1.Investigation on quality of life in patients with inflammatory bowel disease and their family members in Fujian Province

Objective To investigate the quality of life (QOL) and its main influencing factors in patients with inflammatory bowel disease (IBD) and their families members in Fujian Province,and to provide an evidence for guiding the clinical strategy.Methods From September 2015 to March 2016,114 IBD patients who met the standards,their 97 family members,and 113 irritable bowel syndrome (IBS) patients were enrolled.The QOL was assessed with 36-item short form health survey questionnaire (SF-36).T test,inspection rank and Chi-square were performed for statistical analysis in two groups and multiple group.The influence factors of QOL using multiple linear stepwise regression analysis.Results There was no difference in QOL scores between CD and UC patients ((62.82 ± 18.55) vs (62.60 ± 17.94);t=0.065,P=0.948).The QOL scores of IBD patients were lower than those of IBS patients (62.72± 18.18 vs 67.25 ± 16.26),and the difference was statistically significant (t=-1.975,P=0.049).The QOL scores of IBD patients with medical insurance were higher than those of patients without medical insurance (67.13 ±18.18 vs 57.06 ± 16.68),and the difference was statistically significant (t=3.034,P=0.003).The dimension scores of body pain,general health,social function,emotional function,mental health in IBD patients relatives were lower than those of the general population ((72.29±17.27) vs (85.61±18.37),(60.92±17.08) vs (69.55±21.32),(80.80±15.32) vs (86.85±17.07),(68.38±37.38) vs (76.45±38.45),(61.97±15.30) vs (72.65 ± 16.81),respectively),and the differences were statistically significant (t=-7.597,-4.979,-1.870,-2.125,-6.875,all P<0.05).The total QOL scores of families members of IBD patients with operation history was lower than those of without operation history (64.62± 16.74 vs 73.98±14.15),and the difference was statistically significant (t=-2.540,P=0.013).Conclusions The QOL of IBD patients was lower than that of IBS patients,and also The QOL relates with payment types for medical care,and the QOL of family members of IBD patients also decreases.Psychological interventions should be in plemented in both IBD patients and their family members in clinical treatment.

Toll-like receptor 4(TLR4)is a member of the TLRs superfamily,mainly capable of identifying bacterial endotoxin,lipopolysaccharides and lipooligosaccharides of Gram-negative bacte?ria cell walls to prevent microbial invasion. Activation of TLR4 can induce production of proinflammato?ry cytokines and inflammatory chemokines and regulate natural immunity. However,dysregulation of TLR4 can lead to autoimmune diseases. This review summarized the biological structure of TLR4 and recognition mechanisms between TLR4 and its ligands,surveyed TLR4 ligands including lipid A ana?logues,natural products and synthetic small molecules,discussed the structure-activity relationship of TLR4 modulators and the ligand-receptor and protein-protein interactions in the complex,and outlined the prospect of future research and development of TLR4 ligands.
3.Efficacy of aminophylline for prevention of negative chronotropic effect of remifentanil

Objective To evaluate the efficacy of aminophylline for prevention of the negative chronotropic effect of remifentanil.Methods Forty patients undergoing general anesthesia,aged 20-55 yr,with body mass index of 18-25 kg/m2,of ASA physical status Ⅰ or 1Ⅱ,were randomly divided into control group (group C) and aminophylline group (group AP),with 20 patients in each group.At 30 min before induction of anesthesia,normal saline 20 ml was infused intravenously in group C,and aminophylline 3 mg/kg was infused intravenously in group AP.Midazolam 0.1 mg/kg was then injected intravenously.Remifentanil 2 μg/kg was injected intravenously after the patients fell asleep and then artificial ventilation was performed.Vecuronium 0.1 mg/kg,fentanyl 3 μg/kg,and etomidate 0.3 mg/kg were injected intravenously 7 min later.The patients were then endotracheally intubated and mechanically ventilated.HR and MAP were recorded before induction of anesthesia (T0),after falling asleep (T1),and at 0,1,3,5 and 7 min (T2-6) after the end of remifentanil injection.Low frequency (LF) and high frequency (HF) were recorded at T0,T1 and T2,and the ratio of LF/HF was calculated.Results Compared with the baseline value at T0,MAP and HR were significantly decreased at T2-6 in the two groups,and LF and HF were decreased at T1,and LF/HF ratio was increased at T1 in group AP.HR and LF/HF ratio were significantly higher,and LF and HF were lower at T1 in group AP than in group C.Conclusion The efficacy of aminophylline for prevention of the negative chronotropic effect of remifentanil on the hearts of patients is good.
4.Corneal histopathological changes and interleukin-6 level in aqueous humor of a rabbit model of penetrating corneal trauma combined with seawater immersion*

BACKGROUND: Following trauma caused by seawater, cells often exhibit special pathological changes because of the special physico-chemical properties of seawater.OBJECTIVE: To observe corneal histopathological changes and interleukin-6 level in aqueous humor of a rabbit model of penetrating corneal trauma combined with seawater immersion.METHODS: The rabbit eye models of penetrating corneal trauma caused by firecrackers were established in 16 adult healthy grey rabbits. A 3-mm whole-layer incision was made in the cornea. The right eyes served as experimental sides and the left eyes served as controls. Seawater was injected into the aqueous humor of the right eyes via the corneal incision. The eye surface was flushed with seawater for 30 minutes. Physiological saline was used for the left eyes. RESULTS AND CONCLUSION: Optical microscopy results showed that at 1, 2, 3 days after model establishment, corneal cells on the experimental side exhibited severe necrosis and abscission, obvious swelling of substantia propria layer complicated by cellular infiltration. At 1 and 2 days after model establishment, the pathological changes on the control side were the same as the experimental side, but they were mild, but at 3 and 5 days, they were obviously alleviated. At 1, 2, 3 days after model establishment, interleukin-6 level in aqueous humor was significantly higher on the experimental side than on the control side (P < 0.05). These findings suggest that the degree of injury on the experimental side was more severe than that on the control side, indicating that seawater may be an important causative factor of corneal injury.
5.The value of endothelial function of brachial artery in predicting coronary artery disease in elderly patients

Objective To evaluate the value of endothelial function of brachial artery in predicting coronary artery disease in elderly patients. Methods Flow-mediated vasodilation (FMD)and nitroglycerin mediated dilation (NMD) were measured by high-frequency ultrasound in 98 patients with coronary artery disease and 37 controls. All the subjects were diagnosed by coronary arteriography. They were divided into coronary artery blockages < 50% subgroup, 50%-75%subgroup, >75% subgroup or coronary blockages in 0, 1, 2, 3 branch (es) subgroups. The correlations between FMD or NMD and the pathological extent or branches of coronary artery were studied and the predictive value of FMD and NMD were analyzed with ROC curve. Results In coronary artery disease group, FMD and NMD were significantly decreased as compared with the control group. They were closely related with coronary artery disease (r= -0. 54, -0. 52, P<0.01), and were weakly correlated with severity and range of coronary arterial lesion (P<0. 01).With ROC curve analysis, FMD and NMD could be used to predict coronary artery disease.Conclusions Endothelial function can be used as an index to predict coronary artery disease.
6.Effect of penehyclidine hydrochloride pretreatment on NF-κB mRNA expression and SOD activity in lung tissues in rats with acute lung injury induced by LPS

Objective To investigate the effects of penehyclidine hydrochloride (PHCD) pretreatment on NF-κB mRNA expression and SOD activity in lung tissues in rats with acute lung injury (ALI) induced by LPS.Methods Thirty-two male SD rats, 2 months old, weighing 230-280 g, were randomly divided into 4 groups (n=8 each): control group (group C), ALI group, low dose PHCD group (group LP) and high dose PHCD group (group HP). ALI was induced by intravenous LPS 5 mg/kg via tail vein. Group LP and HP received intraperitoneal PHCD 0. 3 and 1 mg/kg respectively 30 min before LPS administration. The rats were killed at 6 h after LPS administration. The lungs were removed immediately for determination of W/D lung weight ratio, lung water content, NF-κB mRNA expression, TNF-α and MDA content, and SOD activity and microscopic examination. Results NF-κB mRNA expression, TNF-α and MDA content, W/D lung weight ratio and lung water content were significantly higher, while SOD activity was significantly lower in group ALI, LP and HP than in group C (P < 0.05). NF-κB mRNA expression, TNF-α and MDA content, W/D lung weight ratio and lung water content were significantly lower, while SOD activity was significantly higher in group LP and HP than in group ALI and HP than in group LP (P < 0.05). The LPS-induced changes were mitigated by pretreatment with low and high doses of PHCD in group LP and HP.Conclusion Pretreatment with PHCD attenuates LPS-induced ALI by downregulating NF-κB mRNA expression, decreasing local inflammatory response and enhancing anti-oxidant activity.
7.DNA extraction method of pathogenic fungi and the optimization of simple sequence repeat PCR system

BACKGROUND: Extraction of fungal DNA plays an important role in fungal genetic engineering and molecular biology research.The result of experiment is affected seriously by the efficiency of extracting DNA especially the quality of DNA. OBJECTIVE: To develop a method for extracting genomic DNA of pathogenic fungi and discuss the optimal combination of components in simple sequence repeat PCR (SSR-PCR) system. DESIGN, TIME AND SETTING: A comparative analysis of DNA extraction methods and an orthogonal experiment were conducted in the Mycology Research Lab of Department of Pathogenobiology in Norman Bethune College of Medicine, Jilin University from July 2004 to December 2006.MATERIALS: Clinical specimens were inoculated on Potato dextrose agar, Potato dextrose broth and Yeast extract peptone dextrose and cultivated under the temperature of 28 ℃ for 3-7 days, after which suspectable colonies were selected to be isolated and purified.METHODS: Three kinds of methods of extracting DNA(beading-salt fractionation method, CTAB method and Gene TLE~(TM) extraction method ) were compared in terms of their effects on DNA quality; Experiment was performed with orthogonal design to four factors (Taq DNA polymerase, template DNA, dNTP, primers) in three levels on the basis of L9 (3~4) orthogonal table, The appropriate annealing temperature and cycles were determined through PCR.MAIN OUTCOME MEASURES: The optimal reaction system determined according to the polymorphism and specificity of amplification of banding pattern.RESULTS: The objective fragments were all amplified by Gene TLE~(TM) extraction method, and the banding patterns obtained were clearer and brighter compared with the other two methods. The result of orthogonal experiment on SSR-PCR system showed that,according to the value of R, the significance of factors followed by ascending were template DNA (2.67), Taq DNA polymerase (2.00), dNTP (0.67) and primers (0.33). According to the value of ki, the optimal level of each factor combination was 30 mg/L template DNA, 1U Taq DNA polymerase, 150 μmol/L dNTP, 0.5 μmol/L primer. However, because primers were nonsignificant factors, which was presented by their small R value, we took A level of primer as 0.25 μmol/L. The best reaction condition was 55 ℃ annealing temperature and 35 cycles.CONCLUSION: The Gene TLE~(TM) method shows higher efficiency of extracting DNA and its operation is fast and simplel According to the results of orthogonat experiment, the optimal SSR-PCR system was 30 mg/L template DNA, 1U Taq DNA polymerase, 150 μmol/L dNTP and 0.25 μmol/L primer. The best reaction condition was 55℃ annealing temperature and 35 cycles.

Objective To construct a recombinant expression vector of human interleukin-24(hIL-24) gene and express it in E.coli M15,and to evaluate the bioactivity of IL-24 fusion protein.Methods The human IL-24 cDNA fragment was amplified from plasmid by polymerase chain reaction(PCR),and sequenced.PQE/hIL-24 was constructed by gene rearrangement,then it was transformed into E.coli M15.The expression of the target protein was induced with IPTG and purified by Ni2+-NTA agarose column.The expressed recombinant hIL-24(rhIL-24) was identified by SDS-PAGE and Western blotting.Normal peripheral blood mononuclear cells(PBMCs) were cultivated with the expression protein for 48 and 72 h,the levels of IL-6,IFN-? and TNF-? of PBMCs stimulated with rhIL-24 were detected by ELISA.Results The recombinant prokaryotic expression vector PQE/IL-24 was constructed successfully and expressed stably in E.coli M15.At about 18 400 of molecular weight,there was an induced protein band.The levels of IFN-?,IL-6 and TNF-? in the group of cultivated with the expression protein were obviously higher than those in the groups without the expresson protein(P
9.Effects of pancreatic kininogenase on expression of TGF-?_1 and collagen remodeling in spontaneously hypertensive rats

Objective To detect the effects of pancreatic kininogenase on the expression of TGF-?1 and collagen remodeling in myocardiom of spontaneously hypertensive rats(SHR).Methods Twenty-four male SHR(aged fifteen weeks)were randomly divided into three groups:SHR group,pancreatic kininogenase group and captopril group(n=8),8 male Wistar Kyoto rats with normol blood pressure was considered as control group.Pancreatic kininogenase was given by peritoneal injection(7.2 U? kg-1? d-1),captopril was given by intragastric administration(10 mg? kg-1? d-1),the rats in SHR group and control group were administered with 0.9% NaCl(2 mL ? kg-1? d-1)through peritoneal injection.After four-week experiment,the pressure was measured in rats througth carotid artery,the rats were sacrificed and left ventricular mass index,collagen volume fraction,peripheral vascular collagen area were measured.Myocardial tissue was stained with VG and pathological changes were observed.The expression of TGF-?1 were detected by immunohistochemical technique(SP method).Results The systolic blood pressure,left ventricular mass index,collagen volume fraction,peripheral vascular collagen area and the expression level of TGF-?1 in SHR group were obviously higher than those in control group(P0.05).Conclusion Pancreatic kininogenase can obviously control pressure and reverse myocardial fibrosis probably by decreasing the expression of TGF-?1 in SHR.

OBJECTIVE: To develop a method for the determination of levodopa in human plasma and study aarodopa's pharmacokinetics.
