2.Quality study about the cooperation experiences of doctors and nurses from the Infectious Department

Objective To understand the cooperation experiences of doctors and nurses from the Infectious Department,that may be contributive to promote a harmonious relationship between doctors and nurses.Methods Sixteen nurses and nine doctors came from Infectious Department were interviewed.The data were collected through face-to-face in-depth semi-structured interviews.Thematic analysis based on Qualitative Research method,qualitative content analysis was used for data interpretation by analysis and management.Results Three themes were identified and described as the cooperation experiences between doctors and nurse,the cooperation troubles,suggestions about promoting a harmonious relationship between doctors and nurses.Conclusion Harmonious relationship is particularly important in the clinical work,but in fact,there are many factors affect the harmonious relationships,so it is recom-mended that hospital leaders should take measures to build a harmonious relationship between doctors and nurses.
3.Antineoplastic molecular mechanisms on bufalin

There is a long history of Toad venom in the treatment of cancer in China.Bufalin,extracted from Toad venom,is one of the biologically active compounds of anticancer.We elaborate the molecular mechanism of bufalin on anticancer activity from several aspects such as inducing cell apoptosis and differentiation,inhibitting cell proliferation and angiogenesis,enhancing the sensitivity of chemotherapeutics,which can provide the basis for in-depth study of Toad venom and its development and clinical medication.
4.Analysis of Differentially Expressed Proteome in Urinary Exosome from Non-small Cell lung Cancer Patients

Urine provides an alternative to blood plasma as a potential source of disease biomarkers. Exosomes was separated by ultracentrifuge at 200000 g in normal persons and non-small cell lung cancer (NSCLC) patients′ urine. For proteomic analysis of urinary exosome, 1D sodium dodecylsulfonate-polyacrylate gel electrophoresis(SDS-PAGE) was carries out and cut the gel 31 kDa-20 kDa bands in normal group and disease group′s. These gel blocks were subjected to in-gel trypsinization, and the extracted peptides were analyzed HPLC-CHIP-MS/MS. Approximately 24 unique proteins were identified in the UniProtKB/SWISS-PORT. The difference expression proteins were found in urinary exosome from NSCLC patients, including three fragment of the immunoglobulin kappa, two kinds of Ras related proteins, glutathione S-transferase A2, serum amyloid P-component precursor and phosphatidylethanolamine-binding protein 1.
5.In vivo and in vitro study of aescin in L1210 murine acute lymphoid leukemia cells

Objective To explore aescin's inhibition of in vitro proliferatim in L1210 murine acute lymphoid leukemia cells and its in vivo antileukemic effects.Methods MTT assay was used to determine the anti-proliferative effect in vitro;Leukemia transplant models were used to test the antileukemic effect. Mice inoculated with L1210 cells were treated with ten daily doses of aescin or/and each other day of DOX, and were observed for survival.Antileukemic effects were assessed by calculating the extension of lifespan. Results Aescin(20~60?g/mL) was found to be able to inhibit the proliferation of L1210 cells.The effects were in a dose- and time- dependent manner.The IC_(50) value of aescin in L1210 cells after 72 h was (24.86?2.23)?g/mL.In vivo study showed that after treating the L1210 cells bearing mice with higher, middle,and low dosage(4.5,3.5,and 2.5 mg/kg) of aescin for ten consecutive days,The extension of lifespan were 25.8%(P
6.Risk factors analysis of liver dysfunction in patients with colorectal cancer after chemotherapy

Objective:To investigate the risk factors, clinical features and prognosis of abnormal liver function after receiving oxaliplatin-containing chemotherapy regimen in patients with colorectal cancer, and to provide a relevant basis for clinical diagnosis and treatment.Methods:The clinical data of 108 colorectal cancer patients who received XELOX (oxaliplatin+capecitabine) or mFOLFOX6 (oxaliplatin+leucovorin+ 5-fluorouracil) chemotherapy regimen from October 2017 to May 2019 in the First Hospital of Shanxi Medical University were analyzed retrospectively. According to the liver function indexes after chemotherapy, the patients were divided into abnormal liver function group and normal liver function group. The observation indexes included alanine aminotransferase, aspartate aminotransferase, total bilirubin, direct bilirubin, alkaline phosphatase, γ-glutamyl transpeptidase. The clinical characteristics of liver dysfunction after oxaliplatin-containing chemotherapy were analyzed and the related factors that might lead to liver dysfunction were analyzed by using multivariate logistic regression analysis.Results:Among 108 patients receiving chemotherapy, there were 67 (62.0%) cases of abnormal liver function. The main grades of liver dysfunction were grade 1 and grade 2, including 49 cases of grade 1 (73.1%) and 16 cases of grade 2 (23.9%). After chemotherapy, the abnormal liver function usually began in 1-4 cycles, of which 22 cases were 1 cycle (32.8%), 17 cases were 2 cycles (25.4%), 20 cases were 3 cycles (29.8%), and 4 cases were 4 cycles (6.0%). Univariate analysis showed that the age <60 years old, chemotherapy cycle >6, the use of mFOLFOX6 regimen, unprotected hepatoprotective drugs were related to liver dysfunction ( χ2 values were 3.910,4.799, 12.861, 4.044; all P < 0.05). Multivariate logistic regression analysis showed that mFOLFOX6 regimen and unprotected hepatoprotective drugs were independent risk factors of abnormal liver function ( HR = 3.405, 95% CI 1.266-9.159, P = 0.015; HR = 2.348, 95% CI 1.012-5.477, P = 0.047). Conclusions:For patients with colorectal cancer who have a high risk of liver dysfunction after chemotherapy, it is recommended to prefer XELOX regimen among oxaliplatin-containing chemotherapy regimens and to take preventive liver protection treatment.
7.Clinical significance and changes of inflammatory cytokines in patients suffered from progressive cerebral ;infarction

Objective To observe and analyze the changes of inflammatory cytokines levels in patients with progressive cerebral infarction (PCI ).Methods The patients with progressive cerebral infarction (progressive cerebral infarction group),the patients with non-progressive cerebral infarction(non progressive cerebral infarction group)and the healthy individuals(control group)were selected as the research subjects.Sixty individuals were included in each group.The levels of IL-8,hs -CRP,SAA of the research subjects were detected and observed. Results The levels of IL -8[(201.89 ±46.41)mg/L],hs -CRP[(16.55 ±4.49)mg/L],SAA[(20.65 ± 4.17 )mg/L]of the progressive cerebral infarction group were significantly higher than those of non -progressive cerebral infarction group[(132.02 ±40.26 )mg/L,(8.09 ±4.06)mg/L and (15.35 ±4.07 )mg/L).]The differ-ences were statistically significant (t=4.289,P=0.000;t=2.276,P=0.006 and t=2.269,P=0.031).Compared with the control group[(50.70 ±31.14 )mg/L,(0.70 ±0.11 )mg/L and (5.15 ±2.21)mg/L],the levels of IL-8, hs-CRP,SAA in the progressive cerebral infarction group and non -progressive cerebral infarction group were significantly higher on the first day (PCI group vs control:t=3.614,P=0.000;t=3.406,P=0.007;t=2.559, P=0.009;NPCI group vs control:t=3.602,P=0.000;t=3.181,P=0.009;t=1.520,P=0.026).Conclusion The changes of the inflammatory factors levels such as IL-8,hs-CRP,SAA have certain predicative value on the development of the PCI.
8.Analysis of RR-Lorenz Plot in Patients of Sinus Rhythm With Long RR Interval

Objective: To explore the advantage of RR-Lorenz plot (RR-LP) in analyzing the patients of sinus rhythm with long RR interval.
Methods: A total of 308 RR-LP patients with long RR interval were retrospectively studied. The patients were divided into 7 groups according to the type of long RR intervals. ① Sinus bradycardia with arrhythmia group, n=63,②Repeated transient sinus arrest group, n=16, ③Ⅱ° sino-atrial block group, n=14, ④Ⅱ° atrial ventricular block (Ⅱ° AVB) group, n=47, ⑤ Un-passed atrial premature beats (APB) group, n=28, ⑥ Atrial premature beats group, n=72 and ⑦ Premature ventricular beats group, n=68. We analyzed the patients of RR interval greater than 1500 ms with ambulatory electrocardiogram record.
Results: ①RR-LP of sinus bradycardia with arrhythmia group showed a single distributing area with 1500 ms for the origin of transverse and longitudinal axis with B line slope at 1, tilt angle of 45°.②RR-LP of repeated transient sinus arrest,Ⅱ° sino-atrial block,Ⅱ° AVB and APB groups showed special four distributing areas with B line slope at (0.51 ± 0.01), tilt angle of (23.04 ± 0.50) °, B line slope at 0.6, tilt angle of (27°), B line slope at (0.57 ± 0.21), tilt angle of (25.69 ± 9.59)° and B line slope at (0.50 ± 0.01), tilt angle of (22.59 ± 0.54) ° respectively.③RR-LP of premature beats groups showed special four regional distributing areas, B line slope for atrial premature beats was at (0.38 ± 0.12), tilt angle of (17.06 ± 5.22) ° and B line slope for ventricular premature beats was at (0.07 ± 0.05), tilt angle of (3.02 ± 2.39) °.
Conclusion: RR-LP in patients of sinus rhythm with long RR interval had speciifc morphology and distribution features, the local abnormality could be found in a plane via all RR intervals which provided a differential diagnosis for repeated occurrence of short RR interval.
9.A meta-analysis to compare LC +LCBDE with ERCP+LC for patients with concomitant gallstones and common bile duct stones

Objective To compare the safety and efficiency of laparoscopic cholecystectomy (LC) + laparoscopic common bile duct exploration (LCBDE) with endoscopic retrograde cholangiopancreatography (ERCP) /endoscopic sphincterotomy (EST) +LC for patients with concomitant gallstones and common bile duct stones.Methods The Pubmed,EMBASE and CNKI were searched for randomized controlled trials published from 1990 to March,2012.Revman 5.1 was used to perform the meta-analysis.The main outcomes were stone clearance rate,treatment morbidity,operative time and length of hospital stay.Results 9 studies with 1021 patients were included into the final analysis.Meta-analysis demonstrated that LC+LCBDE was similar to ERCP/EST+LC in the stone clearance rate (OR 1.55,95%CI 0.95~2.52,P=0.08),treatment morbidity (OR 1.12,95%CI 0.75~1.67,P=0.58),operative time (WMD-54.44,95%CI-107.7~-1.17,P=0.05) and length of hospital stay (WMD-0.22,95%CI-2.6~2.16,P=0.86).The P value of the stone clearance rate was approaching 0.05 with increased number of studies in the analysis.There was no significant difference in the operative time,probably because of small sample size.Conclusions LC-LCBDE was equivalent to ERCP/EST+ LC in stone clearance rate,treatment morbidity,operative time and length of hospital stay.There is a possibility that LC+LCBDE can be superior to ERCP/EST+LC in the stone clearance rate and the operative time with increase in sample size.
10.Minimally invasive surgery technology of gallbladder stones combined bile duct stones

While Choledocholithiasis is concomitant with gallstones in approximately 9.2% ~ 14.3% of the patients,and its treatment methods are mainly by surgery,including:(1)Open cholecystectomy and open choledocholithotomy 、T-tube drainage (OC-OCHTD) ; (2) Endoscopic retrograde cholangiopancreatography (ERCP) and Endoscopic sphincterotomy (EST),and followed by Laparoscopic cholecystectomy (LC) second stage (ERCP/EST + LC) ; (3) Laparoscopic cholecystectomy combined with laparoscopic common bile duct exploration (LC +CBDE).These three methods all have their own characteristics.The last two methods are minimally invasive surgery treatment.Practice has proved that LCBDE and ERCP + EST + LC treatment methods are feasible and show certain advantages,compared with OC-OCHTD reflecting the development of minimally invasive surgery.Practice has proved that LCBDE and ERCP + EST + LC treatment methods are feasible and show certain advantages,compared with OCHTD reflecting the development of minimally invasive surgery.But the indications of operation are need to be summarized constantly in clinical practice,while operation skills are also need to be improved.At present,many reports of the two methods of minimally invasive treatment have been published because of the controversy,such as surgical indications,hospitalization expense,operation time,therapy danger,complications and the length of hospital stay.Both LCBDE and ERCP + EST + LC show some advantages and disadvantages compared with each other.On proper indications,LCBDE embodies the superiority of minimally invasive surgery technology as a one-stage operation,which is preferred for patients of choledocholithiasis with concomitant gallstones.For patients,no matter what kind of operation method,less invasion,lower risk and less complications is the key for best treatment choise.
