Objective To observe the expressions of serum insulin-like growth factor (IGF)- Ⅰ ,Ⅱ and IGF binding protein (IGFBP) 3, 5 and to explore the clinical significances in patients with clear cell carcinoma of kidney. Methods Enzyme-linked immunosorbent assay (ELISA) methods were adopted to examine serum expressions of IGF-Ⅰ , Ⅱ and IGFBP 3, 5 in 40 cases with clear cell carcinoma of kidney (renal carcinoma group) and 16 cases with hydronephrosis (control group) from May 2007 to December 2009. Results IGF- Ⅰ , Ⅱ and IGFBP 3,5 in renal carcinoma showed higher expressions before operation (985. 7 μg/L, 1154.0 μg/L,46.6 μg/L and 9.6 μg/L, respectively)than after operation (431.4 μg/L, 632.6 μg/L, 26.7 μg/L, and 6.7 μg/L, respectively, all P＜0. 05 ～0.01). There were no significant differences in those indexes between pre- and post- operation in control group (P＞ 0. 05). Conclusions There are high expressions of serum IGF-Ⅰ , Ⅱ and IGFBP 3, 5 in renal carcinoma patients, and IGF- Ⅱ has clinical significance in diagnosis.

Colorectal cancer (CRC) is one of the most common malignant tumors.Accumulating evidences suggests that gut microbiota play an important role in CRC initiation and progression.Chinese herbal medicine has certain regulation effects on disorders of gut microbiota.In current review,we discuss the changes of gut microbiota in CRC patients,and summarize the effects of gut microbiota on CRC progression and their underlying mechanisms through barrier function,inflammation,bacterial enzymes,toxic metabolites,as well as the effects of Chinese herbal medicine on gut microbiota.This review provides a basic scientific and clinical application of gut microbiota in CRC.

Objective? To explore the establishment and effects of preventive nursing sensitivity indicators for venous thromboembolism (VTE) in orthopedic patients. Methods? Preventive nursing sensitivity indicators for VTE in orthopedic patients were established using Delphi expert consultation method. Totally 160 patients before the preventive nursing sensitivity indicators for VTE were carried out (May 2017) were selected as the control group, while another 201 patients after the indicators were carried out (November 2017) were selected as the study group. Patients in the control group received conventional preventive nursing care for VTE, whereas patients in the study group received monitoring and management using the self-developed preventive nursing sensitivity indicators for VTE. Patients' incidence rate of VTE, satisfaction with VTE prevention, accuracy of VTE risk assessment, awareness rate of health education knowledge, implementation rate of physical prevention measures, achievement rate of nurses' VTE-associated knowledge and skills before and after the indicators were carried out. Results? Patients' incidence rate of VTE was 25‰ before the indicators were carried out, while that after the indicators were carried out was 5‰ (P=0.175); patients' satisfaction before the indicators were carried out was 78.12%, while that after the indicators were carried out was 93.53% (χ2=18.344, P＜0.001); and there were statistically significant differences in accuracy of VTE risk assessment, awareness rate of health education knowledge, implementation rate of physical prevention measures and achievement rate of nurses' VTE-associated knowledge and skills before and after the indicators were carried out (P＜ 0.01). Conclusions? The establishment and application of preventive nursing sensitivity indicators for VTE can effectively improve the nursing quality of VTE prevention as well as patient's satisfaction.

Objective:To explore the clinical characteristics of patients with cholelithiasis in Jinshan district of Shanghai city. Methods:The clinical characteristics of 3455 patients with cholelithiasis treated in Jinshan Branch of Shanghai Sixth People′s Hospital from Jul 2010 to Dec 2012 were analyzed.Results:Among them,3127 patients (90.5%)had clinical symptoms,and 328 patients (9.5%)had no symptoms.The incidence rate of symptomatic cholelithiasis in male patients was lower than that in female patients,while the incidence rate of asymptomatic cholelithiasis in male patients was higher than that in female patients (P <0.05).The incidence rate of asymptomatic cholelithiasis in patients decreased with the increase of age,and the difference was statistically significant(P <0.05).The incidence of cholecystolithiasis was 54.5%.The appearance of symptoms in patients with cholelithiasis was not correlated with the body mass index(BMI)and the location of cholelithiasis(P >0.05).Conclusions:Jinshan district of Shanghai city has high incidence of cholelithiasis.The appearance of Symptoms in patients with cholelithiasis is correlated with gender and age,but not correlated with BMI and location of cholelithiasis.

Objective To understand the quality status of salt iodine monitoring results at the county level and checking results at the city level from 2017 to 2021 in Suzhou，and to provide a scientific basis for the development of iodine deficiency disease（IDD）prevention and control strategies and measures.  Methods During the past five years, Suzhou CDC randomly selected 15 samples of 300 monitored salt samples from each county/district CDC each year for spot checks. The data were processed by comparative analysis of relative error and grouped data t test.  Results In the conformity of iodized salt and non-iodized salt, the sample monitoring results of some counties/districts were non-iodized salt, while the city-level test results were iodized salt. In the conformity of iodized salt and seaweed iodized salt (or enhanced salt), the sample monitoring results of county/district level were iodized salt, while the test results at the city level were seaweed iodized salt (or enhanced salt). The non-conformity was relatively concentrated in some county/district laboratories, and the total amount of non-conformity tended to increase year by year. The total number of samples with relative error of >20% between the county/district and city-level results was the least in 2021, with 6, and the most in 2018, with 25. The number of samples with relative error of >30% between the county/district and city-level results was the least in 2017 being 0, while there were 12 samples in 2018, fluctuating within a small range in the past three years. The t-test results of grouped data showed that the average value of the checking results at the city level was generally higher than the monitoring results at the county level. There were 21 pairs of data with no statistical significance (P>0.05) and 29 pairs of data with statistical significance (P<0.05) between the results at the county/district level and at the city level. Among them, there were 7 counties/districts whose sample monitoring results were not significantly different from the city-level test results in 2020, while there were 9 counties/districts whose monitoring results were statistically different from the city-level test results in 2021.  Conclusion  The data analysis results show that the monitoring data of iodized salt in Suzhou is basically reliable and accurate, but there still exist some problems. Therefore, in the future work it is necessary to strengthen the monitoring system of IDD, increase training efforts, continuously monitor and check the quality of iodized salt to provide a scientific basis for effective prevention and control of IDD.

Objective: To integrate the best evidence of venous thromboembolism (VTE) prevention into practice, improve nurses′ compliance, and reduce the incidence of VTE in orthopaedic department. Methods: Guided by the standard procedure in the JBI-Paces program, collected the best evidence from online databases and set the standards. Collected data through field observation, interviews and review of nursing records. A total of 167 patients and 36 nurses we recruited in the study and audited twice. We judged the result by the knowledge level of nurses and patients, nurses′ compliance with the evidence and the incidence rate of VTE. Results: In baseline review, three criteria of poor compliance, "the correct situation of daily inspection of VTE risk assessment", "Wear gradient pressure stockings correctly for VTE high-risk patients", "Intermittent pneumatic compression device (IPCD) used twice a day for 30 minutes each time", were significantly improved after the application of evidence, increased from 47.22% (17/36) ,36.11% (13/36) ,36.11% (13/36) to 86.11% (31/36) , 86.11% (31/36) , 91.67 (33/36) respectively (χ²=12.25, 18.94, 24.08, all P < 0.05); The incidence of VTE decreased from 2.7% to 0 (P=0.195); the correct rate of VTE risk assessment, awareness rate of health education among patients, implementation rate of physical preventive measures and qualified rate of VTE-related knowledge and skills among nurses increased from 82.43% (61/74) ,74.32% (55/74) ,83.78% (62/74) ,77.03% (57/74) to 97.85% (91/93) ,91.40% (85/93) ,97.85% (91/93) ,96.77% (90/93) respectively (χ²= 8.86-15.24, all P < 0.01) . Conclusion The integration of best evidence standardize nurses′ behaviors, reduce the incidence rate of VTE, improve the quality of clinical care and promote the satisfaction of patients continuously.
