1.The clinical study of stent implantation and platelet activation in acute coronary syndrome

Objective To explore the change of platelet activation before and after stent implantation in acute coronary syndrome. Methods Detection of positive expression of platelet membrane glycoprotein CD62p,CD63 and thrombin-sensitive protein (TSP) were performed using flow cytometry (FCM) in 82 patients which included 45 unstable angina pectoris (UAP) and 37 acute myocardial infarction (AMI). Another 45 angiographically normal patients served as the control. Results Expression of CD62p, CD63 and TSP after stent implantation in ACS group. On the other hand, the expression of the above factors was higher in ACS group compared with the control group. Conclusion Acute thrombogenesis induced by platelet activation is a basic pathophysiologic process in ACS. Endothelial injury and plaque rupture produced by stent implantation may be a cause of platelet activation.
2.Treatment of the Pipkin fractures

Objective To investigate the treatment of Pipkin fractures and curative effect. Methods From January 2003 to November 2009, we treated 19 cases of posterior dislocation of the hip with fracture of the femoral head,with type Ⅰ 8 cases, type Ⅱ6 cases, type Ⅲ 1 case, type Ⅳ4 cases. Seventeen patients were treated according to the illness with internal fixation treatment. Results Among type Ⅰ 8 cases, 2 cases were good with conservative treatment, 3 were excellent and 3 were good with surgical treatment;Among type Ⅱ6 cases, 3 were excellent, 2 were good, 1 was fair; 1 patient of type Ⅲ was fair; In 4 cases of type Ⅳ, 1 case was excellent, 2 cases were good, 1 case was fair. Conclusions Surgical treatment as soon as quickly is preferred for posterior dislocation of the hip with fracture of the femoral head( Pipkin fractures) , but the surgical time and methods should be chosen according to patients' detailed illness. It is important reserving the hip bone for preventing traumatic arthritis, and surgical skills to protect blood supply should not be ignored.
3.Severe complications after microwave ablation in 7 403 cases with liver cancer

Objective To identify the types,frequency and risk factors of major complications using microwave ablation (MWA) to treat liver malignancies in a single center.Methods Using the electronic medical record system,patients with liver cancer who were treated with MWA from January 2010 to November 2015 were retrospectively studied,and the risk factors of severe complications were analyzed.Results Of the 7 403 patients who were treated with MWA (12 558 ablations).96 cases experienced severe complications (1.3%) and 5 patients died (0.07%).The complications included liver abscess (n = 34,0.46%),pleural effusion (n = 20,0.27%),bile duct injury (n = 15,0.20%),hemorrhage (n = 6,0.08%),liver failure (n = 6,0.08%),extrahepa-tic organ injuries (n = 5,0.07%),cancerous cells implantation of abdominal walls (n = 2),cardiac arrest (n = 1),vascular thrombosis (n = 1),renal insufficiency (n = 1),and patients with other associated diseases which developed after the MWA therapy (n =2).Metastatic liver cancer and earlier treatment of MWA sessions were associated with a higher major complication rate (P < 0.05).Conclnsions MWA is a well-tolerated local treatment for liver cancer,with an acceptably low incidence of severe complications.Liver abscess was the most common postoperative severe complication.The surgeons' experience and tumor type were associated with the incidence of severe complications,however,not relavant with the frequency of preoperative ablations.Appropriate measures can be taken to effectively reduce the risk of serious complications.
4.A retrospective study after treatment of proximal humeral fractures with locking plates

Objective The purpose of our study was to evaluate the treatment of proximal humeral fractures with AO Philos plate. Methods From June 2007 to May 2009, 29 cases of proximal humeral fractures were fixed with AO Philos plate. According to Neer classification, 11 cases were two-part fractures, 16 cases three-part fractures and 2 cases four-part fractures. Results Twenty-nine cases were followed 9-16 months (averaging 11.3 months). The results were evaluated with Neer's evaluation system: 13 patients were graded as excellent,12 as good, 4 as fair. Conclusion AO Philos plate is less invasive comparing with traditional method. It can provide solid fixation, permit early functional rehabilitation and lead to a compromising clinical result.
5.The Optimal Concentration of Drinking Water in Eastern Guangdong

Objective: The objectives were to explore the optimal concentration of drinking water in Eastern Guangdong and to provide information for the health administrative department to set up the criteria for fluoride concentration. Methods: Totally 1237 9~ 17 and 35~44 years old residents in Eastern Guangdong who drinking water containing 0.1~2.5mg/L fluoride were recruited to investigate the occurrences of dental caries and dental fluorosis. Their background information and perceived appearances of teeth were also recorded. Results: The major factor influencing dental fluorosis was fluoride concentration in drinking water. Dental caries decreased when the fluoride concentration increased but dental fluorosis also increased at the same time. It was showed by calculation that the Dental Fluorosis Index would be 0.4 and 0.6 when the fluoride concentration was 0.57 mg/L and 0.7 mg/L respectively. The acceptation to the severity of dental fluorosis among the study subjects was in accordance with the Dean's criteria. Conclusion: It suggests that the optimal fluoride concentration in Eastern Guangdong is 0.57 mg/L and should not exceed the upper limit of 0.7 mg/L.
6.Application of ovarian transposition during hysterectomy.

OBJECTIVETo study the optimal position and method for ovarian transposition and its benefits and indications.
METHODSWe performed ovarian transposition in 34 patients from August 1989 to December 2000. Twelve patients were diagnosed with stage Ib to IIa cervical cancer, 4 had stage Ia endometrial carcinoma, 12 had stage III to IV endometriosis, 4 had myoma of uterus, 1 had dysfunctional uterine bleeding, and 1 had an ovarian granulosa cell tumor. Surgery went as follows: the ovary was dissociated by clamp, the skin was incised and a tunnel was made, then the ovary was translocated to the subcutaneous site. In the cases of benign lesions, the ovarian vessel pedicel went in through the abdominal cavity, but in malignant tumors, it went out through the peritoneum.
RESULTSIn both cases (benign lesions or malignant tumors), the short-term and long-term endocrine function of the translocated ovary remained normal. Furthermore, patients could supervise their translocated ovary themselves.
CONCLUSIONSSubcutaneous ovary transposition might prevent not only implantation of gastrointestinal cancer but also the extension of pelvic carcinoma to the ovary. Because of the shallow transposition and the incision scar, it is easy for patients to supervise themselves. Moreover, the site of the ovary is easy to locate for ultrasound examinations. Thus, it can obtain the goal of early prevention for cancer. Subcutaneous ovarian transposition with skin incision is the optimal selection and suitable for all patients with various gynecologic diseases in which ovary removal is not necessary.

7.Comparison of blood lipid levels and influencing factors among normotensive young and middle-aged people with different brachial ankle pulse wave velocity

Objective To compare the blood lipid levels of different brachial-ankle pulse wave velocity (baPWV) in young and middle-aged people with normal blood pressure and to explore the related factors affecting baPWV.Methods From January 2014 to December 2017,the clinical data of one thousand two hundred and sixty-eight middle-aged and young people with normal blood pressure who underwent physical examination in Dongying People's Hospital were retrospectively analyzed.Using baPWV< 1 400 cm/s as the standard of normal arterial stiffness,the patients were divided into normal arterial stiffness group (normal group,1 128 cases),abnormal arterial stiffness group (abnormal group,baPWV ≥ 1 400 cm/s,140 cases).The blood lipid indexes of the two groups were analyzed and compared.Logistic regression analysis was used for multivariate analysis and linear correlation analysis was used for linear correlation analysis.Pearson correlation analysis was used.Results Compared with the normal group,TC ((4.99 ± 1.10) mmol/L vs.(4.48 ± 1.03) mmoL/L,t =5.830),TG ((1.62 ± 0.27) mmol/L vs.(1.49 ± 0.23) mmol/L,t=5.102),LDL-C[(3.25±0.23) mmol/L vs.(3.11±0.16) mmol/L,t =4.712),Apo B((0.96 ±0.07) g/L vs.(0.87±0.08) g/L,t =4.297)in abnormal group all increased,and HDL-C((1.15±0.09) mmol/L vs.(1.27±0.07) mmol/L,t =4.712) decreased,and the differences were statistically significant (P<0.05).Smoking,high FPG,high LDL-C,high Apo B,low HDL-C were the independent factors affecting baPWV abnormality (P< 0.05).TC,TG,LDL-C,Apo B and baPWV in abnormal group were positively correlated(P<0.05),and HDL-C and baPWV were negatively correlated(P<0.05).There was a linear regression relationship between LDL-C,Apo B and baPWV (P<0.05).Conclusion The increase of LDL-C and Apo B are closely related to early arterial disease in the low-risk populations of normotensive young and middle-aged people,even the risk of blood lipid may already exist within the normal range.
8.Can Preoperative Peak Expiratory Flow Predict Postoperative Pulmonary Complications in Lung Cancer Patients Undergoing Lobectomy?

Background and objective Postoperative pulmonary complications (PPCs),especially postoperative pneumonia (POP),directly affect the rapid recovery of lung cancer patients after surgery.Peak expiratory flow (PEF) can reflect airway patency and cough efficiency.Moreover,cough impairment may lead to accumulation of pulmonary secretions which can increase the risk of PPCs.The aim of this study is to investigate the effect of preoperative PEF on PPCs in patients with lung cancer.Methods Retrospective research was conducted on 433 lung cancer patients who underwent lobectomy at the West China Hospital of Sichuan University from January 2014 to December 2015.The associations between preoperative PEF and PPCs were analyzed based on patients' basic characteristics and clinical data in hospital.Results Preoperative PEF value in PPCs group (280.93±88.99) L/min was significantly lower than that in non-PPCs group (358.38±93.69) L/min (P<0.001).According to the binary logistics regression analysis,PEF and operative time were independent risk factors for PPCs.Further,ROC curve showed that PEF=320 L/min was the cut-off value for predicting the occurrence of PPCs (AUC=0.706,95%CI:0.661-0.749).The incidence of PPCs in PEF≤320 L/min group (26.6%) was significantly higher than that in PEF>320 L/min group (9.4%)(P<0.001).Conclusion Preoperative PEF and PPCs are correlated,and PEF maybe used as a predictor of PPCs.
9.A prospective clinical study of endoscopic foam sclerotherapy combined with rubber band ligation in the treatment of grade Ⅱ-Ⅲ internal hemorrhoids (with video)

Objective:To evaluate the safety and long-term effectiveness of endoscopic foam sclerotherapy (FS) combined with endoscopic rubber band ligation (ERBL)in the treatment of grade Ⅱ-Ⅲ internal hemorrhoids.Methods:Consecutive patients diagnosed as having grade Ⅱ-Ⅲ internal hemorrhoids in Xinhua Hospital Affiliated to Shanghai Jiao Tong University School of Medicine from January to December 2020 were prospectively enrolled in the study, and randomly divided into ERBL group and FS combined with ERBL group. The 24 h visual analogue scale (VAS) for pain and 1-week degree of bleeding were evaluated after the treatment. After follow-up of 6 months, the effectiveness of treatment was evaluated.Results:A total of 84 patients with age of 54.4±7.9 years were enrolled, 57.1% (48/84) males, and 73.8% (62/84)grade Ⅱ internal hemorrhoids. Forty-three patients were assigned to the ERBL group and 41 to the FS combined with ERBL group. There was no significant difference between the two groups in baseline data ( P>0.05). In the FS combined with ERBL group, the mean amount of polidocanol foam was 13.8±2.5 mL, the mean number of injection site was 4.7±1.2, and the median scores of VAS was 0 (0, 3), which was significantly lower than that of ERBL group [2 (0, 4), Z=-2.116, P=0.034]. The bleeding rate 1 week after treatment in the ERBL and FS combined with ERBL group were 20.9% (9/43) and 29.3% (12/41), respectively, and mild bleeding was the main symptom. There was no significant difference between the two groups in the bleeding degree ( U=807.0, P=0.378). After 6 months of follow-up, the total effective rates in the ERBL group and the FS combined with ERBL group were 81.4% (35/43) and 90.2% (37/41), respectively ( U=684.5, P=0.044). Conclusion:FS combined with ERBL can effectively relieve post-treatment perianal pain, and improve the long-term effectiveness.
10.Clinical value of manual massage in treatment of grade Ⅰ internal hemorrhoids under endoscopic foam sclerotherapy

Objective To evaluate the effect of manual massage on complications after endoscopic foam sclerotherapy injection for the treatment of internal hemorrhoids.Methods Consecutive 113 patients with grade Ⅰinternal hemorrhoids were prospectively enrolled and completed endoscopic foam sclerotherapy.The patients were randomly divided into a massage group(n=65)and a control group(n=68).Massage group performed manual perianal massage,Visual analogue scale(VAS)was used to evaluate perianal pain.The postoperative bleeding,short-term and long-term efficacy were also compared.Results The median VAS of 24 h postoperation was 1.0(0.0,3.0)in massage group,which was significantly lower than 2.0(1.0,4.0)in control group,the difference was statistically significant(P=0.014).The no bleeding rate of one week postoperation was 84.6%in massage group,which was significantly higher than 64.7%in control group,the difference was statistically significant(P=0.009).After 12 weeks,6 months and 12 months of follow-up,there were no significant differences in cure rate and remission rate between the two groups(P>0.05).Conclusion Manual massage after endoscopic sclerosing agent injection is beneficial to relieve postoperative pain of grade Ⅰ internal hemorrhoids and reduce bleeding.
