1.Effect of CYP2A6 genetic polymorphisms on serum concentration of sodium valproate

Objective To investigate the effect of cytochrome P_ 450 2A6 (CYP2A6) genetic polymorphisms on serum concentration of sodium valproate. Methods A total of 98 epileptic patients receiving sodium valproate after a period of more than 5 half-time were recruited. The genotypes of CYP2A6 of the patients were detected by nested-primer polymerase chain reaction (PCR) to examine the alleles CYP2A6*1 and CYP2A6*4. Fluorescence polarization immunoassay (FPIA) was used to measure the serum concentration of sodium valproate. Results Of the 98 cases, 73 (74.5%) were wild genotypes, 24 (24.5%) were CYP2A6*1/*4 genotypes and 1(1.0%) was CYP2A6*4/*4 genotype. According to the genotypes of CYP2A6 the patients were divided into two groups,one was group A (CYP2A6*1/*1) and the other was group B (CYP2A6*1/*4 or *4/*4). 3.Expression and role of PN-1, thrombin and PAR-1 in rats brain edema tissue after intracerebral hemorrhage

Objective To investigate expression and role of protease catenin-1(PN-1), thrombin(thrombin), protease activated receptor-1(PAR-1) in rats brain edema tissue after intracerebral hemorrhage(ICH).Methods Adult male SD rats 80 were randomly divided into sham group,ICH group after number, 40 in each group, ICH group autologous arterial method of making a rat model of experimental ICH in the right caudate nucleus unit injection.The degree of neurological dysfunction between 2 groups was evaluated at 12 h,24 h and 120 h after post-operation.Observed the morphology of brain cells by HE staining.Changes of PN-1,thrombin, PAR-1 index in rat brain tissue at 3,6,10,12,24,48 and 120 h were detected by Western blot.Results Neurological dysfunction score ICH rats after modeling 12,48,120 h were significantly lower than the sham group(P<0.05);ICH in rats after modeling 3,6,10,12,24,48 and 120 h of brain tissue PN-1, thrombin, PAR-1 compared with sham-operated group were significantly increased (P<0.05); ICH rat brain tissue PN-1, thrombin, PAR-1 appeared in 12 h after modeling shwed a gradual downward trend (P<0.05). Conclusion ICH hematoma surrounding brain tissue in rats PN-1 has the effect of inhibiting thrombin and PAR-1 overexpression, cause nerve damage.
4.Research the Expression of MDR1 Genes in Vaginal Candida Albicans Resistance to Azole Agents

Objective To investigate the expression levels of MDR1 gene from azole resistant strains in vaginal Candidaalbi-cans.Methods 60 strains of Candida albicans from recurrent vagintis patients were collected and identified by CHROM agar from Aug.2011 to Aug.2012.The drug resistance was detected by disk diffusion method.According to the result of drug sensitivity test,the strains were diveded into four groups:sensitive group moderately susceptible or resistant to flucon-azole group moderately susceptible or resistant to detoconazole group and moderately susceptible or resistant to miconazole group.Each group was selected 12 strains randomly.The expression of the MDR1 gene in these 48 strains were detected by real-time RT-PCR and analysed the datas by t test statistics.Results There were 22 sensitive strains,26 strains of moder-ately susceptible or resistant to detoconazole,12 strains of moderately susceptible or resistant to fluconazole and 38 strains of moderately susceptible or resistant to miconazole in 60 strains of Candidaalbicans.The relative expression of MDR1 gene in sensitive group was 0.41±0.47,in moderately susceptible or resistant to detoconazole group fluconazole group and micon-azole group were 3.32±4.46,2.27±3.05 and 0.9±0.81 respectively.Compared with the sensitive group,t values of mod-erately susceptible or resistant to detoconazole group,fluconazole group and miconazole group were-2.177,-2.130 and-2.094.The expression level of MDR1 gene had no statistical significance between the sensitive strains and moderately sus-ceptible or resistant strains(P>0.05).Conclution The relationship between MDR1 expression and the resistance to azole agents in vaginal Candida albicans requires further study.
5.Relationship between PCSK9 gene polymorphorism rs11206510 with blood lipid level and coronary heart disease

Objective To investigate the relationship between PCSK9 gene polymorphism with lipid metabolism and CHD risk among population within Tianjin, China. Methods Patients from Tianjin Chest Hospital were selected (n=778) and divided into coronary heart disease (CHD) group (n=502) and control group (n=276). Immediately after admission, tri?glyceride (TG), high density lipoprotein (HDL-C), low density lipoprotein (LDL-C), apolipoprotein A (ApoA 1) and apolipo?protein B (ApoB) were tested and genotype TT,TC and CC were determined. Results In CHD group, LDL-C level is higher in TC genotype individuals than that in TT genotype individuals with statistical significance (P<0.05). CHD group and the control group were dominated by TT genotype with no statistical difference between their allele frequency distribution nor genotype frequency distribution(P>0.05). The total population allele frequencies were T 96.1%(1 495/1 556), C 3.9%(61/1 556);genotype frequencies were TT 92.2%(717/778), TC 7.8%(61/778). Multivariate Logistic regression analysis revealed that ApoA1 and HDL-C were inversely associated with coronary heart disease, therefore are protective factors for coronary heart disease. Conclusion The study found that TC genotype is correlated with serum LDL-C increase in group CHD, and the incidence of allele C showed no obvious connection with CHD.
6.Application of Integrative Medicines of TCM and WM Combined with Fast-track Surgery Concept in Perioperative Nursing in Colorectal Surgery

Objective To study the clinical effect and significance of integrative medicines of TCM and WM combined with fast-track surgery concept on perioperative nursing in colorectal surgery.Methods One hundred and twenty patients with surgery were randomly divided into the observation group and the control group,60 cases in each group.The control group was treated with conventional nursing,while the observation group was treated with fast track surgery nursing of integrative medicines of TCM and WM.Results Bowel sound recovery time,anus exhaust time,defecation time,length of hospital stay and postoperative complications of observation group was significantly less than the control group (P<0.05).Nursing total satisfaction of observation group was significantly higher than the control group (P<0.05).Conclusion Integrative medicines of TCM and WM combined with fast-track surgery concept on perioperative nursing in colorectal surgery can reduce stress,speed up the recovery,reduce complications.So it is a safe and effective way of perioperative nursing,and worthy of clinical popularization and application.
7.Pharmacokinetics of Berberine in Puerariae and Scutellariae and Coptidis Decoction in Vivo of Dog

OBJECTIVE:To assay serum berberine in Puerariae and Scutellariae and Coptidis Decoction and coptidis rhi?zoma single decoction in dog and to analyze the effect of the compound recipe(whole recipe)on the physiological disposition of beberine.METHODS:Serum concentrations of berberine in dog were determined at different time by HPLC and the phar?macokinetic parameters were computed with WinNonlin software.RESULTS:The concentration-time curves of berberine in Puerariae and Scutellariae and Coptidis Decoction and coptidis rhizoma single decoction in vivo of dog conformed to one-compartment model,the main pharmacokinetic parameters of berberine in Puerariae and Scutellariae and Coptidis Decoction vs.in coptidis rhizoma single decoction were as follows,AUC (0-∞) (1.25?0.06)vs.(14.71?0.54)(?g?h)/ml,t max (1.92?0.31)vs.(3.03?0.07)h,C max (0.17?0.01)vs.(1.79?0.03)?g/ml.CONCLUSIONS:As compared with single-item recipe,the plasma concentration and bioavailability of berberine in the compound recipe of Puerariae and Scutellariae and Coptidis Decoc?tion were all lower.
8.Changes in vimentin and vascular endothelial growth factor expression in a rat model of thin endometrium established by 95% ethanol

BACKGROUND: Ethanol chemical damage method can be used to establish thin endometrium rat model. Expression of proliferation-related proteins is decreased in thin endometrium animal model, which contributes to the study of thin endometrium-related mechanism and provides guidance for clinical treatment. OBJECTIVE:To explore the changes in vimentin and vascular endothelial growth factor expression in rat modelsof high concentrations of ethanol-induced thin endometrium. METHODS: 0.3 mL 95% ethanol was slowly injected in the right uterus of female Sprague-Dawley rats to establish rat models of thin endometrium. An equal volume of physiological saline was injected into the left uterus as control sides. RESULTS AND CONCLUSION:The thickness of endometrium in the injured uterus was significantly thinner than the control ones. Number of glands reduced, structure became disorder, and vimentin and vascular endothelial growth factor expression markedly diminished. These results suggest that 95% ethanol injury can effectively establish rat models of thin endometrium. Proliferation-related protein expression altered in the thin endometrium.
9.The influence of CYP4F2(rs2108622)gene polymorphism on the warfarin dose in old patients

Objective To observe the effect of CYP4F2(rs2108622)polymorphism on the dose of warfarin in old patients(65 to 75 years old)who were treated with atrial fibrillation.Methods Sixty cases of old patients with atrial fibrillation were enrolled in the study.All the subjects had taken warfarin for 3 months,and the international normalized ratio(INR)maintained between 1.6 and 2.5.And the CYP4F2(rs2108622)variant were detected by PCR.Results The patients with CYP4F2(rs2108622)allele C/C scored significantly lower warfarin dose than patients with variant allele C/T and T/T (P < 0.05 ).Conclusion CYP4F2 (rs2108622)gene polymorphism have been related with warfarin dose in old patients.
10.Diagnosing value of fasting blood glucose on MODSE caused by pulmonary infection in elderly patients with diabetes

Objective To evaluate the clinical value of fasting blood glucose (FBG) in the diagnosis of multiple organ dysfunction syndrome (MODS) caused by pulmonary infection in elderly patients with diabetes. Methods Patients over 65 years old with diabetic pulmonary infection admitted to Department of Cadres Ward of Bethune International Peace Hospital of PLA from July 2015 to December 2016 were enrolled. According to the patient's highest FBG level during pulmonary infection, all patients were divided in two groups. The cases whose FBG ≥8.83 mmol/L were served as suspicious multiple organ dysfunction syndrome in the elderly (MODSE) group, and those had FBG < 8.83 mmol/L were served as suspicious non-MODSE group. The incidence of MODSE in suspicious MODSE group and suspicious non-MODSE group was compared. The receiver operating characteristic curve (ROC) was drawn, and the clinical value of highest FBG in the diagnosis of MODSE caused by pulmonary infection in elderly patients with diabetes was assessed. Results 119 elderly patients with diabetes and pulmonary infection were enrolled in the analysis. All patients were male, with age of 76-105 years with an average of (89.41±4.16) years. Among 119 patients, 55 of them with suspected MODSE, 64 of them with suspected non-MODSE. The incidence of MODSE in suspicious MODSE group was significantly higher than that in suspicious non-MODSE group (78.2% vs. 12.5%), with statistically significant difference (χ2 = 52.108, P = 0.000). The area under the ROC curve of the highest FBG for diagnosis of MODSE was 0.895, and 95% confidence interval (95%CI) was 0.839-0.951 (P < 0.001). When using FBG ≥ 8.83 mmol/L to diagnose MODSE induced by pulmonary infection in elderly patients with diabetes, the sensitivity was 84.3% and the specificity was 82.4%. Conclusion FBG ≥ 8.83 mmol/L, as an indicator reflecting the changes of metabolic function, have a diagnostic value for MODSE caused by pulmonary infection in elderly patients with diabetes.
