1.The Roles of Health Education in the Prevention and Treatment of Gout and Hyperuricemia

Objective To elucidate the significances of health education in the prevention and treatment of gout and hyperuricemia.Method Health education scientific knowledg was done through approaches of specific proclamation in clinic out patients,screening the directly-related members of patient family,training medical staffs and popular vehicle propaganda etc.Results Health education improved the diet structure and life mode of patients,promoted early diagnosis and treatment of patients.It also had favourness in timely detection of other patients in directly-related members of ones family.Conclusion Health education combined with conventional pharmacological therapy could produce a better efficacy than that of single pharmacological ones.
2.Intraarticular injection of sodium hyaluronate plus nerve and pain block with betamethasone in treatment of periarthritis of shoulder

BACKGROUND: Very satisfactory clinical effects have been achieved in treatment of osteoarthritis of knee with intraarticular injection of sodium hyaluronate (SH) allied with steroid hormones for nerve block. How will such allied treatment be used for periarthritis of shoulder?OBJECTIVE: To probe into the improvement of pain and joint function in periarthritis of shoulder treated with integration of intraarticular injection of SH with nerve and pain block with betamethasone.DESIGN: Self-control was designed before and after experiment.SETTING: Beijing Tiantan Hospital, Capital University of Medical Sciences PARTICIPANTS: Twenty patients with periarthritis of shoulder were included, aged from 40 to 65 years from Pain Clinic of Beijing Tiantan Hospital affiliated to Capital University of Medical Sciences from January to October 2004.METHODS: Integration of intraarticular injection of SH with nerve and pain block with betamethasone was applied to treat 20 cases of periarthritis of shoulder and the record was done before treatment and 1in which, "no any pain sensation" was 0 score, "unable to bear pain with imagination" was 10 scores. Pain score was recorded during quiet involved. 4 scores indicated being impossible completely, 3 scores indicate being possible with help, 2 scores indicate being possibled reluctantly and mostly feeling inconvenient, 1 score indicated being possible reluctantly without inconvenience and 0 score indicated being possible as healthy person.of daily life activity.RESULTS: According to practical management and analysis, 20 cases enment compared with that before treatment [(4.34±1.33),(8.27±1.37)scores,compared with that before treatment [(4.73±1.31), (8.49±1.35)scores,ment compared with that before treatment [ ( 1.10 ±0.74), (3.30 ±0.48 )scores, P < 0.01]. CONCLUSION: Integration of intraarticular injection of SH with nerve and pain block with betamethasone treats effectively periarthritis of shoulder, without remarkably harmful reaction and it is worth in popularization in treatment of pain.
3.PDCA Model for Management of Infectious Disease Prevention and Control in Hospital

OBJECTIVE To explore a management method for the prevention and control of infectious diseases in hospital. METHODS We applied the PDCA (plan,do,check and action) model to establish a management strategy for the prevention and control of infectious diseases for three years. The components of the management model included providing educational program for medical staff,designing and realizing direct network system for infectious disease reporting,monitoring diagnosed infectious diseases ,analyzing their epidemic tendency,guiding medical staff to prevent and control infectious diseases,and strengthening the surveillance,feedback,rewards and punishment. RESULTS The rate of delayed or missed report of infectious diseases was decreased from 5% to 1.1% by monthly check and from 6.8% to 0% by seasonally cross check. Although 46 cases of AIDS,5 cases of caesarean birth with HIV possitive and 1445 cases of hand-foot-mouth disease were admitted in our hospital,No medical staff and other patients was crossly infected. CONCLUSIONS Our way of management based on PDCA model can strengthen the prevention and control of infectious diseases and assure medical quality and patient safety.
4.Distribution of Nosocomial Infection Pathogens

OBJECTIVE To study the distribution and drug resistance of nosocomial infection(NI) pathogens.METHODS The NI pathogens collected between Jan 2002 and Dec 2008 were completely surveyed and analyzed.RESULTS Totally,9518 samples were cultured to find NI pathogens,in which 6703 NI pathogens were isolated.In the pathogens,G-pathogens occupied 59.5%,G+ cocci 25.0%,and fungi 15.5%.The rate of ESBLs in E.coli was reached to 47.8%,and that in K.pneumoniae was 29.4%;The first two predominant G+ pathogens were Staphylococcus aureus(11.7%) and Enterococcus(5.7%).The rate of MRSA in S.aureus was 78.2%,and that of VRE in Enterococcus was 2.3%,The pathogens of fungus were mainly Candida albicans(10.2%).CONCLUSIONS The G-NI pathogens are markedly increased year by year and become the first important instead of G+ cocci,meanwhile the multiple drug resistance and ESBLs pathogens are also markedly increased.
5.Application of poly (N-isopropylacrylamide) and its derivatives in tissue engineering.

Poly-N-isopropylacrylamide (PNIPAAm) is a new kind of intelligent material. It shows favorable thermo sensitivity because of the structure of hydrophilic acrylamino and hydrophobic isopropyl. PNIPAAm also shows good biocompatibility and non-toxicity. All the characters as above make it an ideal extra cellular matrix material for tissue engineering. This paper reviews its application in tissue engineering.

6.Protective effect of hepatocyte growth-promoting factor against liver ischemia-reperfusion injury in rats and its mechanism of action

ObjectiveTo investigate the protective effect of hepatocyte growth-promoting factor (PHGF) against liver ischemia-reperfusion injury in rats and its mechanism of its action. MethodsA total of 80 healthy male Sprague-Dawley rats were randomly divided into experimental group (PHGF group) and control group (NS group), with 40 rats in each group. A rat model of liver ischemia-reperfusion injury was established by 70% liver ischemia caused by the occlusion of blood flow in the middle and left lobes of the liver, with an ischemia time of 21 minutes. The rats in the PHGF group were given intraperitoneal injection of PHGF for intervention before surgery, and those in the NS group were given an equal volume of normal saline. Serum and liver tissue samples were collected before surgery and on days 1, 3, 5, and 7 after surgery, and the levels of alanine aminotransferase (ALT), aspartate aminotransferase (AST), and total bilirubin (TBil) were measured; HE staining was used to observe pathological changes; real-time PCR was used to measure the mRNA expression of mitochondrial transcription factor A (TFAM) in the liver. The independent samples t-test was used for comparison of continuous data between two groups. ResultsHE staining showed that compared with the NS group, the PHGF group had significantly lower degrees of hepatocyte swelling, inflammatory cell infiltration, and hepatocyte necrosis under a light microscope. Liver biochemistry showed that on days 1, 3, 5, and 7 after surgery, the PHGF group had significantly lower serum levels of ALT, AST, and TBil than the NS group (t=11.879, 16.019, 22168, 10.235, 9.041, 12.936, 18.759, 8.142, 10.108, 11.014, 13.245, and 9.968, all P<0.001). Real-time PCR showed that on days 1, 3, and 5 after surgery, the PHGF group had a significantly higher mRNA level of TFAM in the liver than the NS group (t=7998, 14.764, and 13.861, all P<0.001). ConclusionPHGF preconditioning exerts a protective effect against liver ischemia-reperfusion injury in rats, possibly by upregulating the expression of TFAM to alleviate liver ischemia-reperfusion injury.
7.The Changes of Neurocognitive Function in Early Stage in Patients with First-Episode Schizophrenia

Objective:To evaluate the changes of neurocognitive function in early stage in patients with first-episode schizophrenia.Methods:In this study,73 cases of patients with first-episode schizophrenia (research group) and 75 cases of health person (control group) were selected from January 2015 to January 2016 in our hospital.The neurocognitive function was evaluated by neuro-psychological testing tool and the data between two group were compared.Results:Scores of delayed recall,total recall,3 trial,2 trial and 1 trial of research group were lower than those of control group in BVMT-R test,and the difference was statistically significant (P<0.05).In HVLT-R test,the scores of total delay,3 trial and 2 trial of research group were significantly lower than those of control group (P<0.05).The consuming time of dominant hands and subdominant hands in pegboard tasks were significantly higher in research group than in control group (P<0.05).Conpletion time of connection test A,color connection 1 and 2 in connection test of research group were significantly higher than those of control group (P<0.05).Attempt number and correct number in research group in PASAT test were significantly lower than in control group (P<0.05).Number of search errors in research group was higher than in control group,while number of search correct,search total score and digital sign score were significantly lower than in control group (P<0.05).Total number of words,color and color / word count in research group were lower than in control group in Stroop color word test,and the difference was statistically significant (P<0.05).WMS-Ⅲ test results between two group had no significant difference (P>0.05).Conclusion:The neurocognitive function in early stage in patients with first-episode schizophrenia has been extensively damaged.
8.Antagonism of obidoxime on sarin induced miosis and visual impairment in rabbits

OBJECTIVE The antagonism of obidoxi me on sarin induced miosis and visual impair-ment was evaluated and its antagonistic mechanism was investigated.METHODS ① 30 min after sarin (2 μg /0.1 mL per eye)was given as an eyedrop,the ability of the 2.5%,5.0%,7.5% obidoxi me and 1 .0% atropine to reverse effects of sarin on pupil dia meter and light reflex were evaluated at different ti mes.② Another 36 rabbits received sarin and at 30 min afer sarin exposure,the drugs above were ad-ministrated and their effects on pupillary light reflex,as well as the AChE activity of cornea,iris and reti-na were recorded 4h after the treatment.RESULTS ① Miosis and impaired pupillary light reflex oc-curred soon after sarin exposure but the abnormal pupil width and pupillary light reflex had disappeared by 48 h after sarin exposure;Subcequent to 1 .0% atropine treatment,the pupil dilatedinstead while the impaired light reflex did not i mprove significantly;unlike atropine,soon after ad ministration of 2.5%, 5.0%,7.5% obidoxi me,the pupil dia meter and light reflex were significantly increased(P <0.01 )and then had beco me normal totally by 24 h post-dose,much faster than those of the control and atropine treatment group.However,there was no significant difference in the recovery ti me between the different dose groups of obidoxi me.② 4h after treatment,the AChE activity in cornea and irisof sarin-treated group were (42 ±4)%,(26 ±2)%,respectively;the AChE activity in cornea of 2.5%,5.0%,7.5%obidoxi me were (74 ±1 1 )%,(81 ±10)% and (74 ±7)%,respectively,and the AChE activity in iris were(39 ±10)%,(43 ±8)% and (43 ±8)%,respectively ,co mpared with sarin-treated group,AChE activities of cornea and iris as well as light reflex of the obidoxi me-treated group were significantly increased(P<0.01 ).But there was no difference in light reflex and AChE activity between the sarin-treated and atropine-treated groups.CONCLUSION Obidoxi me showed better antagonism of sarin-induced ocular effects than that of the commonly used drug,atropine;the antagonistic mechanism is likely closely related to its rapid reactivation of the inhibited AChE in the cornea and iris.
9.The clinical application of symmetrical occluder in treating intercristal ventricular septal defect

Objective To investigate the interventional treatment strategy for occluding the intercristal ventricular septal defect (VSD) in order to improve the surgical safety and success rate. Methods During the period from January 2012 to December 2013, a total of 31 patients with intercristal VSD were admitted to authors’ hospital to receive interventional catheter occlusion therapy. Preoperative color Doppler ultrasound echocardiography showed that on the short axis view of the aorta the VSD interrupted port was situated at 12:00 - 1:00 o’clock region. Left ventricular and above aortic valve angiography indicated that the VSD location, shape and size, the split vent size on the left ventricle side and its distance from the aortic valve could be correctly measured when the VSD shunt was visualized , which were very helpful in guiding the operator to select the suitable occluder as well as to adjust the release pattern of the occluder. Postoperative imaging findings of the left ventricular and above aortic valve angiography were compared with the preoperative ones. Results Successful occlusion of VSD was obtained in 22 patients , in 13 among them the left ventricular angiography showed that the direction of blood flow beam at the defect hole was from the left ventricle to the right ventricle in an obliquely upward direction. The basal width of the defect on the left ventricle side was (5.12 ± 1.38) mm, and(6 - 10) mm occluder was employed. In the remaining 9 patients the left ventricular angiography showed that the direction of blood flow beam at the defect hole was from the left ventricle to the right ventricle in a direction almost parallel to the aortic valve , and the basal width of the defect on the left ventricle side was (7.18 ± 1.26) mm, and (9 - 12) mm zero-bias occluder was adopted. Interventional occlusion of VSD was unsuccessful in 9 cases as the intercristal hole was rather larger, and two of them had coexisting aortic sinus aneurysm complicated by mid-to-severe degree aortic valve regurgitation. Conclusion Based on the precise analysis of angiographic images by experienced radiologists optimal treatment scheme can be worked out. If conditions permit, symmetrical occluder should be employed so far as possible in order to reduce the degree of operation difficulty and improve the surgical safety and the success rate as well.
10.Study on the relationship between the level of D-dimer and prognosis in patients with acute exacerbation of chronic obstructive pulmonary disease

Objective To explore the role of D-dimer (D-D) level in patients with acute exacerbation of chronic obstructive disease (AECOPD) in predicting the prognosis of patients.Methods 99 patients with AECOPD in our hospital from July 2008 to December 2010 were selected as the subjects,and were divided into observation group (n=50) and control group (n=49) according to the expression of D-D in patients with positive (D-D level cutoff value of 500 g/L).The basic information and survival of the two groups of patients were investigated,and the level of serum D-D was detected by scatter turbidimetry.The receiver operating characteristic curve (ROC) was used to evaluate the predictive value of D-D on the survival of the patients.The survival time of the two groups was compared by Kaplan-Meier.Results There was no statistically significant difference in gender,age and GOLD score between the two groups (P>0.05).The level of D-D,the time of first hospitalization,the number of re hospitalization,the first hospital mortality and re hospitalization mortality in the observation group were significantly higher than those in the control group (P<0.05).The ROC curve showed that the truncated value of D-D was 1 171.34 g/L and 893 g/L,respectively.The average survival time of patients in the observation group was 30.37 months and the control group was 46.37 months,respectively;Kaplan-Meier study showed that the survival time of patients in the observation group was significantly lower than that in the control group (x2 =6.371,P<0.05).Conclusion The level of D-D in patients with AECOPD is of great value in predicting the prognosis of patients,which can be used as an independent indicator of disease progression.
