1.Clinical analysis of central nervous system infection in systemic lupus erythematosus

Objective To investigate the clinic features and risk factors for central nervous system (CNS) infection in patients with systemic lupus erythematosus. Methods Clinical data of 18 cases of hospitalized lupus patients with CNS infection were analyzed retrospectively.Thirty-three lupus patients without CNS infection were selected as control. Results There were significant differences between the CNS infection group and control group in the dosage of corticosteroid used for pulse therapy (22% vs 3%, P<0.05), the aver-age dosage of corticosteroid used within one year (35±18 vs 24±17, P<0.05), peripheral blood leukocyte count (4.4±3.4 vs 6.7±2.9, P<0.05) and lymphocyte count (0.7±0.6 vs 1.5±0.7, P<0.01), and clinical outcome (mortality rate was 22% vs 0, P<0.05). Conclusion The clinical manifestations of CNS infection may be atypical. Large-dosage corticosteroid therapy, higher daily dosage of corticosteroid used, decreased peripheral lymphocytes and leucocytes counts are risk factors of central nervous system infection in lupus patients.
2.Effects of early administration of high-dose rosuvastatin on coronary microvascular function and short-term outcome in patients treated with primary percutaneous coronary intervention for acute myocardial infarction

Objective To investigate effects of early administration of high-dose rosuvastatin(40 mg) on coronary microvascular function and short-term outcome in patients treated with primary percutaneous coronary intervention (PCI) for acute myocardial infarction(AMI). Methods Ninety-four consequent AMI patients treated with primary PCI were divided into rosuvastatin group (50 patients) and control group (44 group). The infarct-related artery flow of epicardium was classified in compliance with the TIMI criteria. Myocardial and microvascular perfusion was assessed using the TMPG. The incidence of the MACE and the cytotoxicity and hepatotoxicity of rosuvastatin was respectively recorded in 30 d follow-up period. Results Either patients in the rosuvastatin group or in the control group showed better TMPG immediately after PCI (P<0.05), compared with that before treatment. However, the post-PCI TMPG of the rosuvastatin group was obviously much better than that of control group (P<0.05). Compared with that in control group, the 30-day composite MACE rate was lower in rosuvastatin group and in the TMPG 3 patients of rosuvastatin group:12.0%(6/50) vs. 34.1%(15/44), P<0.05;11.1%(3/27) vs. 42.9%(6/14). There was no cytotoxicity and hepatotoxicity in two groups. Conclusions Early administration of high-dose rosuvastatin (40 mg) can improve coronary microvascular function and short-term outcome in patients treated with primary PCI for AMI, and it is efficient and safety.
3.Clinical Studies on Emotional Disorder in Patients After Acute Stroke

[Objective] To investigate the occurrence rate, manifestations and influencing factors of emotional disorder in patients after acute stroke. [Methods] Emotional disorder in 107 cases of acute stroke was assessed by 17-item Hamilton Depression Scales (HAMD), 14-item Hamilton Anxiety Scales (KAMA), Mini-Mental State Examination (MMSE) and Barthel index (BI), and the relationship between emotional disorder and influencing factors were also analyzed. [Results] The occurrence rate of emotional disorder after acute stroke was 41.12% , in which depression was 18.69% , anxiety 9.35% and concurrence of depression and anxiety 13.08% . Emotional disorder usually occurred in the dominant hemisphere of young patients with lower BI after repeated attacks of stroke; its manifestations relieved with the cure of primary diseases. Its dominant syndrome pattern was classified as stagnation of liver-Qi. [ Conclusion ] Emotional disorder, possibly being a stress reaction, is the common complications of acute stroke and usually classified as stagnation of liver-Qi; early effective rehabilitation is the best preventive method for emotional disorder acute stroke.
4.Effect of α1-microglobulin and β2-microglobulin on calcium oxalate urine stone formation

ObjectiveTo define the difference of urine analysis result,α1-microglobulin (α1-MG) and β2-microglobulin ( β2-MG) between those patients suffering from calcium oxalate stone,non-calcium oxalate stone and non-urolithiasis controls at the same time period.MethodsData from 100 patients admitted to the Department of Urology,First Affiliated Hospital of Anhui Medical University from July,2010 to September,2010 were reviewed.66 patients (45 men,21 women) suffered from urolithiasis,and 34 patients (22 men,12 women) were non-urolithiasis.Patients' ages in urolithiasis group varied from 13 to 78 years and the male to female ratio was 2.1∶1.0.The patients in non-urolithiasis controlgroup aged from 12 to 80 years and the male to female ratio was 1.8∶1.0.Blood and urine were taken from the patient the next morning after admission.The biochemistry from blood and 24 h urine were measured by automatic biochemistry analyzer.The α 1-MG and β2-MG content were measured by radioimmunoassay.The stone compositions were analyzed by infrared spectroscopy.ResultsThere was difference in the levels of serum creatinine and blood urea nitrogen among three groups ( P < 0.05).In controls,those with calcium oxalate stone had higher level of urinary α1-MG and β2-MG,but there were no differences in the urinary electrolyte levels.Group of non-calcium oxalate stones urinary uric acid levels were higher than calcium oxalate and control groups,the difference was statistically significant.ConclusionsIn the formation of uric acid stones,uric acid increased as independent risk factors.α1-MG,β2-MG may promote the formation of calcium oxalate stones.
5.Characteristics and prognosis of visual field of G11778A mutation Leber hereditary optic neuropathy

Objective:To analyze the characteristics and prognosis of visual field of Leber hereditary optic neuropathy (LHON) with G11778A mutation.Methods:A retrospective clinical study. Twenty-two (44 eyes) of LHON patients diagnosed with G11778A site mutation by mt-DNA examination from May 2008 to February 2018 in Ophthalmology Department of Dongfang Hospital of Beijing University of Chinese Medicine, were enrolled in this study. All patients underwent best corrected visual acuity (BCVA), visual field and optical coherence tomography (OCT). The BCVA examination was performed using the international standard visual acuity chart, which was converted into logarithm of the minimum angle of resolution (logMAR) BCVA for record. The thickness of the retinal nerve fiber layer (RNFL) in the 200μm×200μm annular region 1.73 mm outside the optic disc was measured by OCT. At least 7 visual field examinations were performed within one month before and after 2, 4, 8, 12, 18, 24 and 30 months of the course of disease by using Octopus 101 perimetry. Among 44 eyes, 27 eyes were detected with G2 procedure (G2 group) and 17 eyes were detected with LVC procedure (LVC group). The mean field defect (MD) and mean optical sensitivity (MS) were used as the main outcome indexes. According to the onset age, the patients were further divided into the ≤14 years old group and>14 years old group. There was a significant difference in initial logMAR BCVA between the G2 group and LVC group ( t=4.994, P=0.000), but there was no significant difference in gender ( χ2=1.896, P=0.169) and age ( t=0.337, P=0.708) between the two groups. Independent sample t test was used for comparison between groups, paired t test was used for comparison within groups, and one-way analysis of variance was used for comparison between groups. The statistical data were compared by χ2 test. Results:In the G2 group, the MD value of the subgroup of children (≤14 years old) decreased gradually during the follow-up period, and the MD value since 18 months after onset was significantly lower than the value of 2 months after onset ( t=3.813, 4.590, 5.033; P=0.002, 0.001, 0.000). No obvious visual field index changes were seen in other subgroups ( P>0.05). The central scotoma was the most common type of visual field defect in the early stage, and the diffuse defect was the most common type of visual field defect in the late stage. There was a significant difference in the types of visual field distribution between the early and late stage in G2 group ( χ2=17.414, P=0.015). There was no significant difference in the type of visual field distribution between the early and late stage in LVC group ( χ2=4.541, P=0.474). The MD value in the G2 group remained stable within 8 months after onset, but significantly improved after 18 months after onset ( t=2.100, 3.217, 3.566; P=0.046, 0.003, 0.001). The MS in the LVC group did not significantly improve during follow-up ( P>0.05). The average visual acuity of the G2 group was significantly improved from 12 months ( t=3.039, 3.678, 4.264, 5.078; P=0.008, 0.002, 0.001, 0.000). The visual acuity of the eyes in the G2 group was better than that of the LVC group during all follow-up periods ( P≤0.05). The RNFL thickness of all patients continued to decrease after onset, but the RNFL thickness was significantly higher at 4, 8, 18, 24, 30 months in the G2 group than those in the LVC group ( t=2.471, 2.269, 2.474, 2.509, 2.782; P=0.018, 0.028, 0.017, 0.016, 0.008). Conclusions:The main types of visual field defect of LHON with G11778A mutation are the central scotoma in the early stage, while the diffuse defect and central scotoma are both very common in the later stage. The visual field of LHON patients examined by G2 procedure is significantly improved during the follow-up, as well as the visual acuity improved significantly, and the visual field improvement in younger cases (≤14 years old) is better than that of older cases (>14 years old), but the visual field of the LVC procedure cases did not improve during follow-up.
6.Study on the nutritional risks in elderly patients with prostate cancer and its influencing factors

Objective To study the nutritional risks in elderly patients with prostate cancer and explore its related factors.Methods 346 elderly patients with prostate cancer in Wuhan area from May 2013 to May 2014 were chosen as the objects in this study.Nutritional risk screening 2002 was used to evaluate nutritional risk.The sleep quality,anxiety,lower urinary tract symptoms,differentiation and other data of patients were collected.The influencing factors for nutritional risk in the patients were analyzed.Results The ratio of nutritional risk in elderly advanced prostate cancer patients was 35.3% (122/346).The nutritional risk was increased with age (P<0.05).The prostate cancer patients with nutritional risk had a higher international prostate symptom score (IPSS) (P< 0.05).Monovariate factor analysis showed that sleep quality,operation,pathology staging,radiotherapy,chemotherapy,lower urinary tract symptoms (LUTS) were the impact factors for nutritional status in elderly patients with prostate cancer (P<0.05).Logistic regression analysis showed that age (OR=0.29),sleep quality (OR=0.25) were the protective factors for the nutritional status,while surgery (OR=12.67),pathological staging (OR=1.65),radiotherapy (OR=2.65),SPSS (OR=1.55),chemotherapy (OR=1.85) were the risk factors for nutritional status (P< 0.05).Conclusions The incidence of nutritional risk is high in elderly prostate cancer patients.Age,sleep quality are the protective factors,and operation,pathology staging,SPSS,chemotherapy,and radiotherapy are the risk factors for nutritional status.

To analyze the potential limitations of hospital-level research projects management by summarizing its performance status from 2008 to 2011,including but not limited to lack of time for conducting research,insufficient financial funds,inadequate funding application,inadequate supervision.Thus,since 2014,our hospital has adopted some new measurements for projects management,including full-time research,hierarchical management,full mobilization,the establishment of reward and punishment measures,which significantly improved the quality of hospital-level research projects,and the rate of longitudinal follow-up project,research enthusiasm of medical staff,and sustainable development of hospital science and technology.
8.The negative effects of Trim34α on activation of luciferase reporter gene containing NF-κB promoter

Objective To investigate the effects of Trim34α on the activation of luciferase reporter gene containing NF-κB promoter induced by adaptor proteins TAB2. Methods The total RNA was isolated from HeLa cells. After amplification with RT-PCR, the target sequences were cloned into 5'-Flag-pcDNA3.1 (+) vector. The recombinant vector was confirmed by restriction enzyme digestion, colony PCR and sequencing. It was transfected into HEK293T cells to detected Trim34α expression by Western blot. Simultaneously, the effects of Trim34α on the NF-κB activation induced by TAB2 were determined by dual-luciferase reporter assay. Results Restriction enzyme digestion, colony PCR and sequencing confirmed the vector was constructed successfully, furthermore it expressed Trim34α protein in HEK293T cells. Moreover, trim34α could form high-molecular-weight oligomeric protein, and here we called it trimsome. Interestingly, dual-luciferase assay showed that Trim34α could effectively block TAB2-induced NF-κB activation. Conclusion Trim34α was involved in negative regulation of TAB2-induced NF-κB activation and could form high-molecular-weight oligomer.
9.Clinical manifestations of portal vein thrombosis and related risk factors in patients with liver cirrhosis

ObjectiveTo investigate the clinical manifestations of portal vein thrombosis (PVT) and related risk factors in patients with liver cirrhosis. MethodsA total of 541 patients with liver cirrhosis who were admitted to the General Hospital of Ningxia Medical University from April 2008 to April 2015 were included in the study; 76 patients with PVT were enrolled in the study group, and another 76 patients without PVT matched for sex, age, and Child-Pugh class were enrolled in the control group. The clinical data and related indicators were analyzed and compared between the two groups. The t-test was applied for comparison of continuous data between groups, the chi-square test was applied for comparison of categorical data between groups, and the unconditional logistic regression model was used to determine the independent risk factors for PVT in liver cirrhosis. ResultsIn the patients with liver cirrhosis and PVT, 421% (32/76) had an insidious onset and 579% (44/76) had obvious clinical manifestations. Most patients had Child-Pugh class B and C cirrhosis. There were significant differences between the two groups in platelet count, blood glucose, percentage of neutrophils, severe esophageal and gastric varices, plasma D-dimer, portal vein width, and thickness of the spleen (all P<0.05). The unconditional logistic regression model analysis showed that percentage of neutrophils (OR=1.044, P=0.040), plasma D-dimer (OR=0.091, P=0.000), portal vein width (OR=0.030, P=0.008), and thickness of the spleen (OR=0.427, P=0.003) were the influencing factors for PVT. ConclusionPVT may have an insidious onset in patients with liver cirrhosis, or have different clinical manifestations. Cirrhotic PVT usually occurs in patients with advanced liver cirrhosis, and plasma D-dimer, portal vein width, thickness of the spleen, and percentage of neutrophils are the independent influencing factors for PVT in patients with liver cirrhosis.
10.Preparation of the polyclonal antibodies of CDPK 5 gene from toxoplasma gondii and the identification of its functions

Objective Screening the immune polypeptide sequence of toxoplasma (Tg) CDPK5 gene ,which were synthesized and then immunized the New Zealand white rabbit to prepare antiserum ,and identification its function .Methods Bioinformatics a‐nalysis was used to determine the immune peptide of Tg CDPK5 sequence ,which were artificially synthesized to immune white rab‐bit to prepare antiserum .The titers of antibodies were determined by ELISA and the polyclonal antibodies were verified with CD‐KP5 antigen by Western blot .The sub‐cellular localization of Tg CDPK 5 were obtained by immunofluorescence assay .Results 17 bp peptide sequence from the Tg CDPK5 N‐terminal were chosen as immune polypeptide by bioinformatics analysis .Synthetic pep‐tide were used to immune rabbit to obtain polyclonal antiserum .The result showed that the titer of the obtained ployantibody were 1∶640 000 ;Western blot demonstrated that the antiserum could specifically recognize Tg CDPK 5(75 .4 × 103 );Immunofluores‐cence assay revealed this antibody could specifically recognize the endogenous Tg CDPK 5 of Toxoplasma gondii .Conclusion Ac‐cording to the analysis of Tg CDPK5 sequence information ,this study successful obtained Tg CDPK5 polyclonal antibody .
