1.Antibiotic Resistance of Staphylococcus Isolated from Burn Patient Wounds

OBJECTIVE To study the antibiotic resistance of Staphylococcus isolated from the burn patient wounds. METHODS The sensitivity to 14 common antimicrobial agents was deteded by K-B method. MRSA and MRCNS were detected by cefoxitin disk;the inducible resistance to clindamycin was detected by D-test. RESULTS There were 378 strains of Staphylococcus isolated from the burn patient wounds. The isolation rates of Staphylococcus epidermidis,S. aureus and S. haemolyticus were 43.1%,41.6% and 11.1%. Antibiotic resistance rate to vancomycin,teicoplanin,minocycline,rifapin and nitrofurantoin was 0.0-19.0%,the resistance rate to other antibiotics was 54.1-100.0%. The detection rate of MRSA was 81.5%,the detection rate of MRCNS among S. epidermidis and S. haemolyticus was 55.6% and 66.7%. The total positive rate of D-test was 48.2% in Staphylococcus which were sensitive to clindamycin but resistant to erythromycin. The positive rate of S. aureus,S. epidermidis and S. haemolyticus were 49.1%,44.2% and 57.1%. CONCLUSIONS Staphylococcus are ones of the important pathogens which make burn wound infection happen and the antimicrobial resistance is increasingly severe. The high positive results of MRSA and D-test make great trouble to therapy,and that should have taken more attention of clinic.
2.Uitrasonographic diagnosis of horseshoe kidney of foetus

Objective To explore the horseshoe kidney of prenatal ultrasonography for detecting birth defects.Methods A total of 3260 women during 22~42 gestational weeks underwent real-time color Doppler ultrasongraphy to find out fetal mati kidney.All the cases were tracked within two month after delivery.Results In all 3260 pregnant women,104 cases were diagnosed on ultrasonography of 100%accordance with clinical diagnosis.Conclusions A systemic prenatal ultrasonographic examination in the second-trimester pregnancy is important for prenatal diagnosis of congenital malformations,which play a very significant role in decreasing fetal birth defects and improving new born population quality.
3.PDCA Model for Management of Infectious Disease Prevention and Control in Hospital

OBJECTIVE To explore a management method for the prevention and control of infectious diseases in hospital. METHODS We applied the PDCA (plan,do,check and action) model to establish a management strategy for the prevention and control of infectious diseases for three years. The components of the management model included providing educational program for medical staff,designing and realizing direct network system for infectious disease reporting,monitoring diagnosed infectious diseases ,analyzing their epidemic tendency,guiding medical staff to prevent and control infectious diseases,and strengthening the surveillance,feedback,rewards and punishment. RESULTS The rate of delayed or missed report of infectious diseases was decreased from 5% to 1.1% by monthly check and from 6.8% to 0% by seasonally cross check. Although 46 cases of AIDS,5 cases of caesarean birth with HIV possitive and 1445 cases of hand-foot-mouth disease were admitted in our hospital,No medical staff and other patients was crossly infected. CONCLUSIONS Our way of management based on PDCA model can strengthen the prevention and control of infectious diseases and assure medical quality and patient safety.
4.Effect of Continuous Blood Purification on Inflammatory Factors and Immune Function in Patients with Sepsis

Objective:To investigate the effect of continuous blood purification (CBP) on inflammatory factors and immune function in patients with sepsis,in order to provide reference for clinical treatment of sepsis.Methods:A total of 76 patients with severe sepsis,who were treated in ICU of Daqing Oilfield General Hospital from June 2014 to December 2016,were selected.The patients were divided into conventional group (n=36) and CBP group (n=40) according to the received treatment method.The patients in the conventional group were treated with targeted therapy and life support,while the patients in the CBP group were treated with CBP on the basis of targeted therapy.The levels of interleukin-6 (IL-6),interleukin-10 (IL-10),tumor necrosis factor-α (TNF-α) and C reactive protein (CRP) of the patients in the two groups were detected and compared before treatment and after 3d of treatment.The CD3+,CD4+,CD8+T cell and the CD4+/CD8+ ratio before treatment and after 3 d of treatment were calculated and compared between the two groups.Results:There was no significant difference in the level of serum inflammatory factors between the two groups before treatment (P>0.05).The levels of serum inflammatory factors in the two groups were significantly decreased after 3 d of treatment,and the levels of IL-6,TNF-α and CRP in the CBP group were significantly lower than those in the conventional group (P<0.05).There was no significant difference in immune function indexes between the conventional group and the CBP group before treatment (P>0.05).After 3d of treatment,CD3+,CD4+ and CD4+/CD8+ ratio were significantly increased in the two groups,and the CD4+ and CD4+/CD8+ ratio of the CBP group were sig-nificantly higher than those of the conventional group (P<0.05).Conclusion:The application of CBP in the treatment of patients with sep-sis can obviously improve the expression of inflammatory factors of patients,and can improve the immune function and promote the re-covery of immune function.
5.Effects of chloride channel blockers on H_2O_2 induced apoptosis in pancreas RIN-m beta cells

Aim To investigate the effects of chloride channel blockers on the apoptosis of RIN-m? cells of pancreatic islet induced by H2O2.Methods The apoptotic model was made by H2O2 exposed for six hours with a concentration of 500 ?mol?L-1.The chloride channel blockers:DIDS,NPPB and NFA were administered to pretreat the samples respectively.The cell viability,morphological changes,and apoptosis rate were observed.Results Chloride channel blockers alone have no marked effects on the cell viability of RIN-m? cell.However,they elevated the cell viability of RIN-m?cell disposed of by H2O2.Compared to H2O2 group,the groups of DIDS +H2O2,NPPB+ H2O2 and NFA+H2O2 have significant difference in cell viability and apoptosis rate(P

AIM:To compare the effects of remifentanil and fentanyl for posterior scoliosis correction and the influences on postoperative pain.METHODS:Forty patients scheduled for posterior scoliosis correction(ASAⅠ-Ⅱ,aged 11-18 years)were randomly divided into group remifentanil(group R)and group fentanyl(group F).Anesthesia was maintained with propofol,vecuronium bromide and sevoflurane.Intravenous remifentanil(0.2 ?g?kg-1?min-1)and fentanyl(1 ?g?kg-1?h-1)were continuously administered for analgesia.Vecuronium bromide and analgesics were discontinued around 30 min before the wake-up test.The wake up time from propofol and sevoflurane discontinuation to movement of the patient's toe were recorded.In group R,once remifentanil infusion was stopped before surgery ended,the patients were injected with fentanyl 2 ?g/kg.All of the patients received intravenous fentanyl(10 ?g?kg-1?min-1)for postoperative analgesia and were interviewed for pain degree during the first 24 h after surgery.RESULTS:The wake-up time were(12.3?5.7)min in group R and(21.6?6.5)min in group F(P
7.The value of esomeprazole test in diagnosing gastroesophageal reflux disease:A randomized multi-center controlled trial

Objective To evaluate the diagnostic value of esomeprazole test in patients with gastroesophageal reflux disease.Methods Consecutive patients with heartburn and/or acid regurgitation in three medical centers were enrolled in a prospective,randomized,double-blind study with a placebo-control design.After upper endoscopy examination,patients were divided into non-erosive reflux diseases and erosive esophagitis.All patients underwent 24-hour ambulatory esophageal pH monitoring.Those who were diagnosed as gastroesophageal reflux diseases had either esophagitis under endoscopy or abnormal esophageal acid exposure in 24-hour ambulatory esophageal pH monitoring.The patients were randomly divided into treatment group and control group.The patients in treatment group were received 40 mg of esomeprazole daily while the patients in control group were given placebo once daily for 14 days.The severity and frequency of heartburn were recorded both by doctors and patients before and during treatment.Results Two hundred and seventeen patients were completed the study(105 patients in treatment group and 112 patients in control group).If the symptom of heartburn which disappeared in the sixth and seventh day of the first week,it was defined as positive for esomeprazole test,the sensitivity and specificity in treatment group were 87.7% and 42.5%,respectively If the symptom of heartburn disap peared in the second week,it was defined as positive for esomeprazole test.The corresponding sensitivity and specificity in treatment group were 84.6% and 45%,respectively.The Youden index was 0.362 and 0.296 for criteria of one and two weeks,respectively.If the heartburn score improved by more than 50%,75% or 100% from the baseline score after treatment,it was defined as positive for esomeprazole test,the sensitivity and specificity were 95.4% and 32.5%,87.7% and 32.5%,84.6% and 45%,respectively.Conclusions The esomeprazole test has high value in diagnosis of GERD with sensitivity of 87.7% and specificity of 42.5%.Positive definition of heartburn disappeared in the sixth and seventh day is superior in cost-effective.

Cross-talk of intracellular signaling pathways that share common components (hubs) is organized in form of a bow-tie network topology.Signaling cross-talk is functionally pleiotropic for target genes regulation, resulting in functional redundancy, synergism and antagonism, which should be precisely controlled to prevent signaling 'leaking' or 'spillover'.Thus, the biological system has evolved multiple insulating mechanisms to achieve stimulus-specific response that maintains intracellular homeostasis.The insulation mechanism of signaling cross-talk suggests: (1) the functional duality of cross-talk molecules that determine cell fate requires selectively targeting dysregulated cross-talk molecules while protecting the normal ones from off-target or unintended effects, and we propose them as the targetable cross-talk molecules;(2) cross-talk molecules are usually carried on the macromolecular complex as their functional platforms, thus the structural plasticity of conformational changes at the interaction surface of cross-talk molecules asks for intensive work on the relationship study between drug binding and biological activity, which we propose as the accessible cross-talk molecules.Therefore, signaling cross-talk and its insulation mechanism play instructive leading roles in resolving the bottlenecks of current drug R&D and improve the clinical outcome.
9.Reno protective effect of alprostadil on renal injury caused by repeated use of the contrast media

Objective To study the effect on renal function about repeated use of contrast media , and whether alprostadil has protective effect towards contrast-induced nephropathy ( CIN) .Methods 80 adult patients who had ever received contrast examination and scheduled to have PCI within 1 month were randomly divided into two groups: the simple hydration group and the hydration plus alprostadil therapy group.The serum level of creati-nine,urea, Cystatin C, Urineβ-microglobulin and creatinine clearance were recorded and compared between the two groups , and were observed before and after repeated exposure of contrast medium.The incidence of CIN was analyzed .Results Compared with pre-contrast levels , serum levels of urea, creatinin, Cystatin C and Urine β-microglobulin all elevated after single and repeated contrast media use in patients in the simple hydration group ( P<0.05 ) .The incidence of CIN did not differ after single or repeated contrast used (2.5%vs.15.0%, P>0.05).After repeated contrast exposure compared with patients with simple hydration , patients in the alprostadil group had repeated serum levels of urea [(7.4 ±2.3) mmol/L vs.(9.1 ±2.6) mmol/L], creatinia [(87.2 ±25.6) μmol/L vs.(96.9 ± 25.8) μmol/L], Cystatin C [(0.8 ±0.3) mg/L vs.(1.4 ±0.3) mg/L] and Urine β-microglobulin [(207.0 ±31.9 ) μg/L vs.(279.3 ±37.3 ) μg/L] were all lower with higher creatinin clearance [(92.2 ±24.2) ml/min vs.(78.2 ±27.5) ml/min](all P<0.05).The incidence of CIN in patients with alprostadil did not differ after single or repeated contrast used (2.5%vs.7.5%, P>0.05).The incidence of CIN in patients treated with alprostadil had no difference compared with patients with simple hydration after repeated contract (7.5% vs.15.0%, χ2 =0.501,P=0.479).Conclusions Contrast media can cause damage to renal function .Short-term repeated use of contrast media can further worsen renal function without significant increase in CIN rates .Alprostadil may have renoprotective effect towards CIN .
10.Effects of Electroacupuncture on Expression of COX-2 in the Dorsal Horn of Spinal Cord of Arthritic Rats

Objective: The present study was to observe the expression of COX-2 protein in the dorsal horn of the spinal cord in rats with chronic inflammatory pain to investigate whether COX-2 is involved in the mechanism of electroacupuncture (EA) analgesia. Methods: After EA treatment, the pain score and expression of COX-2 protein in the dorsal horn of spinal cord in rats with complete Freund's adjuvant (CFA)-induced arthritis were measured by the behavioral test and immunohistochemistry technique, and compared with those of the normal group, model group and medicine group. Results: EA could decrease the pain score significantly and repeated EA treatments had the remarkable cumulative analgesic effect; and with the increase in EA treatments, the number of COX-2 immunoreactive cells was obviously decreased. Conclusion:EA at acupoints Yanglingquan (GB 34) and Kunlun (BL 60) had remarkable analgesic effect on adjuvant arthritis rats; and in the meantime, could inhibit the expression of COX-2 protein in the dorsal horn of spinal cord.
