1.Perioperative nursing of patients with limb long bone defects treated with orthofix limb reconstruction system

Objective To summarize the nursing methods of Orthofix limb reconstruction system in the treatment of limb long bone defects. Methods Thirty-two patients with long bone defects were collected between January 2012 to January 2015. The patients were all treated with the Orthofix limb reconstruction system and perioperative nursing. Results All the cases were treated by the method for 8 . 5 to 18 . 2 months , averaged 15 . 40 ± 3 . 20 months . The bones extended 4 ~ 21 cm , averaged ( 8 . 06 ± 3 . 06 ) cm . The assessments of bone transport treatment showed 31 cases were excellent and 1 good. Conclusions The limb long bone defects with or without soft tissue defects caused by various causes can be treated by the Orthofix limb reconstruction system. Patients should pay attention to psychological counseling and nutritional support preoperatively. It is important for patients to pay attention to bone transport and functional exercise guidance postoperatively.
2.Effect of modified closed negative pressure drainage on abnormal healing of abdominal incision

Objective To explore the curative effect of modified closed negative pressure drainage on abnormal abdominal incision. Methods Sity-three patients with abnormal healing of abdominal incision in our hospital from January 2012 to June 2013 were selected as the observation group. Another 50 patients from January to December 2011 were assigned to the control group. The former was treated after debridement with human recombinant surface growth factor and modified closed negative pressure drainage,while the latter after debridement with anti-infection and dressings of ethacridine or gentamicinsolution as well as with infrared therapy. The two groups were compared in terms of the frequency,time and cost of changed dressings,and the healing time of the incisions.Results There was insignificant difference in the cost of changed dressings between the two groups(P>0.05). However,the differences were statistically significant in terms of the frequency and time of changed dressings and the healing time of the incisions(P<0.05). The observation group was significantly superior to the control group.Conclusions Modified closed negative pressure drainage is effective in accelerating the healing in abnormal abdominal incisions,reducing the frequency and the time of changed dressings,and easing the economic burdens of the patients,which is suggested to be popularized and applicable in the pimary hospitals.
3.Effect of Omaha system measures combined with voice training in the treatment of patients with functional vocal dyspraxia caused by glottic insufficiency

Objective To explore the effect of Omaha system combined with voice training in the treatment of patients with functional vocal dyspraxia caused by incomplete glottic closure. Methods From January 2017 to December 2017,eighty-eight patients with vocal dystonia due to glottic insufficiency who were treated in the Second Hospital of Shaoxing were randomly divided into control group and observation group by random number table method, with 44 cases in each group.The control group was treated with voice training.The observation group was given Omaha system measures on the basis of treatment in the control group.After 8 weeks,the therapeutic effects,GRBAS index scores,VHI scores,changes in acoustic parameters,and laryngoscope scores were compared between the two groups. Results After treatment,the total effective rate of the observation group (90.91% ) was significantly higher than that of the control group(59.09% ) (χ2 =9.317,P<0.05).After treatment,the scores of total deafness,roughness and other indicators were significantly reduced in both two groups,and the scores of GRBAS in the observation group were significantly lower than those in the control group(t=8.674,P<0.001;t=2.169,P=0.032;t=2.489,P=0.014;t=32.832,P<0.001;t=9.381,P<0.001).After treatment,the index scores and total VHI scores of both two groups were significantly decreased,and the scores of the observation group were significantly lower than those of the control group(t=2.222,P =0.028;t =3.144,P =0.002;t =2.003,P =0.048;t =5.763,P <0.001). After treatment,the maximum phonation time(MPT),basic frequency( Jitter),and lowest sound intensity( I-Low) were significantly reduced in the two groups, which in the observation group were significantly lower than those in the control group( t =4.037,P <0.001;t =4.789,P <0.001; t =4.717,P <0.001). The highest basic frequency (F0-High) and DSI index significantly increased in both two groups,which in the observation group were signifi-cantly higher than those in the control group(t=4.497,P<0.001;t=4.794,P<0.001).Conclusion The combi-nation of Omaha system measures and voice training can significantly improve functional vocal disturbances caused by glottic insufficiency,significantly improve the quality of patients'voices,improve glottic closure,and contribute to the recovery of patients'voices.It is worthy of promotion in clinical practice.
4.Effect of lncRNA PP7080 on the migration and proliferation of gastric cancer cell MGC803 by regulating TMSG-1 expression

Objective:To observe the expression of long non-coding RNA (lncRNA) PP7080 in gastric cancer tissues and cell lines, and clarify its effect on the migration and proliferation of gastric cancer cell MGC803 and its molecular mechanism.Methods:Real-time PCR was used to detect the expression of lncRNA PP7080 in gastric cancer tissues and adjacent tissues, gastric cancer cell lines and immortalized normal gastric mucosal epithelial cell lines. The gastric cancer cell line with the least expression was selected, and the expression plasmid of lncRNA PP7080 and the negative control plasmid were transfected into gastric cancer cells, respectively, and named as lncRNA PP7080 group and NC group. Real-time PCR to verify the effect of transfection. Cell scratch test and CCK-8 test were used to detect the regulation of lncRNA PP7080 on the migration and proliferation of gastric cancer cells.The statistical saftware SPSS 20.0 was used for statistical analysis, the measurement data of normal distribution were expressed as ( Mean± SD). Real-time PCR and Western blot were used to detect the expression of tumor metastasis suppressor gene 1 ( TMSG-1) in the transfected cells. The t test was used for comparison between groups. Results:The expression of lncRNA PP7080 in gastric cancer tissue was less than that in adjacent tissues [(0.85±0.34) vs (5.33 ± 0.76), P<0.01]. The expression of lncRNA PP7080 in gastric cancer cell lines is less than that of immortalized normal gastric mucosal epithelial cells ( P<0.05), and the least expression in MGC803 cells ( P<0.01). The expression of lncRNA PP7080 in the lncRNA PP7080 group was significantly higher than that in the NC group, and the difference was statistically significant ( P<0.01). The cell migration rates of NC group and lncRNA PP7080 group were (72.67±6.39)% and (26.45±6.63)%, respectively, and the cell migration ability of lncRNA PP7080 group was significantly reduced ( P<0.01). Compared with the NC group, the cell proliferation ability of the lncRNA PP7080 group was significantly reduced ( P<0.05). Compared with the NC group, the expression of TMSG-1 in MGC803 cells of the lncRNA PP7080 group was significantly reduced ( P<0.01). Conclusion:lncRNA PP7080 is lowly expressed in gastric cancer tissues and cell lines. lncRNA PP7080 can inhibit the migration and proliferation of gastric cancer cell MGC803 by promoting the expression of TMSG-1 gene.
5.Determinations of mifepristone and its metabolites and their pharmacokinetics in healthy female Chinese subjects.

The aim of this study is to establish an HPLC method for simultaneous determinations of mifepristone and its metabolites, mono-demethylated mifepristone, di-demethylated mifepristone and C-hydroxylated mifepristone in plasma and to evaluate the pharmacokinetic characteristics of mifepristone tablet. Twenty healthy female Chinese subjects were recruited and a series of blood samples were collected before and after 0.25, 0.5, 1.0, 1.5, 2.0, 4.0, 8.0, 12.0, 24.0, 48.0, 72.0 and 96.0 hours administration by a single oral dose of 75 mg mifepristone tablet. Mifepristone and its three metabolites were extracted from plasma using ethyl acetate and determined by high performance liquid chromatography. The main pharmacokinetic parameters of mifepristone and its metabolites, including Cmax, tmax, MRT, t(1/2), V, CL, AUC(0-96 h) and AUC(0-infinity), were calculated by Drug and Statistical Software Version 2.0. The simple, accurate and stable method allows the sensitive determinations ofmifepristone and its metabolites in human plasma up to 4 days after oral administration of 75 mg mifepristone tablet and the clinical applications of their pharmacokinetic studies.
6.Risk factors and treatment outcome of recurrent acanthamoeba keratitis after corneal transplantation

Objective:To investigate the risk factors and treatment outcome of recurrent Acanthamoeba keratitis (AK) after corneal transplantation. Methods:A serial case-observational study was carried out.Twenty-eight eyes of 28 patients with AK who underwent corneal transplantation in Shandong Eye Hospital from January 2012 to January 2019 were enrolled.All the eyes received corneal transplantation from failing to respond to topical and systemic anti- Acanthamoeba medical therapy, including 13 eyes that received penetrating keratoplasty (PKP) and 15 eyes that received lamellar keratoplasty (LKP). The corneal lesion was removed by a trephine with a diameter of 0.5 mm over infiltration area during PKP or LKP.The clinical features of recurrent AK were summarized, including recurrence time, site and signs, and the risk factors of AK recurrence were analyzed.Local and systemic anti- Acanthamoeba medical therapy was performed in all relapsed eyes, and secondary surgery was performed for the eyes with poor response to medication.The therapeutic outcome of recurrent AK was evaluated.The study adhered to the Declaration of Helsinki.This study protocol was approved by an Ethics Committee of Shandong Eye Hospital (No.201112). Results:In the 28 eyes, 7 eyes (25%) appeared recurrent AK after keratoplasty, including 2 eyes after PKP and 5 eyes after LKP.There was no significant difference in the recurrence rate between the two methods ( P=0.396). The recurrence rate of eyes that had used glucocorticoids drugs before operation was 57.14% (4/7), which was significantly higher in comparison with 14.29% (3/21) of eyes without glucocorticoids before surgery ( P=0.043). The recurrence rate of eyes with ulcer diameter ≥8.2 mm was 50.00% (5/10), which was significantly higher than 11.11% (2/18) of eyes with ulcer <8.2 mm ( P=0.036). The recurrent lesions began at the edge of implant bed accounted for 85.71% (6/7), and the recurrent lesions located below graft accounted for 14.29% (1/7). In 7 eyes with recurrent AK, 6 eyes were completely cured.Among recurrent AK eyes after LKP, 2 eyes were cured by long-term medical therapy, and 2 eyes were cured by extended-diameter LKP, and another 1 eye was cured by conjunctival flap covering surgery.One eye with recurrent AK after PKP was cured by extended-diameter PKP. Conclusions:The risk factors of recurrent AK after surgery are application of glucocorticoids before surgery and big lesions.Recurrent AK after surgery is curable by individualized therapy targeting to different clinical characteristics.

ABSRACT The development of hepatocellular car-cinoma(HCC)is closely related to the formation of tumour blood vessels.VEGF-mediated angiogenesis is a major driver of the immune escape response in tumours.VEGF binds to vascular endothelial growth factor receptor2(VEGFR2)on endothelial cells,promoting endothelial cell proliferation and migration,inducing vascular changes in HCC,and thus promote the growth of hepatocellular carcino-ma cells.Anti-VEGF and its receptor-targeted mo-lecular drugs are currently effective new treat-ments for HCC.Monoclonal antibodies against VEGF and small-molecule tyrosine kinase inhibitors targeting VEGF have been shown to block its angio-genic activity,alleviate the inhibitory effect of the tumour microenvironment,and ultimately achieve tumour regression.This article provides a review of the research progress of VEGF/VEGFR inhibitors in HCC treatment.
8.Diagnosis, treatment and research progress of hypersensitivity pneumonitis in children

Hypersensitivity pneumonitis (HP) is an immune-mediated interstitial lung disease (ILD). It is a disease with highly heterogeneous clinical manifestations, severity and outcomes, which are associated with individual sensitivity, as well as property, dosage, duration and frequency of exposure to the antigens.The 2020 adult HP guideline reclassifies it and describes its radiographic features in detail.HP often occurs in adults, also affects the pediatric population and is one of the most common ILDs in children.The most common factors causing HP in children are avian and fungal antigens in the home environment.The diagnosis of HP is based on clear antigens, typical symptoms and characteristic radiological manifestations.The serum-specific IgG antibody, bronchoalveolar lavage fluid, and pulmonary function tests can help diagnose HP clearly, and lung histopathology is required for children whose diagnosis cannot be confirmed.Early diagnosis and adequate avoidance of antigen exposure are the keys to its treatment and prognosis.
