1.The efficacy of 89 dilated cardiomyopathy cases treated by the carvedilol

Objective To analyse the therapeutic effect of Carvedilol in patients with severe heart failure.Methods The clinical data of 89 Carvedilol patients treated with dilated cardiomyopathy were retrospective analysis.Results The overall therapeutic effective rate was 98.88%,because of one death related to discontinued use of medication.Before and after treatment,systolic left ventricular cavity diameter (LVDD) were (68.25 ± 2.15) mm,(56.89 ±4.21) mm,respectively,and left ventricular ejection fraction(LVEF) were (28.63 ±0.33)%,(40.56 ±3.98) %,respectively,there were significantly differences (t =7.215,16.877,all P < 0.05).Conclusion Carvedilol is the cornerstone in the drug treatments of patients with dilated cardiomyopathy.
2.Isolation of 9 HIV-1 strains from the Infected Persons and Study of Their Biological Characteation

Aim To isolate HIV-1 from the blood specimen of HIV-infected persons, and study on the features of the isolates. Methods PBMCs from 10 heparinized blood specimens of HIV-1 infected persons were collected to isolate the virus with PBMC cocultivation method. Neuramindase was added to the culture medium to raise the HIV isolation rate. The supernatant of cocultures were detected by p24 antigen capture assay and also IFA test was used to identify the isolates. Cell tropism of the HIV-1 isolates was tested byuse of H9 and MT4 cell lines, respectively. Results HIV-1 was isolated from9 of the 10 specimens, the isolation rate could reached 90%. The 9 isolates were used to infect MT4 cells, respectively, and all of them could cause cell syncytias.8 of the isolates showed only a transient infection in MT4 cells, but the isolate FJ113 could infect MT4 successively, OD values kept no changes even it passed 15 generations, therefore, it is consider as a rapid/high replicating virus. Conclusions Addition of neuramindase into T cell culture medium may useful to raise HIV isolation rate. Most of HIV-1 isolates in Fujian are M tropic.
3.Degenerative changes in adjacent segments after lumbar fixation and fusion:transforaminal endoscopic spine system

BACKGROUND:Spine minimaly invasive technique through foraminal mirror is the method to treat lumbar disc herniation with minimal wound. This technique can be conducted under local anesthesia, and does not need to resect the smal joint or destroy the vertebral plate, and has smal damage to the spine. OBJECTIVE: To explore the short-period effects of transforaminal endoscopic spine system for adjacent-segment degenerative changes-caused low back pain after lumbar fixation and fusion. METHODS:A total of 31 patients with degenerative changes after posterior lumbar bone graft fusion fixation, who required secondary surgery, were enroled in this study, including 23 males and 8 females, at the age of 45-81 years old. The postoperative time was 1.1-5.7 years. There were 3 cases of L3-4 single segment, 15 cases of L4-5 single segment, 8 cases of L5S1 single segment, and 5 cases of multi-segment. These patients were treated with transforaminal endoscopic spine system, and folowed up for 6 months. Visual Analogue Scale score and lumbar function Japanese Orthopedic Association score were observed. RESULTS AND CONCLUSION: Lumbar and leg pain symptoms were relieved noticeably during the operation. The patient could walk immediately after the surgery, and the postoperative recovery was quite satisfactory. Visual Analogue Scale score was lower immediately, 1, 3 and 6 months after treatment compared with pre-treatment. Lumbar function Japanese Orthopedic Association score was higher immediately, 1, 3 and 6 months after treatment compared with pre-treatment. Results verify that transforaminal endoscopic spine system for degenerative changes after posterior lumbar bone graft fusion fixation has some advantages such as high safety, short operation time, less hemorrhage, less complications, rapid restoration and easily accepted by patients.
4.Echocardiographic and methodological study on left atrial spontaneous echo contrast in patients with atrial fibrillation

Objective To investigate the independent echocardiographic pathogenesis associated with left atrial spontaneous echo contrast(SEC) formation in patients with atrial fibrillation(AF).Methods Twenty-one patients with valvular AF and twenty-three patients with nonvalvular AF undergoing transthoracic and transesophageal echocardiography(TTE/TEE) were studied.Morphological and functional parameters of the heart in patients with AF were detected by TTE,while the sizes of the left atrial appendage(LAA),its Doppler flow patterns,Doppler flow patterns of the left upper pulmonary vein and integrated backscatter(IBS) values of left atrial SEC relative to those of the left ventricle were measured by TEE.Results There were the following 10 variables significantly associated with the left atrial SEC,some of which associated positively with SEC were left atrial diameter(r=(0.40252),P=(0.0061)),diameter of the open mouth of LAA to the left atrium(r=(0.37816),P=(0.0161)),while the others of which associated negatively with SEC were antegrade/retrograde blood flow velocity peak,integrate(RVi) and retrograde blood flow velocity mean(RVm) values,and S/D wave peak,S wave integrate(Is) and D wave mean(Md) values,their r values ranged from(-0.40379) to(-0.32832),their P values between(0.0018) and(0.0386).Collinearity analyses suggested that RVi,RVm,Is,Md represented completely all the blood flow velocity parameters of LAA and the left upper pulmonary vein,respectively.Multivariate analyses demonstrated that RVm and Md were the independent predictors of the left atrial SEC in patients with AF(both P
5.Application of heteroduplex mobility assay in genetic subtyping on human immunodeficiency virus type 1.

OBJECTIVEUsing heteroduplex mobility assay (HMA) to subtype human immunodeficiency virus type 1 (HIV-1) for the purpose of understanding HIV-1 subtype epidemic in Fujian province.
METHODSDNA fragments of HIV-1 env gene were amplified from peripheral blood mononuclear cell (PBMC) cocultures of HIV-1 infected individuals by nested polymerase chain reaction (PCR). Heteroduplexs were formed through hybridizing PCR products from the samples and reference plasmid. According to the mobility of heteroduplexs in polyacrylamide gel electrophoresis, HIV-1 subtype from that sample was characterized and further confirmed by nucleotide sequencing analysis.
RESULTSThirteen of 15 (86.67%) samples were successfully subtyped by HMA, except 2 failures. Subtype E and B took up 80% (12/15) and 6.67% (1/15) respectively. Results indicated a high concordance between HMA and nucleotide sequencing analysis and concordance rate was 86.67% (13/15).
CONCLUSIONSSubtype E appeared to be the major epidemic strain of HIV-1 in Fujian. HMA showed the characteristics of fastness, easiness, economic and with high specificity, and can be used in the surviellance for the epidemic strain of HIV-1.
