1.Effect of enteral nutrition on T lymphocytes-mediated immune function in patients with acquired immune deficiency syndrome

Objective To investigate the effect of enteral nutrition (EN) on the T lymphocytes-mediated immune function in patients with acquired immune deficiency syndrome (AIDS). Methods Totally 79 AIDS patients were randomly divided into enteral nutrition ( EN ) group ( supported with EN daily in addition to conventional treatment; n = 46) and control group (underwent conventional treatment only; n = 33 ). T lymphocytes including CD3, CD4, and CD8 cells as well as blood biochemical parameters including alanine aminotransferase ( ALT), aspartate aminotransferase (AST), glucose ( Glu ), total protein (TP), albumin ( ALB ), blood urea nitrogen (BUN) , Cr, and prealbumin (PA) were determined immediately before management (T0) and on the 30th day(T1). Results ALT, AST, Glu, TP, ALB, BUN, Cr, and PA showed no significant differences between these two groups before management ( all P > 0. 05 ). The levels of TP ( P = 0. 015), ALB ( P = 0. 007 ), and PA ( P =0. 022 ) were significantly higher in EN group than those in control group at T1. The cell counts of CD3, CD4, and CD8 were not significantly different at T0, while the cell count of CD4 was significantly higher in EN group than that in control group at T1 ( P < 0. 05 ). Conclusion EN can improve the nutritional status and T lymphocytesmediated immune function in AIDS patients.
2.Relationship between sub-stage of initial high-grade T1 bladder urothelial carcinoma and prognosis

Objective To evaluate the impact of sub-stage on the clinical prognosis of T 1 G3 and evaluate the feasibility of the T1me system in pT1G3 sub-staging.Methods The clinical data,pathological specimen and follow-up data were collected from 56 patients out of 87 patients diagnosed with initial high-grade T1 bladder urothelial carcinoma .The patients were divided into Group A [ T1-microinvasive (T1m),17 cases] and group B [T1-extensive-invasive (T1e),39 cases] according to pathological evaluation after transurethral resection.Clinical parameters were analyzed with Chi-square test,and recurrence-free and progression free survival were obtained by Kaplan-Meier analysis and Log-rank test.Results Age,tumor size,number and intravesical instilled medication showed no significant differences between the 2 groups ( P>0.05 ) .There were significant differences of 5-year recurrence-free rate ( P =0.037 ) and progression-free survival rate ( P =0.045 ) between the 2 groups, and the prognosis of group A was significantly better than that of group B .Conclusion The pathological sub-stage is an important predictor in initial high-grade T1 bladder urothelial carcinoma patients , and the T1me system is objective and feasible.
3.Lung cancer Ⅳ recipe combined with chemotherapy in treating non-small cell lung cancer in the elderly: a case-control study

Objective To explore the therapeutic effect of lung cancer Ⅳ recipe combined with chemotherapy on treating middle and advanced stages of non-small cell lung cancer (NSCLC) in the elderly.Methods A total of 77 NSCLC patients in middle and advanced stages aged 70 years and over who had heat-toxin syndrome in traditional Chinese medicine (TCM) were selected from Apr.2013 to Oct.2014 in our hospital.Patients were randomly divided into chemotherapy group (n=39) and chemotherapy + herbs group (n=38).Karnofsky assessment scale (KPS),effective rate,symptom and sign in traditional Chinese medicine,changes in the levels of high-sensitivity c-reactive protein (hs-CRP),carcino embryonic antigen (CEA),neuron specific enolase (NSE),tumor special growth factor (TSGF) were observed before and 1 and 56 days after treatment.Results There was no significant difference in effective rate between the two groups (P>0.05).The KPS and symptom and sign in TCM were improved significantly in chemotherapy + herbs group as compared with chemotherapy group (36.7 % vs.26.5 %,80.0 % vs.58.8 %,respectively (x2 =2.721,6.731,both P<0.05).There were no significant differences in hs-CRP,NSE,CEA,TSGF levels between two groups before and after treatment (P>0.05),but hs-CRP level in both groups was decreased after treatment as compared to pre-treatment and the decvease was more obvious in chemotherapy + herbs group (51.47% vs.23.77%,x2 =19.713,P<0.05).Conclusions Lung cancer Ⅳ recipe can improve the main clinical symptoms,quality of life and tolerance to chemotherapy in NSCLC patients with heat-toxin manifestation in TCM.There is a correlation between heat-toxin syndrome and hsCRP level in NSCLC patients,which shows that hs-CRP probably is the material basis of the heattoxin syndrome in TCM.
4.CT imaging and pathological features of maxillofacial malignant myoepithelioma

Objective To evaluate CT imaging and pathological features of maxillofacial malignant myoepithelioma (MME). Methods A total of eight patients with pathologically confirmed MME of the maxillofacial region were underwent non-enhanced and contrast-enhanced CT scans.CT features of the lesions were assessed as follows:location,number,shape,size,enhancement pattern and CT number on non-enhanced and contrast-enhanced scan.CT features were compared with the pathological results.Results All the lesions were unilateral,in which mixed type with clear cells and spindle cells in 5 cases,mixed type with epithelioid cells and plas-ma cells in 2 cases,and clear cell type in one case.8 cases were manifested as lobulated mass.On non-enhanced scan,the density of all the lesions were hetergeneous with cystic area,1 case with calcification and 2 cases with bone destruction.On the contrast-en-hanced CT,all the lesions were showed hetergeneously moderate or obvious enhancement,and cystic,line and crack areas without the enhancement were observed.Minor vessel and spiculate protuberance of marginal zone on the arterial phase were observed in most le-sions.Conclusion MME has different pathological subtypes and characteristic CT features.CT is an effective method to diagnose MME.
5.Screening Effective Components from Fructus Citri Sarcodactylis by Cell Membrane Chromatography and HPLC/MS

Objective To screen the active component from Fructus Citri Sarcodactylis which have aortic vascular relaxant effect. Methods The active components from Fructus Citri Sarcodactylis were screened by combining vascular smooth muscle/cell membrane chromatography (VSM/CMC) and liquid chromatography/mass spectrometry (LC/MS) with pharmacological assay in vitro. Results Bergapten was the active component of Fructus Citri Sarcodactylis that functions in vascular. The pharmacological experiment showed that bergapten had vasodilating effect. The capacity factor of this compound on the VSM/CMC model was found to be significantly correlated with its pharmacological effect. Conclusion Association between compound and membrane protein (receptor) can be reflected by using the VSM/CMC model. VSM/CMC-offline-LC/MS technology can be used to quickly screening new active components from natural medicine.
6.A study on correlation of obstructive sleep apnea-hypopnea syndrome and perihematoma edema of hypertensive cerebral hemorrhage

Objective To analysis the correlation of obstructive sleep apnea-hypopnea syndrome (OSAHS) and perihematoma edema of hypertensive cerebral hemorrhage.Methods One hundred and forty-four patients with hypertensive cerebral hemorrhage were collected and 78 of these patients were suffered from OSAHS.The patients were divided into two groups,control and OSAHS group,according to whether were accompanied by OSAHS or not.Both of the groups received the routine treatments including dehydration,reducing blood press,protecting the cerebral cells and so on.Cerebral CT scan was taken on admission.Night polymonography (PSG) was done within 24 hours of admission.Twenty-four hours and 4 days after admission,cerebral CT scan was taken again.The volumes of cephalophyma and perihematoma edema were calculated according to the results of CT scan.The changes of cephalophyma and perihematoma edema were dynamic observed.Results No difference in patients' age,sex,body mass index,serum glucose,blood lipid and so on,was observed between the two groups.The relative edema index became significantly different until 4 days after admission (0.40 ± 0.45,0.96 ± 1.35 in control and OSAHS group respectively,t =4.149,P =0.000).Similarly,the alternation edema index of OSAHS was obviously higher than that of control group only in 4 days after admission.While the analysis of the correlation between different degree OSAHS groups and edema indexes showed that at 24 hours after admission the edema volumes for different degree OSAHS groups were consistent (1.05 ± 0.65,0.84 ± 0.48,1.20 ± 0.54,1.10 ±0.40 in control,slight,moderate and severe groups respectively,F =1.061,P =0.374).At 24 hours and 4 days after admission,the edema volumes were positively correlated with the degree of OSAHS.Alternation edema index was significantly correlated with apnea hypopnea index according to the result of Pearson' s correlation analysis (r =0.652,P =0.000).Conclusion OSAHS complication can promote the progression of perihematoma edema of hypertensive cerebral hemorrhage,and the degree of edema aggravation is positive correlated to the degree of OSAHS.
7.The effect of TLR4/MyD88/NF-κB signaling pathway on proliferation and apoptosis in human nasopharyngeal carcinoma 5-8F cells induced by LPS.

OBJECTIVE:
To evaluate the effects of NF-κB activation on the proliferation and apoptosis throughTLR4/MyD88 signaling pathway in human nasopharyngeal carcinoma (NPC) 5-8F cell lines.
METHOD:
TLR4 induced by LPS is inhibited by PE anti-human. Real-Time Quantitative PCR and Western blot were employed to evaluate the efficacy of mRNA level and protein expression. The growth inhibition rate of 5-8F by Celecoxib was evaluated with MTT method. The cell cycle and apoptosis were measured with flow cytometric method (FCM).
RESULT:
By using the specific inhibitor, the protein and gene expression of NF-κB and MyD88 were both significantly lower than the control group (P<. 05). Meanwhile, the down-rugulation of NF-κB could inhibit proliferation of NPC 5-8F cells and promote their apoptosis (P<0. 05).
CONCLUSION
By inhibiting TLR4 / MyD88 signaling pathway, the expression of NF-κB in NPC 5-8F cells could decrease, then the cell proliferation was inhibited and cell apoptosis was induced. The results showed that TLR4 / MyD88 / NF-κB induced by LPS is an important pathway in the genesis and development of NPC. This study provides evidence for targeting research of NPC.

8.Short-term outcomes after lateral meniscus posterior root repairs in patients undergoing anterior cruciate ligament reconstructions

Objective:To evaluate the clinical, radiological and arthroscopic outcomes after surgical repair for chronic lateral meniscus posterior root (LMPR) avulsion combined with anterior cruciate ligament (ACL) reconstruction.Methods:From July 2015 to June 2017, a total of 33 patients who underwent transtibial pull-out suture repair for chronic LMPR avulsion combined with anatomic single-bundle ACL reconstruction with hamstring graft were retrospectively reviewed. There were 30 males and 3 females with an average age of 27.7±7.5 years (range 17-45 years) and a mean BMI of 25.2±3.7 kg/m 2 (range 19.4-36.7 kg/m 2). All patients were available for at least two years of follow-up. A second-look arthroscopy was performed to evaluate the healing status of the repaired meniscus. Subjective knee function was assessed through Lysholm and Tegner scores. Objective knee stability was evaluated using KT-1000 arthrometer side-to-side difference (SSD) and pivot shift test under anesthesia. The tibiofemoral relationship was evaluated by anterior tibial subluxation (ATS) measured on axial MRI. Between patients with preoperative ATS ≥6 mm (18 patients in the ATS positive group) and <6 mm (15 patients in the ATS negative group), the postoperative ATS and the reduction of ATS was also compared. Results:After a mean follow-up of 27.5±4.0 months (range 24-39 months), the LMPR avulsion completely healed in 23 (70%) cases, partially healed in 9 (27%) cases, failed to heal in 1 (3%) case on second-look arthroscopy. The Lysholm score was increased from 60.4±13.6 to 82.7±11.1 at 1 year and to 91.4±9.1 at 2 years operatively ( F=155.996, P<0.001). The Tegner score was increased from 3(2, 5) to 4(3, 5) at 1 year and 6(4, 6) at 2 years postoperatively (χ 2=47.791, P<0.001). The KT-1000 SSD was decreased from 9.1±3.3 mm to 2.0±1.7 mm ( t=11.197, P<0.001). The result of pivot shift test was also improved (10 grade I, 20 grade II, 3 grade III, preoperatively vs 30 grade 0, 3 grade I, postoperatively, U=5.161, P<0.001). The ATS was reduced from 5.7±3.9 mm to 3.5±3.2 mm ( t=3.530, P=0.001). However, there was no statistically significant decrease in the ATS of the ATS negative group ( t=0.400, P=0.695). The ATS of the ATS positive group was reduced from 8.7±1.8 mm to 5.0±3.3 mm ( t=4.765, P<0.001), and the ATS reduction of the ATS positive group was greater than that of the ATS negative group (3.7±3.3 mm vs 0.3±2.8 mm, t=3.115, P=0.004). Conclusion:In patients undergoing ACL reconstruction, the transtibial pull-out suture repair for chronic LMPR avulsion yielded meniscus healing rate of 97% with improved subjective knee function and objective knee stability and better restored the tibiofemoral relationship for patients with excessive ATS.
9.Application of SARIMA model in foodborne diseases prediction in Hubei Province

Objective To investigate the feasibility of the autoregressive integrated moving average model (ARIMA) in analyzing foodborne diseases in Hubei Province and predicting the future trend of foodborne diseases in Hubei Province. Methods Based on the number of reported cases of foodborne diseases in Hubei Province for eight consecutive years (2014-2021), an ARIMA model was constructed using Python software to fit the data. The model was validated and parameters were optimized with data from January 2022 to December 2022. The optimal fitting model was used to predict the incidence and trends of foodborne diseases in 2023. Results The incidence of foodborne diseases in Hubei Province showed seasonal periodicity, and the peak of epidemic was usually in July every year. SARIMA (0,1,0) (2,2,1)12 was determined as the best fitting model. The model extrapolation effect was verified with 2022 data, and MAPE was 23.90 %, indicating that the model extrapolation effect was effective. Conclusion The SARIMA model is effective for short-term prediction of foodborne disease incidence trends in Hubei Province, and can provide a scientific basis for the formulation of foodborne disease prevention and control policies in the coming year.
10.Neuroprotective Effect of Chinese Medicinal Compounds Effective in Supplementing Qi and Resolving Phlegm on Alzheimer's Disease: A Review

Alzheimer's disease (AD) is a common chronic degenerative neurological disease in the elderly and is mainly manifested by the impairment of cognition, memory, and behaviors. At present, the etiology and pathogenesis of this disease have not been fully unraveled, and the research on related drugs for alleviating the disease progression is still in clinical trials. Traditional Chinese medicine (TCM) believes that the elderly have reduced visceral function. Deficiency of vital Qi and turbid phlegm obscuring orifices are the core pathogenesis of AD. Qi deficiency and phlegm obstruction run through the whole pathological process of AD, and the important role of therapeutic principles of supplementing Qi and resolving phlegm is emphasized in treatment. In recent years, the Chinese medicinal compounds effective in supplementing Qi and resolving phlegm represented by Kaixin Powder have been widely used in the clinical and basic research of AD. As reported, in addition to the improvement of the cognitive function of AD, it can also reduce β amyloid (Aβ) deposition, inhibit tau hyperphosphorylation, improve neurotransmitter activity, regulate neuronal synaptic plasticity, resist oxidant stress injury, and inhibit the central inflammatory response and neuronal apoptosis. Therefore, this article reviewed and analyzed the theoretical basis of the treatment of AD by supplementing Qi and resolving phlegm and the mechanism of Chinese medicinal compounds effective in supplementing qi and resolving phlegm against AD to provide theoretical support and a scientific basis for the clinical application of TCM in the prevention and treatment of AD.
