1.Advances on continuing care in patients with ankylosing spondylitis

2.The practice and exploration of integration of clinical teaching pattern management in traditional Chinese medicine

Current undergraduate education in traditional Chinese Medicine combines classroom teaching with a small number of practical courses, which has showed its limitations of increasingly bad medical environment and employment situation. Nanjing University of Chinese Medicine explored new ways and means of medical education in recent years. Therefore, reforming based on the integration of clinical teaching pattern for TCM Bachelor has been carried out step by step. It meant that classroom teaching and clinical practice were fully integrated by moving classroom in the affiliated hospitals. It has been proved to work. With the experiences of the development, we have successfully explored a mature mean for the integration clinical teaching pattern. Here, we discussed the codes of such practice in order to strive to maximize the effectiveness of such practice.
3.Clinical Study on Infliximab for Treating Refractory Ulcerative Colitis

Background:Patients with refractory ulcerative colitis( UC ) cannot benefit from the conventional aminosalicylic acids,steroids and immunosuppressants. The emergence of biological agents offers new opportunities for these patients. Aims:To evaluate the clinical efficacy of infliximab for treating patients with refractory UC. Methods:Twelve patients with moderate to severe active UC admitted from Dec. 2012 to Jan. 2014 at the Second Affiliated Hospital of Harbin Medical University were retrospectively enrolled. All of them were steroids-refractory or steroids-dependent and infliximab therapy was then administered. Modified Mayo score was used to evaluate the disease activity and the efficacy of infliximab. Results:All patients completed the infliximab therapy and a 38-week follow-up. After treatment,the clinical manifestations were improved with varying degrees,and the efficacy at the 6th,22nd and 38th weeks was 66. 7%,83. 3% and 91. 7%, respectively,with significant differences between any two time points(P﹤0. 05). Conclusions:Infliximab is effective and practical for the treatment of refractory UC.
4.N-Terminal pro-brain natriuretic peptide and ischemic stroke

Brain natriuretic peptide (BNP) is one of the most sensitive and specific laboratory indicators during cardiac dysfunction.N-Terminal pro-brain natriuretic peptide (NT-proBNP) is the N-terminal precursor of BNP.The functions of both are not consistent,but its physicochemical properties are superior to BNP.Now it has been widely used in the diagnosis and treatment monitoring of cardiovascular diseases.In the field of stroke,the testing and application of BNP,especially NT-proBNP is less.This article reviews the roles of NT-proBNP in cardio-cerebrovascular diseases,especially in ischemic cerebrovascular disease.
5.The investigation on the study effect of new teaching methods of acupuncture on non-acupuncture master postgraduate in the background with dual-track in resident standardization training

A total of 60 non-acupuncture master postgraduates were divided into control class and experimental class randomly, 30 postgraduates in each class. The control class was taught by traditional teaching methods and experimental class was taught by new teaching methods. The test was conducted after one month later. All the scores were analyzed by SPSS17.0. The result showed that the oretical score ,the practical score and disease recognition rates in the experimental class were significantly higher than those in the control class (P<0.05). The questionnaires showed experimental class had obvious advantage than control class (P<0.05). The new teaching methods of acupuncture has obvious advantage than traditional teaching methods and can provide a new way for teaching acupuncture during resident standardization training.

Under the background of dual-track training mode, the current clinical program of professional master's graduate education lacks complete and unified content and requirements. We set out a unified teaching scheme of subspecialty, in the main subject acupuncture. We identified the teaching management and innovated the teaching and examination methods in the scheme. Our research gradually formed a multidisciplinary talents training system, as the foundation of developing the standardization training. This method could help to cultivate qualified graduates as the high quality talents for the national medicine development.
7.Expression of MUC15 and PI3K/Akt in gastric carcinoma and its association with clini-copathological characteristics and prognosis

Objective:To analyze the expression of MUC15 and PI3K/Akt in gastric carcinoma and its association with clinicopathologi-cal characteristics and prognosis. Methods:The expression of MUC15 and Akt was detected in 144 cases of gastric carcinoma tissues and corresponding para-carcinoma tissues by tissue microarray and immunohistochemistry. Results:The positive expression rate of MUC15 in gastric carcinoma was 79.8%, higher than that of para-carcinoma tissues (22.2%, P<0.01). The positive expression rate of Akt protein in gastric carcinoma was 80.6%, higher than that of para-carcinoma tissues (16.7%, P<0.01). The expression of MUC15 and Akt was statistically associated with the grades of differentiation, invasion depth, lymphatic metastasis, TNM stage of tumor tissues (P<0.05), and the positive correlation between the two protein expression that appear in the gastric tumor tissue (P=0.001). Univariate survival analysis showed that the over-expression of either MUC15 or Akt was inversely correlated with the survival time (P<0.05 and P<0.01, respectively). Cox multiple regression analysis indicated that patients with over-expression of both MUC15 and Akt had the worst prognoses (HR=3.115, P<0.05). Conclusion:MUC15 may be involved in the occurrence, development, invasion, and metastasis of gastric cancer through the PI3K/Akt cell signaling pathway, and the expression of MUC15 combined with Akt is a powerful predictor for the prognosis of gastric cancer.
8.An evaluation of the effectiveness of implementation of national food safety standard for “Iodine Concentration in Edible Salt” (GB 26878-2011)

Objective To observe the implementation of national food safety standard for “Iodine Concentration in Edible Salt”(GB 26878-2011) and its effectiveness on iodine nutritional status of key populations. Methods Information of iodine concentration in edible iodized salt of various provinces (autonomous regions and municipalities, including Xinjiang Production and Construction Corps) was collected using Baidu Searching Engine through the establishment of key words. Sal t samples were collected in Tianjin City and Aksu Region of Xinjiang , and the salt iodine concentration in both places was 30 mg/kg. In Tianjin, Hongqiao, Tanggu and Hangu, Beichen were selected as representatives of the downtown areas, the coastal areas and the suburbs, respectively and counties of Baodi and Ji were iodine deficiency areas in history. Sampling work was carried out from August 2012 to March 2013 in Tianjin. In Aksu, Yatuoer Township and Charqi Town in Baicheng County, Aotebeixi and Aketuohai Townships in Wushen County were chosen as iodine deficiency areas, and the survey was carried out from January to September 2013 . Random urine samples of school-age children ( 8 - 10 years old ) , pregnant women and lactating women were collected; urinary iodine was measured following the Method for Determination of Iodine in Urine by As3+-Ce4+ Catalytic Spectrophotometry(WS/T 107-2006) and iodine in edible iodized salt was measured following the General Test Method in Salt Industry Determination of Iodideion ( GB/T 13025 . 7-1999 ) . Results Fourteen of the provinces(autonomous regions and municipalities, including the Corps of Xinjiang) chose 25 mg/kg as their iodine concentration in edible iodized salt and 13 provinces chose 30 mg/kg. Besides, there were another 5 provinces providing 30 mg/kg particularly for pregnant women and lactating women while 25 mg/kg for other populations. In Tianjin, the medians of iodine concentration in edible iodized salt were ranged from 24.4 - 32.1 mg/kg in retail stores and 26.4 mg/kg at households. The household coverage rate of iodized salt and the proportion of households using adequately iodized salt were 78.5%(168/214) and 62.6%(134/214), respectively. The median ranges of urinary iodine were 178.2 - 183.9 μg/L in school children, 124.3 - 130.9 μg/L in pregnant women and 72.7 - 109.5 μg/L in lactating women. In Aksu, the medians of iodine concentration in edible iodized salt were 27.1 and 26.5 mg/kg in retail stores and households, respectively. The household coverage rate of iodized salt and the proportion of households using adequately iodized salt were 100.0% (363/363) and 98.9%(359/363), respectively. The median ranges of urinary iodine were 174.8 - 293.0, 154.9 - 230.0 and 135.8 - 239.3 μg/L among school children, pregnant women and lactating women, respectively. The median of iodine concentration in a special edible iodized salt sample reached 49.1 mg/kg, and qualified rate was 0(0/11) in Aksu. Conclusions All provinces , municipalities and autonomous regions ( including the Corps of Xingjiang ) in China have adjusted the iodine content in edible iodized salt in accordance with GB 26878-2011. However, in Tianjin the household iodine concentration in edible salt is lower than the local standards; the household coverage rate of iodized salt and the proportion of households using adequately iodized salt are lower than the national standards; pregnant women and lactating women are at risk of mild iodine deficiency.
9.Association of prostasin gene rs12597511 polymorphism with outcomes of pregnancy with severe preeclampsia.

OBJECTIVETo assess the association of prostasin gene rs12597511 polymorphism with clinical features and pregnancy outcomes among patients with severe preeclampsia.
METHODSClinical manifestations, pregnancy outcomes and the genotypes of 179 patients with severe preeclampsia [early-onset group (≤34 gestational weeks): 79 cases; Late-onset group (>34 gestational weeks): 100 cases] and 222 normal-term pregnant women (control group) were collected.
RESULTSIn the early-onset group, the patients with TC or CC genotype at rs12597511 had higher incidences of total complications, liver dysfunction, neonatal asphyxia, neonatal intracranial hemorrhage and perinatal mortality compared with those with TT genotype (P>0.05). Multiple logistic regression analysis showed that the complication rates of severe preeclampsia patients are closely related to TC or CC genotypes, 24 h urinary protein and gestational weeks of onset (OR=1.049, 95% CI:1.007-1.093, P=0.021; OR=1.031, 95% CI: 0.350-0.883, P=0.013; OR=0.733, 95% CI: 0.566-0.950, P=0.019), and the perinatal mortality is related to gestational weeks at delivery (OR=0.542, 95% CI: 0.331-0.887, P=0.015).
CONCLUSIONPolymorphism of the prostasin gene is closely associated with poor pregnancy outcomes of early-onset severe preeclampsia.
