6.0%),then was successively at the 15-44 years old age,5-14 years old age,≥60 and 45-59 years old age.CONCLUSIONS The fracture patients or those experiencing surgical operation are easy to have hospital infection.It is a multifactorial result.For instance,the environment of ward,the disinfection measure in operation,the skilled degree of surgical operators,the length of operation time,rational selection antibiotics and so on,are all the important factors to prevent and cure hospital infection.Pay attention to the quality controlling every link,can reduce the rate of hospital infection among fracture patients.
2.Expression of pituitary tumor transforming gene and p53 protein in large intestinal adenoma and large intestinal carcinoma tissues

ObjectiveTo investigate the expression and relationship of pituitary tumor transforming gene (PTTG) and p53 protein in large intestinal adenoma,large intestinal carcinoma and normal large intestinal mucosa tissues.MethodsImmunohistochemistry was employed to determine the expression of PTTG and p53 protein in 50 cases with large intestinal adenoma tissues,42 cases with large intestinal carcinoma tissues and normal large intestinal mucosa tissues.The relationship of the expression of PTTG and p53 protein with the clinicopathological characteristics was analyzed.ResultsThere was no positive expression of p53 protein in normal large intestinal mucosa tissues,while the positive rate of PTTG expression was 7.14%(3/42).The positive rates of PTTG and p53 protein expression were 82.00%(41/50) and 90.00%(45/50) in large intestinal adenoma tissues,88.10% (37/42) and 95.24% (40/42) in large intestinal carcinoma tissues.The positive rates of PTTG and p53 protein over expression were 45.24%(19/42) and 69.05%(29/42) in large intestinal carcinoma tissues.The positive rates of PTTG and p53 protein expression in large intestinal carcinoma tissues were higher than those in large intestinal adenoma tissues and normal large intestinal mucosa tissues,the positive rates of PTTG and p53 protein expression in large intestinal adenoma tissues were higher than those in normal large intestinal mucosa tissues,and there were significant differences(P < 0.05).The expression of PTTG was not correlated with p53 protein in large intestinal carcinoma tissues(P> 0.05 ),while the positive relationship was found between the expression of PTTG and p53 protein in large intestinal adenoma tissues (P < 0.05 ).The over expression of PTTG was significantly correlated with lymph node metastasis (P < 0.01 ),but the over expression of p53 protein was not correlated with lymph node metastasis(P > 0.05) in large intestinal carcinoma tissues.Conclusions The expression of PTTG is significantly correlated with p53 protein in large intestinal adenoma tissues,and their co-expression may be used as markers for carcinogenesis of large intestinal adenoma tissues.The over expression of PTTG and p53protein is found in large intestinal carcinoma,and the over expression of PTTG is correlated with lymph node metastasis.The over expression of PTTG may be used as a marker for lymph node metastasis of large intestinal carcinoma.
3.Validity and reliability of the Chinese version of the Kutcher Adolescent Depression Scale

Objective:To assess the validity and reliability of the Chinese version of the eleven-item Kutcher Adolescent Depression Scale (KADS-11)in Chinese adolescents,calculate its optimal cut-off value and the sensi-tivity and specificity,and explore the possibility of providing a useful tool to assess the severity of adolescent de-pressive symptoms.Methods:Totally 3180 students aged 11 -17 years were selected from schools in 6 provinces and Shanghai.All of them were asked to complete the KADS-11 and Children Depression Inventory (CDI). Students whose CDI scores were above 19 (including 19)were diagnosed with the DSM-IV criteria of depressive disorder,73 students from Shanghai sample were assessed with KADS-11 and CDI to analyze the test-retest reliabil-ity 1 month later.Results:Exploratory factor analysis showed that KADS-11 had 2 factors,and confirmatory factor analysis tested proved the 2-factor model fit better than the one-factor model.The KADS-11 total scores were posi-tively correlated with CDI total scores (r =0.74,P <0.01 ),and the KADS-11 scores were higher in depressive group than those in non-depressive group.The mean area under the curve (AUC)of KADS-11was 0.94,the mean area under the curve of each item ranged from 0.7 to 0.9.The optimal cut-off point of KADS-11 was total score≥9,sensitivity and specificity were 89% and 90% respectively.The Cronbach's alpha coefficient of the KADS-11 was 0.84,the spilt-half reliability coefficient was 0.71 (P <0.01),and the test-retest coefficient was 0.77 (P <0.01).Conclusion:The KADS-11 is appropriate for Chinese adolescents because of its good validity,reliability and diagnosis accuracy,it could be used to assess depressive symptoms for adolescents.
4.Disease Course and Outcomes in Patients With the Limited Form of Neuromyelitis Optica Spectrum Disorders and Negative AQP4-IgG Serology at Disease Onset:A Prospective Cohort Study

Background:
and Purpose Patients presenting with clinical characteristics that are strongly suggestive of neuromyelitis optica spectrum disorders (NMOSD) have a high risk of developing definite NMOSD in the future. Little is known about the clinical course, treatment, and prognosis of these patients with likely NMOSD at disease onset.
Methods:
This study prospectively recruited and visited 24 patients with the limited form of NMOSD (LF-NMOSD) at disease onset from November 2012 to June 2021. Their demographics, clinical course, longitudinal aquaporin-4 immunoglobulin G (AQP4-IgG) serology, MRI, therapeutic management, and outcome data were collected and analyzed.
Results:
The onset age of the cohort was 38.1±12.0 years (mean±standard deviation). The median disease duration was 73.5 months (interquartile range=44.3–117.0 months), and the follow-up period was 54.2±23.8 months. At the end of the last visit, the final diagnosis was categorized into AQP4-IgG-seronegative NMOSD (n=16, 66.7%), AQP4-IgG-seropositive NMOSD (n=7, 29.2%), or multiple sclerosis (n=1, 4.2%). Seven of the 24 patients (29.2%) experienced conversion to AQP4-IgG seropositivity, and the interval from onset to this serological conversion was 37.9±21.9 months. Isolated/mixed area postrema syndrome (APS) was the predominant onset phenotype (37.5%). The patients with isolated/mixed APS onset showed a predilection for conversion to AQP4-IgG seropositivity. All patients experienced a multiphasic disease course, with immunosuppressive therapy reducing the incidence rates of clinical relapse and residual functional disability.
Conclusions
Definite NMOSD may be preceded by LF-NMOSD, particularly isolated/ mixed APS. Intensive long-term follow-up and attack-prevention immunotherapeutic management is recommended in patients with LF-NMOSD.
5.Biological function and molecular mechanism of URI in HepG2 cells.

OBJECTIVETo explore the effect and molecular mechanism of the unconventional prefoldin RPB5 interactor (URI) in hepatocellular carcinoma HepG2 cells.
METHODSThe cDNA sequence and shRNA of URI were obtained and sub-cloned into eukaryotic expression vectors. Then those vectors were transfected into HepG2 cells to obtain stable transfection cell line. The cell proliferation and anchor-independent growth in URI-overexpressing and knockdown HepG2 cells were determined by CCK-8 and soft agar colony assay. Flow cytometry was applied to detect the cell cycle and apoptosis of γ-ray irradiated cells. Apoptosis related genes were detected by Western blot.
RESULTSThe pCDNA3.1-URI and pGPU6-URIi eukaryotic expression vectors were constructed successfully and corresponding stable transfection cell lines were obtained. Cell proliferation rates of the HepG2, pCDNA3.1-URI-HepG2 and pGPU6-URIi-HepG2 cells were (588.78 ± 32.12)%, (959.33 ± 58.8)% and (393.93 ± 39.7)%, respectively (P < 0.05). The number of cell clones of HepG2, pCDNA3.1-URI-HepG2 and pGPU6-URIi-HepG2 cells were 43 ± 7, 85 ± 5 and 20 ± 4 (P < 0.05), respectively. After γ-ray irradiation, the URI-overexpressing cell line showed a significantly lower apoptosis rate and G(2)/M phase arrest than those in the URI-depleted cell line (P < 0.05). In the HepG2 cells, the relative protein expression levels of URI, Bax and Bcl-2 were 0.92 ± 0.03, 1.11 ± 0.13 and 0.82 ± 0.01 (P < 0.05). In the pCDNA3.1-URI-HepG2 cells, the relative protein expression levels of URI, Bax and Bcl-2 were 1.79 ± 0.12, 0.48 ± 0.01 and 2.20 ± 0.30 (P < 0.05), respectively. In the pGPU6-URIi-HepG2 cells, the relative protein expression levels of URI, Bax and Bcl-2 were 0.50 ± 0.04, 1.52 ± 0.20 and 0.38 ± 0.01 (P < 0.05), respectively. The expression of Bax was down-regulated and Bcl-2 was up-regulated in the URI-overexpressing cell line. However, on the contrary, expression of Bax was up-regulated and Bcl-2 was down-regulated in the URI-depleted cell line.
CONCLUSIONSURI may promote the growth of hepatocellular carcinoma cells via inhibition of cell proliferation and reducing the apoptosis in hepatocellular carcinoma cells in vitro. After the impairment of URI expression, the proliferation ability of hepatocellular carcinoma cells is suppressed and the ability to resist γ-ray irradiation is reduced. URI may become a potential new target for cancer therapy of hepatocellular carcinoma.

6.Risk factor analysis of plasma donation related vasovagal reaction

【Objective】 To investigate the risk factors of vasovagal reactions(VVR) related to plasma donation, so as to put forward clinical suggestions for early identification and accurate intervention of high-risk groups to ensure the safety of plasma donation. 【Methods】 The demographic characteristics(i.e. gender, age, weight) and records of plasma donors(donation history, pulse before plasma donation, duration of collection, etc.) were collected from July to December 2019 in a region of Sichuan. Based on logistic regression analysis, the correlation between these factors and the risk of VVR was explored. 【Results】 The information of 69 172 donors was collected, and the incidence of VVR was 7.04‰. The risk of VVR was reduced by 99% in the group with plasma collection duration less than 30 minutes compared with the group with plasma collection duration more than 50 minutes(OR, 0.01; 95% CI, 0.00~0.01; P<0.001). The risk of male group was 94 % lower than that of female group(OR, 0.06; 95% CI, 0.04~0.10; P<0.001). Compared with the 45~50 kg group, the risk of weight greater than 80 kg group decreased by 80%(OR, 0.20; 95% CI, 0.09~0.42; P<0.001). The risk of repeated donation group was 34 % lower than that of the first time donation group(OR, 0.66; 95% CI, 0.47~0.91; P<0.001). The risk of VVR in the group with pulse greater than 90 bpm before plasma donation was 2.43 times that in the 60~69 bmp group(OR, 2.43; 95% CI, 1.75~3.36; P<0.001). 【Conclusion】 Duration of plasma collection, gender, weight, frequency of plasma donation, pulse before plasma donation and donor status are independent risk factors for plasma donation-related VVR. Among them, plasma collection duration, gender and weight were the main independent risk factors for plasma donation-related VVR. For donors with plasma collection duration more than 50 minutes, female and low weight, higher risk of VVR was presented and more preventive intervention should be given.

8.Effect of light and riboflavin concentration on photodynamic inactivation of pathogens by riboflavin

Objective :
To investigate the effect of riboflavin on inactivation of pathogens under different light time conditions.
Methods :
Plasma containing vesicular stomatitis virus (VSV) was divided into 4 groups,and the final concentrations of riboflavin were 50,100,150 μmol / L and 0 μmol / L was used as the control group.The growth titers at 10,15,20 and 25 min were observed.The control group was treated with no light.The growth titers of VSV culture before and after inactivation were calculated by Reed-Muench method,and the effect of different concentrations of riboflavin on the inactivation effect was evaluated by log reduction factor after inactivation.
Results :
Under the condition of no light,the addition of riboflavin had no pathogen inactivation effect.With the increase of light time from 15 min to 25 min,the inactivation effect of VSV in the non-riboflavin group increased.When the light was less than 15 min,there was no significant difference in the inactivation effect of 50 to 150 μmol / L riboflavin VSV.When the light duration was 15 to 25 min,the inactivation effect of VSV was enhanced after the addition of riboflavin,but there was no difference between different concentrations of riboflavin groups.
Conclusion
The inactivation effect of riboflavin pathogen within 25 min is better with the increase of light time,and has little influence on riboflavin concentration.
