OBJECTIVE:To discuss the economic efficiency of original equipment manufacture(OEM)on drugs.METH?ODS:The economic efficiency of OEM on drugs was evaluated on the basis of transaction cost theory by analyzing the realistic and economic significance of OEM on pharmaceuticals and combining the current situation of supervision.RESULTS&CONCLUSION:The OEM on drugs should be encouraged at the proper time,by which the problem of repeated construction and low percentage of utilization of asset will be solved,and the rationalization of pharmaceutical industrial organization should be realized.

Objective To study the relationship of the plasma levels of high sensitive C-reactive protein(hs-CRP),adrenomedullin(ADM),brain natriuretic peptide(BNP)and matrix metalloproteinases (MMPs)with severity of heart falure, the left ventricular ejection fraction and the relationship among them in the elderly patients with congestive heart failure(CHF). Methods The blood samples were obtained from 50 patients with congestive heart failure and 24 healthy controls.The plasma level of hs-CRP was measured by immunoturbidometry.The plasma levels of ADM and BNP were determined by radioimmunoassay.The plasma levels of MMP-2 and MMP-9 were measured with ELISA.Left ventricular ejection fraction(LVEF)was measured by echocardiography. Objective:To investigate the effect and the mechanism of electroacupuncture(EA)on corpus striatum white matter injury in rats with focal cerebral ischemia(FCI).Methods:Forty-four specific-pathogen-free Sprague-Dawley rats were divided into a normal group(n=10),a sham-operation group(sham group,n=10),and a modeling group(n=24)using the random number table method.The normal group was a blank control.In the sham group,only the vessels and vagus nerve were isolated without embolization.The FCI rat model in the modeling group was replicated using the middle cerebral artery occlusion embolization method.The 20 successfully modeled rats were randomly divided into a model group and an EA group,with 10 rats in each group.Rats in the model group did not receive further treatment.Rats in the EA group received EA stimulation at Baihui(GV20)and the left Zusanli(ST36)24 h after the successful modeling,30 min each time,once a day for 14 d.On the 14th day of the experiment,rats in each group were scored for neurological deficits and then sacrificed,and brain tissues containing corpus striatum around the ischemic focus were paraffin-embedded from 5 rats in each group.Luxol fast blue(LFB)staining was used to detect damage changes in the white matter.The positive immunoreactive expression of myelin basic protein(MBP),myelin-associated growth inhibitor A(Nogo-A)and its receptor(NgR)in rat corpus striatum tissue was detected by immunohistochemistry staining,and then the protein expression of MBP,Nogo-A,and NgR in the corpus striatum tissue around the ischemic focus was determined by Western blotting.Results:Compared with the normal group and the sham group,the model group had a significantly higher neurological deficit score(P<0.05)and fiber bundle injuries in the corpus striatum white matter,evidenced by a significantly lower mean optical density value of corpus striatum LFB staining(P<0.05),a significantly lower MBP expression level(P<0.05),and significantly higher Nogo-A and NgR protein expression levels(P<0.05).Compared with the model group,the neurological deficit score was significantly lower(P<0.05),the mean optical density value of LFB staining was significantly higher(P<0.05),the MBP expression level was increased(P<0.05),and the expression levels of Nogo-A and NgR proteins were decreased(P<0.05)in the EA group.Conclusion:EA reduces the ischemia-induced corpus striatum white matter injury and improves neurological deficits.The mechanism may be related to the inhibition of Nogo-A/NgR activation.
8.Effect of Small Interfering RNA Silencing the Fatty Acid Synthase Gene on Lipid Metabolism in Human Hepatic Cell Line HepG2

Objective: To investigate the effect of the gene interfering technology on fatty acid synthase (FAS) gene silencing for lipid contents in human hepatic cell line HepG2 and to study the lipid metabolism related gene expression in HepG2 cells. Methods: A total of 3 pairs of small interfering RNA (siRNA) targeting different sequences of FAS mRNA were synthesized as FAS-siRNA-1, FAS-siRNA-2 and FAS-siRNA-3, meanwhile, 2 controls were established as Blank control group, in which HepG2 cells were not treated, and Negative control group, in which HepG2 cells were transfected by non-effective siRNA. The mRNA, and protein expression levels of FAS in HepG2 cells were examined by real-time lfuorescence quantitative RCR and Western blot analysis to screen the most effective pair of siRNA for FAS gene silencing; and that speciifc siRNA was transtected to HepG2 cells for 48 hours to detect the intra-/extra-cellular TG, TC levels and the mRNA expression related to lipid metabolism in HepG2 cells. Results: The screening experiment indicated that FAS-siRNA-3 was most effective for FAS gene silencing. Compared with Blank control group, the mRNA and protein expressions in FAS-siRNA-3 transfected HepG2 cells (Transfected group)decreased to (52.33 ± 3.07) % and (51.57 ± 3.14) % respectively. Compared with Blank control group, Transfected group had the reduced intra-/extra-cellular TG levels and reduced extracellular TC level; while increased mRNA expression of hepatic lipase,P<0.0001 and decreased mRNA expression of TG transfer protein in HepG2 microsome,P<0.05. Conclusion: FAS gene silencing could signiifcantly decrease the intra-/extra- cellular TG level and extracellular TC level in HepG2 cells, those ifndings need to be conifrmed by furtherin vivo andin vitro studies.
9.Effect of boiled and fried dumplings on postprandial blood glucose in diabetes patients assessed by continuous glucose monitoring

Objective To compare two different dumping cooking methods (boiling vs.frying) in their effect on postprandial glucose level in diabetic patients using continuous glucose monitoring (CGM).Methods 10 type 2 diabetes mellitus (T2DM) in-patients in the Department of Endocrinology of Peking Union Medical College Hospital between February and May 2011 were enrolled,whose fasting and preprandial glucose levels were controlled with a insulin pump.On day 2 and day 4 in the study period,the patients were given fried dumplings and boiled dumplings for lunch respectively,with the same nutrient contents.The starch digestibility of these two kinds of dumplings were compared using in vitro resistant starch digestion,measuring the concentrations of rapidly digestible starch,slowly digestible starch,and resistant starch.CGM was used to record blood glucose changes,in order to evaluate glycemic effect of these two dumpling cooking methods on postprandial glucose levels at 9 time points (0,15,30,60,90,120,150,180,and 240 minutes),peak blood glucose,and area under the curve (AUC) in 4 time periods (0-60,63-120,123-180,and 183-240 minutes).Results The percentage of rapidly digestible starch was remarkably lower in fried dumplings than in boiled dumplings (30.8% vs.77.0%),but the content of slowly digestible starch in fried dumplings was higher than that in boiled ones (63.7% vs.20.7%),and the content of resistant starch in both dumplings were similarly low (1.9% and 2.3%).The average time to the peak glucose value was shorter in fried dumplings compared with boiled dumplings [(93 ± 53) minutes vs.(156 ± 61) minutes,P =0.02],but the average glucose levels at all the 9 time points and the AUC in all the 4 time periods were not significantly different (all P > 0.05).Conclusions Compared with fried dumplings,boiled dumplings show faster starch digestion,but long time to the peak postprandial glucose level.Fried dumplings may raise the glucose level faster than boiled dumplings do in T2DM patients.
10.Analysis of research hotspot concerning publications on organ transplantation

Objective To analyze the current research focus of solid organ transplantation in order to provide information for Chinese clinicians and researchers for research direction and topic selection.Method By using bibliometric method,the Chinese Science Citation Database and Web of Science were thoroughly searched and highly cited papers concerning solid organ transplantation were summarized and analyzed from 2010 to 2013.With the year of publication,national and institutional distribution,source journals,column type,research content as indicators,all the keywords of high-frequency were analyzed via co-word analysis method in order to summarize the characteristics of hot research subject in the field of solid organ transplantation.Result From 2010 to 2013,top100 highly cited articles in Chinese had a preference for liver transplantation,followed by kidney transplantation,immunosuppressant,organ donation,postoperative complication and rejection,while articles in English mainly focused on surgical technique,recipient survival,transplant immunity and rejection.Top 100 highly cited articles were from 50 and 58 institutions for Chinese and English,respectively.There were 47 papers from top 10 institutions in Chinese,accounting for 45.6% of the total citations.There were 43 papers from top 10 institutions in English,accounting for 47.4% of the total citations.Top 100 highly cited papers in Chinese were distributed in 27 journals.The top 10 journals published 75 papers,which have been cited for 316 times.Top 100 highly cited papers in English were distributed in 36 journals.The top 10 journals published 70 papers,which have been cited for 7 694 times.Conclusion Domestic experts and scholars should pay continuous attention to the organ donation issues related to research in the future,with reference to the advanced experience of Europe and the United States and other countries.Solid organ transplantation registry,data center report and update,transplant recipient prognosis and long-term survival should be highly focused.
