1.Simultaneous Determination of Glycyrrhizic Acid and Glycyrrhetinic Acid in Yaotongning Capsule by HPLC

OBJECTIVE:To establish a method for simultaneously determination of glycyrrhizic acid and glycyrrhetinic acid in Yaotongning capsule. METHODS:HPLC was performed on the column of Agilent TC-C18 with mobile phase of methanol-0.2 mol/L ammonium acetate(gradient elution) at a flow rate of 1.0 ml/min;detection wavelength was 250 nm and column temperature was 25 ℃ and injection volume was 10 μl. RESULTS:The linear range was 0.007 1-0.178 0 mg/ml(r=0.999 8)for glycyrrhizin acid and 0.354 8-8.720 0 μg/ml of glycyrrhetinic acid(r=0.999 8);RSDs of precision,stability and reproducibility tests were no more than 1.74%,average recoveries were 95.49%-100.62%(RSD=1.98%,n=9)and 96.80%-102.26%(RSD=1.83%,n=9),respec-tively. CONCLUSIONS:The method is simple,reproducible,accurate and reliable,and can be used for the simultaneous determi-nation of glycyrrhizic acid and glycyrrhetinic acid in Yaotongning capsule.
2.Repair of intestinal irradiation injury by Flk-1~+ MSCs and SP cells

Objective Comparing biological characteristics and the roles in repairing of intestinal irradiation injury of bone marrow derived Flk-1+ MSCs and small intestinal derived SP cells.Methods Phenotypes of the two cell population were determined by flow cytometery.Flk-1+ MSCs and intestinal SP cells were infused into irradiated mice,distribution and differentiation of these cells were determined.Histopathological change and fibrosis were evaluated.Results The phenotypes of Flk-1+ MSCs and intestinal SP cells were quite different.Both Flk-1+ MSCs and intestinal SP cells could home to injury tissue rapidly,participate intestinal epithelial repair,and alleviate fibrosis and collagen deposition after irradiation.SP cells mostly engrafted into intestinal epithelium,while Flk-1+ MSCs partly engrafted into interstitial region as well as epithelium.Conclusion Flk-1+ MSCs and intestinal SP cells may alleviate symptoms effectively,though their mechanisms are disparite.
3.Total thyroidectomy plus functional neck lymph node dissection for the treatment of papillary thyroid carcinoma

Objective To explore the pattern of cervical nodal metastasis and the clinical significance of total thyroidectomy plus functional neck lymphadenectomy in papillary thyroid carcinoma patients. Methods Clinical and pathological data of 172 patients with papillary thyroid cancer who underwent total thyroidectomy plus functional neck lymph node dissection at Jinan Military General Hospital were retrospectively reviewed, including patient demographics, extent of surgery, parathyroid hormone level,recurrence, tumor pathology, such as tumor size, multifocality, capsular invasion, vascular invasion,extrathyroidal extension, and lymph node status. Results Of the 172 functional neck dissection patients (47 ambilateral), the incidence of lymphonodus metastasis in regions Ⅵ, Ⅳ and Ⅲ was 96. 3% ,78. 5%and 62.1% respectively. Rate of nodal metastasis was higher in patients with extracapsular invasion than in patients with no invasion ( P < 0. 05 ). Serum parathyroid hormone levels significantly decreased immediately postoperatively in total thyroidectomy plus functional neck dissection and remained low for several weeks thereafter ( P < 0. 01 ). The 5-, 10- and 15-year survival rate was ( 98. 83 ± 0. 82) %, (98. 23 ± 1.02 ) % and (96. 42 ± 1.43 )%, respectively. Conclusions Therapeutic neck lymph node dissection for papillary thyroid cancer is recommended for cervical nodal metastasis patients. Total thyroidectomy plus functional neck dissection is important in the treatment of papillary thyroid cancer.
4.Change of Caspase-3 and PSD-95 Expression in Hippocampus of Mice with Chronic Ketamine Addiction

Objective To study the changes of hippocampal caspase-3 and PSD-95 expression levels in the mice exposed to ketamine 30 mg/(kg·d)for three months. Methods Forty C57BL/6 mice were randomly divided into two groups,and the chronic ketamine addiction model was established by giving mice a three month course of daily intraperitoneal injections of ketamine. Immunohistochemical study and Western blot-ting were applied to observe the expression of caspase-3 and PSD-95 protein. Results There were more expression of caspase-3 and less of PSD-95 in ketamine group as detected by immuohistochemistry. Western blotting results showed caspase-3 active fragment level significantly increased com-pared to saline group,but PSD-95 protein level was decreased. Conclusion The increased level of caspase-3 protein and reduced expression of PSD-95 are observed after long-term ketamine administration. These findings may provide an evidence for the neurotoxicity in mouse hippocampus of chronic ketamine addition as a recreational drug.
5.Progress and challenges in optical cochlear implant.

Optical cochlear implant has been occuring as a new cochlear implant which utilizes laser pulses to stimulate hearing. Compared to electronic cochlear implant, it has demonstrated higher spatial selectivity and less radiation scattering, which could lead to higher fidelity cochlear prostheses. At present, most investigations have focused on experiments in vivo. Although a lot of exciting results have been obtained, the mechanisms of laser stimulation is still open. In this paper, a brief review on the recent new findings of optical cochlear implant is given, and possible mechanisms are discussed. In the end, new experimental proposals are suggested which could help to explore the mechanisms of laser-cochlea stimulation.

Objective To evaluate the safety of the da Vinci Si surgical system in thyroid surgery and to accumulate operation experience .Methods The da Vinci Si surgical system consists of a surgeon's console, a patient-side robotic cart, and high-definition 3D vision system.The robot arm tips were introduced via a single axillary incision or small chest wall ports and attached to the arms of the robot .The surgeon , sitting at the con-sole, manipulated the EndoWrist instruments and endoscope view that transfer the surgeon 's movements to the arm tips.The so called EndoWrist'technology offers seven degrees of movements ( up, down, left, right, twist, et al) , thus exceeding the capacity of a surgeon's hand in open surgery .The da Vinci Si surgical system was used to perform thyroidectomy in two small pigs .The animals were intubated and kept anesthetized with halothane .Re-sults Four surgical procedures were done using the da Vinci Si system from beginning to the end , including 2 thyroidectomies and 2 thymectomies .No conversions to open or laparoscopic procedure was used .The procedure length was 61 minutes and 50 minutes, respectively.Conclusions Robotic thyroidectomy can be performed safe-ly by the experienced surgeons after short-term da Vinci Si surgical system training .The learning curve for robotic thyroidectomy is shorter .
7.Robotic thyroidectomy with central neck dissection using axillo-bilateral-breast approach: a comparison to open conventional approach.

OBJECTIVETo evaluate surgical outcomes and the feasibility of robotic thyroidectomy and central neck dissection (CND).
METHODSThe clinical data of 40 patients of papillary thyroid microcarcinoma underwent total thyroidectomy (or lobectomy and isthmusectomy) and CND using the Da Vinci system through axillo-bilateral-breast approach in Jinan Military General Hospital of People's Liberation Army from February to December 2014 were analyzed retrospectively (robotic group). Other forty patients of papillary thyroid microcarcinoma underwent total thyroidectomy (or lobectomy and isthmusectomy) and CND by open approach were selected as the control (open group). Cosmetic satisfaction was assessed after a month postoperation by the numerical score system. t-test and χ(2) test were used to compare the clinical characters, total operative time, intraoperative estimated blood loss, postoperative hospital stay, number of lymph nodes removed, visual analogue scale for pain, postoperative complications, and cosmetic effect between the 2 groups.
RESULTSAll 80 patients were diagnosed of papillary thyroid microcarcinoma. The total thyroidectomy (or lobectomy/isthmusectomy) with CND of 40 patients were successfully performed by da Vinci Si surgical system. The numbers of total thyroidectomy of robotic group and the open group were 36 and 37, respectively. The numbers of metastatic lymph nodes of robotic group and open group were 14 and 15, respectively. The operation time of the robotic group was (130±12) minutes, which was longer than that of open group (98±11) minutes (t=12.432, P<0.05). The study showed statistical significant difference between the two groups regarding the visual analog scale pain assessment (1.9±0.9 vs.3.9±1.1, t=8.900, P<0.05). There were no statistical significant difference of intraoperative estimated blood loss, postoperative hospital stay, number of lymph nodes removed, and the complication rate between the 2 groups.Postoperative cosmetic result was more satisfying on the robotic group (9.1±0.5) than open group (4.8±1.5) (t=17.200, P<0.05).
CONCLUSIONSThe robotic total thyroidectomy (or lobectomy and isthmusectomy) and CND has similar surgery safety and feasibility as open procedures. The robotic thyroidectomy is a good alternative surgical modality for patients with papillary thyroid microcarcinoma who wish to avoid neck scars.

Objective To investigate the diagnostic value of combining carotid intima-media thickness(cIMT)with serum Galectin-3(Gal-3)and Pentraxin 3(PTX3)in patients with psoriasis complicated with cardiovascular disease(C VD).Methods Thirty-eight patients with psoriasis com-plicated with CVD were included in CVD group,51 patients with psoriasis alone were included in pso-riasis group,and 60 healthy subjects were selected as control group.Clinical data were collected from each group.The cIMT was measured using carotid ultrasound,and serum Gal-3,PTX3,and inflamma-tory markers[high sensitivity C-reactive protein(hs-CRP),procalcitonin(PCT),tumor necrosis fac-tor-α(TNF-α)]were detected using enzyme-linked immunosorbent assay.Multivariate Logistic regression analysis was used to identify the influencing factors of CVD in patients with psoriasis.The receiver operating characteristic(ROC)curve was plotted to evaluate the diagnostic value of serum Gal-3,PTX3,and cIMT for CVD.Results The cIMT was greater in the CVD group and the psoriasis group than in the control group,and the CVD group was greater than the psoriasis group(P<0.05).The serum levels of Gal-3,PTX3,hs-CRP,PCT,and TNF-α were higher in the CVD group and the psoriasis group than in the control group,and their levels were higher in the CVD group than in the psoriasis group(P<0.05).The psoriasis area and severity index(PASI)score was higher in the CVD group than in the psoriasis group(P<0.05).Pearsons correlation analysis showed that cIMT,Gal-3,and PTX3 were separately positively correlated with hs-CRP,PCT,and TNF-α(P<0.05).Multivariate Logistic regression analysis showed that PASI score,cIMT,Gal-3,PTX3,hs-CRP,and TNF-α were independent influencing factors for CVD in patients with psoriasis(OR=1.250,1.451,1.432,1.365,1.413,1.465;P<0.05).The diagnostic model based on cIMT combined with serum Gal-3 and PTX3 had higher diagnostic value for CVD in patients with psoriasis than single diagnosis.The area under the curve was 0.922,with sensitivity and specificity of 0.91 and 0.85,respectively.Conclusion The cIMT,serum Gal-3,and serum PTX3 are inde-pendent influencing factors for CVD in patients with psoriasis.The combination of the three indica-tors has higher diagnostic value for CVD in patients with psoriasis.

10.Design and Construction of a Specialized Clinical Research Database for Inflammatory Demyelinating Diseases of the Central Nervous System

Purpose/Significance To construct a specialized database for inflammatory demyelinating disease of the central nervous system(CNS),so as to contribute to clinical research and improve the diagnostic and treatment capabilities of primary healthcare institu-tions.Method/Process Using the internet to collect medical data,after processing and analysis,the CNS inflammatory demyelinating disease database is constructed.Using statistical analysis,natural language processing(NLP),artificial intelligence(AI)image recog-nition and data visualization and other technologies,the database information is integrated and analyzed.Result/Conclusion A standard-ized big database for CNS inflammatory demyelinating diseases is constructed,which enables visualization of clinical research data,pro-vides patient education and specialist training,and facilitates multi-center teleconsultations.The establishment of a specialized database for the CNS inflammatory demyelinating disease can promote the transformation of medical research achievements,provide references for future real-world clinical research,optimize the process of diagnosis and treatment,and improve the clinical capability of primary healthcare institutions.
