1.Study on the Relative Bioavailability of Yifuxian'an Capsules in Human Body

OBJECTIVE:To study the relative bioavailability of yifuxian'an capsules.METHODS:Concentrations of ri-fampicin,isoniazid and pyrazinamide in plasma were determined on12volunteers respectively after a single oral dose of yifux-ian'an capsules(tested preparations)and rifater(reference preparation)by RP-HPLC method;the pharmacokinetics parame-ters were processed by3p97process.RESULTS:The AUC 0~t of the tested preparation and reference substance of rifampicin were(70.69?36.32)and(70.77?32.26)(?g?h)/ml respectively,t max of which were(1.82?0.28)and(1.79?0.25)h re-spectively,C max of which were(12.83?4.98)?g/ml and(12.84?3.95)?g/ml respectively;The AUC 0~t of which were(14.30?8.25)and(13.76?6.16)(?g?h)/ml respectively,t max of which were(1.54?0.36)and(1.58?0.38)h respectively,C max of which were(5.42?2.52)and(5.46?2.46)?g/ml respectively;The AUC 0~t of which of pyrazinamide were(243.56?128.98)and(238.07?117.63)(?g?h)/ml respectively,t max of which were(1.58?0.64)and(1.69?0.55)h respectively,C max of which were(29.45?12.45)and(27.46?11.02)?g/ml respectively.The relative bioequivalence of rifampicin,isoniazid and pyrazi-namide of the tested preparation were(99.88?12.92)%,(103.84?24.10)%and(102.57?12.31)%respectively.CONCLU-SION:Bioavailability of the2preparations are bioequivalent.
2.Ischemic hepatitis: influence factors and clinical features

Objective To investigate the incidence,influencing factors and clinical features of ischemic hepatitis.Methods Changes of serum transaminase and lactate dehydrogenase in patients with shock and respiratory failure were recorded in our hospital.Ischemic hepatitis was identified according to diagnosis guidelines,and its incidence was counted.Clinical data of patients with ischemic hepatitis were recorded.Results There were 53 cases diagnosed as ischemic hepatitis in 317 severe patients,including hemorrhagic shock,heart failure,respiratory failure,et al.The incidence was high in elderly patients aged over 70 years and male.Levels of alanine aminotransferase (ALT),AST and lactic dehydrogenase (LDH) were significantly increased 1-3 days after shock and respiratory failure,then reached to peak value 1-5 days [averaged (3.35±0.89) days] after shock and respiratory failure.Serum ALT was 869-4354 U/L,AST was 773-4833U/L and LDH was 1021-2662 U/L.Total bilirubin level was increased in 16 cases and the total bilirubin was 14-54 μmol/L.AST was declined to a point within 2 times the upper limit of normal range after 9-16 days,averaged (13± 1.6) days.Elevated transaminase level had no correlation with sex,age and pre-existing liver diseases.The time of enzyme changes had no correlation with sex but age and liver diseases.Patients aged ≥ 70 years had later AST peak and the recovery time was longer than younger patients (P<0.01).Conclusions Ischemic hepatitis occurs mainly in men and elderly people.But advanced age and underlying liver diseases can affect the recovery time of transaminase in patients with ischemic hepatitis.
3.FOXO3 expression in lung tissue and lung cancer stem cells and its implication

Purpose To detect the expression of FOXO3 in lung cancer specimens and lung cancer stem cell, for diagnosis and treat-ment of lung cancer. Methods Immunohistochemistry, semi-quantitative PCR and Western blot were used to detect FOXO3 protein expression in clinical specimens of lung cancer. PCR and Western blot verified FOXO3 mRNA and protein expression of lung cancer cells. Results Immunohistochemistry showed that the FOXO3 was significantly lower expressed in lung cancer tissues and the corre-sponding lung cancer stem cells;Detection of FOXO3 mRNA and protein by RT-PCR and Western blot showed both had significantly lower expression;as progression of the disease, there was a clear decreasing trend, in which negative expression rate of FOXO3 protein accounted for 66. 7% in all patients;at the same times, the rate of the expression gradually decreased with loss of differentiation and metastasis of the tumor. Conclusions FOXO3 shows low expression in lung tissue and lung cancer stem cells, while it is closely relat-ed to the degree of malignancy and lung cancer metastasis, which may be a new target for the treatment.
4.Analysis in nosocomial infection in elderly patients with intra-aortic balloon pump after coronary artery bypass grafting and nursing

Objective To summarize the nursing experience on nosocomial infection in elderly patients with intra-aortic balloon pump (IABP) after coronary artery bypass grafting. Methods The clinical data of elderly patients who underwent coronary artery bypass grafting with application of IABP from April 2006 to February 2009 were retrospectively collected and analyzed. Results 29 patients suffered nosocomial infection in all 111 cases. The hospital mortality was much higher among patients with nosocomial infection than in patients without nosocomial infection (19 cases vs 11 cases). 87 positive strains were separated from 29 patients with nosocomial infection, which included 47 strains of gram-negative bacterial, 23 strains of gram-positive bacterial and 17 strains of fungi. Conclusions The nosocomial infection was one of the risk factors for hospital mortality in elderly patients with intra-aortic balloon pump after coronary artery bypass grafting. It is important to take effective nursing strategies to prevent the postoperative nosocomial infection in this population.
5.Action of lowering blood fat of Agaricus blazei Murill ?-6 polyunsaturated fatty acid on hyperlipemia rats and mice

Objective To observe the action of lowering blood fat of Agaricus blazei Murill ?-6 polyunsaturated fatty acid(?-6APFA) on hyperlipemia rats and mice.Methods Sixty rats were randomly divided into control group,model group,natural soybean phospholipids capsule(NSPC)0.70 g?kg~(-1) group,?-6APFA 0.28,(0.14,)0.07 g?kg~(-1) groups(n=10).The other groups except control group were given high-fat diets for 14 d,on the fourteenth day the rats were administered orally,the control group and model group were administered distilled water 10 mL?kg~(-1) at the same volume,14 days after continuous administration,rats were anesthetized,the blood were extracted from abdominal artery,T-CHO,TG,HDL-C,LDL-C in sera were determined. At the same time,the activity of SOD in liver and the content of MDA were determined,the fat accumulated coefficient was calculated.72 male mice were randomly divided into control group,model group,NSPC 1.0 g?kg~(-1) group,(?-6APFA)0.4,0.2,0.1 g?kg~(-1) groups(n=12).Mice were administered continuously,16 h before the last administration,except control group,the mice in the other groups were injected 75% yolk physiological salt solution 0.5 mL through the abdominal cavity,and began to starve,1 h after the last administration,blood was extracted from eyeball,serum T-CHO and TG were determined.Results Compared with model group,T-CHO and TG in rats treated with NSPC 0.70 g?kg~(-1) and ?-6APFA 0.28 g?kg~(-1) all reduced and HDL-C raised obviously(P0.01).)Compared with model mice,T-CHO and TG in acute hyperlipemia mice treated with NSPC 1.0 g?kg~(-1) and ?-6APFA0.4,0.2 g?kg~(-1) reduced obviously(P
6.Preparation and application on compound excipient of sodium stearyl fumarate and plasdone S-630.

The compound excipient containing sodium stearyl fumarate and plasdone S-630 was prepared by applying spray drying method. The basic physical properties of compound excipient were studied by solubility test, scanning electron microscope, differential scanning calorimeter, X-ray diffraction and Fourier transform infra-red spectroscopy. The effect of compound excipient on moisture absorption and ferulic acid in vitro dissolution of spray drying power of angelica were investigated. The results showed that the chemical constituents of compound excipient did not change before and after spray drying. The water soluble compound excipient can improve significantly moisture absorption and has application prospect.
7.Computed tomography in the diagnosis of portal venous and intestinal wall gas in patients with ischemic bowel disease

Objective To investigate radiological features on computed tomography (CT) in the di agnosis of portal venous and intestinal wall gas in patients with ischemic bowel disease.Methods The clinic-pathological data of 17 patients with portal venous and intestinal gas associated with ischemic bowel diseases from Wenzhou People's Hospital (n =6),Yueqing People's Hospital (n =5),Shanghai Xuhui Dahua Hospital (n =3) and the Second Affiliated Hospital of Wenzhou Medical University (n =3) from January 2013 to October 2016 were analysed retrospectively.All the patients have been fasting for 8 h prior to CT scans.Enhanced CT study was performed following routine CT with no abdominal pressure for breath less scanting.Portal venous gas,intestinal wall gas,intestinal thickness and density,mesentery thickness,celiac effusion,and severity of intestinal wall enhancement were recorded.Results All the 17 patients ex perienced abdominal distension and pain.Additionally,nausea and vomiting was observed in 9 patients,di arrhea in 7,melena in 7,periumbilical tenderness in 11 and rebound tenderness in 8.CT scans of these 17 patients showed portal venous gas,including massiveprune-tree signs of hepatic vein and portal vein (n =11) and scanty gas shadows in distal hepatic vein (n =6).Intestinal gas sign was determined in all these patients (n =17),including single bubble shadow (n =8),multiple bubble shadow (n =7),and band-shaped bubble (n =2).Furthermore,CT study indicated extensive intestinal wall thickening with edema (n =13),predominate luminal extension of thinner bowels (n =4),scanty celiac effusion (n =3).Enhanced CT scans demonstrated 8 patients with decreased enhancement of intestinal wall and mesentery with diseases,target and halo signs observed in enhanced scans.Conclusions Portal venous and intestinal wall gas may demonstrate distinctive CT imaging.CT study could have superior sensitivity and spe cialty in clinical diagnoses of ischemic bowel diseases.
8.Quantitative detection of cytomegalovirus DNA by real-time PCR in transplant recipients

Objective To evaluate the advantages of cytomegalovirus (CMV) real-time PCR, to monitor CMV infection in a population of transplantation recipients in comparison with the qualitative PCR method.Methods 150 plasma samples and leukocytes samples collected from 59 bone marrow transplant recipients and 9 liver transplant recipients were tested by qualitative CMV PCR assay in parallel; Real-time CMV PCR using Roche Light Cycler was performed with 150 plasma samples and 54 control plasma samples.Results Using real-time PCR as the reference standard, the sensitivity and specificity of the qualitative CMV PCR assay with leukocytes samples were 62.5% and 95.5%, with plasma samples were 57.5% and 99.1% respectively. The recipients with the level of CMV DNA over 5?10~3 copies/ml by real-time PCR had higher percentage of developing CMV disease than those below 10~3 copies/ml.Conclusion The quantitative detection of CMV DNA by real-time PCR with plasma is a rapid, specific and sensitive method to monitor CMV infection in patients after transplantation and to guide antivirus therapy.
9.MRI and pathological findings of low-grade fibromyxoid sarcoma

Objective To investigate the MRI characteristics of low grade fibromyxoid sarcoma (LGFMS).Methods The MRI imaging of 13 cases with operation and pathology proven low grade fibromyxoid sarcoma were retrospectively analyzed.Comparative study of the relationship between the signal,boundary,cystic,and the enhancement degree with tumor tissue components was performed.Results The tumors were located in thigh (n=7),lower leg (n=3),elbow,shoulder and upper arm (n=l for each).The average diameter of tumor was (7.01 ±2.89) cm (range from 2.5 to 12.0 cm).The tumors with maximal diameter larger than 5 cm were seen in 11 patients.All tumors had well-defined margin.Necrosis and cystic change was seen in 5 tumors,tumor compartment in 3 cases,intratumoral fluid fluid level in 2 cases,and peritumoral edema in 3 cases.T1WI showed hypointensity in 6 cases,iso-,hopointensity in 4 cases,isointensity with slightly hyperintensity in tumor center in 4 cases,heterogeneous intensity in 1 case.T2WI showed hyperintensity in 6 cases,central higher intensity in 4 cases,hyper-,iso-,hypo-intensity in 4 cases,and hyper-,hypo-intensity in 3 cases.T2WI with fat suppression sequence showed hyper-,iso-,hypo-intensity in 8 cases and high intensity solid part and much higher intensity in tumor center in 5 cases.Heterogeneous enhancement of the tumors was noted,including mild enhancement in 8 cases,moderate enhancement in 4 cases,and marked enhancement in one case.Pathological results showed that the tumors were gray and white or yellow,well-defined,and hard texture,in which 5 cases cystic degeneration with tremelloid material can be seen in 5 cases.Microscopy results showed alternating distribution of fibrous and myxoid areas.Nine cases had rosettes formation and characteristic arcuate vessels were seen in 2 tumors.Immunohistochemical results showed negative Vimentin,Desmin,and S-100 in 5 patients,while 3 patients were positive for SMA and CD34.Conclusions LGFMS has some characteristic MRI finding.Combination of MRI and the clinical findings can suggest the diagnosis of LGFMS,however,the final diagnosis depends on pathological examination.
10.Protective Effects of Extract of Periplaneta Americana on Liver Fibrosis in Rats

Objective To investigate the protective effects of extract of Periplaneta americana (APA) against carbon tetrachloride (CCl4)-induced hepatic fibrosis in rats. Methods Seventy SD rats were divided into five groups:normal con-trol group (C), model control group (M), extract of APA 1 group (APA1), extract of APA 2 group (APA2) and reduced glutathi-one group (R). The liver fibrosis model was induced by injecting 40%CCl4-olive solution subcutaneously for seven weeks in M, APA1, APA 2 and R groups. Drugs were given at the same time. Rats were killed at 7-week (C, M, APA1 and R groups), and were killed at 9-week (APA2 group). The serum values of transaminase (ALT, AST), albumin (ALB), hyaluronic acid (HA) and laminin (LN), and hepatic inflammation and fibrosis were detected respectively. The expressions of apoptotic relat-ed gene Bcl-2 and Bax protein in central veins and portal areas of hepatic lobules were detected by immunohistochemical method. Results Rats showed poor nutritional status, and significantly increased transaminase, HA and LN in M group, but the serum level of ALB was significantly lower than that in C group. There was extensive necrosis of liver cells, obviously fi-brosis or cirrhosis in model rats’liver tissues. The serum contents of ALT, AST, HA and LN were significantly decreased, the serum level of ALB were significantly increased in APA1,APA2 and R groups (P<0.05). There were complete hepatic lobule and no obvious fibrous tissue hyperplasia in liver biopsy of APA1 and R groups. The values of Bcl-2/Bax proteins in central vein of liver tissues were significantly higher in APA1,APA2 and R groups than those of M group (P<0.05), but the ratio of Bcl-2/Bax was lower in periportal area of liver tissues (P<0.05). There were significantly higher body weight and ALB levels in APA1 group than those in R group (P<0.05). The level of transaminase was slightly higher in APA1 group than that of R group. The degeneration of liver cells was found mostly in R group. Conclusion The extract of Periplaneta americana has a role in protecting liver cells, inhibiting the progression of hepatic fibrosis, and improving the nutritional sta-tus of trial rats.
