1.Application of needle -based confocal laser endomicroscopy in diagnosis of pancreatic cystic lesions

Pancreatic cystic lesions (PCLs)have various pathological types,extremely different biological behaviors,and different progno-sis.Current diagnosis of PCLs mainly depends on a comprehensive analysis of the patient's medical history,clinical manifestations,imaging examinations,serological examinations,fine needle aspiration biopsy,and cytological examination of cyst fluid.Needle -based confocal la-ser endomicroscopy (nCLE)is a novel method of microscopic endoscopy and is compatible with 19 -gauge puncture needle.Under the guid-ance of endoscopic ultrasound,nCLE can achieve real -time in vivo imaging of the cellular and subcellular structures of diseased tissues and thus helps with the accurate diagnosis of PCLs.This article reviews the application of nCLE in the diagnosis of PCLs.
2.Diagnosis of Gastric Cancer and Precancerous Changes by Confocal Laser Endomicroscopy

At present,the diagnosis of gastric cancer and precancerous changes mainly depends on the histological examination of biopsy samples.Confocal laser endomicroscopy could enable realtime observation of tissue,cellular,and subcellular structure of gastrointestinal tract,and differentiate normal,hyperplastic,and neoplastic mucosa.Confocal laser endomicroscopy has shown the superiority and value in in vivo diagnosis of gastric cancer and precancerous changes, including atrophic gastritis,intestinal metaplasia,and intraepithelial neoplasia,et al.This article reviewed the advances in studies of the diagnosis of gastric cancer and precancerous changes by confocal laser endomicroscopy.
3.Combined use of mosapride and polyethylene glycol for colonoscopy preparation

Objective To compare the clearing effect between combination use of mosapride and polyethylene glycol (PEG) and polyethylene glycol only for colonoscopy preparation of patients with constipation caused by functional diseases.Methods A total of 212 adult patients who underwent colonoscopy examination were recruited to this randomized single blinded controlled study.Patients in control group received 3 liters of PEG only,whereas patients of experimental group received 10mg mosapride 30min before 3liter PEG.The patients were surveyed for the tendency of constipation.Experimental variables including time of first defecation and completion of bowel cleansing,defecation frequency,and degree of discomfort during colonoscopy preparation were recorded.Results The times of first defecation and completion of bowel cleansing of experimental group were shorter than those of control group ( P < 0.05 ),but there were no significant differences in defecation frequency,the degree of discomfort and the bowel cleansing quality between 2 groups ( P > 0.05 ).The percentages of patients with constipation in control and experimental groups were 26.61% and 26.67%,respectively.The defecation frequency and the degree of discomfort of constipative patients were similar in both groups ( P > 0.05 ).The time of first defecation and time of completion of bowel cleansing of constipative patients in experimental group were shorter than those in control group ( P < 0.05 ).The bowel cleansing quality of constipative patients in experimental group was better than that of control group ( P < 0.05 ).There was no difference in defecation frequency and adverse reaction between 2 groups (P < 0.05).Conclusion Combination of mosapride and PEG for colonoscopy preparations of patients with functional constipation tends to shorten times of first defecation and completion of bowel cleansing,and to increase the bowel cleansing quality.
4.In-vitro Antibacterial Activity of the Combination of Herba Violae and Herba Taraxaci in Different Proportions

【Objective】In-vitro antibacterial activity of the combination of Herba Violae(HV)and Herba Taraxaci(HT)in different proportions was compared and analyzed for the optimization of the combination proportion of Herba Violae and Herba Taraxaci and the influencing factors.【Methods】The minimum inhibitory concentration(MIC)of the combination of HV and HT was determined by agar double dilution method and the minimum bactericidal concentration(MBC)by broth dilution method,and bactericidal curve was traced.Meanwhile,the resistance of the combination of HV and HT induced by sub-inhibitory concentration was analyzed,and the influencing factors of in-vitro antibacterial activity were investigated.【Results】The combination of HV and HT in the proportion of 1∶4 had a better antibacterial activity on four kinds of gram-positive bacteria and on four kinds of gramnegative bacteria,and exerted an bactericidal effect on Escherichia coli and Klebsiella Pneumoniae(both MBC_(50)/MIC_(50) and MBC_(90)/MIC_(90) being 2).The bactericidal curve showed that 2?Log cfu/mL bacteria strain could be killed by the combination of the proportion being 1∶4 at 8~(th) hour,the effect being superior to that of other two combinations.Inoculation dosage,serum amount and pH value had no effect on in-vitro antibacterial activity.【Conclusion】The combination of HV and HT in the proportion of 1∶4 has a better antibacterial activity.
5.Diagnostic value of hTERC gene expression in cervical intraepithelial neoplasia in gynecologic cytology

Objective To investigate the expression of human telomerase RNA component (hTERC)gene in cervical scraping samples, and the relationship between hTERC gene and cervical lesions. Methods The fluorescent in situ hybridization (FISH) was used to detect the expression of hTERC oncogene in cervical scraping samples in 100 cases, including 76 cases with cervical intraepithelial neoplasia, meanwhile, high risk human papillomavirus (HR-HPV) were detected by Hybrid Capture 2 (HC2) . The other 24 cases were negative for intraepithelial lesion (NILM). Results The positive expression rates of the hTERC gene in NILM,ASCUS, ASC-H, LSIL, HSIL and cervical cancer were 0, 32.25 %, 75.00 %, 35.71 %, 81.81 % and 100.00 % respectively. hTERC gene positive expression in normal group was less significantly than that in abnormal groups (P <0.05). There was a significantly difference between HSIL and ASCUS, LSIL (x2 = 6.1736, x2 =5.0004, P <0.05). The positive expression rates of the HR-HPV in ASCUS, ASC-H, LSIL and HSIL or higher lesions were 54.80 %, 75.00 %, 60.70 %, 90.90 % and 100.00 % respectively. HR-HPV positive expression in HSIL or higher lesions were more significantly than that in ASCUS group (x2=4.1767, P<0.05). Conclusion The overexpression of hTERC gene may play an important role in cervical carcinogenesis, therefore it can be used as a better tumor marker for prospective in early cervical cancer progresses.

7.The prevention of epigallocatechin gallate on colon preneoplastic lesion

Objective To investigate wheter epigallocatechin gallate (EGCG) could prevent the information of abrant crypt loci (ACF) induced by isoquinoline (IQ)and its possible mechanisms. Methods Sixty male BALB/cA nude immunological deficit mice were divided into five groups. Except control group, the other four groups were received IQ to induce ACF. The rats in low, medium and high dose groups were received 5, 10 and 20 mg/kg of EGCG,respectively. The mice were sacrificed six weeks later. Hematoxylin-eosin (HE) staining and 0.2% methylene blue staining were used to observe the routine histology and ACF, respectively. Immunohistochemistry (IHC) was used to detect Nrf2 protein level and RT-PCR was used to detect Nrf2 and UGT1A10 mRNA levels in colon tissue. Results The body weights of model group decreased significantly compared to high-dose group (21.70±0.13 vs. 24.37±0.07, P<0.01). Compared to model group, the degree of atypical hyperplasia and even canceration of colon mucus and the number of total ACF and total AC in high-dose group were decreased significantly (18.00±7.51 vs. 64.20±45.18, P<0.05;63.90±18.58 vs. 168. 80±35.34, P<0.01). The protein level of Nrf2 increased (0.3114±0.0037 vs. 0.1660±0.0021, P<0.01). The mRNA levels of Nrf2 and UGT1A10 in high-dose group was increased (both P value<0.01). Conclusions EGCG has protective effect on IQ induced preneoplastic lesions through reducing the number of ACF. This effect may be caused partly through the signal pathway Nrf2-UGT1A10.
8.Construction of Formin-like 2 specific small hairpin RNA expressing plasmid and its effect on proliferation in SW620

Objective To construct the specific small hairpin RNA(shRNA)plasmids for Formin-like 2(FMNL2)gene,and to observe its effect on proliferation of SW620.Methods Plasmids FMNL21 shRNA and plasmids FMNL22 shRNA complimentary to the sequence responsible for the function domain of human FMNL2 were constructed and transfected into SW620,respectively.The inhibition rates of FMNL2 expression were detected by re-al-time PCR after transfection with PGenesil-FMNL21 and PGenesil-FMNL22,respectively.The expression of FMNL2 was detected by real-time PCR at 12,24,48,72 and 96h after transfection.At 48 h and 72 h after transfection,the proliferation of SW620 was detected using MTT assay.Resuits The transfection rate in SW620 was(60.8±2.8)%for PGenesil-FMNL21,(58.7±2.9)%for PGenesil-FMNL22,and(62.6±1.7)%for PGenesil-HK(P>0.05).The inhibition rate of FMNL2 expression after transfection with PGenesil-FMNL21 was 77.1%,which was the highest.The inhibition rates of FMNL2 expression were 13.5%,57.3%,80.3%,62.4%,and 33.2%at 12,24,48,72and 96h after transfection respectively.The proliferat capability of SW620 was obviously lower than that of control cells(P<0.05).Conclusion Both plasmids FMNL21 shRNA and plasmids FMNL22 shRNA can inhibit the expression of FMNL2,of which Pgenesil-FMNL21 execs the greatest role.FMNL2 could be related to the proliferation of SW620.
9.The expression and clinical significance of formlin-like 2 in colorectal carcinoma

Objective To analyze the relationship between the expression of formlin-like 2(FMNL2)and colorectal carcinoma metastasis through the detection of the expression of FMNL2 in colorectal carcinoma tissue and paracancerous tissue.Methods ①Immunohistochemistry was used to detect the expression of FMNL2 in 175 cases of paraffin wax specimens(including 30 cases of normal mucosa,25 cases of adenoma,75 cases of colorectal carcinoma and 45 cases of metastatic tumors).②Real time-PCR was used to detect FMNL2 mRNA expression in 32 cases of fresh specimens of carcinoma and 32 cases of paracancerous tissue.Results The positive detection rate of FMNL2 was higher in colorectal carcinoma tissue than in paracancerous tissue(P=0.003),higher in lymphatic metastasis than in primary carcinoma tissue(P=0.037),and was aslo higher in metastatic carcinoma tissue than in non metastatic carcinoma tissue(P=0.011).The expression of FMNL2 mRNA was not related to the infiltration and differentiation of carcinoma tissue but its concentration in metastatic carcinoma tissue was 2.5 times higher than in non metastatic carcinoma tissue.Conclusion FMNL2 gene may play an important role in the metastasis of colorectal carcinoma.
10.Interventional effect of fluoxetine on the quality of life in depressive patients with breast cancer

BACKGROUND: There are many studies on the quality of life (QOL) of patients with breast cancer at home and abroad at present. But the effect of antidepressants on the QOL of depressive patients with breast cancer is not clear yet.OBJECTIVE: To investigate the effect of fluoxetine on the QOL of depressive patients with breast cancer.DESIGN: A self-controlled and observational comparative study of taking the patient as the subjects.SETTING: The research institute of medical psychology and psychiatric specialty in two universities and a psychiatric department in a city hospital.PARTICIPANTS: Sixty-three female post-operative patients with breast cancer hospitalized in the First Affiliated Hospital and Second Affiliated Hospital of China Medical University and Dalian Tumor Hospital, with an average age of (50 ± 9) years.INTERVENTIONS: Sity-three depressive patients with breast cancer were treated with fluoxetine for 8 weeks. Treatment emergent symptom scale (TESS) and relative laboratory examinations were used to evaluate the safety of fluoxetine to depression and anxiety of depressive patients with breast cancer. QOL of WHO scale was used to assess the QOL of the patients before and after administration.MAIN OUTCOME MEASURS:①Score of every index of QOL of WHO;②Side effects and adverse effects.RESULTS: The scores of physiology, psychology and independence in QOLafter 8 weeks' administration(16.30 ±2.17, 14.41 ± 1.85, 16. 90 ± 1.53)were significantly higher than those before grouping (13.61 ±2.46,12.98 ± 2.65, 13.65 ± 2.45) ( t = 6.52, P < 0.01; t = 2.41, P< 0.05; t= 8.93, P < 0.01 ). No obvious side effect was observed during administration.CONCLUSION: Fluoxetine can significantly improve the QOL of patients with breast cancer and there are fewer side effects during the treatment.
