1.Effect of evidence-based nursing in patients with acute respiratory distress syndrome

Objective To explore the effect of evidence-based nursing in patients with acute respiratory distress syndrome(ARDS).Methods A total of 106 ARDS patients admitted in our hospital were divided into control group (n =53) and study group (n =53) with random number table principle.The control group adopted routine nursing measures,while the study group applied evidence-based nursing intervention.The changes of respiratory function,and organ function,and hospitalization time were observed before and after intervention.Results The respiratory function index PEEP was lower in the study group than in the control group (P < 0.05).Levels of SaO2 and p(O2)/FiO2 were higher than that in the control group (P < 0.05).Marshll and Murray scores were lower in the study group than that in the control group (P < 0.05).The duration of hospital stay was (16.50 ± 2.25) d,which was lower than (19.50 ±-2.50) d of the control group (P < 0.05).Conclusion Evidence-based nursing has significant effect in patients with ARDS,which can effectively improve the function of respiratory function and organ,speed up the recovery rate and is suitable for clinical promotion.

3.Research on the expression of CD28 and CD160 in patients with chronic HIV infection

Objective To investigate the expression and clinical significance of CD28 and CD160 in patients with chronic HIV infection.Methods 50 patients with HIV from January 2016 to January 2017 were selected as the observation group, and 50 healthy volunteers were recruited as control group.Observe and record general information of all participants, the expression of CD28, CD160 in CD4+and CD8+T cells, initial T cells (TN), the expression of CD160 in central memory T cells (TCM), effector memory T cells (TEM), end effector memory T cells (TEMRA), mean fluorescence intensity (MFI), viral load of two kinds of the cells, analyze the correlation between the expression level of CD28 and CD160 and CD4+T cell count and viral load.Results With the increase of CD160 expression of CD4+T cells, CD4+T cells showed a downward trend, there is a negative correlation between them (r=-0.561, P<0.05), CD8+T cell number is on the rise, there is a positive correlation between them (r=0.619, P<0.05), and HIV-RNA copy number increased with the increase of CD160 expression on CD4+T cells and CD8+T cells, both positive (r=0.684, P<0.05, r=0.459, P<0.05);with the increase of CD28 cells on the expression of CD4+T, CD4+, CD8+T cells showed a rising trend, there is a positive correlation between them (r=0.621, P<0.05, r=0.527, P<0.05, HIV-RNA) and the copy number decreased with the increase of the expression of CD28 and CD4+T on CD8+T cells, there is a negative correlation between them (r=-0.634, P<0.05, r=-0.582, P<0.05).There was no significant difference in the positive rate of expression in TEMRA subgroup and MFI of CD160 in CD8+T cell in two groups (P>0.05).The positive rate and MFI of CD8+T cell CD160 in TN, TCM and TEM subgroups in observation group were significantly higher than those in control group (Tcm), with statistical significance.Conclusion The expression of CD28 in patients with chronic HIV infection is decreased, and the expression of CD160 is increased, which may be related to the decrease of HIV CD4+T and CD8+T cells, in which CD160 mainly affects the memory CD8+T.
4.Expression of chemokine receptors on Th17 cell in peripheral blood of patients with chronic hepatitis B

Objective To investigate the expression of chemokine receptors CCR4,CCR6,CXCR3 on Th17 cell,and analyze the correlation between chemokine receptors and serum biochemical parameters of liver function test and hepatitis B virus (HBV) DNA load in peripheral blood of chronic hepatitis B (CHB) patients.Methods Thirty patients with CHB (CHB group) and 15 healthy persons (control group) were enrolled in the study.CCR4,CCR6,CXCR3 expression levels on Th17 cell were assayed by flow cytometry.The correlation between chemokine receptors and alanine aminotransferase (ALT),total bilirubin (TBil),and HBV DNA load were analyzed using Pearson' s correlation analysis,respectively.Results CCR4,CCR6,CXCR3 expression levels on CD4 + Th17 cell in CHB group were higher than that of control group.Moreover,the difference between them was statistically significant (P < 0.05).CCR4,CCR6 expression levels on CD8 + Th17 cell in CHB group were higher than that of control group (P < 0.05).Although CXCR3 expression level on CD8 + Th17 cell in CHB group was higher than that of control group,there was no significant difference between them (P > 0.05).CCR4,CCR6 expression levels were positively correlated with serum ALT,HBV DNA load,respectively (P < 0.05),but were not correlated with TBil (P > 0.05).Conclusion Expression levels of chemokine receptor on Th17 cell were increased in patients with CHB and were positively associated with degree of liver inflammation.Therefore,CCR4,CCR6 expression on Th17 cell might be involved in liver injury resulted from Th17 cell.
