1.Epidemiological features and disease spectrum of hepatitis C in Qinghai Province,China:analysis of 1 10 cases

Objective To investigate the epidemiological features and disease spectrum of inpatients with hepatitis C in recent years and to provide reference for the control of hepatitis C.Methods A retrospective study of demography,epidemiology,and laboratory examination was performed in 1 10 hospitalized patients with hepatitis C.Results Of the 1 10 patients,60 were male and 50 were female,with a male-to-female ratio of 1.2:1.The main transmission routes included blood product transfusion (33.6%),surgery (30.0%),intravenous drug use (14.5%),and sexual transmission (4.5%).The other 19 cases (17.4%)were of unknown transmission route.The annual numbers of cases from 2008 to 2011 were 16,18,32,and 44,respectively.The genotype proportions in patients were as follows:1b,60.7%(17/28);2a,28.6%(8/28);1b/2a mixed type,7.1%(2/28);3b,3.6%(1/28).Of all patients,13.6%(15/110)were found with a-cute hepatitis C,67.3% (74/110)with chronic hepatitis C,19.1% (21/110)with liver cirrhosis,and 10.9% (12/110)with decom-pensated cirrhosis.Conclusion The incidence of HCV infection has been increasing year by year.The major HCV genotypes are 1 b and 2a.Hepatitis C is apt to develop into liver cirrhosis.It is necessary to reinforce monitoring and to carry out specific epidemiological study a-mong the target population.
2.Inhibitory Effects of Pharbitidis Semen on Rat Hapatoma Induced by N-nitrosodiethylamine

Objective To study the inhibitory effect of Pharbitidis Semen on rat hepatoma induced by N-nitrosodiethylamine ( NDEA) . Methods SD rats were divided into normal control group, model control group and Pharbitidis Semen group. In model control group and Pharbitidis Semen group, 0. 01% NDEA was applied for 90 days to induce hepatoma, and rats in Pharbitidis Semen group concomitantly received feed containing 6% Pharbitidis Semen at the dosage of 40 g·kg-1 ·d-1 . Thirty days after the hepatoma inducement and Pharbitidis Semen administration, the rats were sacrificed to observe the pathological changes in liver, number of hepatoma nodules and liver weight. The changes of liver/body weight, serum alanine aminotransferase (ALT), γ-glutamyl transferase (γ-GT), and alkaline phosphatase (ALP) were compared. One-way ANOVA (LSD Test) was employed for statistical analysis. Results In the normal control group, the number of hepatoma nodules was 0. 0±0. 0, the liver weight was (9. 87±1. 30) g, the ratio of liver/body weight was (2. 62±0. 24)% and the level of serum ALT was (64. 10±12. 71) U·L-1,γ-GT was (0. 80± 0. 42) U·L-1, and ALP was (121. 20±37. 57) U·L-1. In the model control group, the number of hepatoma nodules was (27. 4±9. 5), the liver weight was (21. 38±7. 29) g, the ratio of liver/body weight was (5. 82±2. 31)%, the level of serum ALT was (175. 70±48. 75) U·L-1, γ-GT was (41. 80±15. 38) U·L-1, and ALP was (200. 50±35. 78) U·L-1. In the Pharbitidis Semen group, the number of hepatoma nodules was (8. 6± 5. 3), the liver weight was (13. 91±3. 55) g, the ratio of liver/body weight was (3. 86±0. 76)% and the level of serum ALT was (113.10±45.35) U·L-1, γ-GT was (13. 40± 6. 15) U·L-1, and ALP was (155. 80±30. 26) U·L-1. The results showed that all indices of Pharbitidis Semen group were higher than those of the normal control group, and lower than those of the model control group (P<0. 01 or P<0. 05). Conclusion Pharbitidis Semen can reduce NDEA-induced injury to the liver cells, and inhibit the overgrowth of the hepatoma.
3.Effect of Astragalosides on Gastrin Receptor in Gastric Parietal Cells of Rats with Spleen Deficiency

[Objective] To observe the regulatory effect of astragalosides (AST) on the binding capacity of gastrin receptor in gastric parietal cells (GPC) of rats with spleen deficiency. [Methods] SD rats were given gastric gavage of Radix et Rhizoma Rhei (Dahuang) decoction 10 g/kg to induce spleen deficiency. Gastric parietal cells were isolated and purified by Lewin gastric sac method and Percoll density gradient separation method. 2 mL suspension of GPC from rat models was incubated in vitro with AST 20 ?L at the concentrations of 10, 20 and 30 g/L respectively. Suspension of GPC from rat models incubated with phosphate buffered saline (PBS) served as the model group and suspension of GPC from the normal rats incubated with PBS as the normal group. The binding capacity of gastrin receptor in GPC was detected by radioligand receptor assay. 4.A new combined nutrition strategy in the prevention and treatment of patients with severe burn sepsis

Objective To analyze the effect of a combined strategy of enteral nutrition in the prevention and treatment of patients with severe bum sepsis. Methods 46 cases were random divided into combined group (group C) and routine group (group R). Besides general supportive treatment,group C was treated with early enteral feeding (EEF) ,recombinant human growth hormone (rhGH) and gluta-mine (Gln), while group R was given general supportive treatment only. The plasma concentration levels of pre-albumin (PA) , C-reactive protein (CRP), procalcitonin (PCT) and tumor necrosis factora (TNFα) were measured immediately after hospitalization and postbum day (PBD)7, 14,28. Results The survival rate of group C was significantly higher than group R. The incidence of sepsis, the complication of heart and gastrointestin in group C was significantly lower than group R ( P < 0. 01 ~ 0.05). The days in hospitalization of group C was shorter than that of group R. Plasma PA levels was decreased in two groups and was markedly decreased in the group R( P < 0. 01 ~ 0.05). Plasma CRP, PCT and TNFαlevels were both increased in two groups and markedly increased in the group R ( P < 0.001 ~ 0.05). Conclusions We have shown that a combined method including rhGH, EEF, Gln and anisodamin can effectively reduce the morbidity and mortality rates of burn sepsis and improve prognosis.
5.Measurement of the angular velocity and perpendicular ground reaction force of the ankle joint in parachute landing simulation

Objective To measure the angular velocity and perpendicular ground reaction force of the ankle joint under different heights with half-squat jumping in parachute training simulation,providing a reliable experiment basis for the preventing of ankle injury.Methods A total of 18 volunteers participated in this study.The experimental group included 9 male with experience of parachute landing,while the other 9 male without experience of parachute landing were assigned to the control group.Each subject was instructed to jump off a platform with a height of 30 cm and 60 cm and land on a hard surface in a half-squat posture.The dynamic landing process was recorded with a high speed camera and the biomechanical data was collected and analyzed,including perpendicular ground reaction force,angular displacement,velocity and acting time.Results From 30 cm's height,the ankle angular displacement of the control group was significantly larger than the experimental group (25.73°± 8.13° vs 20.05°± 12.27°,P < 0.05).The perpendicular ground reaction force of the control group was significantly smaller than the experimental group (3 372.4±748.6 N vs 5 181.5±1 726.2 N,P < 0.05).The acting time of the control group was significantly longer than the ex perimental group (0.049±0.015 s vs 0.012±0.004 s,P < 0.05).The buffer time of the control group was significantly shorter than the experimental group (1.397±0.746 s vs 1.737±0.451 s,P < 0.05).From 60 cm's height,the ankle angular velocity of the control group was significantly higher than the experimental group (25.45± 15.01 °/s vs 16.51 ±4.18 °/s,P < 0.05).The perpendicular ground reaction force of the control group was significantly smaller than the experimental group (4 616.0±1 124.7 N vs 7 119.5±2 307.4 N,P < 0.05).The acting time of the control group was significantly longer than the experimental group (0.048±0.013 s vs 0.015±0.006 s,P < 0.05).The buffer time of the control group was significantly shorter than the experimental group (0.922±0.347 s vs 1.617±0.547 s,P < 0.05).Conclusion Jumping from different heights,the experinental group was larger in perpendicular ground reaction force but smaller in the angular velocity and displacement than the control group.There was a shorter acting time and a longer buffer time in the experimental group than the control group.
6.Effects of astragaloside on TRPC6 expression on mouse podocyte induced by TGF-β1

Objective:To explore the possible mechanism of astragaloside involved in the mouse podocytes injury induced by TGF-β1 in vitro.Methods:Mouse podocytes were cultured in vitro and then all cell were divided into 5 groups:normal control group , TGF-β1 treatment group ,TGF-β1 treatment +astragaloside low dose group ,TGF-β1 treatment +astragaloside middle dose group and TGF-β1 treatment +astragaloside high dose group.The proliferation rate of each group was investigated by MTT assay ,the expression of TRPC6 protein and mRNA were measured by Western blot and RT-PCR respectively after 48 hours.Results:TGF-β1 can significantly inhibit the proliferation of podocytes ( P<0.05) ,fusions of foot processes or even effaced of podocytes were observed .TGF-β1 could also increase the expression of TRPC6.Astragaloside could reduce the inhibition of TGF-β1 to the proliferain of podocytes significantly ,make the cell shape tend to be normal,and reduce the expression of TRPC6 mRNA and protein with dose-effect relation.Conclusion:TRPC6 play an impor-tant role in the TGF-β1 induecd podocytes injury .Astragaloside can alleviate podocytes injury by reduce the expression of TRPC 6.
7.Role of transient receptor potential cation channel 6 (TRPC6) channel in mouse podocyte injury induced by TGF-β1

Objective To investigate the effects of transient receptor potential cation channel 6 (TRPC6) on the expression of nephrin, desmin and caspase 9 and on the apoptosis of podocytes in a mouse model of podocyte injury induced by TGF-β1.Methods Conditionally immortalized mouse podocytes were cultured in vitro and divided into four groups: control, TGF-β1 treatment, TGF-β1+PGPU6/GFP/Neo-TRPC6-mus-581 (TRPC6 knockdown) and TGF-β1+PGPU6/GFP/Neo-NC (negative control).Real-time RT-PCR and Western blot analysis were performed to detect the expression of nephrin, desmin and caspase 9 at mRNA and protein levels, respectively.Flow cytometry was used to analyze the apoptotic rate of podocytes.DAPI fluorescent staining was used to observe the morphological changes of apoptotic podocytes.Results Green fluorescent protein (GFP)-expressing podocytes at 48 hours after transfection were significantly more than those at 24 hours after transfection.The level of TRPC6 in mouse podocytes transfected with PGPU6/GFP/Neo-TRPC6-mus-581 was significantly decreased as compared with that of the control group (P<0.05).No significant difference in the expression of TRPC6 was observed between the negative control group and the control group.Compared with the control group, the TGF-β1 treatment group showed increased expression of desmin and caspase 9 at both mRNA and protein levels (P<0.01), but decreased expression of nephrin at mRNA and protein levels at 48 hours after TGF-β1 intervention (P<0.05).The up-regulated desmin and caspase 9 and the down-regulated nephrin induced by TGF-β1 could be inhibited by the means of TRPC6 knockdown.The apoptosis rate of podocytes in TGF-β1 treatment group was (14.0±2.1)%, while that in TRPC6 knockdown was (10.90±0.56)% (P<0.05).The apoptosis rate of podocytes in negative control group was higher than that in TGF-β1 treatment group (P>0.05).More apoptotic cells with typical morphological features of apoptosis were observed after exposure to TGF-β1 for 48 hours.Conclusion TGF-β1 could induce the apoptosis of podocytes, inhibit the expression of nephrin and enhance the expression of caspase 9 and desmin, the possible mechanisms of which may be related to TRPC6 signal pathway.These changes in TGF-β1-treated podocytes could be alleviated by inhibiting the expression of TRPC6, which might have a protective effect on podocyte injury.
8.A novel lesion detection algorithm based on multi-scale input convolutional neural network model for diabetic retinopathy

Objective To propose a multi-scale convolutional neural network ( CNN) based lesions detection method of fundus image,and evaluate its application in diabetic retinopathy ( DR) assisted diagnosis. Methods A multi-scale CNN based on lesions detection method of fundus image was proposed. Compared with the existing detection methods,the problem of poor robustness based on threshold segmentation and morphological segmentation was overcome. The idea of multi-scale grids detection without relying on manual pixel-by-pixel labeling was adopted in this algorithm,and the detection performance of small lesions was significantly improved. In addition, multiple DR lesions with high accuracy could be detected by the proposed loss function under the condition of weak labels and small data sets. Results At the level of lesions,the sensitivity and specificity of hard exudation lesions detection were 92. 17% and 97. 17%,respectively. Compared with single-scale method,the sensitivity and accuracy of multi-scale method proposed in this paper increased by 7. 41% and 5. 02%,respectively,and compared with other algorithm using the same public dataset IDRiD, the specificity of this algorithm increased by 55. 82%. This method could effectively detect the lesions in fundus images,and could give the basic range of the lesions. The average detection time of fundus images with a large number of lesions was 1. 59 seconds. Conclusions The DR lesions in the fundus image can be quickly and reliably identified,the location information of the lesions can be marked,and the influence of subjective factors can be reduced by using this algorithm, and it can be used to assist the clinician to conduct more effectively.
9.Effect of tail vein transplantation of human amniotic mesenchymal stem cells with different transforming growth factor-β expressions on sciatic nerve function in xenografted mice

Objective To investigate the effect of tail vein transplantation of human amniotic mesenchymal stem cells (hAMSCs) with different transforming growth factor (TGF)-β expressions on recovery of sciatic nerve function in peripheral nerve xenotransplantation mice.Methods The hAMSCs were isolated from amnion membranes by healthy mother donors and identified by fluorescence activated cell sorter.The up-regulated and down-regulated TGF-β lentiviral plasmids were constructed and transfected into the purified hAMSCs;hAMSCs with stable up-regulated or down-regulated TGF-β expression were constructed.The sciatic nerves of C57BL/6 mice were isolated and cut out,and sciatic nerves of SD rats were isolated and transplanted into the sciatic nerve defected C57BL/6 mice to construct peripheral nerve xeno-transplanted mice models;these mice models were divided into 4 groups (n=10)according to random number table:control group,hAMSCs treatment group,high-expressed TGF-βhAMSCs treatment group,and low-expressed TGF-β hAMSCs treatment group;one d before modeling,phosphate buffer saline (PBS) or hAMSCs re-suspension were drawn with a syringe and slowly pushed into the tail veins of mice for transplantation treatment;14 d after treatment,DigGait analysis system was used to evaluate the recovery of sciatic nerve function in each group of mice.Result Fourteen d after treatment,the sciatic nerve function index (SFI) of the high-expressed TGF-β hAMSCs treatment group (-25.820±0.286) was significantly higher than that of the low-expressed TGF-β hAMSCs treatment group (-33.413±0.920) and hAMSCs treatment group (-30.755±0.421,P<0.05).Conclusion The tail vein transplantation of hAMSCs with TGF-β high expression can effectively improve the sciatic nerve function in peripheral nerve xenotransplantation mice,which may be a new breakthrough in the treatment of peripheral nerve defects.
10.Risk factors of anal function after transabdominal intersphincteric resection for low rectal cancer.

OBJECTIVETo explore the risk factors of anal function after transabdominal intersphincteric resection(ISR) for low rectal cancer.
METHODSClinical and follow-up data of 96 patients with low rectal cancer who underwent transabdominal ISR in our department from January 2005 to December 2012 were analyzed retrospectively. The Wexner scoring scale was used to evaluate the anal function and the risk factors of anal function were analyzed by the Cox proportional hazard model.
RESULTSNinety-six patients completed Wexner scoring scale with mean follow-up of 32.7 months. Eighty-three cases(86.5%) presented good continence with a Wexner score less than 10. There was negative correlation between Wexner score and follow-up duration (Pearson coefficient, -0.078, P=0.003). Univariate analysis suggested the distance less than 5 cm from tumor to anal verge(P=0.043), height less than 2 cm from anastomosis to anal verge (P=0.001) and neoadjuvant chemoradiotherapy(P=0.001) were the risk factors. Multivariate analysis revealed that distance less than 2 cm from anastomosis to anal verge(P=0.020) and neoadjuvant chemoradiotherapy(P=0.001) were independent risk factors for fecal incontinence.
CONCLUSIONSMost patients have good continence after transabdominal ISR. A distance of less than 2 cm from anastomosis to anal verge and neoadjuvant chemoradiotherapy are independent risk factors for poor anal function after transabdominal ISR.
