2.Application of appreciative intervention in humanistic nursing of patients undergoing renal biopsy

Objective To explore the effect of appreciation intervention on anxiety and complications in patients with renal biopsy. Methods A total of 197 cases of renal biopsy patients were divided into the control group (n=96) and the observation group (n=101). The observation group was given comprehensive nursing intervention with appreciation, the control group was only given comprehensive nursing care, the two groups were evaluated with Zung Self-rating Anxiety Scale (SAS) upon admission and 1 week after puncture. Results After intervention, the SAS score of the observation group was 30.87 ± 4.52, and the SAS score of the control group was 32.32 ± 3.72, and the difference between the two groups was statistically significant (t=2.45,P<0.05). Totally 97 patients in the observation group and 78 cases in the control group received bed urination exercises, and the difference between the two groups was statistically significant (χ2=9.412,P<0.01). The complications of two groups were collected, the complications in the observation group were 27 cases, and the complications in the control group were 50 cases, the difference between the two groups was statistically significant (χ2=13.285,P<0.01). Among the postoperative complications, 8 of the patients in the observation group had urinary retention, and 19 of the control group had urinary retention (t=2.446,P=0.015). In the observation group, 3 cases had abdominal distention, and 10 cases in the control group suffered from abdominal distension (t=2.117, P=0.035). In the observation group, 1 case had low back pain, and 6 cases in the control group suffered from lumbago (t=2.004, P=0.047); the difference was statistically significant. Conclusions Appreciate intervention for patients with renal puncture biopsy has a positive impact on the bed urination exercise in puncture before surgery. It can alleviate the anxiety of patients received renal biopsy, reduce urinary retention, abdominal distension and pain complications, and improve bedridden experience.
3.Chemical Elements Burden in Soil of High Prevalence Areas of Birth Defects in Shanxi Province

Objective To find the chemical elements whose contents are abnormal in the soil of the high prevalence areas of birth defects in Shanxi Province. Methods 131 soil samples were collected from the high and low prevalence areas of birth defects respectively in 2005, the contents of 16 elements were determined with inductively coupled plasma (ICP)machine. The data were analyzed with regression and nonparametric tests. Results The total content of elements in the soil of the high prevalence areas was significantly higher than that of the low prevalence area. Compared with the low prevalence area, in a high prevalence area, the content of Mo, As, Pb, Ni, V and Se in soil was abnormal, Mo, As, Ni and Pb showed a significant relationship to the prevalence rate of birth defects, in another high prevalence area, the content of Sn, Se, Mo, Zn, Sr, Pb, Ni, Fe, V, Ca, Cu, Al, Mg and Na was abnormal, Pb, Mg, Ca, Al and Zn showed a significant relationship to the prevalence rate of birth defects. Conclusion Maybe the abnormal content of Mo, As, Ni, Pb, Mg, Ca, Al and Zn in the soil is one of the risk factors for birth defects.
4.The literature research on evaluation standard of TCM efficacy of angina pectoris

Objective: 139 documents (10,646 cases of angina pectoris in patients with coronary heart disease) which were treated with traditional Chinese medicine (TCM) of angina pectoris were researched, the application and existing problems of TCM evaluation standards of the efficacy of angina pectoris were studied to determine constructive items (candidate indicatrix) of TCM evaluation standards. Methods: The efficacy evaluation database of angina pectoris was established by the literature search and the software Epidata2.0, recorded in the database, and statistical analysed. Results: There are many kinds of evaluation standards of TCM efficacy of angina pectoris, but the utilization rate of each standard is lower than 40%. The main constructive item in evaluation standards of angina pectoris should include: the effect of the disease, syndrome effects, physical and chemical indicatrix, important clinical event, quality of life, and so on. Conclusion: The current evaluation standards of TCM efficacy of angina pectoris can not meet the clinical needs. A received evaluation standard should be established. The evaluation of TCM efficacy of angina pectoris should adopt integrated multi-dimensional index system.
5.Cross-species Transmission of Avian Leukosis Virus Subgroup J.

Avian leukosis virus subgroup J (ALV-J) is an avian retrovirus that can induce myelocytomas. A high-frequency mutation in gene envelope endows ALV-J with the potential for cross-species transmission. We wished to ascertain if the ALV-J can spread across species under selection pressure in susceptible and resistant hosts. First, we inoculated (in turn) two susceptible host birds (specific pathogen-free (SPF) chickens and turkeys). Then, we inoculated three resistant hosts (pheasants, quails and ducks) to detect the viral shedding, pathologic changes, and genetic evolution of different isolates. We found that pheasants and quails were infected under the selective pressure that accumulates stepwise in different hosts, and that ducks were not infected. Infection rates for SPF chickens and turkeys were 100% (16/16), whereas those for pheasants and quails were 37.5% (6/16) and 11.1% (3/27). Infected hosts showed immune tolerance, and inflammation and tissue damage could be seen in the liver, spleen, kidneys and cardiovascular system. Non-synonymous mutation and synonymous ratio (NS/S) analyses revealed the NS/S in hypervariable region (hr) 2 of pheasants and quails was 2.5. That finding suggested that mutation of isolates in pheasants and quails was induced by selective pressure from the resistant host, and that the hr2 region is a critical domain in cross-species transmission of ALV-J. Sequencing showed that ALV-J isolates from turkeys, pheasants and quails had moved away from the original virus, and were closer to the ALV-J prototype strain HPRS-103. However, the HPRS-103 strain cannot infect pheasants and quails, so further studies are needed.

6.Cystatin M inhibits the proliferation and migration of gastric cancer SGC-7901 cell lines

Objective To encode the protein cystatin M with CST6 gene and construct a CST6-overexpression vector,and transfect it into the gastric cancer cells SGC-7901 in order to observe the effect of cystatin M on the proliferation and migration abilities of SGC-7901 cells.Methods There were three groups in the experiment:pcDNA3.1(+)-CST6 group,pcDNA3.1 (+)group and non-transfected group.RT-PCR and Western blot were used to identify the RNA expression of CST6 in SGC-7901/CST6 cells.The proliferation and migration abilities of the transfected cells were detected by MTT and cell wound healing assay,respectively.Results SGC-7901/CST6 cells stably expressed CST6 gene RNA and cystatin M protein.The proliferation and migration of pcDNA3 .1 (+)-CST6 group cells were reduced compared with those of cells in the other two groups (P<0.05).Conclusion Cystatin M can inhibit the proliferation and migration of SGC-7901 cells.
7.Correlation between syndrome factor combination and cardiac function as well as blood-lipid in coronary heart disease

OBJECTIVE: To discuss the distribution laws of traditional Chinese medicine (TCM) syndrome factor and their combination in coronary heart disease (CHD), and to study the correlation between the TCM syndrome factor combination and cardiac function as well as blood-lipid. METHODS: The parameters of the cardiac function of 300 patients with a final diagnosis of CHD by coronary angiography were measured by echocardiography, and the levels of blood lipids in the CHD patients were detected. An analysis of the correlation was done between the TCM syndrome factor combination and cardiac function as well as blood-lipid in CHD. RESULTS: The TCM syndrome factor combinations of CHD were blood stasis due to qi deficiency, qi and yin deficiency, intermingled phlegm and blood stasis, and yang deficiency and blood stasis. The ejection fraction of CHD patients with yang deficiency and blood stasis was markedly decreased. The levels of triglyceride and low-density lipoprotein cholesterol in CHD patients with intermingled phlegm and blood stasis were markedly increased, and the level of triglyceride in CHD patients with qi and yin deficiency was markedly increased too. CONCLUSION: The treatment of CHD should aim directly at the symptoms and causes. It is also proved that some compound traditional Chinese herbal medicines for supplementing qi and activating blood circulation, nourishing yin and resolving phlegm, and activating yang should be used in treatment of CHD. In cases of CHD with low cardiac function, particular emphasis should be laid on activating yang and blood circulation, while in cases of CHD with blood-lipid disturbance, particular emphasis should be laid on resolving phlegm and activating blood circulation, replenishing qi and nourishing yin.
8.Characteristics of coronary arteriography and traditional Chinese medicine syndrome of 1,069 patients with coronary artery disease

OBJECTIVE: To study the characteristics of coronary arteriography and traditional Chinese medicine syndrome of 1,069 patients with coronary artery disease (CAD). METHODS: One thousand and sixty-nine patients with CAD were investigated by epidemiological method. The patients were divided into young patients (n=82, aged 45 years or younger) and middle-aged and old patients (n=987, older than 45 years). The characteristics of the two groups were analyzed, including clinical data, coronary arteriography and traditional Chinese medicine syndrome. RESULTS: Compared with middle-aged and old patients, proportion of male, triglyceride, total cholesterol, smoking patients, acute myocardial infarction and family history of CAD in young patients were significantly higher (P<0.05). Patients accompanying with hypertension and diabetes in middle-aged and old patients were more than those in young patients (P<0.05). Occurrence rates of morbidity of left circumflex coronary artery, left main coronary artery and multi-branch were higher in middle-aged and old patients (P<0.05), however, the occurrence rates of morbidity of single and double-branch were higher in young patients (P<0.05). The occurrence rates of syndromes of qi stagnation and phlegm turbidity in young patients were higher than those in middle-aged and old patients (P<0.05). But the proportions of cold coagulation, yin deficiency, yang deficiency and kidney deficiency in middle-aged and old patients were obviously higher (P<0.05). CONCLUSION: The traditional Chinese medicine syndrome and pathological changes of CAD in young patients are different from those in old patients.

Two pairs of primer targeting to the genes encoding the major outer membrane protein (MOMP) and the polymorphic membrane protein (PMP) of Chlamydophila psittaci (Cps) designed and used in the fluorescent quantitative PCR assay in the detection of this microorganism were compared in the their sensitivity and specificity.It was demonstrated that both pairs of primer could be used successfully to detect the presence of Cps of the epidemic strains isolated in China.The specificity of PMP primer used was higher than that of MOMP primer. In case of high target concentration in samples, the sensitivity of the former was also superior to that of the latter, but the amplification efficiency of the former was lower than that of the latter. The MOMP primer seemed to be more suitable to detect Cps by using the real-time quantitative assay.As demonstrated by the real-time quantitative PCR assay, great difference in the genomic copy numbers of the PMP gene existed in the epidemic strains of Cps isolated in China.
10.Early application of clean intermittent catheterization in children with neurogenic bladder

Objective To investigate the effect of early application of clean intermittent catheterization(CIC) in infants with neurogenic bladder(NB).Methods Eighty-seven children with NB diagnosed in our urodynamic center were less than 1 year old when they first came to hospital from January 2007 to January 2010, and CIC was carried out at different age.Sixty-four patients were followed up for a long time and divided into early CIC group(less than 1 year old children) and late CIC group(more than 3 years old children) according to the treatment time.Early CIC group included 29 patients [19 boys and 10 girls with the mean age of (7.5 ±2.8) months].And 4 cases were suffering from postoperative spina bifida manifesta;22 cases with spina bifida occulta;2 cases with sacral dysplasia;1 case with meningitis.Late CIC group included 35 patients [20 boys and 15 girls with the mean age of (8.0 ±2.9) months].2 cases were suffering from postoperative spina bifida manifesta;28 cases with spina bifida occulta;4 cases with sacral dysplasia;1 case with postoperative pelvic surgery.Before the treatment, there were no significant differences of the bladder compliance (BC), the maximum cystometric capacity (MCC) and the safety bladder capacity (SBC) between two groups.Urodynamic parameters and complications of 64 patients who were successfully followed up for 6 years were compared.Results After 3 years follow up, BC, SBC and MCC in early CIC group [(8.5 ± 1.9) ml/cmH2O, (140 ±25) ml, (142 ±29) ml]were significantly higher than those of late CIC group [(7.0 ± 2.2) ml/cmH2O, (110 ± 31) ml, (120 ± 28) ml;all P < 0.05].After 6 years follow up, BC, SBC and MCC in early CIC group [(12.0 ±2.5) ml/cmH2O, (210 ±26) ml, (230 ±30) ml] were significantly higher than those of late CIC group [(9.3 ± 2.3) ml/cmH2O, (192 ± 31) ml, (205 ± 35) ml;all P < 0.05], and the vesicoureteral reflux rate [24.1% (7/29)] in early treatment group was significantly less than that in late treatment group [54.3% (19/35), P < 0.05].Increases in BUN and serum creatinine were found in 6 cases (20.7%) in early CIC group and 17 cases (48.6%) in late CIC group, the difference was significant (P < 0.05).Conclusion For NB patients, the effect of early CIC is better than that of late CIC.
