1.The relationship between serum cystatin C and coronary artery disease in type 2 diabetes mellitus patients with normal uric protein

Objective To investigate the relationship between serum cystatin C and coronary artery disease in type 2 diabetes mellitus (T2DM) patients with normal uric protein.Methods According to the coronary artery lesion diagnosed by 320-dynamic volume CT,the 126 T2DM patients with normal uric protein were divided into three groups:no coronary stenosis group (group A,32 cases),coronary atherosclerosis group(group B,38 cases),coronary heart disease group (group C,56 cases).Then the serum cystatin C etc were compared among the three groups.Results The levels of serum cystatin C in group A,B,C were (0.89 ± 0.27),(1.31 ± 0.53),(1.54 ± 0.62) mg/L.With the increase of coronary artery lesions,it gradually increased,there was significant difference among the three groups (P < 0.05).The patients were divided into three groups according to the level of serum cystatin C quartile.The incidence of coronary artery lesion in creased with the increased levels of serum cystatin C.The level of serum cystatin C increased from 75th percentile to 100th percentile,the incidence of coronary heart disease increased significantly (OR =8.32,P <0.05).The result of multiple Logistic regression analysis showed that history of hypertension (regression coefficient 4.135,P =0.000),glycosylated hemoglobin (regression coefficient 1.257,P =0.002),low density lipoprotein-cholesterol (regression coefficient 3.381,P =0.015),cystatin C (regression coefficient 2.046,P =0.030) were the independent risks of coronary heart disease in patients with T2DM.Conclusion The level of serum cystatin C may be a predictor for coronary heart disease in T2DM patients with normal uric protein.
2.Observation and nursing effect of pediatric pneumonia with dyspneic cough adjunctively treated with orientation of traditional Chinese medicine drug therapy and multiple frequency vibration therapeutic instrument

Objective To evaluate the nursing effect of orientation of traditional Chinese medicine drug therapy combined with multiple frequency vibration therapeutic instrument in treating pediatric pneumonia with dyspneic cough. Methods A total of 277 children (pneumonia with dyspneic cough syndrome of wind-cold invading lung, wind-heat invading, lungphlegm-heat congesting lung) were enrolled in this study. They were divided into the orientation of traditional Chinese medicine drug therapy combined with multiple frequency vibration therapeutic instrument group (lung therapy and sputum excretion group,100 cases), the orientation of traditional Chinese medicine drug therapy group (lung therapy group, 94 cases) and the multiple frequency vibration therapeutic instrument group (sputum excretion group, 83 cases) by random digits table method.The main symptoms improvements, therapeutic evaluation, the comfortable level of the children, family satisfaction degree of the 3 groups were evaluated before and after 5 days of treatment. Results After 5 days, the main symptoms score was (7.22±2.74) points in the lung therapy and sputum vibration group, and (8.98±3.40), (8.78±2.91) points in the lung therapy group and sputum excretion group, there were significant differences among 3 groups, F=9.756, P<0.01. The nursing effect in the lung therapy and sputum vibration group was better than that in the other two groups,and there were significant differences among 3 groups, Z=10.376, P<0.01. There were no significant differences among 3 groups in the comfortable level of the children,χ2=1.868,P>0.05. The family satisfaction degree in the lung therapy and sputum vibration group was higher than that in the other two groups, and there were significant differences among 3 groups, χ2=9.102, P<0.05. Conclusions The nursing effect of orientation of traditional Chinese medicine drug therapy combined with multiple frequency vibration therapeutic instrument has more advantages in treating pediatric pneumonia with dyspneic cough.
3.Clinical Investigation of Lower-extremity Arterial Disease in Patients with Newly Diagnosed Type 2 Diabetes Mellitus Combined with Nonalcoholic Fatty Liver Disease

[Objective] To investigate the clinical characteristics and risk factors of lower-extremity arterial disease in the patients with newly diagnosed type 2 diabetes mellitus combined with nonalcoholic fatty liver disease (NAFLD). [Methods] One hundred fifty-one patients were investigated respectively. The patients were divided into two groups (NAFLD-Group and non-NAFLD group) by liver ultrasonography and disease history, then their clinical data were collected and compared in order to find the differences of biochemical indicators and the morbidity of lower-extremity arterial disease between two groups. [Results] Ninety-two cases (60.93%) were complicated with NAFLD. NAFLD group had higher levels of fast insulin and C peptide level, postprandial insulin and C peptide level, uric acid, body mass index (BMI), homeostasis model assessment (HOMA-IR) and lower level of high-density lipoprotein cholesterol and insulin sensitive index than those of without NAFLD (P<0.05). One hundred and one cases(66.89%) were complicated with lower-extremity arterial disease. The morbidity of lower-extremity arterial diseases was higher in NAFLD group than that of without NAFLD group (75% vs. 54.24%, P<0.01). [Conclusion] Both lower-extremity arterial disease and NAFLD are common complicated with type 2 diabetes. The morbidity of lower-extremity arterial diseases was higher in NAFLD group than that of without NAFLD group.
4.Analysis of risk factors of various degrees of non-alcoholic fatty liver disease in patients with type 2 diabetes mellitus

Objective To investigate the relationship between dyslipidemia,obesity,insulin resistance (IR)and various degrees of non.alcoholic fatty liver disease(NAFLD)in patients with type 2 diabetes mellitus (T2DM), and the risk factors of NAFLD.Methods Two hundred and sixty-eight patients were divided into three groups(non-NAFLD group,mild NAFLD group,moderate and severe NAFLD group)by liver ultrasonography.Body height(H),weight(W),waist circumference(WC),hip circumference(H)were measured.The levels of fasting blood glucose (FBG),glycosylated hemoglobin A_1c(GHbA_1C),serum total cholesterol(TC),serunl high density lipoprotein(HDL-C),serum low density lipoprotein(LDL-C),serum triglyceride (TG),alanine aminotransferase (ALT)and fasting serum insulin(FINS)were measured.Body mass index(BMI),the waist to hip ratio(WHR)and insulin resistance index(HOMA-IR)were calculated.Unconditional logistic regression model was used to test for the risk factors of NAFLD.Results BMI、WC、WHR、HNS、HOMA.IR、TC、LDL-C、TG and ALT in NAFLD group were significantly higher than those in non-NAFLD group (P<0.05).The levels of BMI、WC、WHR、HNS、HOMA-IR、 TG and ALT increased significantly in moderate and severe NAFLD group compared with mild NAFLD group(P<0.05).TG、WHR and HOMA.IR were the risk factors of NAFLD(P<0.05,OR=2.394,3.273,5.256).Conclusions NAFLD in patients with T2DM had remarkable dyslipidemia,overweight,central obesity and insulin resistance.TG、WHR and HOMA.IR were risk factors of NAFLD.
5.Effect of sulodexide on islet β-cell function in streptozocin induced diabetic rats

Objective To study the effects of sulodexide on islet B-cell function in streptozocin induced di-abetic rats. Methods Sprague-Dawley(SD) rats were randomly divided into normal control group (group C), dia-betic group without treatment(group D), and suledexide treatment group(group S), a single dose of streptozotocin were abdominally injected to establish the diabetic rat models. Each animal in sulodexide treated group was addition-ally fed with sulodexide of 10 mg/(kg·d) for 12 weeks,while the remained group (group C and D) were given normal water in the same period. After 12 weeks of treatment, fasting plasma glucose(FPG),fasting plasma insulin (FINS), activated partial thromboplastin time (APTT), prothrombin time (PT), thrombin time (TT), triglyceride (TG), Low-density lipoprotein cholesterol (LDL-C), serum creatinine rates (SCr) and alanine aminotransferase (ALT) were measured. Insulin sensitivity index(ISI) and insulin resistant index (HOMA-IR) were calculated. Results After 12 weeks, the levels of TG, LDL-C and ALT had no significant difference between group D and group S, but were higher than those in group C (P <0.05);There were no significant difference of SCr levels among the three groups. Compared with the group C, APTT, PT, TT and ISI in group D and S were significantly decreased, HOMA-IR were significantly increased (P < 0.05). APTT, PT, TT and ISI in group S had significantly increased compared with that in group D, HOMA-IR was significantly decreased in group S compared with that in group D (P < 0.01). Conclusions Sulodexide can reduce insulin resistant, improve hypercoagulability and insulin sensitiv-ity in streptozocin induced diabetic rats. The effects to blood lipid, liver and renal functions in diabetic rats are not obvious.
6.Comparison of codon optimizations of cry1Ah1 gene in rice.

cry1Ah1, one of holo-type cry genes, cloned in this laboratory from Bacillus thuringiensis strain has been patented in China, and it encoded a protein with strong insecticidal activity against certain lepidopteran insect pests, such as Chilo suppressalis. cry1Ah1 gene is exhibiting good application prospects. In order to improve the expression level of cry1Ah1 gene in rice, and investigate the effect of codon usage preference of gene expression, we designed five different optimized schemes for cry1Ah1 insecticidal critical fragment in accordance with bias of rice codon, to improve G+C content, removed the shear signal and unstable factors. Optimized cry1Ah1 genes were transformed into Escherichia coli Rosetta (DE3) respectively, and 65 kDa polypeptides was expressed normally in inclusion body separately. All of these expressed polypeptides showed insecticidal activity against 2nd-instar larvae of Plutella xylostella and neonate of Chilo suppressalis. After transformation with modified cry1Ah1 genes into Var nippobare, the transgenic rice seedlings were detected by PCR, the positive rate containing target gene was more than 87%. Afterwards, the results of real-time RT-PCR and ELISA assay indicated that the highest expression level of five modified cry1Ah1 genes was that using the highest frequent codons. Average expression amount of Cry1Ah1 polypeptides was 0.104% of total soluble proteins from the positive transgenic rice.

7.Application of pulmonary rehabilitation in elderly patients with acute exacerbation of chronic obstructive pulmonary disease

Chronic obstructive pulmonary disease (COPD) is a common preventable disease characterized by persistent respiratory symptoms and airflow limitation, mainly manifested as chronic cough, expectoration and wheezing. With the aging of the population and the aggravation of environmental pollution, the number of elderly patients with COPD has gradually increased in recent years. Compared with young individuals, the acute exacerbation of elderly COPD patients is more frequent and often complicated with a variety of underlying diseases, even after active drug treatment, the therapeutic effect is still limited. More systematic treatment methods are needed on the basis of conventional drug therapy. The positive role of pulmonary rehabilitation in COPD patients who are in stable period has been confirmed, but in the acute exacerbation of chronic obstructive pulmonary disease (AECOPD) patients is controversial, especially in the elderly AECOPD patients. In this review, we summarized current studies of the application of pulmonary rehabilitation in elderly AECOPD patients.
8.Correlation between inspiratory muscle strength and pulmonary ventilation function in patients with chronic obstructive pulmonary disease

Objective To analyze the correlation between inspiratory muscle strength and pulmonary ventilation function in chronic obstructive pulmonary disease (COPD) patients.Methods A total of 41 COPD patients admitted in the Second Xiangya Hospital from January to September 2018 were screened.The inspiratory muscle strength (MIP) and peak inspiratory flow (PIF) of patients were measured by using POWER breathe K-5 tester.The MIP was also calculated as a percentage of the predicted value.A pulmonary function detector was used to measure the forced vital capacity (FVC),forced expiratory volume in 1 second (FEV1 %),FEV1 as a percentage of FVC (FEV1/FVC%),peak expiratory flow (PEF),forced expiratory flow as a percentage of the predicted value (FEF 50%) at 50% of vital capacity and forced expiratory flow rate as a percentage of the predicted value (FEF 75%) at 75% of vital capacity.According to the Gobal Inistiative for chronic Obstructive Lung Disease (GOLD) classification method of COPD patients,the corresponding MIP value and PIF value of the patients were divided into 4 groups and conducted the comparison between groups.Pearson correlation was used to analyze the correlation between the above-mentioned inspiratory muscle strength test values and lung ventilation function test values,and a scatter diagram was drawn for the ones that had a correlation.Results Among the 41 patients,39 (95.1%) had a decrease in inspiratory muscle strength.According to the GOLD classification,the mean MIP value has a difference among the 4 groups.The mean MIP of grade Ⅲ was [(47.09 ± 29.42) cmH2O],which was higher than that of grade Ⅳ of [(24.72 ± 7.66) cmH2 O] (P < 0.05).There was no difference between PIF.The MIP was positively correlated with FVC (P <0.05).There was no correlation between MIP and FEV1 %,FEV1/FVC%,PEF,FEF50% and FEF75% (P >0.05).The MIP as a percentage of the predicted value had a weak positive correlation with FVC and FEV1% (P <0.05).There was no significant correlation between MIP as a percentage of the predicted value and FEV1/FVC%,PEF,FEF50% and FEF75% (P >0.05).PIF had a weak positive correlation with FVC,PEF,FEF50% and FEF75%,with statistically significant difference (P < 0.05).There was no significant correlation between PIF and FEV1 % and FEV1/FVC% (P >0.05).Conclusions Inspiratory muscle dysfunction was common in patients with COPD.There was a difference in MIP among different GOLD lung function classification in COPD patients.Some values of the inspiratory muscle strength and lung ventilation function had a weak-moderate correlation,while some values had no correlation.Therefore,inspiratory muscle strength test cannot replace the lung ventilation function test in COPD patients.
